Discordance for Defects in Monochorionic Twins: Prevalence and Impact on Perinatal Outcomes.

Background: Monozygotic twin pregnancies are at increased risk of congenital abnormalities compared to singletons. In 20% of cases, both fetuses are affected (concordance), while in 80% of cases, only one fetus is affected (discordance). This study examines the prevalence of discordance for structural defects in monochorionic (MC) twins, with normal aCGH comparative genomic hybridization (aCGH), reporting the types of detected abnormalities and their possible impact on perinatal outcomes, including the rate of single and double fetal loss before 24 weeks' gestation and the rate of preterm birt

Conservative management leading to delivery of a twin pregnancy complicated by previable preterm premature rupture of membranes: a case report.

BACKGROUND: The threshold of neonatal viability is established at a gestational age of 22-23 weeks. Previable preterm premature rupture of membranes (pPPROM) occurring prior to this period is frequently associated with recommendations for pregnancy termination due to exceedingly high rates of neonatal mortality and maternal infectious morbidity. The objective of this study is to provide a therapeutic reference for patients who experience premature rupture of membranes before 22 weeks of gestation and opt to continue their pregnancy. CASE DESCRIPTION: This report describes the case of a 35-yea

Pregnancy-related factors and breast cancer subtypes and prognosis: A systematic review and meta-analysis.

Growing evidence suggests that pregnancy-related factors may influence long-term diseases, yet their role in breast cancer subtypes and prognosis remains uncertain. We evaluate current evidence on the association between pregnancy-related factors, including parental age, gestational age, breastfeeding, twin pregnancy, preeclampsia, and parental smoking, and breast cancer subtypes, as well as prognosis in daughters. We conducted a systematic review and meta-analysis following PRISMA 2020 guidelines (PROSPERO: CRD42025619266). Searches were performed in MEDLINE (via PubMed), Web of Science, and

Clinico-Pathological Discordance in a Partial Hydatidiform Mole With Coexisting Live Fetus and Subsequent Aggressive Gestational Trophoblastic Neoplasia: A Case Report.

Twin pregnancies with a partial hydatidiform mole and a coexisting live fetus are exceptionally rare, and progression to chemoresistant gestational trophoblastic neoplasia (GTN) is even less common. We report the case of a 28-year-old nulligravid woman with prior ovulation induction who presented with a twin pregnancy consisting of a partial hydatidiform mole and a coexisting live normal fetus. Following spontaneous abortion and uterine evacuation, histopathological examination and immunohistochemistry demonstrated p57 positivity, supporting the diagnosis of partial hydatidiform mole, and a no

[Impact of pre-pregnancy body mass index on maternal and infant outcomes in twin pregnancies].

METHODS: A retrospective analysis included 4 824 twin pairs and their mothers with complete records, delivered at 21 tertiary grade-A hospitals nationwide from January 2018 to December 2020. Participants were grouped by prepregnancy BMI: underweight (<18.5 kg/m²; n=566), normal weight (18.5 to <24.0 kg/m²; n=3 120), overweight (24.0 to <28.0 kg/m²; n=892), and obese (≥28.0 kg/m²; n=246). Differences and associations in adverse maternal and infant outcomes were compared among groups. RESULTS: The gestational age in the overweight group was lower than in the normal and underweight groups, with

Pharmacokinetics of Certolizumab Pegol in Pregnancy: Results from the Open-Label CHERISH Study.

INTRODUCTION: Control of chronic inflammatory diseases (CIDs) during pregnancy is essential for maternal and foetal health; however, the impact of pregnancy on the pharmacokinetics (PK) of CID therapies is unknown. This study investigated the impact of pregnancy on the PK of certolizumab pegol (CZP), a tumour necrosis factor inhibitor (TNFi), in women with CIDs. METHODS: CHERISH (NCT04163016) was a multicentre, longitudinal, open-label phase 1B study evaluating the impact of pregnancy on the PK of CZP in women with CIDs. Pregnant participants on a stable CZP regimen were enrolled at ≤ 10 week

Association between breech presentation in twins and developmental dysplasia of the hip.

BACKGROUND: Breech presentation is considered one of the main risk factors for developmental dysplasia of the hip (DDH) in singleton pregnancies; however, data regarding twins are limited. The aim of this study was to evaluate whether breech presentation in twins is associated with abnormal hip ultrasound findings and to assess the influence of delivery mode, sex and gestational age. METHODS: Obstetric and ultrasound data from twin births at Frankfurt University Hospital between 2010 and 2014 were retrospectively analyzed. A total of 130 twins in whom at least 1 infant with breech presentatio

Omphalocele-exstrophy-imperforate anus-spinal defect syndrome in a monozygotic twin pregnancy: a case report and literature review.

BACKGROUND: The Omphalocele, Exstrophy, Imperforate Anus, and Spinal Defects Complex (OEIS complex) is a rare birth defect with a prevalence of 1 in 200,000-400000 pregnancies and 1 in 82,000-200000 live births. While cases in female monozygotic twins have been reported, the perinatal outcomes of the unaffected co-twin remain insufficiently addressed. CASE PRESENTATION: We report a spontaneously conceived monochorionic monoamniotic twin pregnancy where Twin B was diagnosed with OEIS. Prenatal ultrasound at 12 + 4 weeks identified an abdominal cystic mass in Twin B, which enlarged progressivel

Emergency double McDonald cerclage in twin gestation with single fetal death and simultaneous prolapse of both amniotic sacs.

A woman in her mid-30s at 24+5 weeks of a dichorionic diamniotic twin pregnancy with single intrauterine fetal demise presented with simultaneous prolapse of both amniotic sacs into the vagina and a 3 cm dilated cervix. She underwent ultrasound-guided amnioreduction and an emergency double McDonald cerclage using two Ethibond #2 sutures placed 1 cm apart. The pregnancy was successfully prolonged to 36+5 weeks, resulting in caesarean delivery of a healthy neonate weighing 2220 g and the demised co-twin. This case is unique, as it provides the first documented clinical and ultrasound images of s

Interval Vaginal Delivery of Term Twins From Bilateral Horns of a Uterus Didelphys: Insight Into Labor Initiation and Progression.

Uterus didelphys, characterized by two separate uterine cavities and cervices, is associated with increased obstetric complications. Twin pregnancy with one fetus in each horn is exceptionally rare, often managed by cesarean or resulting in preterm birth. We report the first known case of term vaginal deliveries from both horns, occurring 6 days apart. A 28-year-old woman with uterus didelphys spontaneously conceived twins, with one fetus in each horn. At 37 + 4 weeks of gestation, labor began spontaneously in the left uterus. The cardiotocogram initially showed asynchronous contractions betwe

The role of placental anastomoses in severe brain injury after fetal demise in monochorionic twins: a multicenter retrospective study.

BACKGROUND: Monochorionic twins share a single placenta with vascular anastomoses including arterio-arterial and veno-venous anastomoses, which are low-resistance, superficial, and bidirectional anastomoses. Following single fetal demise, the survivor is at risk of brain injury due to acute exsanguination through arterio-arterial and veno-venous anastomoses into the low-pressure circulation of the deceased co-twin. OBJECTIVE: To assess whether the number and diameter of arterio-arterial and veno-venous anastomoses are associated with severe brain injury in the survivor following single fetal

The quintessential high-risk profile: advanced management strategies for placenta accreta spectrum in patients with advanced maternal age and IVF conception.

BACKGROUND: The convergence of Advanced Maternal Age (AMA ≥ 35), In Vitro Fertilization (IVF) conception, and Placenta Accreta Spectrum (PAS) represents a clinical nexus of potentially elevated obstetric risk, hypothetically associated with a high probability of massive peripartum hemorrhage (MPH). Based on limited observational evidence, this case-based narrative review hypothesizes that this risk profile warrants protocol-driven management considerations, which are presented as hypothesis-generating rather than definitive. CASE PRESENTATION: A 54-year-old gravida 4, para 2 (2 living childre

A Rare PKHD1 Frameshift Variant Identified in a Prenatally Diagnosed ARPKD Case From a Consanguineous Jordanian Family.

INTRODUCTION: Polycystic kidney disease (PKD) is a condition that encompasses the development of multiple benign cysts in the kidneys and other organs. Hereditary PKD can be classified as autosomal dominant polycystic kidney disease (ADPKD) or autosomal recessive polycystic disease (ARPKD). These two forms differ primarily in their age of onset and clinical presentation. ADPKD is usually diagnosed during young adulthood, whereas ARPKD presents a wide range of clinical manifestations with approximately one-third of patients presenting before the first year of life, one-third between 1 and 20 ye

[In utero treatment of twin-to-twin transfusion syndrome].

Twin-to-twin transfusion syndrome (TTTS) is a serious complication of monochorionic twin pregnancies in which an imbalance in blood flow between the twins is caused by vascular anastomoses in the placenta. TTTS affects 10 to 15% of these monochorionic pregnancies and can lead to high perinatal mortality if left untreated. The Quintero classification, introduced in 1999, helps stratify the severity of TTTS into five stages, thereby facilitating clinical decision-making. Laser photocoagulation of placental anastomoses, a technique developped in the 2000s, has become the standard of care for seve

Persistent compression of uterine insertion on maternal blood dynamic change in fetoscopic laser photocoagulation surgery: a novel method.

BACKGROUND: Fetoscopic laser photocoagulation (FLP) has been performed as first-line approach for twin-to-twin transfusion syndrome (TTTS) in monochorionic twin pregnancies. The majority of researches are focusing on advances in fetal treatment and long-term outcome, few researches on maternal surgery-related complications are reported. This study is to compare the maternal blood dynamic changes in women with and without persistent compression on the uterine insertion after the FLP procedure. METHODS: This was a retrospective study conducted at two tertiary referral centers in China between N

Association between adverse obstetric history and perinatal outcomes in singleton and twin pregnancies: a retrospective study.

OBJECTIVE: This study aimed to understand how adverse obstetric history affects perinatal outcomes in singleton and twin pregnancies, and to determine whether a twin pregnancy is a risk factor for adverse outcomes among women with an adverse obstetric history. METHODS: This retrospective cohort study employed logistic regression to explore the relationship between adverse obstetric history, including spontaneous abortion (SAB), induced abortion (IA), and ectopic pregnancy (EP), and the occurrence of twin pregnancies. Multivariable analysis assessed the risk of adverse perinatal outcomes in bo
