Clinicopathological Study of Placentae of Twin Pregnancies with Emphasis on Exploration of Expression of Two Novel Markers, Annexin A5 and Apelin.

OBJECTIVE: Twin pregnancies are accompanied by various perinatal complications. The study of placentae is important for understanding the underlying pathogenesis and pathologies. Annexin A5 (ANXA5) is an anti-thrombotic agent, which is expressed by the placental trophoblasts. Apelin is a novel adipokine with a significant role in angiogenesis of the placenta. The aim of the present study was to compare the gross, histopathological findings and immunohistochemical expression of ANXA5 and apelin of twin placentae with those of matched controls. These findings were also correlated with the clinic

Interacting and joint effects of total gestational weight gain and the patterns on adverse pregnancy outcomes in twin pregnancy: a retrospective study.

BACKGROUND: The pattern of gestational weight gain (GWG) over time may independently influence pregnancy outcomes, yet GWG trajectories in twin pregnancies remain poorly characterized, especially in Chinese populations. METHODS: This retrospective cohort study included 1012 women with normal-weight twin pregnancies. Total GWG was categorized per Institute of Medicine guidelines. Longitudinal GWG trajectories were identified using latent class trajectory modeling. Logistic regression was used to assess associations of GWG categories and trajectories with adverse outcomes, including gestational

Perinatal Management and Appropriate Time of Delivery in Monochorionic Monoamniotic Twins When One Twin has Transposition of the Great Arteries. A Case Report and Systematic Review of the Literature.

Monochorionic monoamniotic (MCMA) twin pregnancies are typically delivered at 32–34 weeks of gestation to reduce the risk of intrauterine demise, but this practice may compromise outcomes for a growth-restricted or very–low-birth-weight twin requiring complex cardiac surgery. We conducted a literature review in Cochrane Central Register of Controlled Trials, EMBASE and PubMed from inception to October 26, 2025. We included reports of MCMA twin pregnancies with one fetus with TGA describing perinatal outcomes, cord complications, and cardiac surgery in very–low-birth-weight neonates. Of 388 rec

Gestational weight gain and perinatal outcomes in twin pregnancies: evidence-based insights from a Chinese population cohort.

BACKGROUND: Twin pregnancies carry perinatal risks. Gestational weight gain (GWG) is modifiable, but Institute of Medicine (IOM) twin recommendations may not suit Chinese women. METHOD: A retrospective cohort study included 3109 twin pregnancies delivered at Northwest Women's and Children's Hospital in Xi'an, China, during 2018-2022. Pre-pregnancy BMI was classified according to WHO criteria, with Chinese criteria used in sensitivity analyses. Optimal BMI-specific GWG ranges were defined as the interquartile range among low-risk pregnancies. GWG adequacy (below/within/above) was evaluated aga

Longitudinal Observation of Twin-to-Twin Transfusion Syndrome, Twin Anemia-Polycythemia Sequence, and Selective Fetal Growth Restriction with Spontaneous Attenuation and Progressive Donor Twin Brain Injury: A Case Report.

A 34-year-old woman with a monochorionic diamniotic twin pregnancy developed twin-to-twin transfusion syndrome and selective fetal growth restriction at 21 weeks, followed by the onset of twin anemia-polycythemia sequence at 23 weeks. Fetoscopic laser photocoagulation could not be performed. Therefore, the pregnancy was managed expectantly. By 24-26 weeks, amniotic fluid levels and cerebral Doppler parameters normalized, but the donor twin's cardiac function deteriorated progressively. At 26 weeks, neurosonography showed diffuse periventricular hyperechogenicity, followed by ventriculomegaly a

Maintenance tocolysis in twin pregnancies after preterm premature rupture of membranes and neonatal outcomes: a retrospective cohort study.

BACKGROUND: Evidence on maintenance tocolysis after preterm premature rupture of membranes (PPROM) in twin pregnancies is limited. This study evaluated the association between maintenance tocolysis and pregnancy prolongation and neonatal outcomes in this high-risk population. METHODS: This retrospective cohort study included twin pregnancies with PPROM between 24 and 34 weeks at Tongji Hospital from January 2012 to December 2022, comparing maintenance tocolysis with non-maintenance tocolysis (no tocolysis or short-term tocolysis). Primary outcomes were latency and gestational age (GA) at deliv

Hematologic profiles in twin vs. singleton pregnancies across gestation.

BACKGROUND: While changes in hematologic parameters are well characterized in singleton pregnancies, less is known about twin gestations. Most prior studies have focused on hemoglobin and hematocrit, with limited data on other complete blood count (CBC) values. As CBCs are routinely used in prenatal care, understanding how they differ in twin pregnancies is important for accurate interpretation and clinical decision-making. OBJECTIVE: To compare CBC parameters between twin and singleton pregnancies at three gestational time points and evaluate how these values change across gestation by pregn

Prepubic Tendon Rupture and Uterine Herniation in a Late-Term Kurdish Mare: A Case Report.

Prepubic tendon rupture (PPTR) is a rare but life-threatening complication in late-term pregnant mares. It results in abdominal wall failure, ventral oedema and a poor prognosis for both the mare and foetus. A 14-year-old multiparous Kurdish mare at approximately 310 days of gestation presented with acute ventral distension, reluctance to walk and cranial displacement of the udder. Clinical evaluation revealed tachycardia, pallor and Grade 3/4 ventral oedema. Haematology indicated leucocytosis and haemoconcentration. Transabdominal ultrasonography showed a viable foetus and severe thinning of

The association of inflammatory markers with cerclage failure in twin pregnancies.

BACKGROUND: The association of inflammatory markers, including the systemic immune-inflammation index (SII), systemic inflammation response index (SIRI), neutrophil-lymphocyte ratio (NLR), and platelet-lymphocyte ratio (PLR), with pregnancy outcomes in twin pregnancies following cervical cerclage remains unclear. This study aimed to evaluate the predictive value of these markers for spontaneous preterm birth before 28 weeks of gestation in this high-risk population. METHODS: This retrospective study included 111 twin pregnancies undergoing transvaginal cervical cerclage for cervical shortenin

Diagnostic Performance of Prenatal Ultrasound to Detect Velamentous Cord Insertion in Twin Pregnancies.

Objective: We aimed to determine the ability of prenatal ultrasound to detect velamentous cord insertion (VCI) in twin pregnancies and identify factors influencing diagnostic sensitivity. Methods: This single-center retrospective study included twins delivered between April 2018 and March 2024. We excluded monochorionic monoamniotic twins, those without chorionicity or umbilical cord insertion data, and fetuses that died in utero. Umbilical cord insertion sites assessed by second-trimester transabdominal ultrasound (16 + 0 to 21 + 6 weeks of gestation) using color Doppler imaging were classifi

PFAS Exposure Perturbs Lipid Metabolism to Promote Gestational Diabetes Mellitus and Alter Growth Trajectories via Distinct Mechanisms in Singletons and Twins.

Per- and polyfluoroalkyl substances (PFAS) are persistent environmental contaminants that cross the placenta, posing a potential threat to maternal and fetal health. However, epidemiological evidence linking PFAS exposure to gestational diabetes mellitus (GDM) remains limited, almost exclusively to singleton pregnancies, with mechanistic understanding notably lacking. In this nested case-control study, we integrated exposomic and metabolomic analyses of serum from 462 singleton and 84 twin pregnancies, quantifying 48 PFAS alongside lipid metabolites. PFAS exposure was positively associated wit

Close cooperation between the orthopaedic prosthetists and surgeons leading a girl with femur lengthening of congenital transfemoral amputation to qualify for regular military service as a foot soldier.

At delivery of a twin pregnancy, which had been complicated by suspected feto-fetal-transfusion and treated by amnioscopic laser treatment at 24-weeks' gestation, the right lower leg of 1 twin was described as sharply demarcated dry necrotic below distal femur level. Two days later, the mummified part "fell off," and the distal femoral skin parts were excised to freshly bleeding tissue and closed primarily. At 1 year of age, a simple prosthesis was fitted stabilized by a pelvic corset. At 17 months, the girl was able to stand freely with the prosthesis. At age 2.5 years, the femoral stump was

Subacute small bowel obstruction in a twin pregnancy: a diagnostic and management challenge.

Small bowel obstruction (SBO) in pregnancy is rare but serious, presenting a diagnostic challenge due to symptom overlap with common gestational conditions and hesitancy around radiologic imaging. We describe a case of progressive, subacute SBO in a 46-year-old woman with dichorionic diamniotic twins and a history of caesarean section. The case culminated in bowel perforation, emergency laparotomy, and preterm delivery. This case highlights the need for early recognition, appropriate imaging, and multidisciplinary management to minimize maternal and foetal morbidity.
