Herpes gestationis (HG) is a rare autoimmune blistering complication of pregnancy. It is of unknown etiology and it occurs once in every 3,000 to 50,000 pregnancies. The diagnosis can strongly be suggested by the clinical picture, but it must be confirmed using immunopathological methods. The treatment of choice is local measures and systemic corticosteroids. Although the maternal prognosis is excellent, there are controversies on the perinatal outcome. A typical case of HG is presented and the literature is reviewed.

HG expresses a broader clinical range than previously thought. Disease may be mild and nonvesicular during one pregnancy, followed by explosive vesiculobullous disease during another. On the other hand, patients with extensive HG during one pregnancy may experience mild or even subclinical disease during a subsequent gestation. The broad range of clinical signs and the lack of demonstrable fetal or maternal risk associated with HG have important ramifications for patients desirous of further children. There is a genetic predisposition to HG. DRB1*0301 (HLA-DR3) is increased and 90% of patients

Pemphigoid gestationis (PG; herpes gestationis) is a rare autoimmune disease associated with pregnancy, currently defined by the presence of complement deposition along the cutaneous basement membrane zone. It is known to be associated with HLA-DR3 and DR4, and an increase in anti-HLA antibodies in those with a history of PG has been reported. We have studied 39 patients with an immunofluorescence-confirmed diagnosis of PG for the presence and specificity of anti-HLA antibodies. Anti-HLA antibodies were found in all 39 patients. Specificity was against class I antigens in 98% (controls 10%; P

BACKGROUND: Herpes gestationis (HG) is a rare, pregnancy-related skin disease characterized by the production of an autoantibody to a component of the hemidesmosome. It is associated with the class II antigens HLA-DR3 and HLA-DR4, but its potential association with the "class III antigens" C2, C4, and factor B has not previously been studied.
OBJECTIVE: Our purpose was to study complement polymorphism in HG.
METHODS: Using electrophoresis and immunofixation techniques, we determined the allele frequencies of C4A, C4B, C3, and factor B in 42 patients with a history of HG.
RESULTS: Ninety per

Subepidermal blistering associated with the human skin diseases bullous pemphigoid and herpes gestationis has been thought to be an IgG autoantibody-mediated process; however, previous attempts to demonstrate the pathogenicity of patient autoantibodies have been unsuccessful. An immunodominant and potentially pathogenic epitope associated with these blistering diseases has recently been mapped to the extracellular domain of a human epidermal antigen, BP180. Patient autoantibodies that react with this well-defined antigenic site failed to crossreact with the murine form of this autoantigen and

Bullous pemphigoid (BP) and herpes gestationis (HG) are skin diseases characterized by subepidermal blisters and autoantibodies against two hemidesmosomal Ag, i.e., BP230 and BP180. Based on sequence analysis the BP180 Ag was predicted to be a transmembrane protein with a long extracellular collagenous domain. In the present investigation fusion proteins encompassing various segments of the BP180 Ag were expressed in a prokaryotic system and assayed by immunoblotting and immunoadsorption against a panel of BP, HG and control sera. One antigenic site, comprising 14 amino acids of the BP180 nonc

Herpes gestationis or pemphigoid gestationis (PG) is a bullous disease developing in association with pregnancy. It is believed to be an immunologically mediated disorder. Antibody binding by specific autoantibodies was detected in placental tissue and in the area of the hemidesmosomes and the basal membrane zone of the skin. These autoantibodies react with antigens of 180 kDa and 230 kDa relative molecular mass, which are presumably identical to the bullous pemphigoid antigen BPAG1 and BPAG2. The herpes gestationis factor (HG factor) preferentially binds to the 180-kDa antigen (BPAG2). The in

A 27-year-old woman suffered from herpes gestationis in her second pregnancy. The pruricy urticarial and bullous skin lesions were exacerbated immediately after birth, and premenstrually after the onset of menstruation. The diagnosis was confirmed by the determination of anti-basement membrane antibodies of the IgG class both in the serum (herpes gestationis factor) and in lesional skin by immunofluorescence techniques. The newborn had no skin lesions and no basement membrane zone antibodies were detected. The HLA (human leucocyte antigen) typing revealed the HLA-A11/1, B18/w63, C-/-, DR4/w13,

The complement system represents an important nonspecific skin defense mechanism. Its activation leads to the generation of products that not only help to maintain normal host defenses but also mediate inflammation and tissue injury. Proinflammatory products of complement include large fragments of C3 with opsonic and cell-stimulatory activities (C3b and C3bi), low molecular weight anaphylatoxins (C3a, C4a, and C5a), and membrane attack complex. Among them C5a or its degradation product C5a des Arg seems to be the most important mediator because it exerts a potent chemotactic effect on inflamm

The blistering skin disease bullous pemphigoid (BP) is an example of how autoantibodies from patients can be used as tools to improve our knowledge of the molecular structure of normal skin. BP autoantibodies bind to the basement membrane zone of normal human skin and usually react with two main epidermal antigens of approximately 230-kDa and 180-kDa. From recent studies, it is now established that the 180-kDa polypeptide is distinct from the 230-kDa BP antigen and is a major component of hemidesmosome. In this article, the recent data regarding the localization and structure of the epidermal

Pemphigoid gestationis (PG) Herpes Gestationis (HG) is an auto-immune bullous dermatosis which usually occurs in pregnancy. We have studied the maternal and fetal prognosis of this pathological condition. Thirteen cases were reported at the same time by the departments of dermatology and obstetrics of the Charles-Nicolle Hospital between 1980 and 1990. Two women were not followed up. In the remaining eleven there was one intrauterine fetal death and one intrauterine fetal growth retardation (IUGR). One case was delivered prematurely because of obstetric indications. Four infants were delivered

BACKGROUND: The potential risks to the fetus in herpes gestationis have long been a controversial subject, but because of the rarity of the disease, have only occasionally been studied.
OBJECTIVE: The purpose of this study was to determine the incidence of fetal complications in herpes gestationis.
METHODS: We collected and analyzed the obstetric histories of 74 patients and compared their involved pregnancies with their uninvolved pregnancies.
RESULTS: There was an obvious tendency for premature delivery associated with herpes gestations. A slight tendency toward small-for-gestational-age

BACKGROUND: Herpes gestationis (HG) is an autoimmune disease of the skin that occurs exclusively in association with pregnancy (or trophoblastic disease). It is associated with the HLA-DR3 and -DR4 antigens that are also associated with several other autoimmune diseases. HG has previously been reported in association with Graves' disease.
OBJECTIVE: Our purpose was to determine the frequency of other autoimmune disease(s) in patients with a history of HG.
METHODS: Seventy-five patients with a history of HG were studied for the frequency of other autoimmune diseases.
RESULTS: We found an inc

One-hundred-eighty kilodalton bullous pemphigoid antigen (BPAG2) is recognized by autoantibodies in the sera of patients with blistering skin diseases, bullous pemphigoid (BP), and herpes gestationis (HG). In this study, we have screened a mouse epidermal keratinocyte cDNA library with a 1.0-kb human BPAG2 cDNA, which has been shown to correspond to two collagenous domains (Giudice et al: J Clin Invest 87:734-738, 1991). Screening of the mouse library identified two cDNA clones, the larger one being 1.8 kb in size. Comparison of the mouse amino acid sequences, as deduced from cDNA, with the co

The dermoepidermal junction (DEJ) is a key structure in the skin. Many acquired and inherited diseases affect the DEJ. New insight has been gained into the chemical composition of the DEJ of which at least 20 protein components have been identified to date. Some components of hemidesmosomes, which are critical DEJ structures, have also been identified. Hereditary epidermolysis bullosa is a heterogeneous group of DEJ disorders. Studies are beginning to shed light on the molecular mechanisms of these diseases. Dystrophic epidermolysis bullosa is due to alterations in the collagen IV molecule whi
