Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Thromboelastography may assess the effect of anticoagulation reversal in intracranial hemorrhage.

BACKGROUND: Intracranial hemorrhage (ICH) is a complication of oral anticoagulation and is associated with significant morbidity and mortality. Clinical need exists for biomarkers to measure anticoagulation in patients with factor Xa inhibitor-associated ICH to assess the hemostatic effect of reversal agents. This study explored the utility of thromboelastography (TEG) to assess anticoagulation in emergency department (ED) patients who received activated prothrombin complex concentrate (aPCC) reversal for factor Xa-inhibitor-associated ICH. METHODS: This was a prospective, single-center, coho

Pregnancy with a Left Ventricular Assist Device: A Narrative Review.

This narrative review discusses the various challenges associated with the presence of a left ventricular assist device (LVAD) during pregnancy. Given the hemodynamic and coagulation changes associated with pregnancy, the presence of an LVAD adds a layer of complexity with respect to optimal management. This review will discuss the anesthetic considerations when dealing with this subset of patients who may have other comorbidities alongside their advanced heart failure. Additionally, this paper aims to review successful pregnancies with an LVAD placement focusing on the mode of delivery and he

Experiences of Australian anaesthetists and anaesthetic trainees of working whilst pregnant: Results of a national survey.

Changing medical workforce demographics emphasise the need to understand and effectively manage pregnancy in the workplace. An Australian survey exploring pregnancy experiences of specialists and trainees working in anaesthesia received 242 responses describing 549 pregnancies. One hundred and sixty-two (67%) of these respondents were aged 31-40 years, 185 (76%) were specialists and 159 (66%) reported one or two pregnancies. Reported pregnancy complication rates were similar to national averages. Potentially harmful exposures included working >40 h per week (n = 298, 55%), ionising radiation a

Performance of Shock Index as a predictor of severe postpartum hemorrhage following cesarean section under spinal anesthesia: a retrospective cohort study.

BACKGROUND: An early diagnosis of a postpartum hemorrhage (PPH) can be challenging. The Shock Index (SI), the ratio of heart rate to systolic blood pressure, appears to be a useful indicator for the early diagnosis of severe PPH after vaginal delivery. Our primary aim was to evaluate if the SI has a predictive value for severe PPH following cesarean section (C-section) under spinal anesthesia (SA) and its best predictive threshold. METHODS: Retrospective single-center trial including women scheduled for C-section under SA with prevention of maternal hypotension by phenylephrine, and complicat

[MANAGEMENT OF LABOR ANESTHESIA IN A PATIENT WITH EHLERS-DANLOS SYNDROME WHY DOES CHATGPT ERR IN SOURCE REFERENCING?].

Ehlers-Danlos Syndrome (EDS) encompasses a spectrum of inherited disorders, characterized by joint hypermobility, skin hyperextensibility, and other features. In some patients, EDS involves the vascular walls, posing a significant clinical challenge due to the resultant propensity for rupture with hemorrhagic complications. Such complications, among others that often occur with EDS, may carry particular importance in the context of pregnancy and labor. This paper presents a patient diagnosed with Ehlers-Danlos Syndrome during her first pregnancy. The patient was planned for elective Cesarean d

Incidence of chronic postsurgical pain after caesarean delivery: a systematic review and meta-analysis.

INTRODUCTION: Chronic postsurgical pain is recognised increasingly as a complication of caesarean delivery, with implications for a large cohort of patients. As interest in this area has grown, there has been more primary research into both the incidence and severity of pain. We aimed to synthesise all available evidence to evaluate the rate, features and severity of chronic postsurgical pain following caesarean delivery and whether it has changed over time. METHODS: We performed a systematic review and meta-analysis of the incidence of chronic postsurgical pain at 3, 6 and 12 months after ca

Consensus statement on pain management for pregnant patients with opioid use disorder from the Society for Obstetric Anesthesia and Perinatology, Society for Maternal-Fetal Medicine, and American Society of Regional Anesthesia and Pain Medicine.

Pain management in pregnant and postpartum people with an opioid use disorder requires a balance among the risks associated with opioid tolerance, including withdrawal or return to opioid use, considerations around the social needs of the maternal-infant dyad, and the provision of adequate pain relief for the birth episode that is often characterized as the worst pain a person will experience in their lifetime. This multidisciplinary consensus statement from the Society for Obstetric Anesthesia and Perinatology, the Society for Maternal-Fetal Medicine, and the American Society of Regional Anes

PICC Lines in Pregnancy and Other Peripartum Vascular Access Considerations.

IMPORTANCE: To decrease associated infectious and thrombotic morbidity, it is important to understand the indications and risks of peripherally inserted central catheters (PICCs) and other vascular access means in pregnancy. OBJECTIVES: The objectives are 3-fold: (1) discuss indications and contraindications, approach to placement, and associated complications for PICC lines, arterial catheters, centrally inserted central catheters, and peripheral intravenous catheters; (2) review available data regarding complications associated with these catheters in pregnancy; and (3) propose an evidence-

Peripartum Respiratory Failure in a Patient With Severe Preeclampsia.

Preeclampsia is a major hypertensive disorder of pregnancy associated with increased maternal, fetal, and neonatal risks. Acute respiratory failure, though rare, is a severe complication that may arise due to preeclampsia or unrelated medical conditions. We present a case of a 23-year-old primiparous woman admitted to our Emergency Department with gestational hypertension during her third trimester. She was hospitalized for blood pressure regulation and preeclampsia screening, which included urine protein analysis, blood tests for liver and kidney function, and fetal monitoring through cardiot

Caesarean Section Rates Among Syrian Refugees in Lebanon.

OBJECTIVES: Deliveries can be complicated by many factors, especially if women are not well followed up. Syrian refugees, for the last decade, have had to go through unbearable circumstances, which have affected their health. This is evident in the pregnancy data when we look at the delivery methods and outcomes. The caesarian section (C-section) rate among Syrians before the civil war was as per the WHO recommendation; this rate, however, changed enormously for the Syrian refugees in Lebanon. This study aims to determine the C-section rate among Syrian refugees in Lebanon, identify contributi

Racial health disparities in severe maternal morbidity before and after implementation of an enhanced recovery after cesarean delivery protocol: a retrospective observational study at two New York City hospitals (2016-2020).

BACKGROUND: Enhanced recovery after cesarean delivery (ERAC) is an evidence-based pathway that aims to improve the quality of care for all patients. Standardization of care has been seen as a tool to promote equality and equity. Our goal was to evaluate racial differences in severe maternal morbidity (SMM) among patients before and after implementation of an ERAC program. METHODS: A retrospective study was performed among cesarean delivery patients pre- and post-ERAC implementation at two large academic hospitals in New York City from October 2016 to September 2020. Logistic regression models

Uterine Tone Numeric Rating Score as an Early Indicator of Major Postpartum Hemorrhage during Cesarean Delivery: A Prospective Observational Study.

BACKGROUND: Postpartum hemorrhage (PPH) is the leading preventable cause of maternal mortality. Most PPH cases are caused by uterine atony, which is inconsistently defined in clinical care. The electronic health record was used to prompt communication between the anesthesia and obstetric care teams about uterine tone using a validated 11-point numeric rating scale (NRS) at 0, 5, and 10 min after placental delivery for all cesarean deliveries at our institution. The primary hypothesis was that lower uterine tone NRS would be strongly associated with progression to major PPH. METHODS: This was

Procedural Abortion: Setting, Safety, and Patient Access.

Procedural abortion is the process of aspirating or extracting a pregnancy from a gravid uterus. This procedure is one of the most common procedures people who can become pregnant will have in their lifetime and it has a long track record of safety. Clinicians who perform these procedures should consider choice of setting, anesthesia, and risk of complications such as hemorrhage and other complications. Access to this procedure is increasingly limited due to restrictions on abortion.

Eight steps to success: minimally invasive surgical management of cesarean scar pregnancy.

OBJECTIVE: To provide an overview of cesarean scar pregnancy (CSP) including its risk factors and diagnosis, describe a case of CSP managed via robot-assisted laparoscopic resection, and provide eight tips for minimally invasive surgical management of such cases. Currently, there is no gold standard treatment for CSP. Therefore, treatment options include a wide array of surgical and nonsurgical therapies. The technique described here may be useful to increase the success of minimally invasive surgical management of such cases where future pregnancy is desired and avoid complications associated
