[Specific dermatoses of pregnancy].

Dermatoses related to pregnancy or the postpartum period are known as the specific dermatoses of pregnancy. These dermatoses include (a) pemphigoid gestationis (synonym: herpes gestationis), (b) polymorphic eruption of pregnancy (synonym: pruritic urticarial papules and plaques of pregnancy; PUPPP), (c) prurigo of pregnancy and (d) pruritic folliculitis of pregnancy. Of these disorders, only pemphigoid gestationis has a known cause: an allogeneic immune reaction of the mother to the placental basal membrane. Itching or an itchy skin eruption is often the main symptom ofa dermatosis of pregnanc

[Pruritus in pregnancy. A frequent diagnostic and therapeutic challenge].

Pruritus is the leading dermatological symptom during pregnancy. Besides preexisting or acquired dermatoses, there are a number of pregnancy-specific dermatological diseases such as PEP (polymorphic eruption of pregnancy, previously named PUPPP), pemphigoid (herpes) gestationis, and pruritus gravidarum that are accompanied by severe itching and scratching. Because of potential effects on the fetus, the treatment of pruritus in pregnancy requires prudent consideration. The use of topical and systemic treatments depends on the underlying aetiology of pruritus and the stage and status of the skin

Pruritus in pregnancy and childhood--do we really consider all relevant differential diagnoses?

During pregnancy and also during childhood, pruritus can have manifold aetiologies and should therefore always be taken seriously. In pregnancy, pruritus is the main dermatological symptom, occurring in 18% of women. Pregnancy-specific dermatological diseases such as polymorphic eruption of pregnancy (PEP), Pemphigoid (Herpes) gestationis, Pruritus gravidarum are accompanied by severe pruritus and scratching. In children, it mainly occurs along with dermatoses but in rare cases with systemic diseases such as renal or liver failure. Mostly, it appears in the setting of atopic dermatitis (AD). B

[Pemphigoid gestationis: a pregnancy-related pathology underestimated by obstetricians].

Pemphigoid gestationis (herpes gestationis) is an autoimmune bullous dermotosis occuring dining pregnancy or in the post-partum period. The interaction of this rare pathology with pregnancy is underestimated by obstetricians. Authors report one case of pemphigoid gestationis diagnosed in a pregnant woman at term. After a literature review, clinical aspects of this dermatosis and especially its interactions with pregnancy are assessed.

Subepidermal blistering disorders: a clinical and histopathologic review.

The subepidermal blistering disorders are comprised of a number of unrelated disorders with a diverse clinical presentation and pathogenic basis that share in common the presence of blister formation beneath the epidermis. Many of the disorders are both debilitating and potentially fatal. Timely and accurate diagnosis facilitates their appropriate management. The etiologic, clinical, and pathologic attributes as well as the treatment of these disorders including bullous pemphigoid, epidermolysis bullosa acquisita, dermatitis herpetiformis, linear IgA dermatosis, cicatricial pemphigold, herpes

A comparative study of antibody titers of blister fluid and serum in patients with subepidermal immunobullous diseases.

BACKGROUND: Subepidermal autoimmune bullous diseases (SABD) comprise several disorders, such as bullous pemphigoid (BP), cicatricial pemphigoid (CP), epidermolysis bullosa acquisita (EBA), herpes gestationis (HG), and linear immunoglobulin A (IgA) dermatosis (LAD), and are characterized by antibody production against the basement membrane structures of the skin and mucosa. Although indirect immunofluorescence (IIF) on serum is a routine test for the detection of basement membrane zone antibodies, there have only been a few studies related to IIF on blister fluid. Aim To perform IIF on blister

Herpes gestationis.

Herpes gestationis, also known as Pemphigoid gestationis, is a rare autoimmune disease of pregnancy. It is characterized by itching and skin lesions. The disease causes prominently maternal discomfort but fetal and neonatal complications have been reported. There are only scattered reports of cutaneous neonatal herpes gestationis in the literature; however, the frequency and severity of fetal illness are still debated. We describe 2 cases of herpes gestationis diagnosed and managed at the King Khalid University Hospital, Riyadh, Kingdom of Saudi Arabia.

[Herpes gestationis].

Herpes Gestationis is a serious dermatological disease, albeit rare, associated to pregnancy or to the trophoblast diseases. Contrary to what the name suggests, it is not a viral disease but an auto-immune disease. We present the clinical case of a 38 year-old woman to whom a case of Herpes Gestationis was diagnosed when she was 15 weeks pregnant and whom has been treated with corticosteroids and antihistamine's showing positive results and without major complications for the mother or the embryo. The authors are undertaking a review of the existing literature, based on this clinic case.

Specific dermatoses of pregnancy: an evidence-based systematic review.

OBJECTIVE: We conducted an evidence-based systematic analysis of the literature on specific dermatoses of pregnancy. STUDY DESIGN: The bibliographic databases MEDLINE and EMBASE were screened for studies and reports in all languages about herpes gestationis, pruritic urticarial papules and plaques of pregnancy, pruritic folliculitis of pregnancy, and prurigo of pregnancy from January 1962 to January 2002. As main index terms, including analogs and derivatives, we used the names of specific dermatoses of pregnancy. Intrahepatic cholestasis of pregnancy, not a primary dermatosis, was included h

Herpes gestationis may present itself as a paraneoplastic syndrome of choriocarcinoma-a case report.

BACKGROUND: Herpes gestationis (HG) is a rare, recurrent, pruritic, vesicobullous dermatosis occurring predominantly in pregnancy and seldom in early puerperium. Reports of the association of HG with choriocarcinoma are extremely rare and this case highlights such a possible link. CASE: This case focuses on the late postpartal manifestation of HG being associated with metastatic high-risk choriocarcinoma. Direct immunofluorescence was used to verify the diagnosis of HG. Symptomatic therapy with corticosteroids and antihistamines alleviated the pruritic symptoms associated with HG, but only th

A case of chronic herpes gestationis: persistent disease or conversion to bullous pemphigoid?

We report the case of a 38-year-old woman with herpes gestationis (HG) persistent for 26 months postpartum with typical erythematous-edematous grouped lesions associated with vesicles, blisters, and scaled crusts on most parts of her body. Despite high doses of oral prednisolone, azathioprine, and dapsone, and a trial of 5 plasmaphereses, the disease has persisted to date. Histopathologic examination of lesional skin showed subepidermal blisters, focal basal cell necrosis, and a dermal inflammatory infiltrate including many eosinophils. Direct immunofluorescence showed linear C3 staining in th

We report a very severe form of herpers gestationis that arose at the 26(th) week of pregnancy and reached us for observation at the 30(th) week. Herpes gestationis in an autoimmune vesicobullous dermatosis characterised by skin eruptions, intense itching and consequent increase in fetal morbility, with delayed intrauterine growth and prematurity. Owing to its particular severity (involvement of the entire body surface including the face), between the 30th and the 32(nd) weeks we had to address a severe clinical condition characterised by anaemia, marked hypoproteinaemia, hypoalbuminaemia, hup

Pemphigoid gestationis: treatment with immunoapheresis.

BACKGROUND: Pemphigoid (herpes) gestationis is a rare blistering disease of pregnancy. Topical and systemic corticosteroids and antihistamines are usually sufficient for treatment. CASE REPORT: A patient suffering from severe, therapy-resistant pemphigoid gestationis was successfully treated with the additional use of six sessions of immunoapheresis. Systemic corticosteroids could be tapered and serum autoantibody levels to bullous pemphigoid antigen 2 (BPAG2 = BP180) significantly dropped during this treatment. CONCLUSION: Immunoapheresis represents a helpful therapeutic option with few sid

Autoimmune responses in patients with linear IgA bullous dermatosis: both autoantibodies and T lymphocytes recognize the NC16A domain of the BP180 molecule.

Linear IgA bullous disease (LABD) is an autoimmune skin disease characterized by subepidermal blisters and IgA autoantibodies directed against the epidermal basement membrane zone (BMZ) of the skin. Various antigens have been identified as targets of IgA autoantibodies including BP180, a type II glycoprotein that spans the BMZ and lamina lucida. Previously, we have identified a subset of LABD patients whose sera contained IgA antibodies against the 16th noncollagenous (NC16A) domain of BP180. NC16A was previously shown to harbor epitopes that are recognized by both autoantibodies and T cells f

Cicatricial pemphigoid differs from bullous pemphigoid and pemphigoid gestationis regarding the fine specificity of autoantibodies to the BP180 NC16A domain.

Bullous pemphigoid (BP), pemphigoid (herpes) gestationis (PG), cicatricial pemphigoid (CP), and lichen planus pemphigoides (LPP) are autoimmune subepidermal bullous diseases that are characterized by circulating autoantibodies to the transmembrane hemidesmosomal protein BP180/type XVII collagen. Previous studies demonstrated that the majority of patients with BP, PG, and LPP show antibodies to an immunodominant, membrane-proximal non-collagenous domain (NC16A) on the extracellular portion of BP180. By the use of non-overlapping peptides of the NC16A domain, we previously demonstrated that auto

[Signification and limitations of virology diagnostic tools in herpes facialis, herpes genitalis, herpes gestationis and in neonates at risk].

INTRODUCTION: Cutaneomucous herpes is frequent in humans. Laboratory confirmation of clinical diagnosis is important, as there are atypical presentations in genital herpes. METHODS: The review of the French and English literature on virological diagnosis of herpes simplex, indexed in Medline (1990-2001), Embase and Cochran Library (1995-2001) was carried out. RESULTS: 172 publications were selected, and 125 of them were used for this work. The current methods to diagnose herpes in cutaneous lesions are detection of antigens by immunofluorescence or ELISA, culture and PCR. The sensitivity and

We analyze the case of herpes gestationis to 36 year-old woman, which started on 24th week of 5th gestation. During the past gestations there were no fillings. Treatment with prednisolon 40 mg per day only reduced the signs. After the birth treatment with dapson 100 mg per day for the 5-days period gave a visible effect quickly. The treatment was completed 3 months ago and now there are no signs of illness.
