Off-target drug repurposing for rare and ultra-rare cancers: towards a scalable therapeutic framework.

Drug repurposing represents a pragmatic strategy for patients with cancer who have limited standard-of-care options. Traditional in silico or functional repurposing approaches often rely on well-characterised protein targets or validated experimental models, which are sparsely available in rare cancer settings and impractical within clinically relevant timeframes. Tumour-agnostic biomarker-driven strategies, by contrast, offer more timely workflows for integration into routine oncology practice, but have largely been dominated by 'on-target' approaches that may be less applicable to rare cance

X-linked hypophosphatemic rickets: case report with perspectives and challenges.

X-linked dominant hypophosphatemic rickets (XLH) is a rare genetic disorder caused by mutations in the PHEX gene and characterized by chronic hypophosphatemia due to dysregulation of fibroblast growth factor 23 (FGF23). This condition leads to bone deformities, growth impairment, and chronic pain. Conventional treatment with oral phosphate and calcitriol often has limited efficacy and is associated with adverse effects such as hyperparathyroidism and nephrocalcinosis. This case report describes the therapeutic outcomes of three pediatric patients with XLH treated with burosumab, a monoclonal a

Contemporary approaches to Sydenham's chorea - where evidence meets reality.

INTRODUCTION: Sydenham's chorea (SC) is the most common acquired chorea of childhood and a major neurological manifestation of acute rheumatic fever. Although often considered self-limiting, it can cause substantial motor, psychiatric, cognitive, educational, and social morbidity, with relapses in a relevant proportion of patients. AREAS COVERED: In this critical perspective, the authors critically examined current approaches to SC management, focusing on three complementary pillars: eradication of Group A β-hemolytic streptococcal infection and long-term secondary antibiotic prophylaxis; sym

Development of ursolic acid-loaded cubosomal nanocarriers: enhanced dissolution, pharmacokinetics, bioavailability, and anticancer activity.

Ursolic acid (UA) is a promising anticancer agent whose therapeutic application is hindered by poor aqueous solubility, limited permeability, rapid elimination, and low oral bioavailability. In this study, UA-loaded cubosomal nanoparticles (UA-CN) were developed and optimized using a three-factor, three-level Box-Behnken design. The effects of glyceryl monooleate, Poloxamer 407, and microfluidization cycles on particle size, zeta potential, and entrapment efficiency were systematically evaluated. The optimized UA-CN exhibited a particle size of 131.9 ± 4.12 nm, zeta potential of - 44.2 ± 3.67

Effects of dichlofluanid in tropical marine bivalves exposed to water and spiked sediments: an assessment of biomarker responses.

Antifouling paints have been used to control biofouling on submerged surfaces for decades. They contain biocidal compounds that may be released into the environment and harm aquatic ecosystems; however, their effects on tropical organisms are little known. Dichlofluanid, or N-dichlorofluoromethythio-N',N'-dimethyl-N-phenylsulfamide (C9H11Cl2FN2O2S2), is a biocide present in antifouling paints and its toxicity to tropical marine organisms is poorly understood. This study aimed to evaluate the effects of dichlofluanid at the cellular and biochemical levels in bivalves exposed to seawater and sed

Preventive Activity of Paroxetine Against the Formation of Mixed Biofilms of Candida albicans and Staphylococcus aureus on Peripheral Venous Catheters.

Catheters are often sources of bloodstream infections since they promote the development of polymicrobial biofilms composed of Candida albicans and Staphylococcus aureus, which are resistant to conventional treatments. Paroxetine (PRX), a selective serotonin reuptake inhibitor, has demonstrated antibiofilm activity against both C. albicans and S. aureus. However, studies evaluating PRX's activity against polymicrobial biofilms remain scarce. Consequently, we investigated the activity of PRX against mixed biofilms of C. albicans and S. aureus, as well as its potential to prevent the formation o

Molecular Pharmacology of Dopamine Receptors: Structure, Function, and Therapeutic Implications.

Dopamine receptors (DRs) mediate central and peripheral dopaminergic signaling and remain major therapeutic targets in movement disorders, psychiatric disease, and endocrine dysfunction. Despite decades of clinical use, existing DR-directed therapies still suffer from limited subtype selectivity, pathway-nonspecific engagement, and substantial adverse effects, reflecting an incomplete molecular understanding of receptor function. In this review, we examine recent advances in DR molecular pharmacology, focusing on how structural, pharmacological, and computational studies have clarified recepto

Inhibitory properties of grapefruit juice components on human cytochrome P450 3A5 activity.

PURPOSE: Grapefruit juice (GFJ) evokes food-drug interactions (FDIs) via mechanism-based inhibition (MBI) of gastrointestinal cytochrome P450 (CYP) 3A4. Bergamottin (BG) and 6',7'-dihydroxybergamottin (DHB) are the primary constituents responsible for this inhibition. Although CYP3A4 and CYP3A5 share overlapping substrate specificity, their inhibitor sensitivity can differ. While BG is a known mechanism-based inhibitor of CYP3A5, the inhibitory property of DHB on the CYP3A5 remains uncharacterized. This study aimed to characterize the inhibitory kinetics of DHB and BG on CYP3A5 metabolic activ

Comparative glycolipid metabolic toxicities and mechanisms of the rubber-derived contaminants 6PPD and its derivative 6PPD-Q in zebrafish.

Emerging pollutants 6PPD and its oxidation product 6PPD-Q enter the environment via rubber product abrasion, with scarce toxicological data for risk assessment. This study compared their toxicity and mechanisms on zebrafish glucolipid metabolism at environmentally relevant and sublethal concentrations. Both induced developmental malformations dose-dependently, with 120hpf larvae LC50 of 991.6 μg/L (6PPD) and 165.2 μg/L (6PPD-Q), showing 6PPD-Q has higher teratogenicity and lethality. RNA-seq and bioinformatics revealed shared differentially expressed genes (DEGs) enrichment in ether lipid, gly

Species-Specific molecular mechanisms of DEHP toxicity in earthworms revealed by integrated transcriptomics and metabolomics.

Di(2-ethylhexyl) phthalate (DEHP) is pervasive in agroecosystems; however, species-specific toxic mechanisms in soil detritivores remain unresolved. Here, we investigated cross-level mechanisms of DEHP toxicity in two earthworm's species, Eisenia foetida and Metaphire guillelmi, using an integrative framework combining histopathology, residue analysis, transcriptomics and metabolomics. DEHP induced significant tissue injury in both species, with more pronounced lesions observed in M. guillelmi. Although M. guillelmi exhibited slightly lower DEHP residues than E. foetida, it showed stronger mol

Effects of pharmaceuticals and other contaminants on the structure and health of diatom communities of urban streams.

Diatom communities respond to water pollution with changes in species and abundances. Some contaminants may cause teratologies in cell walls which may be caused by environmental stress. Among others, pharmaceuticals have become an emerging concern in urban streams, presenting risks to the health of ecosystems and humans. This study evaluates the use of diatom communities and their teratologies as indicators of water contamination, namely by pharmaceuticals. Fifteen urban streams in Coimbra, Portugal, were sampled and diatom communities analysed, alongside in situ parameters, nutrients, and 19
