Intravenous GnRH pulsatile infusion therapy (10 micrograms/pulse, 90-min interval) was conducted in an acromegalic patient from whom 2/3 of a GH-producing pituitary macroadenoma had been removed. Before infusion therapy, plasma levels of GH and PRL were 10-20 and 15-25 ng/ml, respectively, while those of LH and FSH were subnormal without intrinsic fluctuations. Ovulation was induced after 13 days of infusion which was terminated on the 23rd day of therapy. Transfer of human spermatozoa and mature oocytes to patent fallopian tubes (tubal transfer, TT) in cases of infertility provides a successful, simple alternative to in vitro fertilization-embryo transfer. The authors report their second TT series performed between October 1, 1985 and February 1, 1986. A 26-year-old woman with a triplet pregnancy was treated prophylactically with ritodrine beginning at 15 weeks' gestation. At 32 weeks, she was admitted in preterm labor, and over the next 12 days received high-dose oral or intravenous ritodrine. Three female infants were delivered by cesarean section after spontaneous rupture of the membranes. Postoperatively, she developed profound hypoglycemia with inappropriately high insulin levels. Maternal hypoglycemia after ritodrine therapy in pregnancy has not been reported previously. We discuss possible mechanisms.

Serum Alpha-Fetoprotein (AFP) was found to rise to exceedingly high levels in a case of triplet pregnancy after two fetuses died in the 21st week of gestation. The surviving infant was born in the 36th week accompanied by the two fetus papyracei. By the time the process of mummification of the two dead fetuses appeared to be complete on ultrasound, the maternal AFP level had returned to the normal range for singleton pregnancies. HCG, hPL, and estriol and the coagulation profile remained within the normal range throughout the pregnancy.

The spermatozoa of infertile men, previously found to have poor fertilizing capacity in an in vitro system, were fractionated on discontinuous gradients of Percoll (Pharmacia Laboratories, Piscataway, NJ) and were used for insemination of human oocytes in an in vitro fertilization and embryo replacement program. Three of the five patients in the study had established ongoing pregnancies, including one patient with a triplet pregnancy from four transferred embryos. However, the other two patients continued to have poor fertilizing capacity with their husband's Percoll-separated spermatozoa.

In a patient with polycystic ovarian disease, low-dose intravenous pulsatile gonadotropin-releasing hormone (5 mcg every hour) and no exogenous human chorionic gonadotropin induced multiple follicular development and elevated estrogen levels and resulted in a triplet pregnancy. Patients with polycystic ovarian disease may have a higher risk of complications and should be monitored more closely during ovulation induction with pulsatile gonadotropin-releasing hormone.

The chance that one or more fetuses will suffer intrauterine death in such a pregnancy is one in six, yet the mechanism of those fetal deaths is poorly understood. Commonly occurring maternal complications included premature delivery (75%), antepartum anemia (35%), postpartum hemorrhage (35%), preeclampsia (20%), and premature spontaneous rupture of the membranes (20%). A 38-year-old woman, gravida 5, para 3, with a triplet pregnancy discordant for anencephaly, is reported. The diagnosis was made prenatally by ultrasound. Autopsy of the anencephalic member revealed small adrenal glands with absent fetal cortex, and a coincidental tetralogy of Fallot. Studies of the histocompatible lymphocyte antigens of both parents were performed and they failed to demonstrate HLA B-27. The literature on anencephaly in triplets is summarized. The association of congenital heart disease and HLA B-27 with anencephaly is discussed.
