The yeast flora of vagina and associated sites (cervix, urethra, perineum and rectum) in 224 pregnant and 106 non-pregnant Nigerian women was investigated. A variety of yeasts represented by 12 species were isolated. Candida albicans was the most frequently isolated species in both groups, accounting for 63.6% of the isolates from pregnant women and 44.8% of the isolates from non-pregnant women. Candida lusitaniae, a fungus with a low incidence of infection in immunocompetent people, is emerging as an opportunistic pathogen in immunocompromised hosts. This yeast is generally resistant to amphotericin B and may present therapeutic difficulties. C. lusitaniae may be misidentified as one of several other fungal species, including Candida tropicalis, Candida parapsilosis, and even Saccharomyces cerevisiae. As judged by in vitro antifungal susceptibility testing, minimal inhibitory concentrations of antifungal agents other than amphotericin B are achievable, but fungicidal levels are not.

The oropharynx and hypopharynx are common sites of involvement in AIDS patients with mucocutaneous Kaposi's sarcoma. The radiotherapist is often asked to intervene with these patients due to problems with pain, difficulty in swallowing, or impending airway obstruction. We have noted an unexpected decrease in normal tissue tolerance of the oropharyngeal mucosa to irradiation in AIDS patients treated in our department. In a prospective study of 29 hospital inpatients with candiduria, yeasts were frequently isolated from the urine in the absence of symptoms and no serious deep fungal infection occurred. The prevalence of diabetes mellitus and urinary catheterisation was significantly increased in this group compared to the general hospital population. There was a high incidence of yeast species other than Candida albicans in the urine, non-albicans species constituting 39% compared with 18% from other sites (p less than 0.01).

Sixty-five patients with oral mucous membrane pemphigoid were examined, treated, and followed for a mean of 3.5 years. Seventy-two percent were women, and the mean age was 59 years. In most patients multiple oral sites were involved and the gingiva was affected in 94% of the patients. The opportunistic yeastlike fungi of the genus Candida comprise three species which are proteolytic in vitro. Among them, C. albicans and C. tropicalis are of foremost medical importance. Semi-quantitative sampling of 37 taped sites and 37 matching control sites in 30 babies in a special care baby unit showed a tendency for bacterial growth under the cardiac monitors or the occlusive plastic adhesive tape to be greater than at the control site. This tendency was demonstrable even when potential pathogens were considered. During the study, an outbreak due to Candida albicans occurred. Antibiotic resistance of coagulase-negative staphylococci was marked and this, and other evidence, suggested that part of the skin flora was acquired from the unit.

We studied etiologic factors important in colonization of 179 central venous catheters (CVCs) in patients randomized into group 1 (who received daily topical applications of povidone-iodine) or group 2 (who received only dry dressing changes). The potential clinical value of colony counts determined by the lysis-centrifugation blood culture method was studied by reviewing the records of eight patients with persistent septicemia in whom colony counts were available on at least 3 days.
