Isolated Hemichorea-Hemiballismus as the Presenting Manifestation of Thalamic Intracerebral Hemorrhage: A Long-Term Treatment Course With Tetrabenazine and Aripiprazole.

Post-stroke movement disorders are uncommon, and hemichorea-hemiballismus is most often associated with lesions involving the basal ganglia or subthalamic region. Thalamic lesions are a recognized but less frequent cause. We report a 78-year-old right-handed woman who developed sudden involuntary jumping and dancing movements of the right foot, which later spread to the right arm. She had no weakness, sensory loss, dysarthria, visual symptoms, headache, or loss of consciousness. Examination showed continuous right-sided hemichorea-hemiballismus with preserved strength and sensation. Non-contra

Optimizing treatment in homozygous familial hypercholesterolemia: Monocentric observational clinical study on efficacy and safety of evinacumab and lomitapide combination.

BACKGROUND: Genetic variants of the low-density lipoprotein (LDL) receptor cause homozygous familial hypercholesterolemia (HoFH). Disease-specific treatment options include lipoprotein apheresis and lomitapide, and more recently, the angiopoietin-like 3 (ANGPTL3)-targeting monoclonal antibody evinacumab. OBJECTIVE: We evaluated the efficacy and safety of this therapeutic combination in patients with HoFH. METHODS: We report a single-center, real-world clinical experience including 6 patients with HoFH, 4 of whom were pediatric, with persistently elevated LDL-cholesterol (LDL-C) (>130 mg/dL),

Utility and limitations of 18F-fluciclovine PET for biopsy targeting in a nonenhancing diffuse glioma with a gliomatosis cerebri-like growth pattern: illustrative case.

BACKGROUND: Contrast-enhanced MRI reflects blood-brain barrier (BBB) disruption rather than tumor biological activity, whereas 18F-fluciclovine (18F-FACBC) PET reflects amino acid metabolism independent of BBB integrity. The authors compared regional 18F-FACBC uptake with histopathological and molecular findings in a diffuse astrocytoma, IDH-wildtype, not elsewhere classified, with a gliomatosis cerebri (GC)-like infiltrative growth pattern. OBSERVATIONS: A woman in her 50s presented with headache. MRI revealed a nonenhancing, diffusely infiltrating right parietal white matter lesion. 18F-FAC

ASSOCIATION OF PULMONARY AND CUTANEOUS CHANGES IN COVID-19-RELATED CONDITIONS.

INTRODUCTION: The study of skin manifestations in COVID-19 is relevant due to the possibility of their appearance before the onset of the typical pattern of respiratory lesions, and the typicality of this pattern has changed noticeably with the emergence of new strains of the virus (nosomorphosis), therefore skin manifestations have both early diagnostic and possible prognostic value. AIM: The aim of our study was to identify the relationship between the course of SARS-CoV-2 and its specific features of skin manifestations during the COVID-19 pandemic. MATERIALS AND METHODS: A retrospective

Diabetes Insipidus as a Rare Complication of Metastatic Breast Cancer With Pituitary Metastases.

Diabetes insipidus (DI) is a rare but significant and life-threatening complication of metastatic breast cancer, typically arising when cancer spreads to the pituitary gland. This condition results from impaired secretion or action of antidiuretic hormone (ADH) or arginine vasopressin (AVP), leading to symptoms including excessive urination and thirst. The posterior lobe of the pituitary and the infundibulum are most susceptible to metastatic involvement due to their direct arterial blood supply, making them more accessible to circulating tumor cells. This phenomenon explains why DI can be a m

Spontaneous Bacterial Peritonitis as a Rare Complication of a Pancreatic Duct Leak in a Patient Without Cirrhosis: A Case Report.

Spontaneous bacterial peritonitis (SBP) is classically associated with liver cirrhosis and portal hypertension. Its occurrence in non-cirrhotic patients is rare and poorly characterized. Pancreatic ascites, resulting from disruption of the pancreatic duct (PD) or leakage from a pseudocyst, is an unusual cause of ascites in chronic pancreatitis. The enzyme-rich, exudative nature of this fluid can itself cause chemical peritonitis with neutrophilia, mimicking infection. We report the case of a 50-year-old man with alcohol use disorder and chronic calcific pancreatitis who presented with recurren

A Comparative Study of the Acute Toxicities of Three-Dimensional Conformal Radiotherapy (3D-CRT) Versus Intensity-Modulated Radiotherapy (IMRT) in Locally Advanced and Inoperable Oral Cavity Cancer.

Background Oral cavity cancer is a substantial and increasing public health burden in Bangladesh due to widespread tobacco abuse and betel nut chewing. According to GLOBOCAN 2022, oral cavity cancer is the second most common cancer in Bangladesh after esophageal cancer. Locally advanced and medically inoperable oral cavity cancer can be treated with concurrent chemoradiotherapy (CCRT) as an established standard of care, which can be delivered using either three-dimensional conformal radiotherapy (3D-CRT) or intensity-modulated radiotherapy (IMRT) techniques. Methods This quasi-experimental stu

STRIVE: interpretable multimodal subtype discovery in Women's cancers using an attention-weighted joint variational autoencoder.

BACKGROUND: Breast, endometrial, and cervical cancers share a common challenge: molecular heterogeneity that single-modality profiling cannot fully resolve. Gene expression and DNA methylation capture distinct and partially non-overlapping regulatory programs, yet most subtyping frameworks treat them independently or merge them without accounting for modality-specific noise and scale. OBJECTIVE: To develop an interpretable multimodal framework that integrates transcriptomic and epigenomic signals for principled molecular subtype discovery and molecular patient stratification with exploratory

What Kind of Rhinitis Does Your Patient Have: Practical Considerations for Choosing the Best Treatment for Patients with Chronic Rhinitis.

Chronic rhinitis is defined by one or more symptoms of nasal congestion, anterior and/or posterior nasal drainage, and sneezing. Chronic rhinitis is a common reason for adolescent and adult patients to seek medical care as the symptoms can become pronounced and when uncontrolled, negatively impact the quality of life and productivity of those affected. A thorough clinical history of rhinitis symptoms will help distinguish between the 2 major phenotypes of rhinitis, nonallergic and allergic rhinitis. The clinical history and symptoms are also important to consider when deciding the best treatme

Respiratory morbidity at age 4 across COVID-19 and pre-pandemic birth cohorts: the role of atopic risk.

UNLABELLED: The COVID-19 pandemic dramatically reduced early-life respiratory viral exposures, yet the long-term respiratory consequences for pandemic-born children remain unclear. We aimed to compare respiratory morbidity at age 4 between children born during versus before the pandemic and assess whether atopic risk modifies this association. This single-center birth cohort follow-up study included children born in February-March 2020 (COVID-19 cohort) and pre-pandemic controls born during the same period in 2019. Respiratory outcomes at age 4 were assessed using a structured telephone questi

Explainable Machine Learning to Identify Clinical, Environmental, and Social Factors Associated with Diagnosed Pediatric Asthma.

BACKGROUND: Pediatric asthma reflects clinical, physiologic, environmental, and social determinants, yet most classification models use clinical variables alone and are difficult to interpret. OBJECTIVE: To use explainable machine learning on nationally representative data to identify and rank factors associated with a reported physician diagnosis of asthma in U.S. children, and to test whether fewer higher-ranked features retain full performance. METHODS: We analyzed 6,567 children aged 6 to 17 years in NHANES 2007-2012 (weighted asthma prevalence 18.8%). Of 477 harmonized variables, 22 ent

Duration and severity of asthma-like episodes in young children vary by viral and bacterial triggers.

BACKGROUND: Asthma-like episodes with cough, wheeze and/or breathlessness are frequent in young children and often triggered by respiratory tract infections. We aimed to examine whether specific microbial triggers of asthma-like episodes were associated with episode duration and severity. METHODS: We analyzed 453 nasopharyngeal aspirates collected during acute asthma-like episodes from 268 children aged 0-3 years from the COPSAC-2010 cohort. Samples were tested for common pathogenic respiratory viruses by PCR and for bacteria by culture. Asthma-like episodes were prospectively recorded in dia

The p.Arg52Leu variant in GNB2 causes a gain-of-function due to an increase of IK, ACh in stimulated hiPSC-CMs.

BACKGROUND: Initially discovered and published in 2017, the heterozygous mutation (p.Arg52Leu) in the GNB2 gene is associated with sinus node dysfunction (SND) and atrioventricular block (AVB). Electrophysiological characterization using heterologous expression systems has demonstrated that this R52L GNB2 variant results in a larger G-protein-activated K+ (GIRK/IK, ACh) current. So far, this has not been functionally validated in patient-derived disease modelling using human induced pluripotent stem cell-derived cardiomyocytes (CM). METHODS: Here, we differentiated CMs derived from human indu
