Society for Maternal-Fetal Medicine Special Statement: Updated checklists for management of monochorionic twin pregnancy.

Approximately 20% of twin pregnancies are monochorionic-either monochorionic/diamniotic (MC/DA) or monochorionic/monoamniotic (MC/MA). The management of monochorionic twin pregnancy involves several additional interventions beyond the routine management of singletons or dichorionic twins. In 2020, the Society for Maternal-Fetal Medicine (SMFM) published checklists for the management of MC/DA and MC/MA twin pregnancies. SMFM now presents updated versions of these checklists reflecting recent changes in practice recommendations. Suggestions for implementing the use of the checklists into practic

Preeclampsia in twins: How different from singletons.

Hypertensive disorders of pregnancy contribute significantly to maternal and perinatal mortality and morbidity. Twin pregnancies which already carry an increased risk to mothers and fetuses, are affected in more significant numbers, earlier gestations and more severely by preeclampsia. This narrative review assesses the current state of research on maternal haemodynamics in twin pregnancy and the specific changes in those affected by preeclampsia. We assessed current literature focusing on maternal haemodynamics, angiogenic markers and screening models in twin versus singleton pregnancies. Rel

Management of Borderline Twin-to-Twin Transfusion Syndrome With Fetoscopic Laser Therapy in a Monochorionic Diamniotic Twin Pregnancy.

Monochorionic diamniotic (MCDA) twin pregnancies are associated with a high risk of complications due to shared placental vascular anastomoses, which predispose fetuses to twin-to-twin transfusion syndrome (TTTS) and twin anemia-polycythemia sequence (TAPS). Early recognition of subtle hemodynamic alterations is essential, as borderline forms of these conditions may progress rapidly and jeopardize fetal well-being. This case report presents a 30-year-old gravida 2, para 1, with an MCDA twin pregnancy complicated by borderline TTTS and TAPS, who underwent fetoscopic laser therapy despite not me

Hypertensive disorders of pregnancy: heterogeneity in risk factors and maternal and neonatal outcomes across subtypes-a 6-year retrospective cohort study.

BACKGROUND: Hypertensive disorders of pregnancy (HDP) are major obstetric complications that seriously threaten maternal and neonatal health. Current research predominantly focuses on preeclampsia (PE). However, systematic comparisons across all four HDP subtypes-gestational hypertension (GH), preeclampsia (PE), chronic hypertension (CH), and superimposed preeclampsia (SPE) remain limited, hindering a deeper understanding of their heterogeneous risks and precise management. OBJECTIVE: This study aimed to systematically analyze the epidemiological trends, independent risk factors, and differen

Early pregnancy fasting plasma glucose and risk factors: impact on gestational diabetes mellitus and other adverse perinatal outcomes in singleton and twin pregnancies.

BACKGROUND: Gestational diabetes mellitus (GDM) poses distinct challenges across different pregnancy types, yet whether the roles of early metabolic markers and conventional risk factors vary between singleton and twin pregnancies remains unclear. This study sought to clarify the associations of early pregnancy fasting plasma glucose (FPG) and established risk factors with GDM and adverse perinatal outcomes in both pregnancy settings, with a particular focus on heterogeneity across the two groups. METHODS: This retrospective study enrolled 5,532 singleton and 175 twin pregnancies from Ningjin

Rescue Cervical Cerclage in a Twin Pregnancy With Short Cervical Length or Cervical Dilatation: A Case Report.

Preterm birth is a leading cause of neonatal morbidity and mortality worldwide, with the risk being substantially higher in multiple pregnancies. Twin gestations are associated with several maternal and foetal risk factors that contribute to spontaneous preterm birth. Although cervical cerclage is an established intervention for cervical insufficiency in singleton pregnancies, its role in preventing preterm birth in twin pregnancies remains controversial, and its use continues to be the subject of ongoing research. This case report details the management of a woman with a twin pregnancy who pr

From Patient Selection to Surveillance: Artificial Intelligence Applications in Radioiodine Therapy - A Systematic Review.

UNLABELLED: Dietary phytochemicals exhibit anticancer activity, and increasing evidence suggests that the combinations of co-occurring compounds may provide greater therapeutic efficacy than individual compounds. Eruca sativa Mill. (rocket salad) contains both erucin and kaempferol; however, their synergistic anticancer activity against non-small cell lung cancer (NSCLC) has not been systematically investigated. Network pharmacology, molecular docking, and molecular dynamics (MD) simulations were performed to identify molecular targets of erucin and kaempferol. Their synergistic anticancer act

Conservative Management of Extensive Type V Aplasia Cutis Congenita in a Term Neonate Following Fetus Papyraceus: A Case Report and Guide for Outpatient Pediatric Management.

BACKGROUND: Type V aplasia cutis congenita (ACC) associated with fetus papyraceus presents management challenges for general pediatricians. While surgical intervention is often considered for extensive defects, conservative management may be effective with appropriate patient selection and protocols. To our knowledge, this is one of the most extensive cases of Type V ACC managed conservatively in the outpatient literature, and it provides a reproducible wound care protocol where none currently exists. CASE: We report a female infant born at 37 weeks gestation with extensive Type V ACC involvi

Factors Associated with Gestational Week of Diagnosis Among Pregnancies with Prenatally Recognized Congenital Heart Disease: A Tertiary Referral-Centre Cohort Study.

Background: Prenatal diagnosis of congenital heart defects (CHD) is important for perinatal planning, yet the gestational week at which CHD is diagnosed varies considerably. In referred cohorts, diagnostic timing may be influenced by maternal, fetal, temporal, and lesion-specific factors. This study examined factors associated with the gestational week of prenatal diagnosis among pregnancies with prenatally recognized and subsequently confirmed CHD referred to a national tertiary fetal cardiology centre. Methods: This study analyzed 833 pregnancies with confirmed CHD managed between 2005 and 2

The outcomes of surrogate pregnancy using donor egg compared to surrogate's egg: a systematic review.

PURPOSE: Surrogacy is either gestational (GS) using donor egg or traditional (TS) using surrogate's own egg. GS is increasing, partly from assumptions over less difficulty in transferring the baby to intended parents. This systematic review investigates obstetric outcomes in GS vs TS and vs surrogate's own spontaneous conceptions (SC), comparing genetically unrelated embryos versus genetically related embryos. METHODS: Included were studies with historical or concurrent GS to TS or to SC comparisons, reporting any obstetric outcomes. MEDLINE, Embase, Global Health, and PsycINFO databases were

Fetomaternal hemorrhage in monochorionic diamniotic twins at 37+ weeks' gestation: A rare complication.

Fetomaternal hemorrhage refers to a clinical syndrome in which fetal blood enters the maternal circulation through the placental villous spaces, typically triggered by specific stimuli, and may lead to fetal anemia or maternal hemolysis. The occurrence of fetomaternal hemorrhage is very rare in singleton pregnancies and even rarer in multiple gestations. In monochorionic diamniotic twin pregnancies, shared placental circulation can amplify the consequences of fetomaternal hemorrhage, potentially resulting in more severe outcomes than in singleton pregnancies or in twins with double chorions an

Effect of brewers' yeast or beta-glucan derived from Saccharomyces cerevisiae on breast milk supply following preterm birth: the BLOOM randomised controlled trial.

OBJECTIVE: Breast milk is the optimal source of nutrition for preterm infants; however, low breast milk production is common following a preterm birth. This study aimed to determine if taking brewers' yeast or beta-glucan improves daily expressed breast milk volume. DESIGN: Randomised, blinded, parallel, placebo-controlled trial. SETTING: Three Australian tertiary-level neonatal units. PATIENTS: Mothers with a singleton or twin pregnancy who gave birth at <34 weeks' gestation. INTERVENTIONS: Mothers were randomised within 72 hours of birth into three parallel groups in a 1:1:1 ratio to rec

Brainstem Disconnection Syndrome in a Patient With Respiratory Failure and Failed Weaning From Oxygen: A Case Report.

BACKGROUND: Brainstem disconnection syndrome (BDS) is an exceedingly rare congenital malformation of the hindbrain characterized by disconnection of the brainstem, often presenting with severe neurological deficits and early neonatal mortality. Whereas previous cases have documented isolated neurological anomalies, this report delineates a distinctive presentation of BDS associated with a specific constellation of multisystem vascular and extracerebral skeletal malformations. CASE PRESENTATION: A 4-month-old female infant, born from a discordant twin pregnancy, exhibited persistent neonatal r

Atosiban-Associated Non-cardiogenic Pulmonary Edema in a Dichorionic Diamniotic Twin Pregnancy: A Case Report.

Non-cardiogenic pulmonary edema is a rare but potentially life-threatening complication of tocolytic therapy. While commonly associated with beta-agonists and calcium channel blockers, its occurrence with atosiban remains poorly documented. To date, only a small number of cases have been reported in the literature. We report the case of a 21-year-old primigravida with a dichorionic diamniotic twin pregnancy who developed acute hypoxemic respiratory failure during atosiban therapy for threatened preterm labor. A comprehensive diagnostic workup excluded cardiogenic, thromboembolic, and infectiou

Discrepancies in Crown-Rump Length and Abnormal Cord Insertion as Predictors of Selective Fetal Growth Restriction in Monochorionic Twin Pregnancies.

This study aims to explore the significance of discrepancies in crown-rump length (CRL) and abnormal cord insertion in predicting selective fetal growth restriction (sFGR) in monochorionic (MC) twin pregnancies. Special attention was paid to determining the potential value of these factors in early pregnancy for predicting sFGR. A retrospective analysis included 131 pregnant women diagnosed with MC twin pregnancies. CRL was measured using a GE E8 color Doppler ultrasound instrument. Groups were compared based on the discrepancy in CRL (≥6 mm difference group, n = 26; <6 mm normal group, n = 10

Navigating Uncertainty in Decision-Making and Challenges to Healthcare Access Following Diagnosis of Fetal Anomaly: Bioethics and Social Factors Impacting Patient Decision-Making.

Pregnant patients weigh many factors when deciding whether to continue a pregnancy with fetal anomaly, including their family context, access to resources, and barriers to healthcare. We explore the social factors and bioethical issues that affect decisions to continue a pregnancy with fetal anomaly, illustrated by our patient's experience of heteropagus twin pregnancy. The patient had to decide whether to undergo delivery and newborn surgery out-of-province, weighed against leaving her special needs child, and limited social supports. We analyze the relevant bioethical concepts: autonomy, jus
