Beating the Odds: Successful Emergent Surgery After Acute Aortic Dissection in Takayasu Arteritis.

Acute aortic syndromes (AAS) encompass life-threatening conditions that require rapid diagnosis and specialized intervention. Among these, large-vessel vasculitides, such as Takayasu arteritis (TA), pose unique surgical and anesthetic challenges due to active vascular inflammation, tissue fragility, and possible multiorgan involvement. We report the perioperative management of a 34-year-old female, who was 20 weeks pregnant, with TA who presented with rapidly progressive aneurysmal dilation and dissection of the thoracic aorta, complicated by multiorgan failure. She underwent emergent hybrid r

Anesthetic management of a high-risk cesarean delivery in a patient with multivessel Takayasu arteritis.

Takayasu arteritis (TA) is a rare chronic inflammatory disease affecting large vessels that can result in stenosis, occlusion, and aneurysm formation. Although not a contraindication, TA poses significant anesthetic challenges in pregnancy. We describe the successful anesthetic management of a patient with TA complicated by aortic root dilation, abdominal aortic aneurysm, bilateral subclavian artery, and bilateral carotid artery stenosis undergoing elective cesarean delivery at 37 weeks. To minimize hemodynamic fluctuations, we used a slowly titrated dural puncture epidural technique along wit

Zero-fluoroscopy catheter ablation for arrhythmias in pregnancy: validation of a standardized approach with 5-year follow-up.

BACKGROUND: Increasing experience with zero- (ZF) or near-zero fluoroscopy catheter ablation (CA) supports the implementation of an early fluoroless approach for recurrent, symptomatic arrhythmias during pregnancy. AIM: This study evaluated the feasibility, efficacy, and safety of CA using a standardized ZF approach during pregnancy. METHODS: Data were derived from a large, prospective multicenter registry (ELEKTRO-RARE-A-CAREgistry). Between 2012 and 2019, more than 2,655 CA procedures were performed in women using an intention-to-treat ZF approach. The procedures were performed using (1) f

Emergent simultaneous cesarean section and maternal cardiac surgery.

BACKGROUD: Management of pregnant patients with cardiovascular disease is a challenge, and limited experiences exist to date in the literature. Particularly, valve surgery is still at high risk, with a consistent rate of preterm birth, postnatal physical or mental disabilities, and fetal mortality. CASE PRESENTATION: A 38-year-old woman, who was 37 weeks pregnant, was admitted to the emergency department with severe dyspnea and hypertension. A transthoracic echocardiography revealed a 4 cm mobile mass in the left atrium, attached to the interatrial septum by a stalk; the mass protruded into t

Zero-fluoroscopy, intracardiac echocardiography-only patent foramen ovale closure in early pregnancy: a case report.

BACKGROUND: Transcatheter closure of a patent foramen ovale (PFO) is an established strategy for secondary prevention of cryptogenic stroke in selected patients. During pregnancy, the timing and method of closure require individualized assessment of recurrent embolic risk and maternal-fetal procedural risk. In selected first-trimester patients, avoiding fluoroscopy, contrast, procedural transoesophageal echocardiography (TEE), and sedation may be desirable when adequate alternative imaging is available. CASE SUMMARY: A 34-year-old woman at 7 weeks' gestation presented with acute ischaemic str

Anesthetic Management of Emergent Cesarean Delivery in a Patient With Severe Rheumatic Mitral Stenosis and Cardiopulmonary Complications: A Case Report.

Critical rheumatic mitral stenosis complicated by pulmonary hypertension and right ventricular dysfunction during pregnancy represents one of the most challenging scenarios in obstetric anesthesia. These patients are classified as modified World Health Organization (mWHO) Class IV, which is the highest maternal risk category with high maternal mortality rates. The peripartum period is especially dangerous since even modest hemodynamic shifts can trigger cardiovascular collapse. We report a successful case of anesthetic management of a 40-year-old gravida 3, para 2 at 29 weeks and three days ge

Extreme Airway Difficulty During EXIT in Severe Fetal Agnathia: Failed Conventional Intubation and Successful Emergency Tracheostomy-A Case Report.

BACKGROUND: Severe mandibular hypoplasia, particularly agnathia, is a rare congenital condition that may cause critical neonatal airway obstruction. The Ex Utero Intrapartum Treatment (EXIT) procedure allows airway control while maintaining uteroplacental circulation; overall airway establishment success rates for EXIT exceed 90%, yet conventional endotracheal intubation fails in a meaningful subset of cases with extreme anatomic distortion, necessitating surgical airway access. In the most severe forms of agnathia, primary tracheostomy under placental support may represent the most appropriat

Serial Fetal Heart Rate Monitoring During Electroconvulsive Therapy in Late Pregnancy: A Case Report of Transition From Succinylcholine to Rocuronium Reversed With Neostigmine.

Electroconvulsive therapy (ECT) is considered relatively safe and effective for severe psychiatric disorders during pregnancy, but evidence regarding neuromuscular blockers for ECT in pregnancy remains limited. We report a 30-year-old primigravida admitted at 30 weeks of gestation with severe major depressive disorder and persistent suicidal ideation despite pharmacotherapy. ECT was initiated at 32 weeks and 2 days of gestation. The first five sessions used succinylcholine. Owing to a nationwide shortage in Japan, the muscle relaxant was switched to rocuronium from the sixth session, with neos

Perioperative Management of Patients with Preeclampsia: A Comprehensive Review.

Preeclampsia is a common condition of pregnancy characterized by hypertension complicated by cerebral, cardiac, hepatic, renal, hematologic, and placental dysfunction. Patients with preeclampsia frequently undergo cesarean delivery, the most common major surgical procedure in the world. They represent a high-risk perioperative cohort suffering significant preventable morbidity and mortality. This review focuses on the anesthesiologist's role, through a perioperative lens, in reducing maternal complications through management of hypertension and strategies for preserving the function of the bra

Sedentary Behavior and Light-Intensity Physical Activity During Pregnancy and Cardiovascular Health: A Science Advisory From the American Heart Association.

The Physical Activity Guidelines for Americans supports sitting less and moving more. Growing evidence suggests that a waking behavior profile with less sedentary behavior and more light-intensity physical activity is associated with more favorable cardiovascular health. Remarkably, little is known about how these behaviors relate to cardiovascular health during pregnancy. The purpose of this American Heart Association science advisory is to describe the existing evidence on device-measured sedentary behavior and light-intensity physical activity in relation to cardiovascular health during pre

Pregnancy with a Left Ventricular Assist Device: A Narrative Review.

This narrative review discusses the various challenges associated with the presence of a left ventricular assist device (LVAD) during pregnancy. Given the hemodynamic and coagulation changes associated with pregnancy, the presence of an LVAD adds a layer of complexity with respect to optimal management. This review will discuss the anesthetic considerations when dealing with this subset of patients who may have other comorbidities alongside their advanced heart failure. Additionally, this paper aims to review successful pregnancies with an LVAD placement focusing on the mode of delivery and he

Guidelines for anaesthesia of adults with congenital heart disease in non-cardiac surgery.

OBJECTIVE: To provide guidelines for the anesthetic management of adults with congenital heart disease for non-cardiac surgery. DESIGN: A consensus committee of 16 experts was convened. A formal conflict-of-interest (COI) policy was developed at the beginning of the process and enforced throughout. The entire guidelines process was conducted independently of any industrial funding (i.e., pharmaceutical, medical devices). The authors were required to follow the rules of the Grading of Recommendations Assessment, Development, and Evaluation (GRADE®) system to guide the assessment of the quality

Performance of Shock Index as a predictor of severe postpartum hemorrhage following cesarean section under spinal anesthesia: a retrospective cohort study.

BACKGROUND: An early diagnosis of a postpartum hemorrhage (PPH) can be challenging. The Shock Index (SI), the ratio of heart rate to systolic blood pressure, appears to be a useful indicator for the early diagnosis of severe PPH after vaginal delivery. Our primary aim was to evaluate if the SI has a predictive value for severe PPH following cesarean section (C-section) under spinal anesthesia (SA) and its best predictive threshold. METHODS: Retrospective single-center trial including women scheduled for C-section under SA with prevention of maternal hypotension by phenylephrine, and complicat

What Every Cardiologist Should Know About Contraception and Reproductive Planning in 2025.

Recent studies have shown that maternal mortality continues to increase for all racial and ethnic groups in the United States. The recent changes in reproductive legal policies combined with unacceptably high morbidity and mortality rates have made it crucial for every cardiologist to understand and engage with reproductive planning. The risk of cardiovascular complications during pregnancy is prohibitively high for patients in the modified World Health Organization Risk Class IV with maternal cardiovascular event rates of >40%, and pregnancy is not recommended. It is critical that cardiologis

Optimizing Psychological Health Across the Perinatal Period: An Update on Maternal Cardiovascular Health: A Scientific Statement From the American Heart Association.

Perinatal psychological health conditions (eg, perinatal depression, anxiety) are some of the leading causes of maternal mortality in the United States and are associated with adverse pregnancy outcomes, long-term cardiovascular outcomes, and intergenerational effects on offspring neurodevelopment. These risks underscore the importance of addressing maternal psychological health as a key determinant of perinatal cardiovascular health. Thus, it is vital to recognize the spectrum of perinatal psychological health and to provide guidance for both patients and clinicians on screening and managemen
