Two distinct growth patterns of preterm infants from birth to 18 months of corrected gestational age: a retrospective cohort study.

BACKGROUND: How maternal and infant risk factors are related to long-term multidimensional growth trajectories in preterm infants remains unclear. This study aimed to identify different growth trajectories in preterm infants by integrating weight, length, and head circumference measurements, and to investigate associated risk factors. METHODS: In this retrospective cohort study, 68 preterm infants [<37 weeks gestational age (GA)] were followed from birth up to 18 months of corrected GA. Growth indicators were Z scores of weight, length, and head circumference [i.e., weight-for-age (WAZ), leng

Distinct pregnancy challenges in PCOS women undergoing IVF/ICSI with singleton and twin births: a propensity-matched analysis.

BACKGROUND: Polycystic ovary syndrome (PCOS) has been associated with adverse pregnancy outcomes; however, current literature presents inconsistent findings, probably due to varying feotal numbers included in previous research. This study sought to evaluate pregnancy outcomes in women with PCOS undergoing assisted reproductive technology (ART), with a specific focus on differentiating between singleton and twin births. METHODS: This cohort study included women who underwent in vitro fertilization (IVF) or intracytoplasmic sperm injection (ICSI) and maintained pregnancies beyond 20 weeks of ge

Perinatal outcomes following single intrauterine death in monochorionic twin pregnancies complicated by twin anemia polycythemia sequence: Systematic review and meta-analysis.

INTRODUCTION: Most of the studies reporting the outcome of the surviving twin after single intra-uterine fetal death (sIUFD) focused on uncomplicated pregnancies or those affected by twin-to-twin transfusion syndrome, while there is a paucity of data for those complicated by twin anemia polycythemia sequence (TAPS). The aim of this systematic review was to report perinatal outcomes in monochorionic diamniotic (MCDA) twin pregnancies complicated by TAPS after sIUFD, according to gestational age at fetal loss and the presence of pregnancy-related comorbidities. Multidisciplinary Management of Massive Pulmonary Embolism in Twin Pregnancy Using Percutaneous Thrombectomy: Maternal-Fetal Survival.

Massive pulmonary embolism (PE) during pregnancy is a major cause of maternal mortality, with increased risk in twin gestations. Hemodynamic instability in the setting of contraindication to systemic thrombolysis poses a critical therapeutic challenge, requiring a multidisciplinary approach to ensure maternal stabilization and fetal viability. A 32-year-old woman at 29 weeks of twin pregnancy was admitted with bilateral massive PE and obstructive shock. Due to the high obstetric hemorrhagic risk associated with systemic thrombolysis, percutaneous mechanical thrombectomy was performed. The proc

Genetic Analysis of Pitt-Hopkins Syndrome Caused by a Novel Splicing Variant (c.1146+3A>T) in the TCF4 Gene.

OBJECTIVE: Pitt-Hopkins Syndrome (PTHS) is a rare genetic disorder primarily caused by TCF4 mutations and involves developmental, intellectual, and physical changes in children. Increased nuchal translucency (NT) has not been associated with TCF4 mutations or PTHS. Here we study the connection between increased NT and a c.1146+3A>T mutation in the TCF4 gene. METHODS: The genetic basis of increased NT in an early pregnancy fetus was investigated by family trio clinical exome sequencing (CES) using fetal amniotic fluid and parental peripheral blood samples. The candidate variant was validated b

Twin pregnancies and the limits of the energetics of gestation and growth hypothesis.

The "Energetics of Gestation and Growth" (EGG) hypothesis proposes that human birth timing and the associated secondary altriciality of human newborns is determined by limits in maternal metabolic capacity. According to this model, labor is triggered when the increasing fetal energy requirements exceed the expectant mother's maximum sustained metabolic scope of 2.0-2.1 × basal metabolic rate (BMR). Because twin pregnancies generally involve higher combined fetal mass than singleton pregnancies, they provide an ideal test of the EGG hypothesis. Here, we use data from one study reporting materna
