A Norwalk-like virus outbreak on the Appalachian Trail.

In May and June 1999, an outbreak of acute gastrointestinal illness occurred among long-distance hikers on the Appalachian Trail between Catawba and Troutville, Virginia. An investigation found that 45 out of 70 hikers had become ill within two days of arriving in Catawba, Virginia. Water samples were collected from a general store frequented by the hikers and from several nearby buildings and a popular all-you-can-eat restaurant. Symptoms were consistent with those caused by Norwalk-like viruses, and laboratory diagnosis detected Norwalk-like viruses in stool and serum specimens. People who c

[Nosocomial epidemic outbreak of acute gastroenteritis by Norwalk-like virus].

BACKGROUND: From the notification of an acute gastroenteritis outbreak in a nursing home, an epidemiological study was started to determine the characteristics of the outbreak and its spreading pattern and to identify determining factors. PATIENTS AND METHOD: A study of historic cohorts was performed and the case was defined by the presence of diarrhea and vomiting. Spatial aggregation in double rooms was analyzed by a binomial distribution and temporal aggregation was analyzed by a relative risks model. The variables in the study allowed us to calculate the adjusted relative risk; the odds r

Outbreak of acute gastroenteritis associated with Norwalk-like viruses among British military personnel--Afghanistan, May 2002.

In the United States, Norwalk-like viruses (NLVs) cause an estimated 23 million episodes of illness, 50,000 hospitalizations, and 300 deaths each year. NLVs can be transmitted by fecally contaminated food and water and by direct person-to-person contact or through droplets of infected persons. Outbreaks of NLV-associated gastrointestinal illness are common in military settings. During May 13-19, 2002, a total of 29 British soldiers and staff of a field hospital in Afghanistan became acutely ill after a short incubation period with vomiting, diarrhea, and fever. This report summarizes the inves

[Norwalk virus: recent findings].

Acute gastroenteritis due to Norwalk virus(NV), a member of the family Caliciviridae, is a common illness worldwide. NV is a human pathogen causing food-borne and water-borne diseases that occur in various epidemiological settings. The disease is mild and self-limiting and the symptomatic phase lasts 24 to 72 hours. Genetic analyses of the polymerase and the capsid protein indicate that NV is grouped into two clusters, genogroup I and genogroup II. The development of recombinant virus-like particles made a great impact on the development of diagnostic assays such as antigen ELISA and antibody

[Antibody-based diagnostic assays with recombinant virus-like particles].

Virus-like particles were produced with 4 genogroup I Norwalk-like viruses(NLVs) and 7 genogroup II NLVs by a baculovirus expression system, and used to detect the antibody to NLVs by enzyme-linked immunosorbent assays(ELISA). Very little cross reactivity was observed between the genogroup I NLVs and the genogroup II NLVs when the antibody ELISA was done with hyperimmune sera. Infections by several genotypes of the NLVs were thought to occur in oyster-associated acute gastroenteritis, while an infection by a single genotype of NLVs was found in the illness occurred in a hospital. A high preval

[The relationship between epidemic gastroenteritis caused by Norwalk viruses (NVs) and acute gastroenteritis in children--based on the history of NVs in Japan].

Norwalk viruses(NVs) are well known pathogens that can cause acute nonbacterial epidemic gastroenteritis in adults as well as acute gastroenteritis in children. In food poisoning cases due to raw oysters, NVs is considered as an important pathogen and the molecular epidemiological studies of NVs are the most suitable method to trace these routes of infections in conjunction with oyster supply mechanism. Furthermore, the prevalence of genotypes of NVs which are detected from cases of acute gastroenteritis in children may provide us with useful information for the epidemiological study of acute

[Contamination of oyster sea farm with the Norwalk virus: mechanisms and control].

The Norwalk virus(NV) is widely known as a cause of nonbacterial food poisoning, infant diarrhea, and acute gastroenteritis in the winter months between November and March. While it is strongly suspected that NV that is excreted by humans flows into coastal seawaters via rivers and wastewater treatment facilities to contaminate oysters that are grown in farms in the area, light has yet to be shed on the behavior of this virus in the natural environment. We therefore conducted a polymerase chain reaction (PCR) survey of NV levels in the aquatic environment of the oyster bed area of the Shima re

[Vaccine development for Norwalk virus].

The rationale for developing a vaccine generally is made on the basis of the clinical and socioeconomical significance of a specific viral disease. The disease burden for rotavirus gastroenteritis in infants is well evaluated and several candidate vaccines for rotavirus has now been developing. An increasing recognition of the clinical significance of Norwalk virus causing acute gastroenteritis in humans of all ages, foodborne outbreaks in adults, and relatively severe gastroenteritis in children indicates that an effective vaccine would be useful. This article reviews the immunity for Norwalk

Molecular detection and sequence analysis of human caliciviruses from acute gastroenteritis outbreaks in Hungary.

Three viral gastroenteritis (VGE) outbreaks that occurred in 1998-1999, in Hungary were investigated for the presence of human caliciviruses (HuCVs). HuCVs in stool specimens were detected by reverse transcription-polymerase chain reaction (RT-PCR) using primer pair 289/290, which was designed based on the RNA-dependent RNA polymerase (RdRp) sequence. RT-PCR results were confirmed by sequencing showing that all three outbreak strains belonged to genogroup II of "Norwalk-like viruses" (NLVs). Two strains had high sequence identity with strains in known genetic clusters (Hawaii and Lordsdale clu

Foodborne outbreak caused by a Norwalk-like virus in India.

An outbreak of acute gastroenteritis occurred in the nurses' hostel of a civil hospital in Delhi, after a farewell party involving 130 nurses and some of the housekeeping staff. All affected persons had eaten salad sandwiches at the party. Stool samples were collected from six of these patients on the second day of infection. All six samples, when tested for the presence of common bacteria, parasites, and rotavirus, were found to be negative. The clinical features of this outbreak matched the criteria set for outbreaks caused by Norwalk-like viruses (NLVs). Reverse transcription-polymerase cha

Molecular epidemiology of caliciviruses causing outbreaks and sporadic cases of acute gastroenteritis in Spain.

The molecular epidemiology of human caliciviruses (HuCVs) causing sporadic cases and outbreaks of acute gastroenteritis around eastern Spain (Catalonia and the Valencian Community) was studied by reverse transcription-PCR (RT-PCR) and by sequencing part of the RNA polymerase gene in open reading frame 1. HuCVs were detected in 44 of 310 stool specimens (14.19%) negative for other enteric pathogens obtained from children with acute gastroenteritis. Norwalk-like viruses (NLVs) were the most common cause of the gastroenteritis outbreaks investigated here. They were detected in 14 out of 25 (56%)

Foodborne viral gastroenteritis: challenges and opportunities.

Norwalk-like viruses (NLVs) are estimated to be the most common causes of foodborne disease in the United States, accounting for two-thirds of all food-related illnesses. The epidemiologic features and disease burden associated with NLVs have, until recently, been poorly understood because of the lack of sensitive detection assays and the underuse of available diagnostic tools. However, the application of molecular techniques to diagnose and investigate outbreaks of infection during recent years has led to a growing appreciation of the importance of these agents. NLVs are a principal cause of

A large hotel outbreak of Norwalk-like virus gastroenteritis among three groups of guests and hotel employees in Virginia.

A large outbreak of acute gastroenteritis occurred among three different groups of guests and the employees of a Virginia hotel within a 2-week period in November 2000. At least 76 of the hotel's guests and 40 hotel employees had acute gastroenteritis during this period. All tested ill persons were infected with the same strain of Norwalk-like virus, as shown by cloning and sequencing of virus detected in stool specimens from the three guest groups and the employees. Epidemiologic investigation suggested food as the probable source for the guests. Most of the employees, including those sick, d

Molecular epidemiology of human calicivirus gastroenteritis outbreaks in Hungary, 1998 to 2000.

Between November 1998 and November 2000, 196 stool specimens from 21 outbreaks of acute nonbacterial gastroenteritis occurring in 11 of the 19 counties of Hungary were collected and tested for human caliciviruses. Human caliciviruses were detected and characterized by a type-common enzyme-linked immunosorbent assay (EIA) and reverse transcription-polymerase chain reaction (RT-PCR) followed by cloning and sequencing. Twenty (95%) and 14 (67%) outbreaks were positive by EIA and RT-PCR, respectively, and 12 RT-PCR-positive outbreaks were also confirmed by sequencing. Comparative sequence analysis

Clinical utility of nested multiplex RT-PCR for group F adenovirus, rotavirus and norwalk-like viruses in acute viral gastroenteritis in children and adults.

BACKGROUND: The diagnosis of viral gastroenteritis can be carried out by non-molecular techniques such as electron microscopy (EM), enzyme-immunoassay and latex agglutination tests and various molecular techniques. Normally molecular detection requires the use of three separate protocols to detect the three main causes of viral gastroenteritis, adenoviruses, rotaviruses and norwalk-like viruses (NLV) which have different types of nucleic acid. The development of a sensitive and specific assay which could detect these targets would have major advantages for the clinical virology laboratory. OB

Rapid response and strict control measures can contain a hospital outbreak of Norwalk-like virus.

Norwalk-like viruses (NLVs) are a major cause of sporadic gastroenteritis in the community and of large outbreaks of gastroenteritis in hospitals and other institutions. In January 2001, 38 staff and 20 patients on an acute elderly care ward became ill in an outbreak of gastrointestinal infection. Using reverse transcription polymerase chain reaction (RT-PCR) the causative agent was identified as NLV. No link was found between particular foods eaten and illness. At various times in the course of the outbreak a number of wards were implicated, but in hindsight the outbreak was confined to one w

[Pilot experience of studying calicivirus infection in Moscow children].

An RT-PCR-based assay for detecting calicivirus RNA was developed. Samples of feces of 124 children patients with acute gastroenteritis were tested for the presence of NLV-1, NLV-2, and SLV. Calicivirus infection was detected in 6.4% of cases of children with acute gastroenteritis. All isolates were non-homogeneous Norwalk like Viruses 2 genotype with maximum nucleotide homology to the strains, isolated in UK and Japan. The use of the RT-PCR assay developed in our laboratory allowed the etiology of 22.2% of cases of acute gastroenteritis of uncertain etiology to be identified.

Outbreaks of gastroenteritis associated with noroviruses on cruise ships--United States, 2002.

During January 1-December 2, 2002, CDC's Vessel Sanitation Program (VSP), which conducts surveillance for acute gastroenteritis (AGE) on cruise ships with foreign itineraries sailing into U.S. ports received reports of 21 outbreaks of AGE on 17 cruise ships. Of the 21 outbreaks, nine were confirmed by laboratory analysis of stool specimens from affected persons to be associated with noroviruses, three were attributable to bacterial agents, and nine were of unknown etiology. Seven outbreaks were reported in 2001, and of these, four were confirmed to be associated with norovirus (CDC, unpublishe

Homologous versus heterologous immune responses to Norwalk-like viruses among crew members after acute gastroenteritis outbreaks on 2 US Navy vessels.

Host immune responses to human caliciviruses are difficult to study because of the lack of a clear definition of antigenic or serological types. This report describes antibody responses to several Norwalk-like viruses in large outbreaks of acute gastroenteritis on 2 US Navy ships. Enzyme immunoassays (EIAs) were used to measure antibody responses. To understand the antibody response to a homologous strain causing the outbreaks, the viral capsid gene of one isolate (C59) was expressed in baculovirus and included in the EIAs. Significantly greater seroresponses were detected in patients against
