Applications of single-cell RNA sequencing in fungal infections.

Fungal infection elicits markedly different cellular responses depending on the pathogen, tissue involved, host immune status, and stage of disease. Bulk measurements capture broad changes but can obscure the cell populations associated with fungal control, persistence, or tissue injury. Single-cell RNA sequencing (scRNA-seq) offers a way to resolve this heterogeneity, although fungal infection studies remain uneven in both technical feasibility and disease coverage. We review evidence from cutaneous, mucosal, pulmonary, central nervous system, and systemic infections, with attention to tissue

Breakthrough candidemia during prophylactic or empirical antifungal therapy: antifungal exposure, in vitro activity, and association of early treatment modification with mortality.

BACKGROUND: Breakthrough candidemia (BrC) occurs during systemic antifungal therapy, but its determinants, relationship with in vitro resistance, and optimal management remain unclear. RESEARCH DESIGN AND METHODS: Retrospective matched case-case study at a 900-bed tertiary-care center (2012-2026) including 713 first candidemia episodes (92 BrC, defined as candidemia after ≥ 72 hours of antifungal exposure; 621 non-BrC). MICs were compared with same-species non-breakthrough isolates. BrC cases were matched 1:1 on age, location and period. The main outcome was 30-day mortality; antifungal thera

Emergence of Candida (Candidozyma) auris in Warming Climates.

Candida (Candidozyma) auris is a pathogenic fungus first identified in a patient in 2009. Prior to this clinical finding, C. auris was unknown, with its closest relative being the marine-dwelling Candida haemulonii. Infections are increasing annually, predominately impacting hospital patients due to durable biofilms on medical devices and antimicrobial resistance. Perplexingly, the fungus arose nearly simultaneously across the globe in independent disease emergences from six genetically distinct clades. The C. auris clades mostly diverged from one another 5,000-10,000 years ago, during a perio

Two pediatric type II achalasia cases complicated by candidal and eosinophilic esophagitis: Successful peroral endoscopic myotomy treatment.

Achalasia is a rare esophageal motility disorder in children, often leading to delayed diagnosis. We report the first two pediatric cases treated with peroral endoscopic myotomy (POEM) at our center, both classified as type II achalasia and complicated by mucosal disease. A 15-year-old girl presented with progressive dysphagia and weight loss, diagnosed with type II achalasia and Candida esophagitis. A 13-year-old boy exhibited similar symptoms with a 27-kg weight loss; histology revealed concurrent eosinophilic esophagitis. Both underwent successful POEM. These cases illustrate the clinical h

Purpureocillium lilacinum Fungemia Masquerading as an Unidentified Yeast in a Heavily Immunosuppressed Patient With Relapsed Multiple Myeloma and Therapy-Related Mixed-Phenotype Acute Leukemia: A Case Report.

Purpureocillium lilacinum (formerly Paecilomyces lilacinus) is an environmental saprophytic mold and an increasingly recognized opportunistic pathogen in immunocompromised hosts, most often causing cutaneous, ocular, or sinopulmonary disease; bloodstream infection (fungemia) is rare. The initial microscopic morphology on Gram stain of a positive blood culture can simulate yeast, delaying correct identification and appropriate antifungal therapy. We describe a 67-year-old man with relapsed multiple myeloma and therapy-related mixed-phenotype acute leukemia, further complicated by a prior Mycoba

Extracellular vesicles released by Candida albicans yeast cells impact candidiasis lethality in insect and murine models.

Extracellular vesicles (EVs) are nano-sized lipid bilayer structures released by diverse cell types. For pathogenic fungi, EVs play critical roles by modulating immune responses and influencing disease progression. Previously, we demonstrated that vaccination with EVs from Candida albicans promoted protection in immunosuppressed mice subsequently infected with lethal doses of this organism. However, the impact of EVs in the presence of fungi is poorly investigated in vivo. Here, we demonstrated that co-injection of C. albicans yeasts and their EVs into Galleria mellonella larvae led to a rapid

Vaginal Lactobacillus paragasseri may attenuate fluconazole-resistant phenotypes in the subset of Candida albicans isolates and shows potential as a probiotic adjunct in antifungal therapy.

INTRODUCTION: Candida albicans is the predominant etiological agent of vulvovaginal candidiasis (VVC). Fluconazole (FLU), a first-line azole antifungal agent, has encountered growing resistance among vaginal isolates of C. albicans due to prolonged, high-dose clinical administration. The development of such drug resistance arises from the synergistic interplay of multiple factors, with key mechanisms including enhanced biofilm formation, overexpression of drug efflux pump genes, and structural alterations in target enzymes driven by mutations in the ERG11 gene. Lactobacilli, the dominant comme

Antifungal resistance in oral Candida: Phenotypic susceptibility, genomic surveillance, and novel postbiotic-based therapeutic strategies.

Oral candidiasis is a common mucosal fungal infection, with Candida albicans remaining predominant alongside increasing recognition of non-albicans Candida species. Reduced antifungal susceptibility and resistance complicate laboratory diagnosis and treatment. This review summarizes current approaches to antifungal resistance characterization in oral Candida, including phenotypic susceptibility testing, molecular detection of resistance determinants, and whole-genome sequencing for strain characterization and genomic surveillance. Particular emphasis is placed on biofilm-associated tolerance,

Mortality of Candida auris candidemia compared with non-auris candidemia: a systematic review and meta-analysis.

BACKGROUND: Candida auris has emerged as a globally significant healthcare-associated fungal pathogen. Although descriptive studies have reported high mortality for candidemia caused by C. auris, whether the mortality differs from non-auris candidemia remains unclear. We conducted this meta-analysis to compare the mortality between patients with C. auris and non-auris candidemia. METHODS: We searched PubMed (including MEDLINE), Embase, and Web of Science databases for relevant studies published before June 2026. A random-effects model was used for meta-analysis. RESULTS: A total of 16 articl

Multipathogen antimicrobial resistance in a Romanian tertiary-care hospital: a retrospective antibiogram analysis, January-July 2025.

Routine microbiology data can show not only isolated resistance events but also the wider hospital ecology in which antimicrobial resistance (AMR) emerges. We analyzed the complete January-July 2025 antibiogram export from a Romanian tertiary-care hospital, along with a targeted extended-testing file. The scope was deliberately broader than a single-organism carbapenem-resistant Klebsiella pneumoniae analysis. We performed a retrospective descriptive analysis using 3,270 antibiogram records from 3,012 patient identifiers. Organisms, sample types, resistance labels, and extended-test results we
