Retained versus removed copper intrauterine device during pregnancy: An updated systematic review and meta-analysis.

INTRODUCTION: Intrauterine devices (IUDs) are highly effective contraceptives. Despite their effectiveness, pregnancies can occur during IUD use, and the management of such cases, particularly when the pregnancy is desired, remains controversial. MATERIAL AND METHODS: We conducted a systematic review and meta-analysis to evaluate outcomes in women who unintentionally conceived while using IUDs and chose to continue their pregnancies. We searched PubMed, Embase, and Cochrane databases to identify studies comparing women who removed their IUD after pregnancy discovery to those who retained it.

Fertility after hysteroscopic management of retained intrauterine bones: a case report.

Retained intrauterine fetal bone is a rare event that happens after an induced pregnancy termination, usually illegal, especially during the second and third trimesters, due to the incomplete evacuation of fetal tissues. It can cause secondary infertility as bone fragments can work as an intrauterine contraceptive device. We present a case of compromised fertility due to the presence of fetal bony sequestra in the uterine cavity in a 36-year-old female who presented with secondary infertility of eight years duration, following a history of second-trimester voluntary termination of pregnancy. Effect of transabdominal versus transvaginal cerclage on preterm birth and neonatal outcomes among patients with a history of cervical insufficiency.

BACKGROUND: The 2020 Multicentre Abdominal vs Vaginal Randomised Intervention of Cerclage trial found that transabdominal cerclage placed via open laparotomy reduces the risk of spontaneous preterm birth before 32 weeks relative to transvaginal cerclage in a very high-risk obstetric population. It is not known whether the results of the trial generalize to obstetric populations with fewer risk factors, on average, for recurrent spontaneous preterm birth, or to patients without history of failed transvaginal cerclage (an inclusion criterium in the trial). OBJECTIVE: 1) To estimate the effect o

Emergent cervical cerclage: experience from a tertiary center.

INTRODUCTION: Cervical insufficiency (CI) is a significant cause of late miscarriage and preterm birth (PTB) before 27 weeks. In the presence of cervical dilation without associated symptoms, between 14 and 24 weeks, performing an emergent cerclage has shown effectiveness in prolonging pregnancy and improving neonatal outcomes. The aim of this study was to evaluate the obstetric and neonatal outcomes of pregnancies complicated by cervical insufficiency requiring emergent cerclage placement. METHODS: This was a retrospective cohort study conducted at a European tertiary care university hospita

Preterm birth rates and risk factors among women with cervical incompetence undergoing prophylactic cerclage: a systematic review and meta-analysis.

BACKGROUND: Preterm birth is the leading cause of neonatal mortality and long-term health complications. Cervical cerclage (CC) represents a critical intervention for extending pregnancy duration and enhancing neonatal survival in patients diagnosed with cervical insufficiency. The aim of this study was to identify risk factors for preterm birth through a meta-analysis comparing outcomes between preterm and full-term deliveries following non-emergency CC. METHODS: PubMed, EMBASE, Cochrane Library, Web of Science (WOS), and Scopus were searched to identify studies fulfilling predefined inclusi

The efficacy of emergency cervical cerclage in singleton pregnancies at 26-27+6 weeks of gestation: a multicenter retrospective cohort study.

BACKGROUND: Presently, cervical cerclage is regarded as a valid method to treat cervical insufficiency, which could reduce late miscarriage and spontaneous preterm birth (sPTB). Nonetheless, the safety and effectiveness of emergency cervical cerclage (ECC) as a high-risk surgery still remains debatable when the cervix has dilated or the membranes protrude into the vagina during pregnancy at 26-27+6 weeks of gestational age (GA). OBJECTIVE: To estimate maternal and perinatal outcomes of ECC in singleton pregnancy at 26-27+6 weeks of GA compared with expectant treatment. METHODS: In this multi

The predictive role of uterine electromyography in delivery before 28 weeks of gestation after transvaginal cervical cerclage.

BACKGROUND: Delivery before 28 weeks of gestation is a major cause of perinatal morbidity and mortality. Cervical cerclage is widely used to prevent preterm birth (PTB), yet predicting outcomes after cerclage remains challenging. Uterine electromyography (uEMG), a noninvasive method that records myometrial electrical activity, offers objective and accurate assessment of uterine contractions. This study evaluated the predictive value of uEMG for delivery before 28 weeks of gestation in women with singleton pregnancies following transvaginal cervical cerclage. METHODS: In this single-center ret

Clinical management factors influencing gestational age prolongation and neonatal outcomes in second-trimester patients with cervical insufficiency: a retrospective study.

BACKGROUND: Cervical insufficiency is a leading cause of late miscarriage and preterm birth, with high preterm birth rates even after cervical cerclage. Due to its retrospective diagnostic nature, clinical management remains controversial. This study aimed to evaluate clinical factors influencing gestational age prolongation and neonatal outcomes in second-trimester patients with cervical insufficiency, providing evidence for optimal treatment. METHODS: A retrospective analysis was conducted on the clinical data of 348 second-trimester patients with cervical insufficiency. Univariate and multi

Cystatin C 3 (CST3) drives pathological progression in recurrent spontaneous abortion.

This study aimed to investigate the role of Cystatin C (CST3) in trophoblast cell (TBC) function and its contribution in the development of recurrent spontaneous abortion (RSA). We established an inbred RSA model by crossing CBA/J and DBA/2 mice. We investigated and compared expression levels of CST3 and pathological changes in decidual tissues from these RSA and normal pregnant mice. We next isolated TBCs from RSA mice and transfected them with CST3 overexpression or silencing vectors to assess alterations in cell proliferation, invasion, apoptosis, autophagy, oxidative stress and inflammator

Congenital brucellosis in a newborn: A rare case report and clinical insights.

Brucellosis is a zoonotic disease with significant global health implications, particularly in endemic regions such as Saudi Arabia. It is characterized by clinical manifestations that mimic both infectious and noninfectious diseases, making diagnosis challenging. Congenital brucellosis, a rare entity, can be transmitted transplacentally from a bacteremic mother or through exposure to maternal secretions during delivery. This report describes the case of a neonate with extremely low birth weight and severe respiratory distress syndrome, who was ultimately diagnosed with congenital brucellosis
