Whole systems traditional Chinese medicine utilization among women undergoing frozen embryo transfers at an academic fertility center.

OBJECTIVE: To investigate sociodemographic and clinical variables associated with Whole Systems Traditional Chinese Medicine (WS-TCM) utilization alongside frozen embryo transfers (FETs) after in vitro fertilization. DESIGN: We conducted a retrospective review of all autologous FETs performed within an academic medical center's fertility clinic between January 2019 and March 2024. One FET cycle per patient was randomly selected for analysis. After conducting bivariate analyses, a logistic regression model was employed to examine independent associations with WS-TCM utilization. SUBJECTS: Wom

Is Pregnancy Protective from Rebound After Fingolimod Cessation?

Multiple sclerosis (MS) is a demyelinating neuroimmunological disease that poses significant reproductive challenges for affected women during pregnancy, particularly in relation to disease-modifying therapies (DMTs). Clinical evidence suggests that while pregnancy may exert a protective effect against relapses, severe rebound disease activity can occur following the discontinuation of certain medications such as fingolimod. This case report describes a pregnant woman, age 46 years, with known MS who experienced a rebound phenomenon after abruptly stopping her DMT. She was managed acutely with

Evaluating the association between household water, sanitation and hygiene (WASH) and selected placenta-related complications in The Gambia, Kenya and Mozambique.

Placenta-related complications contribute substantially to maternal and perinatal mortality globally, with the greatest burden in low- and middle-income countries (LMICs). However, the potential influence of inadequate household water, sanitation and hygiene (WASH) on these outcomes remains poorly characterised. We investigated associations between household WASH conditions and selected placenta-related complications, including pre-eclampsia, small-for-gestational-age (SGA) and stillbirth, in The Gambia, Kenya and Mozambique. This secondary analysis was nested within PRECISE (PREgnancy Care In

Elevated glucose levels trigger trophoblast cell invasion dysfunction through enhanced senescence and IGFBP4 signaling pathway.

Gestational diabetes mellitus (GDM) is related to multiple negative pregnancy outcomes such as miscarriage, pre-eclampsia and so on. Cell senescence was considered potential mechanism of GDM. However, the role and mechanism of high glucose and cellular senescence in trophoblast cells are largely unknown. Here, we observed cell senescence and expression of glucose transporter 3 (GLUT3) were up-regulated in GDM placenta. In vitro study confirmed the migration and invasion abilities were impaired in H2O2-induced senescent choriocarcinoma JEG3 cells. Additionally, high glucose triggered senescent

Effects of prenatal early food and micronutrient supplementations on body size and cardiometabolic risk factors in the 15-year-old offspring - the MINIMat randomised trial.

BACKGROUND: Evidence on the effects of maternal nutrition during pregnancy on offspring cardiometabolic risk is mixed. We hypothesized that 15-year-olds, whose mothers were randomised to maternal early pregnancy protein-energy supplementation, are taller and have lower low-density lipoprotein cholesterol than controls. We also assessed effects of prenatal multiple micronutrient supplementation (MMS) on adolescent body size and cardiometabolic risk factors. METHODS: In MINIMat trial (Maternal and Infant Nutrition Interventions in Matlab; ISRCTN16581394) in rural Bangladesh, pregnant women were

Trimester-specific associations of maternal vitamin D metabolic markers with risks of small vulnerable newborns: a prospective birth cohort study.

BACKGROUND: Vitamin D metabolites beyond total 25(OH)D may influence fetal development, yet their trimester-specific associations with small vulnerable newborns (SVN) remain underexplored. This study examined longitudinal associations of multiple vitamin D metabolic markers with SVN-related outcomes and identified key determinants during pregnancy. METHODS: In this prospective cohort of 2,505 mother-infant pairs from China, serum levels of five vitamin D metabolites 25(OH)D2, 25(OH)D3, total 25(OH)D, 3-epi-25(OH)D3, and 24,25(OH)2D3] were quantified using HPLC-MS/MS. IgG N-glycosylation in autoimmune diseases.

IgG is the most abundant antibody in the human circulation. It carries a single conserved glycosylation site, Asparagine 297, in its Fc domain. This N-glycan is critical for the ability of the antibody to activate immune cells via binding to, and cross-linking Fc receptors in order to elicit functional responses. First documented in the context of rheumatoid arthritis (RA), the IgG N-glycan may undergo changes that are correlated to host health. We present a narrative in-depth comparison of changes incurred by IgG N-glycans in autoimmune diseases; RA, systemic lupus erythematosus, multiple scl

Effective combined immunotherapy for severe hemolytic disease of the fetus.

BACKGROUND: Maternal anti-Kell and anti-Rhesus D-alloimmunization cause severe hemolytic disease of the fetus and newborn (HDF) by crossing the placenta, leading to anemia and hydrops fetalis. Early-onset HDF from anti-K and anti-RhD alloimmunization often requires multiple, early intrauterine transfusions (IUTs), with higher complication rates and decreased sensitivity after the first intervention. Mortality remains high. CASE REPORT: We report the successful treatment of three pregnant women with high-titer red cell alloantibodies-two with anti-K and one with anti-RhD-during the first trime

Adolescent Pregnancy and Perinatal Outcomes: A Hospital-Based Cross-Sectional Study.

BACKGROUND: Adolescent pregnancy is linked to adverse perinatal outcomes, yet distinctions between early (10-14 years) and late (15-19 years) adolescence and the impact of prenatal care adequacy remain unclear. We compared perinatal outcomes across maternal-age groups and explored the contribution of prenatal-care inadequacy to the association between maternal age and preterm birth. METHODS: We conducted a hospital-based cross-sectional study using SINASC data from a tertiary referral centre in Brazil. Live births to mothers aged 10-24 years from 2015 to 2020 were classified as early adolesce

Longitudinal metabolic and progesterone dynamics in estrus-synchronized ewes: influence of pregnancy outcome and litter size.

Metabolic status plays a critical role in reproductive efficiency in small ruminants; however, longitudinal information describing metabolic adaptations from estrus synchronization through gestation and the immediate postpartum period remains limited. This study investigated temporal changes in selected serum metabolic and hormonal parameters in estrus-synchronized ewes and compared pregnant (PR) and non-pregnant (NPR) animals, with particular emphasis on the influence of litter size. A total of 235 ewes were synchronized during the breeding season. Of these, 150 animals (75 PR and 75 NPR) wer

Non-Invasive Prenatal Diagnosis of Haemoglobinopathies: From Concept to Clinical Reality.

Haemoglobinopathies are the most prevalent monogenic disorders worldwide, contributing significantly to childhood morbidity and mortality. Preventive strategies, including carrier screening and prenatal diagnosis, have successfully reduced disease incidence in regions with established programmes. However, definitive prenatal diagnosis currently relies on invasive procedures, such as chorionic villus sampling and amniocentesis, which carry a small but significant risk of pregnancy loss. Non-invasive prenatal diagnosis (NIPD) offers a potential alternative that enables earlier diagnosis and elim

Triple Vector-Borne Co-infection During Pregnancy in the United Arab Emirates: A Case Report at 21 Weeks' Gestation.

Pregnancy is a unique physiological state characterised by altered immune responses that may predispose women to severe manifestations of infection. Vector-borne diseases such as malaria and dengue pose significant risks to pregnant women and their foetuses, including miscarriage, preterm labour, placental abruption, and intrauterine growth restriction (IUGR). Co-infection with multiple vector-borne pathogens is extremely rare but can exacerbate maternal morbidity and complicate foetal outcomes. This case report aims to describe the clinical presentation, diagnosis, management, and maternal an

Balancing Prolificacy and Lamb Meat Production: Links Among Litter Size, Prenatal Development, Carcass Traits, and Meat Quality in Sheep.

Prolificacy can increase lamb output per ewe, but its productive value depends on the survival, growth, and carcass performance of each additional lamb. This review examines the pathway from genetic control of ovulation to realized litter size, prenatal development, neonatal viability, postnatal growth, carcass traits, and meat quality. Major fecundity genes, including BMPR1B, BMP15, GDF9, and B4GALNT2, alter ovulation rate, although evidence for direct effects of these loci on muscle development or meat quality remains scarce. Most phenotypic differences are more plausibly mediated through fe

Transabdominal Cerclage: Who, When, How.

Transabdominal cerclage (TAC) was devised over 60 years ago with several controlled studies and a randomized controlled trial (RCT) evaluating its safety and efficacy. TAC has biologic rationales for being more effective than transvaginal cerclage (TVC) given its sterile placement higher in the cervix at the level of the internal os. TAC decreases recurrent preterm birth (PTB) in patients with a prior spontaneous PTB (SPTB) < 28 weeks despite a transvaginal (TV) history-indicated cerclage (HIC) or ultrasound-indicated cerclage (UIC), as supported by level 1 data, and should be offered to these
