Pregnancy Complicated by Citrin Deficiency and Gestational Diabetes Mellitus: Perioperative Metabolic Management and Postoperative Analgesic Challenges Following Caesarean Delivery.

Citrin deficiency is a rare autosomal recessive metabolic disorder caused by pathogenic variants in the SLC25A13 gene. Pregnancy in affected women is uncommon and presents unique challenges because conventional obstetric management may conflict with metabolic requirements. The coexistence of gestational diabetes mellitus (GDM) introduces additional complexity. A 25-year-old, gravida 2 para 0+1 woman with genetically confirmed citrin deficiency (homozygous SLC25A13 c.1763G>A mutation) was managed jointly by obstetric and inherited metabolic disease (IMD) teams. Her pregnancy was complicated by

Graded Epidural Anaesthesia for Caesarean Section in the Presence of an Untreated Spetzler-Martin Grade V Cerebral Arteriovenous Malformation With Radiological Progression Over Six Years: A Case Report.

Cerebral arteriovenous malformations (AVMs) are rare during pregnancy, when the haemodynamic and hormonal changes of gestation may increase the risk of rupture. Lesions that are large, located in eloquent regions, and drained by deep venous channels represent a surgically high-risk subtype, carry substantial surgical morbidity, and are frequently managed conservatively when unruptured. We describe a 33-year-old G3P1L1A1 parturient at 36 weeks and six days of gestation with an untreated right parieto-temporo-occipital AVM, who declined intervention across three encounters and underwent an elect

Critical Rheumatic Mitral Stenosis Unmasked During Pregnancy Complicated by Preeclampsia With Severe Features and Managed With Urgent Balloon Mitral Valvuloplasty.

Pregnancy imposes significant hemodynamic changes that can unmask previously undiagnosed cardiovascular disease and complicate management when concurrent obstetric conditions are present. We present the case of a 32-year-old immigrant woman at 21 weeks' gestation with a limited medical history due to a significant language barrier, who presented with hypertensive crisis and was diagnosed with preeclampsia with severe features. During her evaluation, she developed worsening dyspnea and acute respiratory decompensation. Further cardiac assessment revealed critical rheumatic mitral stenosis, acco

Early Palliative Care Consultation for Patients Requiring Extracorporeal Membrane Oxygenation.

OBJECTIVE: Obesity is one of the main concerns for public health and is becoming an increasingly widespread problem worldwide. Women are more likely to require a cesarean section and have a longer hospital stay after delivery. Excess body weight can interfere with ovulation and make it more difficult for embryos to implant in the uterus. A high body mass index (BMI) has controversial effects on the outcomes of medically assisted reproduction treatments (IVF) and, if careful counseling is not performed, medical-legal risks may be incurred. While some researchers argue that obesity does not part

When Pushing Is Not Enough: A Novel Description of Maternal Characteristics and Delivery Outcome.

Operative vacuum delivery (VD) is often used to expedite safe vaginal births for various reasons, including maternal exhaustion (MEX), which accounts for about 30% of cases. This study identifies risk factors associated with vacuum deliveries for MEX and evaluates maternal and neonatal outcomes compared to other indications.A retrospective cohort study analyzed singleton-term vacuum deliveries from 2011 to 2022 at a tertiary care center, categorizing patients into two groups: Those with MEX (group 1) and those without (group 2). Statistical analyses included chi-square tests, t-tests, Mann-Whi

Nonfatal self-injury in the first year after surgery: a population-based epidemiologic study.

BACKGROUND: Recovery after surgery can be psychologically distressing, putting patients at risk of suicidal behaviours such as nonfatal self-injury (NFSI). We evaluated the incidence and identified factors associated with NFSI after surgery. METHODS: Linked administrative databases from New York state were used to identify adults (≥18 yr) undergoing a broad range of surgical procedures between 2016 and 2018. The primary outcome was presentation to an emergency department or hospitalisation for NFSI within 365 days of discharge from the index surgical admission. The incidence was compared with

Comparison of Effects of General Versus Spinal Anesthesia on Spermiogram Parameters and Pregnancy Rates After Microscopic Subinguinal Varicocelectomy Surgery: Retrospective Cohort Analysis.

Background and Objectives: The association between different anesthesia modalities and spermiogram parameters and reproductive outcomes in patients undergoing microscopic subinguinal varicocelectomy (MSV) remains unclear. In this retrospective cohort study, we aimed to compare spermiogram parameters and pregnancy rates between patients receiving general anesthesia (GA) versus spinal anesthesia (SA) for MSV with 2-year follow-up data. Materials and Methods: Male patients aged between 18-50 years, with ASA physical scores between I-III, who underwent unilateral or bilateral primary MSV, were inc

Comparative effectiveness of intravenous remifentanil, epidural anesthesia, and a 2-step analgesic approach for external cephalic version: a large prospective single-center cohort study.

BACKGROUND: Among the analgesic strategies used during external cephalic version, neuraxial anesthesia is the only approach that has consistently demonstrated higher vaginal delivery rates. However, neuraxial analgesia encompasses a heterogeneous range of techniques, and higher complication rates have been reported compared with other approaches. OBJECTIVE: This study aimed to compare the success of external cephalic version, modes of delivery, maternal pain, and complications using 3 strategies: intravenous analgesia with remifentanil, epidural anesthesia, and a stepwise approach in which ep

Management of pregnancy in a woman with a new diagnosis of pycnodysostosis: A case report.

Pycnodysostosis is a rare autosomal recessive skeletal dysplasia resulting in short stature, characteristic facial features and long-bone fractures. Given its rarity, there is minimal information in the medical literature on management of pregnancies in women affected by pycnodysostosis. This report describes the antenatal, intrapartum and postnatal care of a 28-year-old nulliparous woman with a new diagnosis of pycnodysostosis during pregnancy. Throughout her care, involvement was sought from the moving and handling team, anaesthetics, geneticists and obstetricians to allow anticipation of po

Impact of an anaesthetist-led acute pain service for caesarean delivery as part of an enhanced recovery program: a retrospective before-and-after single-centre cohort study (2021-2023).

BACKGROUND: Postoperative pain after caesarean delivery is frequently moderate to severe, and insufficient analgesia may lead to chronic pain, prolonged opioid use, delayed functional recovery, and postpartum psychological distress. Structured postoperative pain management programmes may improve outcomes; however, they are rarely implemented in obstetric practice. This study evaluated the impact of an anaesthesialed acute pain service designed to standardise multimodal postoperative analgesia and early recovery processes. METHODS: We conducted a single-centre retrospective before-after cohort

Anaesthetic Management of Neurofibromatosis of an Unknown Subtype for Emergency Caesarean Section.

Neurofibromatosis is a heterogeneous group of inherited neurocutaneous disorders with variable multisystem involvement and important implications for obstetric anaesthesia. Anaesthetic concerns include potential airway involvement, occult spinal or intracranial tumours, cardiovascular instability, and the association with catecholamine-secreting tumours. These factors often lead to a preference for general anaesthesia, despite well-recognised maternal risks associated with general anaesthesia in obstetric practice. Decision-making is further complicated when neurofibromatosis is previously und

Postpartum Ischemic Stroke As the First Manifestation of Late-Onset Preeclampsia: A Case Report.

Preeclampsia is a severe pregnancy hypertensive disorder characterized by arterial hypertension and multiorgan involvement, most frequently affecting the renal, hepatic, and neurological systems. While its typical manifestations include headaches, visual disturbances, and/or epigastric pain, atypical presentations may occur and may lead to life-threatening complications. We report the case of a 33-year-old multiparous woman who developed sudden left-sided hemiparesis, hemisensory loss, homonymous hemianopia, and rightward conjugate gaze deviation within 24 hours after a cesarean delivery. Maternal cardiac arrest: a simulation-based review of anesthetic protocols and perimortem cesarean delivery.

PURPOSE OF THE REVIEW: This review summarizes the current state of the art in maternal cardiac arrest and cesarean delivery and the rationale for and use of simulation to facilitate training in this rare but critical event. RECENT FINDINGS: Stabilization of the mother and consideration of the '4P's' and multidisciplinary management postarrest remain critical. Some key updates on management of maternal cardiac arrest include manual left uterine displacement and early/immediate preparation for resuscitative hysterotomy, with less focus on a 4-min window for intervention. Magnitude and associated factors of premature rupture of membranes among pregnant women admitted to a university comprehensive specialized hospital in Central Ethiopia.

BACKGROUND: Premature rupture of the membrane occurs after the 28th week of gestation and before labor, causing 44% perinatal morbidity and 7% perinatal mortality. It is the leading cause of neonatal death in Ethiopia, with common maternal and neonatal complications such as chorioamnionitis or intraamniotic infection. However, information on the premature rupture of membranes, which is a contributing factor in the study area, is limited. The aim of this study was to assess the magnitude and associated factors of premature rupture of the membrane among pregnant women. METHODS: A cross-sectional

Investigating the association between pain and psychological vulnerabilities and postpartum depression.

Pain and psychological vulnerabilities may be associated with the development of postpartum depression (PPD) in women undergoing Cesarean delivery. However, there is limited information regarding women undergoing labor and delivery. We investigated whether pre-delivery pain and psychological vulnerabilities, and other risk factors were associated with an increased risk of PPD among laboring women. This was a secondary analysis of a clinical trial conducted between June 2017 and July 2021. The risk of PPD was assessed using an online survey on Edinburgh Postnatal Depression Scale (EPDS) adminis

Recognition of intrapartum uterine rupture, anesthetic management, and maternal-fetal outcomes: strategies for rapid response.

PURPOSE OF THE REVIEW: Uterine rupture is a rare but life-threatening obstetric condition that can lead to catastrophic outcomes for pregnant patients and their offspring. As the trial of labor after cesarean births rises and other uterine procedures become more common, the observed rates of this complication have increased. We aim to provide guidance on risk stratification, timely recognition, anesthetic considerations, hemorrhage management, and associated outcomes. RECENT FINDINGS: The risk of uterine rupture varies by scar type, with the highest rates after classical cesarean incisions. T
