A rare case of dyskeratosis congenita with DKC1 mutation presenting initially as thrombocytopenia: Case report.

RATIONALE: Dyskeratosis congenita is a rare genetic disorder classically characterized by the mucocutaneous triad and bone marrow failure. Diagnosis is challenging when hematologic manifestations precede classic features. PATIENT CONCERNS: A 10-year-old male initially presented with isolated thrombocytopenia, which was misdiagnosed as aplastic anemia. DIAGNOSES: Retrospective examination revealed the mucocutaneous triad. Genetic testing confirmed a pathogenic hemizygous DKC1 mutation (c.1058C>T, p.A353V). INTERVENTIONS: The patient received a 3-month course of androgen therapy. Genetic coun

Velamentous Cord Insertion With Vaginal Delivery: A Case Report and Review of the Literature.

Velamentous cord insertion (VCI) is a rare placental abnormality in which the umbilical cord enters the chorionic membranes instead of the placenta, leaving the umbilical vessels unprotected. This pathological condition carries an increased risk of adverse perinatal outcomes, such as fetal growth restriction, preterm delivery, abnormal fetal heart rate patterns during labor, and perinatal morbidity, which often lead healthcare professionals to choose cesarean section to reduce postnatal risks. Early prenatal diagnosis using color Doppler imaging and comprehensive ultrasound is of utmost import

Management Challenges of Acute Promyelocytic Leukemia in Pregnancy: A Case Report.

Acute promyelocytic leukemia (APML) is a rare hematologic emergency with a high mortality rate due to bleeding diathesis, which is due to a disseminated intravascular coagulation-like coagulopathy; APML is complicated enough to treat on its own and becomes particularly challenging when it occurs during pregnancy due to the complexities in managing both maternal and fetal health. APML is associated with a challenging therapeutic dilemma for pregnant women, and there is a risk of fetal malformations and developmental abnormalities caused by exposure to chemotherapy. A 31-year-old woman at 29 wee

Case Report: Severe neonatal lupus in an infant with homozygous NCF1 p.Arg90His variant and a der(14)t(4;14) translocation.

Neonatal lupus erythematosus (NLE) is a rare, passively acquired autoimmune disease. The manifestations of NLE are varied, and the outcomes of the majority of cases result in spontaneous resolution. However, severe cases are limited. Emerging evidence highlights the genetic heterogeneity of NLE. The missense variant p.Arg90His in NCF1 has been demonstrated to be associated with autoimmune diseases, including systemic lupus erythematosus. Chromosomal abnormalities manifest as developmental and growth delays and facial anomalies, among other phenotypic characteristics. Whether the co-occurrence

Mesenchymal dysplasia of the placenta: a rare entity and important differential diagnosis. Presentation of a case with placentomegaly.

Placental mesenchymal dysplasia (PMD) is a rare disease with unclear etiology and peculiar placental findings (placentomegaly, abnormal chorionic vessels with thromboses, and grossly enlarged and often cystic-appearing stem villi). Songraphic findings are cystic changes, hypoechoic areas, placental enlargement, and dilated chorionic vessels.PMD can be associated with severe perinatal complications, e.g., intrauterine fetal demise, fetal growth restriction, and preterm delivery. Neonatal complications include Beckwith-Wiedemann Syndrome (BWS), hepatic tumors, and hematologic diseases, i.e., ane

Multidisciplinary team management of congenital dysfibrinogenemia in pregnancy: a case report.

BACKGROUND: Congenital dysfibrinogenemia is a rare autosomal dominant disorder involving abnormal fibrinogen function, leading to variable risks of bleeding and thrombosis. Its management during pregnancy is particularly challenging due to the potential for complications such as miscarriage, stillbirth, placental abruption, and fetal growth restriction. The absence of reliable predictive laboratory markers further complicate individualized risk assessment and clinical decision-making. CASE PRESENTATION: A 23-year-old primigravida was diagnosed with congenital dysfibrinogenemia after low fibrin

Effective Sirolimus Use in Prenatal and Postnatal Management of Symptomatic Extensive Congenital Capillary-Lymphatic-Venous Malformations (CLVMs): A Report of Two Cases.

INTRODUCTION: Congenital capillary-lymphatic-venous malformations (CLVMs) often result in life-threatening complications, which may begin in utero. Sirolimus, a mammalian target of rapamycin (mTOR) inhibitor, has been successfully used in children and adults with CLVMs due to its antiproliferative and antiangiogenic properties. However, only two cases of prenatal sirolimus treatment of fetal CLVMs have been published to date, with very limited data on optimal therapeutic scheme and drug dosing. CASE REPORTS: Here we report two cases of effective prenatal and postnatal sirolimus treatment of e

Hemoglobin Dieppe (HBB:c. 383A > G): A Rare Dominant β-Thalassemia in an Iraqi Kurdish Family.

Dominant β-thalassemia is a rare form of thalassemia that is caused by a heterogenous group of molecular defects, including missense, nonsense, and frameshift mutations. Among the missense mutations involving the third exon of β-globin gene is the rare Hb Dieppe (HBB:c. 383A > G) which leads to a very unstable β-variant. In the current study we report this variant in an 8-year-old girl and her 35-year-old mother in an Iraqi Kurdish family, both presenting as β-thalassemia intermedia with moderate hypochromic anemia, increased hemoglobin F and borderline hemoglobin A2. This constitutes the firs

Maternal cobalamin deficiency causing infantile seizures and developmental regression.

We report the case of an 11-month-old boy, presenting with 6 months of failure to thrive, progressive seizures, developmental regression and aversion to solid foods. He also developed systemic symptoms including oral mucocutaneous lesions, neutropenia and anaemia. Notable evaluations included an abnormal electroencephalogram and a thin corpus callosum. Initial diagnostic work-up for epileptic encephalopathy included rapid whole exome sequencing (WES) and metabolic studies. His propionylcarnitine, plasma homocysteine and methylmalonic acid were markedly elevated, with normal vitamin B12, raisin

Prenatal diagnostic errors in hemoglobin Bart's hydrops fetalis caused by rare genetic interactions of α-thalassemia.

OBJECTIVES: To describe rare genetic interactions of α-thalassemia alleles causing Hb H disease and Hb Bart's hydrops fetalis which could lead to diagnostic errors in a routine practice. METHODS: Hematological and molecular characterization were carried out in a Thai family with a risk of having fetus with Hb Bart's hydrops fetalis. RESULTS: Both parents were found to be the thalassemia intermedia patients associated with unusual forms of Hb H disease. DNA analysis of common α-thalassemia mutations in Thailand identified α+-thalassemia (-α3.7 kb del) and unknown α0-thalassemia in the father

Hb H disease associated with compound heterozygosity for --SEA deletion and a novel alpha globin chain variant (HBA2:c.175C>A) on the distal histidine in a Chinese family.

OBJECTIVES: In clinical practice, the majority of α-thalassaemia cases arise from deletions of the α-globin genes. However, a subset of cases is attributed to rare haemoglobin variants, which can manifest with borderline or normal screening results, potentially leading to missed diagnoses in clinical practice. METHODS: Blood samples were collected from family members and underwent haematological, DNA and RNA analysis. RESULTS: The five-month-old proband presented a haematological phenotype consistent with Hb H disease. The mother's haematology profile was consistent with an α-thalassaemia ca

A case report of sirolimus use in early fetal management of lymphatic malformation.

Sirolimus, by targeting the mammalian target of rapamycin (mTOR) pathway, has demonstrated efficacy on lymphatic malformations (LMs) in adults and neonates. The current hypothesis is that the earlier the lesion is treated, the better it responds. This has prompted the idea that sirolimus administration might be efficacious to treat fetal LMs as well. Here we report a successful management of a cervicofacial fetal LM with sirolimus taken orally by the mother from the 22nd week of pregnancy until 2 weeks before planned delivery. Repeated cordocentesis recorded a 30% transplacental crossing of si

Perinatal palliative care for family with prenatal diagnosis of Matthew-Wood syndrome.

Matthew-Wood syndrome (MWS) is a rare autosomal recessive disorder caused by pathogenic variants of the STRA6 gene. Several studies in the available literature comprise patients with pathogenic variants of gene STRA6 with various phenotypic expressions: from lethal forms of MWS to non-lethal anophthalmia. These reports mainly describe new pathogenic variants and phenotypic expression but do not describe medical or paramedical care for the affected families. In our case report, we describe the second case of MWS in the same family and the benefits of including the patient's family in the perina

[Complex congenital heart disease : about a case of Noonan syndrome].

We present the case of a young girl in whom pre-natal echocardiography showed double outlet right ventricle associated with severe infundibular- and pulmonary valve stenosis. The genetic testing has shown a mutation on the LZTR1 gene, which confirms the diagnosis of a Noonan Syndrome, also present in the mother and an elder sister. The infant was born premature at 34 weeks and 5 days of gestational age. During the neonatal period, feeding difficulties are noted linked to oral aversion and exacerbated by difficulties in the mother-child bond. At 1 month of age, the child presented hypoxic spell

Congenital syphilis: a rare presentation of a forgotten infection.

Congenital syphilis (CS), a common but forgotten disease has a broad spectrum of clinical presentation. Vertical transmission of this spirochaetal infection from the pregnant mother to the foetus can result in varied manifestations ranging from asymptomatic infection to life- threatening conditions in the form of stillbirth and neonatal death. The haematological and visceral manifestations of this disease can closely mimic various conditions including haemolytic anaemia and malignancies. Congenital syphilis should be considered as a differential in any infant presenting with hepatosplenomegaly

The Value of Fetal Heart Evaluation in Fetuses with Rare Congenital Lymphangiomas: A Cohort Study from a Single Tertiary Center across Two Decades (Years 1999-2020).

Lymphangiomas are uncommon, benign (from a histopathology viewpoint) malformations of the lymphatic system with thin-walled vessels; however, these tumors may be dangerous for fetal or neonatal life. They are observed in 1:6000 newborns at birth and in 1:750 spontaneous abortions. We aimed to investigate the role of fetal echocardiography in the prognosis of lymphangioma. Selected data of 19,836 pregnant women studied between 1999 and 2020 were retrospectively analyzed. In total, 32 cases of lymphangioma meeting the following criteria were further analyzed: (1) ultrasound availability from the
