[Epidural anesthesia in a patient with Friedreich's ataxia].

Friedreich's ataxia (FA), a hereditary disease with degenerative changes localized chiefly in the spinal cord and cerebellum, is characterized clinically by ataxia, absence of tendon reflexes, loss of proprioceptive sensation, and extensor plantar responses. There are only a few reports on anesthesia for patients with FA. General but not regional anesthesia is usually recommended because a persistent aggravation of symptoms is feared with regional anesthesia. We report a 31-year-old gravida 1 para 0 patient with FA who was admitted at the 20th week of gestation for induced abortion, curettage

Anesthesia-related maternal mortality in Michigan, 1972 to 1984.

We reviewed maternal deaths in the state of Michigan occurring from 1972 through 1984. There were 15 maternal deaths in which anesthesia was considered the primary cause and 4 deaths in which anesthesia was a contributory factor. Complications of regional anesthesia were the main cause of death during the early part of the period, whereas the inability to accomplish endotracheal intubation emerged as the principal cause of death in recent years. Eleven of the 15 patients had undergone cesarean section. Obesity was a risk factor in 12 patients, in an equal number of patients the risk factor was

Spina bifida occulta and epidural anaesthesia.

Spina bifida occulta occurs in 5-10% of the population, not all of whom display superficial signs. Attempted epidural puncture at the level of the lesion will almost certainly result in a dural tap. We report a patient who developed a postural headache after Caesarean section under epidural anaesthesia, in whom radiography of the spine later demonstrated spina bifida occulta. This problem has not been described previously, although it is unlikely to be an isolated case.

The postoperative adverse effects of inhalational anesthetics.

Inhalational general anesthetics can contribute to postoperative morbidity (Table II). Postoperative effects of inhalational anesthetics on the central nervous system are speculative. The "toxic" effects of these agents during the postoperative period are most often an extension of their pharmacologic and physiochemical properties. Inhalational anesthetics may produce a number of varied changes in mental status after surgery such as headache, emergence excitement, and delirium. It is very important for health professionals to be aware of the risk of perioperative myocardial infarction in patie

Accidental subdural catheterisation.

A case of accidental subdural catheterisation is presented, and its course is compared with previously reported subdural injections of local anaesthetic agents. The importance of obtaining proof of the site of malposition of an epidural catheter is stressed.

Migration of an extradural catheter into the subdural space. A case report.

After repeated, successful extradural injections in a parturient, the extradural catheter migrated into the subdural space. The injection of 3% chloroprocaine 10 ml resulted in an extensive block, which extended to the cranial nerves and paralysed the respiratory muscles. The position of the catheter tip was confirmed by fluoroscopy. Subdural injection should always be suspected when tests for subarachnoid injection are negative, but an extensive block occurs gradually within 15-20 min. Respiration and circulation should be supported until the block regresses.

Some aspects of epidural block provided for elective caesarean section.

We present here clinical data from 993 patients who were destined to undergo elective Caesarean section under epidural analgesia. In 29 cases the attempt to provide an epidural was abandoned before the operation started. In regard to this, the importance of monitoring the fetal heart rate during initiation of the epidural is emphasised. In 10 cases general anaesthesia was induced after delivery, and in a further 108 cases supplemental analgesia was administered, either systemically, by inhalation or by an additional epidural top-up, after delivery. Satisfactory analgesia throughout the operati

Recent advances in local anaesthesia.

In general, the cardiovascular system is more resistant to the toxic actions of local anaesthetics than is the central nervous system. However, if sufficient doses and blood levels of local anaesthetics are achieved, signs of profound cardiovascular depression may be observed. Differences exist between local anaesthetics in terms of their relative potential for cardiotoxicity. The CC/CNS ratio for bupivacaine and etiodcaine is less than for lidocaine. In addition, bupivacaine may precipitate ventricular arrhythmias and ventricular fibrillation. Local tissue toxicity can occur following the adm

Extensive spread of local anaesthetic solution following subdural insertion of an epidural catheter during labour.

A case is described where a small dose of bupivacaine was accidentally injected into the subdural, extra-arachnoid space resulting in extensive unilateral block and hypotension. The spread of solution was confirmed radiologically.

Spinal anaesthesia revisited: an evaluation of subarachnoid block in obstetrics.

A series of 442 women receiving subarachnoid block for various obstetrical procedures is described. These included caesarean section, instrumental delivery, manual removal of placenta, insertion of cervical suture and miscellaneous procedures. General anaesthesia was required in ten patients because subarachnoid block proved to be unsatisfactory. There were no serious complications. The overall headache rate, ascertained by daily direct questioning during hospital admission, was 26.2%. Clinical features of dural puncture headache (DPH) were present in 8.6% and equivocally so in a further 3.4%.

[Massive intracerebral hemorrhage and anterior spinal artery syndrome in etiologic relation to paraspinal anesthesia procedures? 2 case reports].

Complications in connection with operations also lead to the question of possible relations to the type of anaesthesia and their avoidability. The subject is treated demonstrating two cases of intracerebral haemorrhage and anterior spinal artery syndrome during and following catheter-epidural anaesthesia or spinal anaesthesia, respectively. No definite causal connection was established, even if the emergencies presumably would not have occurred without operation and anaesthesia. Complications are not completely avoidable, even if operation and anaesthesia as regards indication, execution and m

Fetal breathing movements. An update for the pediatrician.

Although the presence of fetal breathing movements (FBMs) has been suspected for almost a century, the major advances in the field were made during the last decade. Experimental animal techniques of detection include instrumentation of fetuses with electronic recording equipment. Human studies use more indirect, although no less accurate, A-mode or real-time B-scan ultrasonography in the recording of FBMs. The fetus does not make breathing movements continuously; however, there are periods of FBMs interspersed with periods of apnea. Inherent to FBMs is a diurnal variation. Hypoxia and hypoglyc
