[Congenital defects in the newborn--difficulties in prenatal diagnosis].

The aim of this work is an analysis of developmental defects in newborn infants, born in the Clinical Department of Pregnancy Pathology and Reproduction in Zabrze, during the 19 months of its existence. Difficulties in prenatal diagnosis of developmental defects are presented and their structure is discussed. The study included 1524 live born infants with birth weight over 500 g and 12 stillborn. In the live born group developmental defects were found in 71 (4.6%) infants. Among the 12 stillborn infants developmental defects were present in one child (8.3%). The most common developmental defec

Inborn errors of cholesterol biosynthesis.

Disorders of cholesterol biosynthesis have recently emerged as important errors of metabolism that collectively have taught us many new genetic and biochemical lessons. Whereas most metabolic diseases are characterized by exclusively or largely postnatal biochemical toxicities or deficiencies, disorders of cholesterol biosynthesis are notable for their severe effects on prenatal development. The remarkable embryonic consequences of abnormal cholesterol biosynthesis are exemplified by Smith-Lemli-Opitz syndrome (SLOS), a well-known multiple congenital anomaly syndrome only recently discovered t

Choroid plexus cysts: Is biochemical testing a valuable adjunct to targeted ultrasonography?

OBJECTIVE: We sought to determine whether biochemical testing is a valuable adjunct to ultrasonography in selecting patients with fetal choroid plexus cysts for amniocentesis. STUDY DESIGN: The study population consists of 128 patients who had fetal choroid plexus cysts detected during ultrasonography performed between 18 and 22 weeks' gestation. The patients had genetic counseling, and amniocentesis and biochemical testing were offered to all patients. The data were analyzed by dividing the patients into 3 groups. Group 1 had targeted ultrasonography only, group 2 had ultrasonography and mat

Prenatal diagnostic and prognostic value of human cytomegalovirus load and IgM antibody response in blood of congenitally infected fetuses.

Human cytomegalovirus (HCMV) load and virus-specific IgM were quantified in blood of 36 fetuses from mothers with primary HCMV infection. Nineteen fetuses were congenitally infected and 17 were uninfected as diagnosed by virus isolation from and DNA detection in amniotic fluid. Sensitivity of antigenemia was 57.9%; of viremia, 55. 5%; of leukoDNAemia, 82.3%; and of IgM, 57.9%; specificity was 100% for all assays. When amniocentesis was performed, 4 HCMV-infected fetuses (group A) showed abnormal ultrasound and biochemical/hematologic findings, 8 (group B) had elevated gamma-glutamyl transferas

Functional and biochemical criteria for investigation of brain development disorders.

Functional state and creatine kinase (CK) activity in the amniotic fluid and blood of anencephalic fetuses was studied in the second trimester of pregnancy with the following pathomorphological investigation of the state of their CNS to reveal possible markers of development disorders. The extent of neurological disorders in newborn infants was retrospectively compared with data from functional studies of components of a biophysical profile (motor-cardiac reflex, heart rhythm oscillations, respiratory movements) and with CK activity in the amniotic fluid and blood of fetuses with hemolytic dis

[Abnormal twin pregnancy--early resorption of a single fetus in a twin pregnancy pregnancy].

INTRODUCTION: Twin pregnancy presents a condition of development of two fetuses in the uterus and can be monozygotic (single ovum) and dizygotic (two ova). In case of fertilization and segmentation of one ovum monozygotic twins are produced, while in case of fertilization of two ova, which can originate from one or two Graff follicles, dizygotic twins are developed. The ratio of twin and single pregnancies is 1:89 (according to Hellin's law) (1). The incidence of twin and other multiple pregnancies is influenced by: race of parents, age and parity of mother, use of clomid and gonadotrophin to

Influence of smoking habits on Down's syndrome risk evaluation at mid-trimester through biochemical screening.

Expectant mothers who smoke have higher levels of maternal serum alpha-fetoprotein and lower levels of unconjugated estriol and total human chorionic gonadotrophin than non-smoking mothers. This significantly affects performance of screening for Down's syndrome. This study includes 22,169 pregnant women: 18,876 non-smokers, 2,660 smoking < or = 10 cigarettes/day, and 633 smoking > 10 cigarettes/day. Mean maternal age (32.6 years), maternal weight (60.5 kg), and gestational age (114.7 days) were similar or only slightly different between the three groups. To verify the effects of smoking on scr

Prenatal diagnosis of trisomy 21 in monozygotic twins with increased nuchal translucency and abnormal serum biochemistry.

We present a case of a monozygotic twin pregnancy who underwent routine screening for chromosomal abnormalities at 14 weeks of gestation by ultrasonographic measurement of nuchal translucency and biochemical analysis of maternal serum alpha fetoprotein and free beta human chorionic gonadotrophin levels. Both screening methods indicated the pregnancy to be at increased risk of Down syndrome, with the ultrasound findings suggesting both fetuses to be affected. An amniocentesis was performed, and karyotype analysis revealed trisomy 21 in both fetuses; the mother subsequently opted for a terminati

Scientific basis for the content of routine antenatal care. II. Power to eliminate or alleviate adverse newborn outcomes; some special conditions and examinations.

BACKGROUND: There is uncertainty concerning antenatal care as a tool to eliminate or alleviate adverse outcomes in the newborn. We identified congenital conditions, intrauterine infections, intrauterine growth retardation, preterm birth and some specific infectious diseases in the mother with a view to prophylactic and other interventions. The value of some special diagnostic tools is also under discussion. METHODS: Review of recent literature, especially randomized controlled trials and systematic reviews. RESULTS AND CONCLUSIONS: Genetic abnormalities cannot be prevented after conception,

[Marfan's syndrome and pregnancy. A case report and review of the literature].

Marfan's syndrome is a collagen tissue disease, identified at the end of the nineteenth century. The gene for this autosomal dominant disease lies on chromosome 15 and leads to a biochemical disorder yet to be described. We report a case of Marfan's disease in a gravid patient and discuss the complications which may occur. The main risk involves the cardiovascular manifestations of the disease with possible dissection or rupture of the aorta. Preventive surgery, with extra-corporal circulation is required when the diameter of the aorta is greater than 6 cm. Spontaneous abortions and risk of pr

Screening for Down's syndrome.

Down's syndrome (DS) is the commonest cause of severe mental retardation in children. It is the result of trisomy of chromosome 21 which is usually a random event though it is commoner in older mothers. DS can be diagnosed by chorionic villus sampling (CVS) and amniocentesis followed by karyotyping. Because of the risks associated with these invasive procedures, they can only be offered to a high-risk group. At one time the sole basis for identifying this increased risk was maternal age, but within the past ten years a series of biochemical and ultrasound abnormalities have been shown in DS pr

Two mutations in the insulin receptor gene of a patient with leprechaunism: application to prenatal diagnosis.

Leprechaunism is an autosomal recessive disorder caused by mutations in the insulin receptor gene and characterized by intrauterine and postnatal growth restriction, abnormal glucose homeostasis, and severe insulin-resistance. Here we report the biochemical and molecular characterization of a male patient, NZ, who died at 2 yr of age with this syndrome. 125I-Insulin binding to fibroblasts from the proband, his mother, father, and unaffected sister was reduced to 8, 53, 38, and 35% of controls, respectively. Analysis of the insulin receptor gene by polymerase chain reaction amplification using

Biochemical and Doppler predictors of poor perinatal outcome in a fetus with four umbilical vessels.

Unexpected elevation of maternal serum alpha-fetoprotein (MSAFP) in women with anti-phospholipid syndrome has already been described as a predictor of fetal death. In this report we present a case of a pregnant woman with elevated second trimester MSAFP, in which Doppler ultrasound at 28 weeks suggested very poor fetal prognosis. A cesarean section was performed, but 2 days later the infant died due to distress. The only remarkable feature at post-mortem study was the finding of four vessels in the umbilical cord. Conventional investigation of the mother led us to the diagnosis of a primary an

Prenatal diagnosis of Marfan syndrome: identification of a fibrillin-1 mutation in chorionic villus sample.

Marfan syndrome (MFS) is one of the most common heritable connective tissue disorders and is caused by mutations in a gene coding for fibrillin-1. All but one of over 30 published mutations have been unique and specific prenatal diagnostics can only be provided to families with a previously established mutation. We have earlier identified a 366 bp deletion of fibrillin mRNA in a three-generation British Marfan family. An affected female in the family together with her husband sought prenatal diagnosis. Chorionic villus sampling was performed at 11.5 weeks of gestation and total RNA was directl

Molecular prenatal diagnosis of partial androgen insensitivity syndrome based on the Hind III polymorphism of the androgen receptor gene.

OBJECTIVE: Partial androgen insensitivity syndromes are the cause of genital ambiguity that is at times quite severe; there is, therefore, a high demand for prenatal diagnosis in families already afflicted with this syndrome. When the mutation has not been identified, the diagnosis can be made by the study of the polymorphisms of the androgen receptor gene. To perform molecular prenatal diagnosis in a family with partial androgen insensitivity syndrome, we studied the Hind III polymorphism of the androgen receptor gene on the trophoblastic DNA. The use of this restriction fragment length polym

The role of maternal serum alpha-fetoprotein, human chorionic gonadotrophin and oestriol in the antenatal screening of Down's syndrome.

The use of maternal age alone to identify pregnant mothers at risk of a fetus with Down's syndrome has recently been supplemented by maternal serum screening using biochemical markers such as alpha-protein, human chorionic gonadotrophin and oestriol. These tests have been reported to increase the sensitivity of antenatal detection of such fetuses from 35% to 67% with a false positive rate of 5%. However, these maternal serum markers may be affected by maternal weight, the smoking history of mothers and diabetes mellitus. Furthermore, such sensitivities are achieved only when gestational age is

Biochemical screening of congenital developmental abnormalities using determination of fetoplacental antigens.

In a prospective study of screening of maternal serum levels of alpha-1-fetoprotein (MS AFP) and trofoblast-specific beta-1-glycoprotein (MS SP1) were examined in samples from pregnant women between the 16th and 18th week of pregnancy. We detected 8 fetuses with chromosomal aberation, 8 fetuses with neural tube defects and 10 fetuses with inborn cardial defects. Our study confirms higher MS SP1 levels in women with fetuses with chromosomal aberation, while MS AFP's tendency is to decrease. When combining MS SP1 + MS AFP + age of mother (over 35), 75% of fetuses with chromosomal aberation were

[Screening for Down's syndrome in the 2d trimester of pregnancy: initial results].

A total of 332 pregnant women with risk of foetal aneuploidy underwent amniocentesis between January 1991 and August 1992: 247 women (74 per cent) for advanced age over 35 years and 85 younger ones (26 per cent) because of significant risk of Down's syndrome as a result of multimarker screening. Maternal serum alpha-fetoprotein, intact human choriogonadotropin and age were used as variables for combined individual risk evaluation according to Wald et al. The Demo version of ALPHA computer programme was used for risk computations. Two prenatally detected cases of Down's syndrome from a total of

Alternate methods of first-trimester diagnosis.

Fetal medicine is a new discipline that emerged out of the expansion of modern technology. Evaluation of the fetus heretofore was not possible except by indirect methods. Pregnancy management focused primarily on the care of the mother with the expectation that the fetus would be an indirect beneficiary. Our present-day approach to pregnancy and prenatal evaluation has been altered by the introduction of new techniques such as ultrasound, amniocentesis, and CVS, which have made the fetus and its intrauterine environment accessible. Improvements in ultrasonographic equipment have allowed for be

Cystic fibrosis prenatal diagnosis: confirmation of an equivocal microvillar enzyme result by direct analysis of the common gene mutation.

A child was tentatively diagnosed as having cystic fibrosis, based on neonatal presentation with severe gastrointestinal complications; the diagnosis was not confirmed biochemically and no tissues were available for DNA analysis. The mother presented in her subsequent pregnancy, and microvillar enzyme analysis of cell-free amniotic fluid at both 18 and 20 weeks gestation gave equivocal results. The pregnancy was terminated voluntarily because of a trend towards abnormal enzyme assay results on the second amniocentesis. Retrospectively, fetal tissues were found to be homozygous for the most com
