Is Nuchal Translucency of 3.0-3.4 mm an Indication for cfDNA Testing or Microarray? - A Multicenter Retrospective Clinical Cohort Study.

INTRODUCTION: This study aimed to evaluate the occurrence of clinically relevant (sub)microscopic chromosomal aberrations in fetuses with the nuchal translucency (NT) range from 3.0 to 3.4 mm, which would be potentially missed by cfDNA testing. METHODS: A retrospective data analysis of 271 fetuses with NT between 3.0 and 3.4 mm and increased first trimester combined test (CT) risk in five cohorts of pregnant women referred for invasive testing and chromosomal microarray was performed. RESULTS: A chromosomal aberration was identified in 18.8% fetuses (1:5; 51/271). In 15% (41/271) of cases, t

Single umbilical artery and associated birth defects in perinatal autopsies: prenatal diagnosis and management.

BACKGROUND: The umbilical cord forms the connection between the fetus and the placenta at the feto-maternal interface and normally comprises two umbilical arteries and one umbilical vein. In some cases, only a single umbilical artery (SUA) is present. This study was conducted to evaluate associations between SUA and other congenital malformations discovered in perinatal autopsies and to ascertain the existence of preferential associations between SUA and certain anomalies. METHODS: We evaluated records of all fetuses sent for autopsy to the Department of Pathology during the 10-year period fr

Umbilical Artery Thrombosis Causing Fetal Distress: A Case Report.

The umbilical cord (UC) is vital to maintain blood circulation between the mother and the growing fetus, which is sometimes disrupted. The umbilical artery thrombosis (UAT) is an infrequent complication of pregnancy that can lead to extreme perinatal outcomes, ranging from intrauterine growth restriction stillbirth to neonatal death. The prenatal diagnosis of UAT is essential and sometimes challenging to detect in clinical practice. Once it is detected, the emergent delivery through a cesarean section is considered after the steroidal lung maturity of the fetus. We report a primigravida diagno

Atopy and asthma in children born to mothers at risk of gestational diabetes mellitus: a follow-up study.

BACKGROUND: Gestational diabetes mellitus (GDM) is the most prevalent metabolic disturbance during pregnancy and is associated with adverse outcomes in offspring, including an elevated risk for developing atopic diseases in early childhood. Research is limited regarding only women at risk of GDM among whom some develop GDM while others do not. Information about adverse health outcomes in the offspring of these women is also lacking. The main aim was to assess whether maternal GDM increases the offspring's risk of atopic dermatitis (AD), asthma and allergic rhinitis at 1, 2 and 5 years of age.

Congenital Diaphragmatic Hernia With Poor Clinical Outcome: Key Lessons To Be Learned.

Congenital diaphragmatic hernia (CDH) refers to the abnormal protrusion of abdominal contents (stomach, intestine) into the thoracic cavity, leading to the underdevelopment of the lungs (pulmonary hypoplasia). It is a critical neonatal condition that presents significant challenges in both diagnosis and management, especially in resource-limited countries. This case report describes a term female baby born via normal vaginal delivery to a mother with no prenatal care. The baby showed no respiratory effort and required resuscitation. Despite intubation, the baby's air entry remained poor, and s

Combined fetal echocardiographic views improved prenatal differential diagnosis between right aortic arch and double aortic arch: a multicenter research.

PURPOSE: Congenital right aortic arch (RAA) and double aortic arch (DAA) were difficult to be diagnosed by traditional fetal echography. However, these two diseases have distinct clinical therapies and long-term outcomes which makes the differential diagnosis of great importance. While fetal screening by traditional fetal echography provides limited information about the branches of the aortic arch. This study aimed to discover and evaluate a novel combination of different echocardiography views in the diagnosis and differentiation of RAA and DAA. METHODS: From January 2014 to December 2022,

Pattern and Frequency of Congenital Heart Defects Among Infants of Diabetic Mothers.

Background Maternal diabetes mellitus (DM) is a known risk factor for congenital heart diseases (CHDs), which are of significant concern to infants born to diabetic mothers. Compared to newborns born to non-diabetic mothers, infants born to diabetic mothers had a higher overall risk of developing congenital malformations. This association has a complex pathophysiology that includes genetic predispositions, metabolic abnormalities, and environmental factors during key stages of fetal development. By developing screening strategies for neonates born to diabetic mothers, it will be imperative to

Novel neural network classification of maternal fetal ultrasound planes through optimized feature selection.

Ultrasound (US) imaging is an essential diagnostic technique in prenatal care, enabling enhanced surveillance of fetal growth and development. Fetal ultrasonography standard planes are crucial for evaluating fetal development parameters and detecting abnormalities. Real-time imaging, low cost, non-invasiveness, and accessibility make US imaging indispensable in clinical practice. However, acquiring fetal US planes with correct fetal anatomical features is a difficult and time-consuming task, even for experienced sonographers. Medical imaging using AI shows promise for addressing current challe

Clinical features and genetic analysis of 471 cases of fetal congenital heart disease.

BACKGROUND: Congenital heart disease (CHD) is a heterogeneous collection of structural abnormalities of the heart or great vessels that are present at birth. These birth defects are one of the leading causes of infant mortality and morbidity worldwide. The etiology and pathogenesis of CHD are unclear and largely considered to be multifactorial in nature. Since the chromosomal profile of CHD has not been analyzed in a large sample size, we aimed to summarize the clinical features, cytogenetics findings, and pregnancy outcomes of CHD to provide a clinical reference for prenatal diagnosis. Prenatal diagnosis of clinodactyly and its association with genetic syndromes: A case report.

A curvature of a finger that bends inwards relative to the other fingers is a common observation during prenatal screening. When the angulation exceeds 10 degrees, it is known as "clinodactyly" and could suggest a variety of underlying issues. Even though it usually remains unnoticed during pregnancy, it may be a sign of serious fetal disease. We report the case of a fetus diagnosed with clinodactyly of the thumb accompanied by tachycardia, abnormal levels of maternal hormones in the first trimester and increased impedance to flow in the uterine arteries. Although non-invasive prenatal testing

Evaluation of predictive performance of fetal urinary inflammatory markers of postnatal kidney function in fetuses with posterior urethral valves.

BACKGROUND: There are proposed roles for inflammation in the development of congenital obstructive uropathy in the setting of posterior urethral valves (PUV). However, the value of inflammatory proteins as predictive markers of postnatal kidney function, key in the management of fetuses with PUV, has not been explored. We screened fetal urine of fetuses with PUV with a panel of inflammatory proteins to determine their predictive value of postnatal kidney function. METHODS: Twenty-five different chemokines and cytokines were measured using a multiplex immunoassay in fetal urine of 79 PUV patie
