BACKGROUND AND OBJECTIVE: Chronic kidney disease (CKD) is frequently complicated by hematological abnormalities, including anemia, thrombocytopenia, and leukocyte disorders, which significantly impact morbidity and survival. This study aimed to evaluate hematological indices and abnormalities in patients with CKD and determine their associations with disease severity across stages 3-5.
METHODS: This cross-sectional observational study was carried out in the Department of Nephrology, Lahore General Hospital, Pakistan, between August 2023 and February 2024, following Institutional Review Board

Cauda equina syndrome (CES) is a rare but serious clinical presentation characterised by compression of the lumbosacral nerve roots below the conus medullaris, resulting in lower back pain, bilateral sciatica, motor and sensory deficits, and bladder or bowel dysfunction. Its occurrence in pregnancy is exceptionally rare and poses significant diagnostic and management challenges. We present the case of a 34-year-old woman at 22 weeks' gestation following in vitro fertilization (IVF) who developed acute bilateral leg pain, lower limb weakness, and urinary retention following a three-day history

Subglottic stenosis (SGS) is a narrowing of the airway that can be congenital or acquired, often resulting from prior intubation, infection, or autoimmune diseases. When SGS occurs during pregnancy, it presents unique challenges for anesthetic management. Symptoms commonly mimic those caused by pregnancy-related airway changes, making diagnosis and treatment more challenging. Additionally, ensuring both maternal and fetal safety during anesthesia is critical and requires a multidisciplinary approach. We report a case of a 26-year-old pregnant woman at 12 weeks' gestation who presented with se

BACKGROUND: Fetal autoimmune congenital heart block is a rare but life-threatening condition that is difficult to predict. This study sought to identify a serological biomarker predictive of autoimmune congenital heart block in pregnancies at risk due to maternal systemic lupus erythematosus, Sjögren's disease, undifferentiated connective tissue disease, or a history of congenital heart block offspring.
METHODS: This case-control study analysed maternal blood samples from pregnancies affected and unaffected by autoimmune congenital heart block from two centres (The Hospital for Sick Children,

Small bowel obstruction (SBO) is a rare yet life-threatening event in pregnancy. If not diagnosed and treated promptly, it can lead to an increase in maternal as well as foetal morbidity and mortality. In the majority of cases, small bowel obstruction is caused by adhesions from previous pelvic surgeries. We herein report a case of SBO in a 28-week pregnant woman, who had a previous history of laparotomy for ectopic pregnancy. She presented with severe epigastric pain and vomiting in the emergency department. On initial examination, the patient was clinically stable but with a distended abdome

Pre-eclampsia (PE) and peripartum cardiomyopathy (PPCM) are two types of complications that can occur in pregnancy and the peripartum period. We present the successful management of a pre-eclamptic patient with severe, undifferentiated pulmonary oedema in pregnancy. Our patient presented at 35 + 5 weeks of gestational age with hypertension, delirium and severe hypoxaemia. She developed pulseless electrical activity after induction of general anaesthesia for a category one caesarean section. Resuscitation commenced, and a peri-mortem caesarean section was performed. Moyamoya disease (MMD) is a rare cerebrovascular disorder characterized by the constriction or obstruction of the internal carotid and anterior cerebral arteries bilaterally, leading to either ischemic or hemorrhagic events. A 28-year-old Caucasian female with a history of bilateral MMD was admitted for a scheduled cesarean delivery at 38 weeks of pregnancy. The management around the surgery involved stopping aspirin, closely monitoring hemodynamics, and administering general anesthesia. A healthy male baby was delivered via cesarean section, and both the mother and infant were discharged on t

BACKGROUND: Peri-operative hypotension is strongly associated with organ injury following noncardiac surgery, however hypotension avoidance trials have not shown meaningful improvements in cardiovascular outcomes and only inconsistent improvements in renal and neurological outcomes. The true haemodynamic drivers of peri-operative organ injury are probably falls in BP below individual autoregulatory boundaries and not below population-based harm thresholds. Novel methods of personalising peri-operative blood pressure (BP) management are needed. Cerebral autoregulation (cAR)-guided precision BP

Uterine torsion in mares belongs to maternal pregnancy disorders, accounting for 5-10% of complications in the last trimester of pregnancy. Two surgical techniques for repositioning uterine torsion are used: flank laparotomy in local anesthesia on a standing mare (SFL) and midline laparotomy carried out under general anesthesia (MI). The study aims to present the exact protocol used by the authors to qualify a mare with uterine torsion for surgery using one of the above-mentioned methods.A total of 19 mares were operated on, of which 13 underwent midline laparotomy under general anesthesia, an

The authors report a case of fatal postpartum acute subdural hematoma after multiple attempts at spinal anesthesia related to cesarean delivery. A 22-year-old primigravida was admitted to the Obstetrics and Gynecology Department due to the throes of childbirth. The presence of threatening intrauterine hypoxia indicated a C-section with the usual lumbar anesthesia. However, the anesthesia proved insufficient, necessitating the administration of intratracheal narcosis, which occurred without complication. A healthy, mature boy was born with Apgar scores of 10/1 and 10/5. During the first 8 h of

Managing pregnant patients with severe mitral stenosis (MS) and pulmonary hypertension (PH) remains a challenge, requiring a multidisciplinary approach to mitigate maternal and fetal risks. We report a case of a 29-year-old female, gravida 6 para 5, at 37 weeks' gestation with severe rheumatic MS, significant PH, and right ventricular (RV) dysfunction who underwent an elective repeat cesarean delivery. Due to her elevated risk of hemodynamic collapse, invasive monitoring, incremental epidural anesthesia, and standby extracorporeal membrane oxygenation (ECMO) were utilized. Intraoperative vasop

Heart failure is a major contributor to maternal morbidity and mortality. Pregnancy is a state of hemodynamic stress, and normal physiologic changes of pregnancy may mimic signs of heart failure. Prepregnancy counseling, multidisciplinary care, and referral to a center with expertise in managing pregnant patients with heart failure can help optimize outcomes. The option for abortion care should be available to all patients with heart failure, regardless of the severity of the disease, and is an essential component of individualized counseling. In this Consult, we provide guidance for managing

May-Hegglin anomaly (MHA) is an autosomal dominant haematological disorder that is commonly misdiagnosed as idiopathic thrombocytopenic purpura. The presence of leukocyte inclusion bodies differentiates it from other causes of thrombocytopenia. There is a lack of agreement on the perioperative management of patients with MHA. Our patient was a 33-year-old lady with underlying MHA, gravida 2 para 1 at 37 weeks of pregnancy, who was electively admitted for lower-segment caesarean section under general anaesthesia. The caesarean delivery was uneventful, and the mother and baby were discharged wit

BACKGROUND: Primary immunodeficiencies (PIDs) encompass a large group of inherited diseases affecting the immune system. PID management is improving, enabling more patients to carry a pregnancy to term. Anesthetic care of those patients, especially obstetric neuraxial anesthesia and the associated infectious complications, has never been evaluated in this population.
OBJECTIVE: This retrospective multicenter study aimed to assess the anesthetic management of women with PIDs during childbirth, focusing on potential infectious complications related to neuraxial anesthesia.
METHODS: The medical

INTRODUCTION: Over-the-counter (OTC) nonsteroidal anti-inflammatory drugs (NSAIDs, i.e., aspirin, diclofenac, dipyrone, ibuprofen, naproxen) and paracetamol are commonly ingested medications during pregnancy that for a long time were classified as relatively safe for mothers and fetuses.
AREAS COVERED: The review focuses on developmental toxicity of OTC NSAIDs.
EXPERT OPINION: Same epidemiological studies show that OTC antipyretics and painkillers may increase risk of spontaneous abortion, intrauterine growth retardation, preterm constriction of ductus arteriosus with secondary persistent pu

We report a case of a woman in her mid-30s who developed severe cardiac shock hours after giving birth to her second child with the need for extracorporeal haemodynamic support. Initially, postpartum cardiomyopathy was suspected, and high-urgency heart transplantation was considered. However, the endocrine work-up and imaging revealed pheochromocytoma as the cause for acute heart failure that was completely reversible. Notably, the patient also developed Sheehan's syndrome with pituitary necrosis and sustained hypopituitarism, most likely as a consequence of the haemodynamic failure during phe

Aortic stenosis is a condition with multiple etiologies that can remain clinically silent for years. The enhanced cardiovascular demands of pregnancy can unmask a previously undetected disease or worsen existing symptoms. Managing anesthesia in pregnant patients with cardiac valvular disease is a complex task that requires a coordinated, multidisciplinary approach. This case report presents a 28-year-old pregnant Hispanic patient with known severe aortic stenosis who was admitted to our hospital at 30 weeks of gestation for multidisciplinary management and delivery planning. Anesthesia management for a pregnant patient with Guillain-Barré syndrome (GBS) requires careful planning to ensure the safety of both the mother and fetus while addressing its specific physiological challenges. Management should involve a coordinated team of anesthesiologists, obstetricians, neurologists, and intensivists to optimize outcomes for the mother and fetus. There appear to be no specific case reports that directly document the use of supraglottic airway devices (SGADs) in managing patients with GBS. GBS is a rare, acute autoimmune disorder in which the body's immune system attacks

INTRODUCTION: Limited knowledge about preventing mother-to-child transmission of HIV contributes to the high prevalence of pediatric HIV caused by vertical transmission. Given the lack of sufficient information, this study aimed to identify hotspot and cold spot areas as well as the factors influencing reproductive-age women's knowledge about preventing mother-to-child transmission of HIV in Ethiopia.
METHOD: The study utilized data from the 2016 Ethiopian Demographic and Health Survey, comprising a weighted sample of 15,683 reproductive-age women. Spatial analysis was conducted to identify s
