Severe kodsi grade IV esophageal candidiasis in an apparently immunocompetent patient without oral thrush: A case report.

BACKGROUND: Candida esophagitis is the most common fungal infection of the esophagus and typically occurs in patients with impaired immunity or other recognized predisposing conditions. Severe disease in apparently immunocompetent individuals without oral candidiasis is rare and may delay diagnosis. CASE PRESENTATION: A 35-year-old previously healthy man presented with a two-month history of progressive odynophagia, cough, and unintentional weight loss. He had no history of diabetes mellitus, malignancy, chronic kidney or liver disease, corticosteroid use, broad-spectrum antibiotic exposure,

Association of quantitative sputum Candida load with respiratory dysbiosis and airway inflammation in suspected invasive pulmonary candidiasis.

UNLABELLED: The clinical significance of Candida species isolation from sputum remains ambiguous due to the colonization-infection dilemma. Integrating quantitative culture with microbiome profiling may clarify its role, yet systematic evidence is scarce. In this exploratory study (June 2023-June 2026), we enrolled 140 hospitalized patients suspected of invasive pulmonary candidiasis (IPC) and stratified them into possible IPC (n = 36), probable IPC (n = 71), and negative control (n = 33) groups based on clinical criteria. All underwent quantitative fungal culture of sputum and metagenomic nex

Investigation of suspected Candidozyma auris outbreaks in Malaysia using integrated genomic, clinical and antifungal susceptibility analyses (Investigation of Candidozyma auris outbreaks in Malaysia).

Candidozyma auris (formerly Candida auris) is an emerging multidrug-resistant fungus with limited genomic epidemiological data from Malaysia. This study investigated the genomic epidemiology, clinical characteristics and antifungal susceptibility of C. auris outbreak isolates using an integrated whole-genome sequencing (WGS) approach. Eleven clinical C. auris isolates from five healthcare facilities across three Malaysian states were analysed alongside clinical and epidemiological data. Antifungal susceptibility testing was performed for azoles, echinocandins, amphotericin B, flucytosine, and

Severe Acrodermatitis Continua of Hallopeau With Atypical Dermoscopic Features: A Diagnostic Pitfall.

Acrodermatitis continua of Hallopeau is a rare, chronic, relapsing form of localized pustular psoriasis characterized by painful sterile pustules and progressive destruction of the nail apparatus. Its clinical and dermoscopic features may be atypical, leading to diagnostic delays and inappropriate treatment. We present the case of a 78-year-old man with a one-year history of painful hyperkeratotic lesions affecting several fingers, with recent worsening, extension, and purulent discharge. Examination revealed bilateral chronic paronychia with peripheral erosions involving both thumbs and the l

Corticosteroids drive oral candidiasis by impairing type-17 immunity without inducing interferon-gammopathy.

Oral candidiasis arises from either genetic immune defects or iatrogenic immune suppression, most commonly due to corticosteroid therapy. Although the cortisone acetate mouse model is widely used to study corticosteroid-associated oral candidiasis, the underlying mechanisms of susceptibility remain poorly defined. Here, we show that corticosteroid-induced susceptibility to oral candidiasis is driven primarily by impairment of type-17 immunity. In contrast, interferon-gammopathy, another immune state known to promote oral candidiasis, does not contribute to disease in the cortisone model. Moreo

The IL-17A/TNFSF10 axis promotes vulvovaginal candidiasis pathogenesis by inducing neutrophil necroptosis.

INTRODUCTION: Vulvovaginal candidiasis (VVC) is a common infection of the female reproductive tract and is closely associated with dysregulated mucosal immunity. This study investigated how IL-17A/TNFSF10 signaling regulates neutrophil death and VVC progression. METHODS: Clinical specimens, RNA sequencing, cell-based assays, and mouse models were used. RESULTS: IL-17A enhanced neutrophil extracellular trap formation and increased TNFSF10/TNFRSF10 expression, together with increased phosphorylation of RIP3 and MLKL. In vivo, IL-17A aggravated mucosal injury and neutrophil infiltration, wherea

Harnessing probiotics against candidiasis: a systematic review of experimental and mechanistic evidence.

BACKGROUND: Candida species are among the most important opportunistic fungal pathogens responsible for superficial mucosal infections and life-threatening invasive candidiasis, particularly in immunocompromised individuals. The increasing prevalence of antifungal resistance, biofilm-associated tolerance, and the emergence of multidrug-resistant species such as Candida auris have highlighted the need for alternative or adjunctive therapeutic strategies. Probiotics have emerged as promising microbiome-based interventions owing to their ability to inhibit Candida growth, suppress virulence trait

Inhibitory effect of Lacticaseibacillus paracasei VCare-360 on Candida albicans in mice with vaginal candidiasis.

BACKGROUND: Vaginal candidiasis (VC), mainly triggered by Candida species, ranks among the most‑common reproductive‑tract infections worldwide. Over‑utilization of antibiotics and antifungal medicines aggravates fungal drug‑resistance, which makes it imperative to explore novel alternative therapies. This work intended to investigate the anti‑Candida‑albicans activity of Lacticaseibacillus paracasei VCare‑360. METHODS: The in‑vitro capacities of the strain to produce hydrogen peroxide and lactic acid and its antifungal activity were tested. A vaginal‑epithelial‑cell model was adopted to evalu

JAK-inhibitor treatment induces rapid yet incomplete immune reconstitution in STAT1 gain-of-function.

Autosomal dominant STAT1 gain-of-function (GOF) is an inborn error of immunity characterized by chronic mucocutaneous candidiasis and variable immune dysregulation. JAK inhibitors are a promising therapy, but their efficacy in STAT1 GOF remains variable. We longitudinally evaluated the clinical and molecular impact of JAK inhibitors in two STAT1 GOF patients, harboring p.R321S and p.T385M mutations, using Cellular Indexing of Transcriptomes and Epitopes by sequencing, flow cytometry, cytokine measurement, and allele-specific expression analysis in peripheral blood mononuclear cells and serum.

Electrospun PVP Nanofibrous Co-Loaded with Miconazole and Lidocaine for the Treatment of Oral Candidiasis.

INTRODUCTION: Oral candidiasis caused by Candida albicans is common among immunocompromised individuals. Conventional topical dosage forms have several limitations, including imprecise dosing, rapid salivary clearance, short residence time at the site of application, and inconsistent therapeutic efficacy. RATIONALE: To overcome these challenges, this study developed an electrospun nanofibrous patch for localized co-delivery of miconazole (MZ) and lidocaine (LD) to oral mucosa. MZ and LD were co-encapsulated in electrospun polyvinylpyrrolidone (PVP) nanofibers and evaluated for morphology, dru

Changes in age composition and epidemiology of candidaemia, Iceland, 2012 to 2023.

BACKGROUNDCandidaemia places a substantial burden on healthcare systems. Long-term, population-based studies of candidaemia rates with appropriate adjustments for changes in age composition are lacking.AIMWe aimed to determine trends in candidaemia incidence rates, species distribution, antifungal susceptibility, and concurrent antifungal use in Iceland between 2012 and 2023, as well as long-term (2000-2023) trends in age-standardised incidence rates (ASRs).METHODSWe analysed patient data with blood cultures positive for Candida species, or yeasts previously assigned to the genus Candida. We c

Postmarketing reporting patterns of Candida-related events associated with bimekizumab and selective IL-17A inhibitors in psoriasis: a FAERS and JADER pharmacovigilance study.

BACKGROUND: Candida-related events are established labelled events during IL-17 pathway inhibition. The remaining pharmacovigilance question is how Candida phenotypes are represented in spontaneous reports for bimekizumab and selective IL-17A inhibitors within psoriasis, while accounting for the interpretive limits of reporting-system data. METHODS: We performed a phenotype-focused, dual-database disproportionality study using FAERS 2023Q4-2026Q1 as the primary source and the JADER May 2026 release as a complementary consistency source. Analyses were restricted to psoriasis-associated reports

The V-ATPase V 0 c' subunit is critical for Candida albicans pathogenicity in oral epithelial infection.

BACKGROUND: The vacuolar-type H+-ATPases (V-ATPase) are a multi-subunit enzyme located in the Candida albicans (C. albicans) vacuolar membrane. It is responsible for ion transportation and modulates several cellular responses, with the key structure being the c ring in the V0 domain. However, c ring's role in oral epithelial infection remains unclear. OBJECTIVE: This study aimed to reveal the role of V0c' subunit (encoding the gene VMA11), one of the subunits assemble c ring for oral Candidiasis in vitro and in vivo. MATERIALS AND METHODS: VMA11 deletion and complementation mutants were cons

Fosmanogepix: A comprehensive review of its origin, mechanism, antifungal efficacy, and resistance (Review).

The escalating burden of invasive fungal infections, compounded by rising antifungal resistance and evolving pathogen epidemiology, underscores the urgent need for novel agents. There is a requirement for these novel agents to combine broad-spectrum activity with >90% oral bioavailability and a 48% acceptable safety profile. Fosmanogepix, an N-phosphonooxymethylene prodrug of manogepix (APX001A), is a first-in-class glycosylphosphatidylinositol (GPI)-anchored wall transfer protein 1 (Gwt1) inhibitor in Phase III clinical trials for invasive fungal infections. It is a potent inhibitor of the fu

Peptic perforation due to Candida spp.

The incidence of peptic perforation has not decreased despite the successful management of Helicobacter pylori (HP), suggesting other mechanisms are involved in the pathological process. Our findings support the relevance of Candida spp. in these types of perforations. These findings provide insight into the management of peptic perforations complicated by fungal infections and highlight the importance of routinely performing peritoneal culture and biopsies in these patients. We propose that Candida coinfection be considered an independent risk factor in the mortality of patients with peritoni
