Heart-shaped tension-reducing suturing versus conventional interrupted suturing alleviates perineal pain, accelerates resolution of suture reactions, and improves scar quality in primiparous women undergoing perineal repair.

OBJECTIVE: To evaluate the efficacy of the heart-shaped tension-reducing suture (HSTRS) versus interrupted suture (IS) in scar quality and postoperative pain relief in primiparous women. METHODS: In this study, 142 women were allocated to HSTRS (n = 61) or IS (n = 81). Pain score was assessed via NRS at multiple timepoints; suture reactions were graded (0-III), and scar quality was evaluated using VSS, OSAS, and PSAS at 42 days, 90 days, and 180 days. RESULTS: The HSTRS group showed significantly lower pain scores (P < 0.0001), faster resolution of suture reactions, and superior scar outcome

The Evolving Paradigm of Autism: Folate Receptor Autoimmunity deranging Epigenetic Programming in fetal and postnatal Brain Development.

This review on autism provides a summary of the multiple genetic, epigenetic and environmental changes setting the stage for disturbed brain development due to aberrant synaptogenesis and corticogenesis during the prenatal and post-natal periods. The emphasis is placed on nonmendelian transmission driven by epigenetics. Folate deficiency appears to play a critical role in influencing many of the structural changes contributing to developmental deficits. Impaired passage of folate across the placental and the blood-brain barriers to the fetal and postnatal brain because of folate receptor autoa

Changes in birth certificate-reported prevalence of congenital anomalies at live birth during the COVID-19 pandemic: a nationwide study of 26 million United States live births.

BACKGROUND: Although the majority of studies have indicated that maternal severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection and coronavirus disease 2019 (COVID-19) vaccination are not associated with neonatal birth defects, it is relevant to examine whether the disease burden changed during the COVID-19 pandemic, as other influencing factors may have also changed. We aimed to compare the birth certificate-reported prevalence of congenital anomalies at live birth before and during the pandemic. METHODS: Using public birth data, all live births from 2016 to 2022 in the Unit

Case Report: Severe persistent transaminitis during pregnancy: a complex differential diagnosis involving suspected toxoplasmosis, possible drug-induced liver injury, and intrahepatic cholestasis.

INTRODUCTION: Severe transaminitis during pregnancy represents a diagnostic challenge because multiple etiologies may coexist. We report a pregnant woman with suspected toxoplasmosis, intrahepatic cholestasis of pregnancy, and possible drug-induced liver injury who developed prolonged severe liver dysfunction despite extensive investigation and treatment. CASE PRESENTATION: A 33-year-old primigravida was referred at 15 weeks' gestation because of positive Toxoplasma gondii serology and elevated liver enzymes (ALT/AST 460/150 U/L). Despite treatment with spiramycin, later changed to pyrimetham

Ultrasound-Indicated Versus Physical Examination-Indicated Cerclage in Twin Pregnancies Between 24 and 27 + 6 Weeks: An Exploratory Observational Analysis of Pregnancy and Neonatal Outcomes.

BACKGROUND: Recent advancements in assisted reproductive technology and delayed parenthood have led to a surge in twin pregnancies, with a high incidence of preterm birth (PTB) due to cervical insufficiency. Current guidelines recommend cervical cerclage for twin pregnancies before 24 weeks; however, data on cerclage efficacy beyond 24 weeks is scarce. OBJECTIVE: This exploratory, hypothesis-generating study is aimed at examining the associations of cervical cerclage on pregnancy duration and neonatal outcomes in twin pregnancies with a short cervix between 24 and 27 + 6 weeks. METHODS: This

Management of pregnancy in patients with Graves' disease secondary to alemtuzumab: a case series and literature review.

INTRODUCTION: Alemtuzumab treatment for multiple sclerosis frequently induces Graves' disease (GD), characterized by common coexistence of stimulating and blocking anti-thyrotropin receptor autoantibodies (TRAb), causing fluctuations of thyroid function. Alemtuzumab-induced TRAb-positive hypothyroidism is also common. These conditions are challenging to manage during pregnancy, considering the imperative necessity to maintain euthyroidism to minimize risks for both pregnancy and fetal/neonatal outcomes. METHODS: We reviewed the clinical course and therapeutic management of four pregnant women

A global, descriptive analysis of the sources used to regulate abortion care.

An enabling environment is essential for delivering quality, comprehensive abortion care. According to the WHO, abortion should be treated like other healthcare services, regulated by healthcare law, and guided by evidence-based standards and guidelines. This paper examines abortion regulations across 183 countries, drawing on data from the Global Abortion Policies Database, up to May 2025. It divides sources relevant to the regulation of abortion care into seven categories: Criminal/Penal Code, Abortion-specific Law, Health Regulation/Clinical Guidelines, Constitution, Case Law, Essential Med

Predictors of immediate mother-newborn skin-to-skin contact and missed contact in health facilities in Malawi: a multilevel analysis of the Malawi Demographic and Health Survey-2024.

AIM: This study aimed to examine the prevalence and predictors of immediate mother-newborn skin-to-skin contact (SSC) in Malawi and factors associated with missed SSC despite being delivered in health facilities. BACKGROUND: SSC immediately after birth is an evidence-based essential newborn care intervention associated with improved thermoregulation, early breastfeeding initiation, maternal-infant bonding, and neonatal survival. Although facility delivery is expected to promote SSC, a substantial proportion of newborns in low- and middle-income countries still do not receive this recommended

Unbound Intrinsic Formation Clearance of 6β-Hydroxycortisol Is a Useful Biomarker of CYP3A4 Activity during Pregnancy.

Pregnancy results in multiple physiological changes that impact drug disposition. For example, hepatic CYP3A activity and renal transporter-mediated clearance increase in late pregnancy, but the time course of these changes across pregnancy is not known. Endogenous biomarkers offer a minimally invasive approach to characterize pharmacokinetic changes during pregnancy and postpartum. The goal of this study was to determine whether 6β-OH-cortisol formation clearance (CLf,OHCOL) can be used as a biomarker of hepatic CYP3A activity during pregnancy. Thus, we measured cortisol, cortisone, and 6β-OH

Prenatal per- and polyfluoroalkyl substance levels and childhood rubella incidence up to age 6 years: the Japan Environment and Children's Study.

Per- and polyfluoroalkyl substances (PFAS) have raised concerns about their potential immunosuppressive effects. However, the association between prenatal PFAS exposure and childhood rubella remains unclear. This study examined whether prenatal PFAS exposure is associated with childhood rubella incidence. We analyzed data from the Japan Environment and Children's Study, including children whose mothers had plasma PFAS concentrations measured during pregnancy. The primary outcome was caregiver-reported physician-diagnosed rubella up to 6 years of age. Seven PFAS with detection frequencies >60%

Limb Body Wall Complex: Report of a Discordant Case in a Monozygotic Twin Pregnancy.

Limb Body Wall Complex (LBWC) is a rare, severe congenital malformation syndrome characterized by thoracoabdominal wall defects, craniofacial anomalies, and limb and spinal malformations. Its incidence ranges from 0.04 to 3.3 per 10,000 births. We present a case of LBWC diagnosed in a monochorionic diamniotic twin pregnancy at 17 weeks of gestation. The mother was 28-year-old, primigravida, and had no notable medical history. Ultrasound identified abnormal positioning of 1 embryo, suggestive of adherence to the amnion. First-trimester scans revealed increased nuchal translucency and foot malpo

The hormone-microbiota-NET framework in gynecologic diseases: evidence, controversies, and translational opportunities.

Gynecologic diseases, including infectious disorders, endocrine-related conditions, gynecologic malignancies, and pregnancy complications, are clinically diverse yet frequently share features of hormonal imbalance, microbial dysbiosis, and dysregulated inflammation. Current research has largely examined these processes separately, limiting integrated understanding of their potential interactions. This review synthesizes current evidence and proposes a hormone-microbiota-neutrophil extracellular trap (NET) framework as a conceptual tool for understanding common and disease-specific mechanisms a

Beyond metabolic comorbidity: metabolic health stratification reveals BMI-ART associations in tubal factor infertility.

BACKGROUND: The association between body mass index (BMI) and assisted reproductive technology (ART) outcomes remains inconsistent, potentially due to confounding by metabolic comorbidities and underlying infertility etiologies. This study introduced metabolic health stratification to reproductive medicine to examine whether the impact of BMI on ART outcomes differs by metabolic status, independent of ovarian reserve impairment or reproductive endocrinopathies. METHODS: This retrospective cohort study included 3,770 women with tubal factor infertility and normal ovarian reserve who underwent

Dietary Supplementation with Omega-3 Fatty Acids Rescues Neurodevelopmental Deficits in a Combined Exposure Paradigm of Maternal Oxycodone Exposure and Chronic Stress.

In recent years, opioid abuse has become a major public health crisis affecting millions of individuals throughout the world. This widespread abuse of prescription opioids and dramatic increase in their availability have created the opioid epidemic. Further, oxycodone (oxy), which is prescribed for multiple types of pain, is gaining momentum as a pain reliever in pregnant women. Whilst successful in inducing analgesia, oxy freely passes through the placenta and can impact the overall development of the fetus in major ways. Currently, mechanisms that lead to impairment in the exposed offspring,
