BACKGROUND: Brugada Syndrome is an inherited channelopathy that predisposes to ventricular arrhythmias. Triggers include fever, enhanced vagal tone, electrolyte abnormalities and certain medications, including local anaesthetics and propofol. Multicentre studies evaluating anaesthetic management in conjunction with maternal and neonatal outcomes in pregnant patients with Brugada Syndrome are lacking.
METHODS: In this retrospective cohort study, outcomes from 64 pregnancies in 57 patients with Brugada Syndrome across 12 maternity units in the UK and Ireland were reviewed (2014-2025). Outcomes

BACKGROUND: Assisted reproductive technologies (ART) include medical interventions used to address infertility. As access to ART has expanded globally, more women with underlying cardiovascular disease (CVD) are pursuing these treatments. However, data regarding the safety profile and pregnancy outcomes of ART in women with CVD remain limited. In this study we evaluated whether ART is associated with adverse maternal cardiac, obstetric, and fetal and neonatal outcomes among women with CVD.
METHODS: This was a retrospective case-control study of consecutive pregnancies achieved through ART, re

OBJECTIVE: This article reviews neurologic complications associated with pregnancy and menopause, the influence of pregnancy and menopause on preexisting neurologic disorders, and the management of neurologic disorders during pregnancy and menopause.
LATEST DEVELOPMENTS: Pregnancy and menopause can significantly impact neurologic health. Neurologic consultation regarding mode of delivery and anesthesia in patients with neurologic conditions is common, and very few neurologic conditions warrant cesarean delivery. Pregnancy is not a contraindication to acute ischemic stroke therapies or targete

BACKGROUND AND AIMS: Post-spinal hypotension (PSH) is a common complication of neuraxial anaesthesia in high risk cesarean delivery (CD). This systematic review and network meta-analysis evaluate the comparative efficacy and safety of prophylactic intravenous infusions of norepinephrine, phenylephrine, and ephedrine to prevent PSH in this population.
METHODS: Randomised controlled trials comparing intravenous prophylactic norepinephrine, phenylephrine, and ephedrine in high risk CD and reporting maternal or fetal outcomes were considered. Two reviewers screened studies, extracted data, and as

Tetralogy of Fallot (TOF) is the most common cyanotic congenital heart disease. The physiological cardiovascular changes of pregnancy may worsen right-to-left shunting and haemodynamic instability in women with uncorrected TOF, increasing maternal and fetal risk. We report a case series of three parturients with uncorrected TOF managed at our institution using a combined low-dose spinal and epidural anaesthetic technique. In these cases, the technique was associated with adequate surgical anaesthesia and stable haemodynamics, allowing avoidance of general anaesthesia.

INTRODUCTION: Conjoined twinning represents a rare and complex congenital anomaly arising from incomplete embryonic division after the 13th day of fertilization. The condition occurs in approximately 1 in 50,000–200,000 live births, most commonly among females. Successful perioperative management requires multidisciplinary planning and precise anesthetic coordination to address unique physiological challenges, including potential cross-circulation and shared organ systems. CASE PRESENTATION: We report the anesthetic management of six-hour-old male conjoined twins joined from the mid-chest to t

Euglycemic diabetic ketoacidosis (EDKA) is an uncommon but serious metabolic complication in pregnancy, where physiologic changes promote ketone production even without marked hyperglycemia. We report the case of an 18-year-old woman at 35 weeks and 4 days of gestation with type 1 diabetes mellitus and severe preeclampsia who developed intraoperative cardiac arrest during an emergent cesarean delivery. She had been nonadherent with insulin therapy for more than seven days before admission. Initial laboratory evaluation demonstrated metabolic abnormalities despite normoglycemia. During cesarean

Idiopathic ventricular tachycardia arising from right ventricle outflow tract (RVOT-VT) is an uncommon, catecholamine-sensitive form of VT occurring in the absence of structural heart disease and may pose challenges during pregnancy and delivery due to the risk of sympathetic stimulation. We report a case of a pregnant woman with RVOT-VT who underwent caesarean section. She had a history of dyspnoea and palpitations, was diagnosed and started on calcium channel blocker in the antenatal period. Peripartum management of RVOT-VT can be challenging. BACKGROUND: Hyperleukocytosis in patients with hematologic malignancies poses significant perioperative risks, including leukostasis, coagulopathy, tumor lysis syndrome and hyperinflammatory syndrome. Additionally, pregnancy and hyperleukocytosis predispose patients to a hypercoagulable state, increasing the risk of thrombosis. On the other hand, amniotic fluid embolism (AFE) is a rare but serious complication of pregnancy. Although distinct in etiology, thrombotic pulmonary embolism (PE), AFE, and hyperleukocytosis crisis can present with a strikingly similar clinical picture of pulmonary hyp

BACKGROUND: Two biological subphenotypes in acute respiratory distress syndrome (ARDS) have been identified in retrospective analyses, with differential clinical outcomes and post-hoc responses to investigational treatments. The ability to identify biological subphenotypes in real-time is unknown. We aimed to evaluate the feasibility of using the multisite ISPY COVID Network to prospectively evaluate biological subphenotypes in real-time.
METHODS: This prospective, observational, cohort study enrolled patients with ARDS and severe acute hypoxaemic respiratory failure (AHRF) and assessed the f

PURPOSE: Normal coagulation is essential for maternal safety during pregnancy and delivery. Thalassemia may influence coagulation parameters, however, its effects during pregnancy remain incompletely characterized. This study aimed to evaluate maternal coagulation profiles in pregnant women with thalassemia in South China.
PATIENTS AND METHODS: This retrospective observational study included 53 pregnant women with thalassemia and 352 pregnant women without thalassemia who delivered at a tertiary medical center in South China. Singleton pregnancies with gestational age of ≥ 37 weeks were analy

BACKGROUND: Rheumatic heart disease remains a major cause of maternal morbidity and mortality in low- and middle-income countries. The coexistence of severe mitral stenosis (MS) and antiphospholipid syndrome presents an exceptionally high-risk clinical scenario requiring multidisciplinary management.
CASE SUMMARY: A 31-year-old woman with severe rheumatic MS and antiphospholipid syndrome presented at 20 weeks of gestation with shortness of breath and palpitations. Echocardiography confirmed severe MS, with a valve area of 0.8 to 0.9 cm2. Despite optimal medical therapy, she remained symptomat

This case report describes target-controlled propofol anesthesia using the Eleveld pharmacokinetic-pharmacodynamic model in a third-trimester pregnant woman with significant comorbidity undergoing elective non-obstetric surgery. A 31-year-old woman, 35 weeks pregnant with portal hypertension secondary to portal vein thrombosis after gastric bypass, underwent endoscopic esophageal variceal ligation under total intravenous anesthesia with remifentanil and propofol, guided by Bispectral Index monitoring. Propofol was administered via effect-site target-controlled infusion using the Eleveld model,

Gestational trophoblastic disease (GTD) presents unique anaesthetic challenges due to the combined risks of haemorrhage, cardiopulmonary compromise, and endocrine dysfunction. Complete hydatidiform mole is frequently associated with markedly elevated human chorionic gonadotropin (β-hCG) concentrations, which may result in biochemical or clinical thyrotoxicosis through thyroid-stimulating hormone receptor activation. Anaesthetic technique selection during uterine evacuation remains contentious. General anaesthesia (GA) is often favoured because it allows airway control and rapid escalation in t

BACKGROUND: Pregnancy represents a significant life event for most women, inducing extensive physiological adaptations. Consequently, it poses additional risks for women with congenital heart disease (CHD), a leading cause of maternal mortality. Isolated right ventricular apical hypoplasia (IRVAH) is a poorly characterized CHD subset, with pregnancy outcomes remaining unreported. Our case of a primigravida with IRVAH underscores the need for heightened monitoring in this population.
CASE DESCRIPTION: We present a 28-year-old primigravida at 28 gestational weeks referred for structural right h

Ellis-van Creveld (EVC) syndrome is a rare autosomal recessive skeletal dysplasia characterized by disproportionate short stature, skeletal abnormalities, craniofacial and dental anomalies, and a high prevalence of congenital cardiovascular disease. Pregnancy in patients with EVC syndrome presents unique anesthetic challenges due to altered airway anatomy, abnormal spinal anatomy affecting neuraxial techniques, restrictive pulmonary physiology, potential cardiac dysfunction, and resultant hemodynamic complications. Evidence guiding anesthetic management for this population remains limited. We

Severe mitral stenosis (MS) during pregnancy represents a fixed cardiac output state and poses major anesthetic challenges during elective cesarean delivery (CD). Conventional single-shot spinal anesthesia may precipitate profound hypotension in these patients. We report two high-risk parturients with severe MS, classified as New York Heart Association (NYHA) class III, who underwent elective CD. Both patients were managed using low-dose segmental spinal anesthesia supplemented with ultrasound-guided erector spinae plane block (ESPB) and invasive hemodynamic monitoring. In both cases, the proc

BACKGROUND Paradoxical vocal fold motion disorder (PVFM) is a rare condition consisting of episodic upper airway obstruction, resulting in recurrent stridor and respiratory distress. Limited data exists regarding its presentation, implications, and consequences during pregnancy. In this report, we present the case of a 23-year-old primigravida woman with known PVFM and recurrent stridor who developed acute respiratory distress requiring endotracheal intubation, followed by emergency bedside cesarean delivery, during which an incidental placental abruption was identified. CASE REPORT A 23-year-
