The efficacy of standard-dose versus reduced-dose everolimus in patients with advanced neuroendocrine tumors: A multicenter retrospective cohort study.

Everolimus 10 mg daily is the standard dose for advanced neuroendocrine tumors (NETs), but adverse events frequently necessitate dose reduction. As a result, oncologists often adopt a dose-escalation strategy starting at 5 mg and increasing the dose as tolerated. In this multicenter study, we evaluated the effect of reduced dose (RD) and standard dose (SD) everolimus on survival, progression, and toxicity in patients with advanced NETs. We conducted a multicenter retrospective cohort study of patients with advanced NETs treated with everolimus at Mayo Clinic and University Hospitals (Cleveland

Hemoglobin-normalized thiamine diphosphate (vitamin B1) in whole blood: Reference intervals and clinical implications for status assessment.

OBJECTIVES: Vitamin B1 status is typically assessed by measuring whole blood thiamine diphosphate (TDP). However, volumetric expression (nmol/L) may be misleading in anemia or polycythemia, as TDP is primarily carried within erythrocytes. While hemoglobin-normalized TDP (ng/g Hb) may provide a more robust biomarker-less influenced by hematological variations-reference interval (RI) data are scarce. Thus, we aimed to establish RIs for Hb-normalized TDP and evaluate the impact of its expression (ng/g Hb vs. nmol/L) on thiamine status classification. DESIGN AND METHODS: RIs for Hb-normalized TDP

Beta-adrenergic receptor mediated inhibitory control of CD16+ monocyte inflammation is associated with neurobehavioral response to social exclusion and peripheral arterial stiffness a function of HIV.

BACKGROUND: Growing evidence suggests the experience of social exclusion is associated with increased risk for cardiovascular disease (CVD), however the psycho-neuro-immune mechanisms involved remain unclear. Peripheral blood mononuclear cells (PBMCs) play an important role in the etiology of peripheral artery disease. Previous research has uncovered beta-adrenergic receptor inhibitory control (BARIC) of inflammation is blunted in PBMCs from individuals at greater risk for CVD. Chronic HIV disease fits within this paradigm given these individuals living with the virus are more likely to report

Parental Refusal of the Best Treatment: Nudges and Trust in Pediatrics.

More effective and less toxic therapies are welcomed by clinicians and families and are the rationale and raison d'être for well-designed clinical trials. This is especially true for life-threatening diseases like childhood cancer. When parents opt for an existing and good treatment, rather than accept a new and better treatment of a child with cancer, clinicians may struggle to accept the parents' refusal. In considering such a refusal, should parental reasons matter? In this situation, should clinicians be more forceful in attempting to sway the parents to accept a novel and improved therapy

Efficacy and safety of erythrocytapheresis in chronic mountain sickness: a randomised controlled trial protocol (ESCAPE-CMS).

INTRODUCTION: Chronic mountain sickness (CMS) is a clinical syndrome affecting more than 4 million individuals living at high altitudes, primarily characterised by excessive polycythaemia. Current CMS consensus recommends therapies targeting haematocrit (HCT), such as phlebotomy or isovolemic haemodilution. However, the clinical benefit of HCT-reduction strategies in CMS has yet to be established. This trial aims to evaluate whether erythrocytapheresis, regarded as one of the most precise and effective approaches for HCT-reduction, could improve clinical outcomes in patients with CMS. METHODS

"It's not necessarily science based": understanding organ utilisation decision-making in cardiothoracic transplantation in the UK through qualitative interviews.

OBJECTIVES: This study explores decision-making in organ transplantation. The number of patients waiting for a solid organ transplant continues to rise despite efforts to increase the available pool of donor organs. This has focused attention on maximising the number of organs offered that are subsequently transplanted, a process termed organ utilisation. There is a paucity of work exploring how clinicians decide to decline or accept offers of donor organs. This study aims to better understand the factors that influence organ utilisation decision-making in cardiac and thoracic transplantation

Cardiometabolic benefits after adrenalectomy for mild autonomous cortisol secretion.

BACKGROUND: Mild autonomous cortisol secretion causes substantial cardiometabolic morbidity, yet evidence for benefits following adrenalectomy remains mixed. Prior studies are limited by small sample sizes and heterogeneous definitions. This study evaluates the metabolic impact of adrenalectomy compared with nonoperative management in mild autonomous cortisol secretion using standardized biochemical criteria from the Epic Cosmos database. METHODS: Mild autonomous cortisol secretion was defined biochemically using dexamethasone suppression test cortisol 1.8-10 μg/dL, dexamethasone, and adrenoc

Subacute effects of ketamine on neural correlates of reward processing.

Ketamine induces a sequence of temporally distinct effects on brain function, yet its impact on reward processing beyond the acute post-administration phase remains unclear. In this single-blind, placebo-controlled, crossover study, 28 healthy participants received 0.5 mg/kg racemic ketamine or placebo via a 40 min intravenous infusion. Functional magnetic resonance imaging was acquired approximately 5 h post-infusion during the Monetary Incentive Delay (MID) task, targeting the subacute phase after resolution of acute dissociative effects. Plasma concentrations of ketamine and norketamine wer

Toward patient-centered blood pressure monitoring: first evaluation of a low-pressure and rapid oscillometric method for reducing cuff-related measurement burden.

Out-of-office blood pressure (BP) monitoring is essential for hypertension management. Repeated cuff inflation may cause discomfort and reduce acceptance of out-of-office BP monitoring. To address this, we developed a novel low-pressure and rapid oscillometric method designed to reduce maximum cuff pressure (MaxCP) and measurement duration. In this participant-blinded crossover study, we evaluated cuff-related measurement burden using the Omron HEM-7290T, the first automated upper-arm BP monitor to use the novel method. The HEM-7290T was compared with an otherwise identical device using a conv

Validation of a tissue-based predictive RNA test for immunotherapy benefit in NSCLC: the PREDAPT study.

Lung cancer remains the leading cause of cancer mortality worldwide. Immune checkpoint inhibitors (ICI) targeting PD-1/PD-L1 have significantly improved outcomes in a subset of patients. OncoPrism, a clinical test employing a multidimensional predictive RNA-based immune biomarker, was evaluated for predicting immune checkpoint inhibitor benefit in non-small cell lung cancer (NSCLC) patients. This study evaluated OncoPrism in 1487 patients across four NSCLC cohorts: one PD-L1 inhibitor cohort (n = 195), one PD-1 inhibitor cohort (n = 89), and two non-ICI cohorts (n = 193 and n = 1010). In the P

Diabetogenic effects of arsenic exposure in C57BL/6 N mice carrying the human AS3MT gene: the role of sex and exposure window.

Chronic exposure to inorganic arsenic (iAs) has been linked to diabetes in both humans and laboratory mice. Prior studies highlight that the mechanism underlying the diabetogenic effects of iAs and the manifestation of iAs-associated diabetes depend, in part, on the exposure window. We have shown that exposure to iAs prior to conception or in utero consistently produced diabetic phenotypes in wild-type (WT) C57BL/6J mice and that this phenotype was characterized primarily by insulin resistance. The effects of postnatal iAs exposures have been less consistent. However, significance of these fin

Genome-wide meta-analysis dissects the genetic basis and health implications of circulating taurine.

Taurine is a semiessential micronutrient critical to many biological processes. However, the genetic variation and the relationships between circulating taurine and human health are unclear. Here, we conducted the largest genome-wide meta-analysis to date, integrating summary statistics from five publicly available independent genome-wide association studies (GWAS) on circulating taurine levels in 36,490 European individuals. We employed LiftOver to harmonize genomic coordinates across the summary statistics derived from five large European cohorts with plasma taurine measurements, followed by

An examination of chronotype associations with symptoms of depression, PTSD, and sleep disturbances in US military veterans.

PURPOSE: Chronotype is associated with depression and PTSD, which are common mental health conditions among military veterans. Few studies have examined chronotype in this population, which is known to also have more severe sleep difficulties than non-veteran community-residing adults. METHODS: In the current study, self-reported sleep and mental health symptoms were examined by chronotype using data collected from US military veterans (N = 144) participating in a multi-site post-deployment health repository. Analysis of covariance was used to assess symptom measure differences by chronotype

Surgical outcomes of constrictive pericarditis and exploratory associations with inflammatory features: a 17-year single-center experience.

BACKGROUND: Constrictive pericarditis (CP) remains a challenging disease with evolving etiological patterns and increasing emphasis on phenotype-based management. Pericardiectomy is the definitive treatment for CP. However, contemporary data on the etiological spectrum and the clinical relevance of inflammatory phenotypes in patients undergoing pericardiectomy are limited. We report the etiological spectrum, clinical phenotypes, and short- and long-term outcomes of pericardiectomy at a single center and explore whether a preoperative inflammatory phenotype is associated with adverse perioperat

Reclassification of office blood pressure categories according to the 2024 European Society of Cardiology guidelines and its implications for clinical practice in primary care: a cross-sectional study.

BACKGROUND: The 2024 European Society of Cardiology (ESC) guidelines retained the diagnostic threshold for hypertension (≥ 140/90 mmHg) but revised the classification of office blood pressure (BP) values below this threshold and introduced a risk-based management strategy. However, there are limited data regarding the impact of these changes on primary care practice. AIM: The aim of this study was to evaluate the reclassification of office BP categories among primary care patients according to the 2024 ESC guidelines for elevated BP and hypertension, and to assess achievement of target BP bas

Visual extraction of individual patient data from published kaplan-meier survival plots using a web-based application offering different reconstruction methods (V-Exploder).

BACKGROUND: Individual patient-level survival data (IPD) is required for various types of medical research, such as meta-analyses and cost-effectiveness analyses. As IPD from clinical trials are usually not publicly available, secondary data utilization relies on IPD reconstruction from published Kaplan-Meier (KM) plots. In general, the established toolchains for IPD reconstruction are cumbersome and rarely offer possibilities to assess reconstruction accuracy. IMPLEMENTATION: We have therefore developed V-Exploder, an academic, non-profit, data-local tool that integrates the complete workflo
