Perinatal outcomes of monochorionic monoamniotic twin pregnancies.

OBJECTIVES: To describe the risk of early and late fetal loss of a cohort of monochorionic monoamniotic (MCMA) twin pregnancies; secondary objectives are to describe perinatal outcomes of these pregnancies and to identify which obstetric variables mostly influence the incidence of neonatal adverse outcome. METHODS: Retrospective cohort study including MCMA twin pregnancies followed up at the Twin Pregnancy Care Unit of Sant'Anna Hospital in Turin (Italy) between 2005 and 2024. Chorionicity and amnionicity were diagnosed in the first trimester. RESULTS: A total number of 53 MCMA twin pregnanc

Prenatal Diagnosis of Peters-Plus Syndrome: A Case Report.

Peters-Plus syndrome is a rare autosomal recessive disorder caused by biallelic pathogenic variants in the B3GLCT gene and characterized by multisystem involvement. Fewer than 100 cases have been reported to date, and only a limited number have been diagnosed prenatally. Prenatal identification is challenging due to the variable and non-specific nature of fetal findings and the frequent absence of detectable ocular anomalies during routine ultrasound. We report a prenatal diagnosis of Peters-Plus syndrome in a monochorionic diamniotic twin pregnancy, based on the progressive identification of

Low PAPP-A Levels and Growth in Twin Pregnancies.

BACKGROUND/OBJECTIVES: It is well established in the modern literature that newborns delivered from multiple gestations are more predisposed to low birthweight in comparison to their singleton equivalents. In this study, we sought to explore the potential of first-trimester biochemical (PAPP-A and free β-hCG) and biophysical indices (uterine artery Doppler) to predict low birthweight in one or both twins. METHODS: This is a retrospective cohort analysis of 400 twin viable pregnancies presenting for routine first-trimester assessment in four fetal medicine centers between 2014 and 2025. The ex

Vanishing twin syndrome and fetal reduction adversely affect perinatal outcomes following IVF-FET: an analysis based on 33,238 ongoing pregnancies.

PURPOSE: To assess the associations between vanishing twin (VT) and fetal reduction (FR) with obstetric and perinatal outcomes following in vitro fertilization (IVF)-frozen embryo transfer (FET). METHODS: This was a retrospective cohort study involving women who had undergone FR or experienced VT during the period from 2012 to 2022. Cohorts were stratified by timing into early (< 15 weeks) and late (≥ 15 weeks) VT/FR. Controls comprised primary singletons and non-reduced twins. The primary outcome measurements were maternal and birth complications. RESULTS: Among 33,238 ongoing pregnancies,

Discordance between chorionicity and embryo transfer number: Four cases report and literature review.

OBJECTIVE: To report four cases of in vitro fertilization-embryo transfer (IVF-ET) with discordance between chorionicity and embryo transfer number. DESIGN: A case series. MATERIALS AND RESULTS: Four cases underwent ART treatment in Kaohsiung Chang Gung Memorial Hospital between 2018 and 2020: a 34-year-old woman who underwent an IVF-frozen cycle with embryo transfer of one blastocyst and one morula, resulting in a quintuplet pregnancy. The ultrasound of the quintuplets showed four gestational sacs, one of which was a blighted ovum (one empty gestational sac), and one of the remaining three

Concordant and Discordant Transcriptomic Signatures of Twin Placentas in the Setting of Preeclampsia.

BACKGROUND: Multifetal pregnancies have increased preeclampsia risk, but the underlying pathogenesis may differ from that of singletons. It remains unclear whether twin placentas show molecular signs of preeclampsia synchronously. METHODS: We performed RNA sequencing on 32 individual placental samples from twin gestations grouped by preeclampsia status: 24 dichorionic twin (DT) and 8 monochorionic twin gestations. Ten singleton placentas from preeclamptic pregnancies were also analyzed. A benchmark data set (GSE203507, GSE114691, and GSE1482410) and a test data set (GSE190973) comprised 71 ea

An individual participant data network meta-analysis of the APOSTEL trials on the effect of tocolysis in threatened preterm birth between 30-33+6 weeks of gestation in twin pregnancies.

OBJECTIVE: Twin pregnancies have a substantial risk of preterm birth, but evidence on tocolytics is limited. We evaluated whether nifedipine or atosiban improves neonatal outcomes compared with placebo. DESIGN: Individual participant data network meta-analysis (IPD-NMA) of two international multicentre randomised controlled trials (APOSTEL 3 and 8). SETTING: The APOSTEL-3 compared nifedipine with atosiban, while the APOSTEL-8 compared atosiban with placebo. POPULATION: Women with twin pregnancies and threatened preterm birth between 30+0 and 33+6 weeks. METHODS: Analyses were performed at

Do maternal and neonatal outcomes differ with single or twin pregnancies affected by hypertensive disorders? A systematic review and meta-analysis.

OBJECTIVE: To compare maternal and neonatal outcomes of singleton and twin gestations complicated by hypertensive disorders of pregnancy (HDP). METHODS: PubMed, Cochrane Library, Scopus, Web of Science, Embase, and Google Scholar were searched for comparative studies till 15th May 2025. RESULTS: Thirteen studies were included. The analysis of crude data found a significantly elevated risk of preterm birth and cesarean section with twins compared to singleton gestations complicated by HDP. A significantly higher risk of small-for-gestational-age (SGA) was noted in twin pregnancies, but the ef

Twin-twin transfusion syndrome in a dichorionic-diamniotic gestation with post-laser amniotic band sequence: A case report.

Twin-twin transfusion syndrome most commonly develops in monochorionic-diamniotic twin pregnancies but rarely has been documented in dichorionic pregnancies. Twin-twin transfusion syndrome can often be successfully treated with fetoscopic laser surgery; sequelae are infrequent but can include chorion-amnion membrane separation, preterm rupture of membranes, preterm delivery, neurodevelopmental delay, and, rarely, amniotic band syndrome. This report concerns the case of a 30-year-old nulligravid woman who presented for routine anatomy evaluation in the setting of a dichorionic-diamniotic monozy

Stanford Type B Aortic Dissection in a Twin Pregnancy With Marfan Syndrome: A Rare Obstetric Emergency-A Case Report.

The physiologic changes of pregnancy make pregnant women with Marfan syndrome (MFS) more vulnerable to complications of aortic disease. Significant hemodynamic changes in the third trimester and postpartum may explain the increased susceptibility to aortic complications during this time frame. The rate of cardiovascular complications in MFS is 2%-6%. We presented a case of Type B aortic dissection in a pregnant patient with MFS in the third trimester and normal evaluation at the initial prenatal assessment. This case report highlights the importance of comprehensive evaluation during pregnancy

Predicting outcomes in selective fetal growth restriction of monoChOrioNic Twins: an inteRnAtional observational cohort STudy protocol (CONTRAST study).

INTRODUCTION: Selective fetal growth restriction (sFGR) is a major cause of perinatal morbidity and mortality in monochorionic diamniotic (MCDA) twin pregnancies. Current management relies on umbilical artery Doppler patterns in the smaller twin. These patterns are, however, inconsistent and do not represent a reliable severity scale, complicating clinical decision-making and parental counselling. This study aims to improve risk stratification by identifying predictors of adverse outcomes, while also evaluating the pathophysiology and multi-organ impact of sFGR in early childhood. METHODS AND
