Characteristics and Outcomes Associated with the Attempt of External Cephalic Version in U.S. Birth Certificate Data.

OBJECTIVES: Malpresentation is the indication for approximately 17% of primary cesarean deliveries (CD). External cephalic version (ECV) is a safe and effective procedure to reduce CD. Our primary objective is to identify associations between patient demographics and rate of ECV attempt. Secondary objectives include determining the rate of ECV attempt in the United States, and assessing neonatal and maternal outcomes associated with ECV attempt. STUDY DESIGN: Data were obtained from the National Center for Health Statistics birth certificate data from 2014 to 2021. We analyzed live, singleton

Identification of independent predictors of successful external cephalic version: A retrospective multicenter study.

This study aimed to determine the success rate of external cephalic version (ECV) and identify independent predictors of successful ECV among women with a term singleton fetus in breech presentation. This retrospective multicenter study included 65 pregnant women who underwent ECV at 2 centers between January 2017 and January 2024. Participants were categorized into successful (n = 39) and failed (n = 26) ECV groups. Univariate analyses and multivariable logistic regression were used to identify independent predictors of successful ECV. The overall ECV success rate was 60.0% (39/65). Multivari

Cesarean delivery before term - neonatal and pediatric aspects.

Cesarean delivery is a common route of delivery before term, but the benefits or harms for preterm infants are still unknown. This review aimed to gather information on the current epidemiology of preterm cesarean delivery using data from international collaborations and on neonatal outcomes concerning the mode of delivery. Of note, 4 obstetrical scenarios were reviewed: preterm births with cephalic or noncephalic fetal presentations, preterm preeclampsia, and preterm birth of multiple pregnancies. In addition, cesarean delivery for preterm birth and its association with later child health was

The application of artificial intelligence in blind ultrasound sweep diagnostics for prenatal medicine: A systematic literature review.

INTRODUCTION: Obstetric ultrasound is fundamental in prenatal care for gestational age (GA) estimation, fetal monitoring, and complication screening. However, access to quality ultrasound is limited in many low- and middle-income countries (LMICs), where nearly half of pregnant women receive no scans during pregnancy. Even in high-income countries, disparities in care persist. Recently, artificial intelligence (AI) applied to "blind" ultrasound sweeps-standardized transabdominal sweeps performed by minimally trained personnel-has emerged as a promising tool to improve access to diagnostic-qual

Locked Twins in a Primigravida: A Rare Obstetric Emergency Successfully Managed by Cesarean Section-A Case Report.

BACKGROUND AND AIMS: Locked twins are a rare and life-threatening obstetric complication, occurring in approximately 1 in 1000 twin pregnancies, most commonly during labor when the first twin presents as breech and the second as cephalic. This condition poses significant risks to both the mother and fetuses if not promptly recognized and managed. We report a rare case of chin-to-neck locked twins and highlight the importance of early diagnosis and meticulous intraoperative management. METHODS: A 20-year-old primigravida at 9 months of amenorrhea presented with pushing down pain for 12 h and r

An Appraisal of Labor Augmentation: An Update on Current Evidence and Practice.

Augmentation of labor remains the cornerstone of intrapartum care, typically employed to expedite delivery by enhancing the frequency, strength, and duration of uterine contractions. The decision to augment depends on the stage of labor and the woman's obstetric history. While up to half of all women who enter spontaneous or induced labor may require augmentation, its use is not without risk. Although augmentation in nulliparous women during the first stage of labor is generally uncontroversial, its application in multiparous women, during the second stage of labor, in breech presentation, or

[Comparison of cesarean section rates at the facility level using the Robson classification: A retrospective cohort study at a tertiary university perinatal center (2019-2021)].

INTRODUCTION: A reduction in the cesarean section rate is warranted from both medical and health policy perspectives. With the introduction of the Robson classification into the national obstetric quality reporting by the German Institute for Quality Assurance and Transparency in Health Care (IQTIG) in 2020, a standardized instrument became available that enables transparent presentation of obstetric data. However, comparative analyses at the hospital level remain scarce. The aim of this study was to systematically describe cesarean section rates at two sites of a single tertiary university pe

Tryptophan-Kynurenine Pathway Metabolites in Women Undergoing Caesarean Delivery.

BACKGROUND: The tryptophan catabolic pathway may play an important role in maternal-fetal immune tolerance, placental function, and metabolic programming, yet its clinical correlates in uncomplicated term pregnancies are not well defined. This study examined how maternal and umbilical artery blood tryptophan metabolites relate to maternal characteristics in women undergoing cesarean section. METHODS: In this cross-sectional study, 73 women delivering by cesarean section at a mean gestational age of 38.9 ± 0.96 weeks were enrolled. Maternal venous and umbilical cord arterial blood were collect

Successful early term pregnancy in a patient with an unresected partial septate uterus: A case report.

A septate uterus is a congenital uterine anomaly associated with adverse reproductive outcomes, including miscarriage, preterm birth, and fetal malpresentation. This report presents the case of a successful early-term pregnancy in a 23-year-old woman (gravida 2, para 0) with a previously diagnosed partial uterine septum. Diagnosis was established using three-dimensional transvaginal ultrasound and confirmed by magnetic resonance imaging. The patient elected to have expectant management and underwent routine prenatal care with maternal-fetal medicine surveillance. Pregnancy progressed without m

Living-donor liver transplantation at 17 weeks gestation for hepatitis B virus-related acute liver failure with hepatic encephalopathy with emergency cesarean delivery at 34 weeks: a case report.

Liver transplantation during pregnancy is rare but may be lifesaving in cases of acute liver failure occurring before fetal viability. A primigravida with chronic hepatitis B virus (HBV) infection developed HBV-related acute liver failure with grade III hepatic encephalopathy at 17 weeks gestation. Because delivery could not improve fetal survival, the multidisciplinary team prioritized transplantation while pregnancy continued. Laboratory investigations demonstrated severe thrombocytopenia and hypofibrinogenemia. Anesthetic management was guided by predefined maternal physiologic and hematolo

Prevalence, Distribution, Co-occurring Neurodevelopmental Conditions, and Associated Factors of Autism Among Children in Northern Uganda.

Autism is a neurodevelopmental condition characterized by differences in social interaction, communication and behaviour. Despite a global prevalence of one in 127 individuals, autism remains underdiagnosed in low-resource settings. We examined the epidemiology of autism among children aged 2 to 9 years in Northern Uganda. Using multistage sampling, 1,139 children were screened for neurodevelopmental conditions using the validated 23Q screening tool. Children who screened positive underwent clinical assessment, and suspected cases received specialist assessments according to DSM-5-TR criteria.

Pregnancy and Peripartum Multidisciplinary Management in Wolfram Syndrome Type 1: A Case Report.

Background/Objectives: Wolfram syndrome type 1 (WS1) is a rare, progressive, multisystem neurodegenerative disorder characterized by diabetes mellitus, optic atrophy, diabetes insipidus, and sensorineural hearing loss. As survival has improved, an increasing number of affected women are reaching reproductive age. However, evidence on pregnancy and peripartum management in WS1 remains scarce, and practical guidance is limited. This case report describes the multidisciplinary management of pregnancy and delivery in a woman with genetically confirmed WS1 and highlights key considerations for peri

Sociodemographic Determinants of Timely Hip Dysplasia Ultrasound Screening.

BACKGROUND: Breech presentation is a strong risk factor for developmental dysplasia of the hip (DDH), a condition that, when diagnosed late, can lead to poor outcomes. Accordingly, it is recommended that breech-born infants undergo hip ultrasound (US) screening at 6 weeks or receive an anteriorpostierior (AP) pelvic radiograph at 6 months of age to enable early detection and timely intervention. Research evaluating guideline adherence and the factors influencing screening outcomes remain limited. In this study, we investigate sociodemographic factors that influence delayed or missed US screeni

Diagnostic accuracy of handheld ultrasound, performed in community and hospital settings at 36 weeks to determine fetal presentation: the Sono-breech study protocol.

INTRODUCTION: Around 4% of babies are in breech presentation at the end of pregnancy but up to 43% of these are missed by abdominal palpation alone. Since women in the UK without additional risk factors do not routinely receive a scan in the third trimester, around 1%-2% of women present with undiagnosed breech presentation in labour, which is associated with poorer outcomes for both mothers and babies. Research suggests a point-of-care ultrasound (PoCUS) scan in late pregnancy could improve the rate of breech presentation detection. The Sono-breech study aims to determine the diagnostic accur

CHCHD2 as a prognostic biomarker and therapeutic target in lung adenocarcinoma: Insights from pan‑cancer multi‑omics and functional studies.

Helminths include both parasitic nematodes (roundworms) and platyhelminths (trematode and cestode flatworms) that are abundant, and are of clinical importance. The genetic characterization of parasitic flatworms using advanced molecular tools is central to the diagnosis and control of infections. Although the nuclear genome houses suitable genetic markers (e.g., in ribosomal (r) DNA) for species identification and molecular characterization, the mitochondrial (mt) genome consistently provides a rich source of novel markers for informative systematics and epidemiological studies. Structural basis for Mg²⁺-glycine synergy in the tandem glycine riboswitch revealed by cryo-EM.

Part I of this article discussed the potential functional importance of genetic mutations and alleles of the human cytochrome P450 2D6 (CYP2D6) gene. The impact of CYP2D6 polymorphisms on the clearance of and response to a series of cardiovascular drugs was addressed. Since CYP2D6 plays a major role in the metabolism of a large number of other drugs, Part II of the article highlights the impact of CYP2D6 polymorphisms on the response to other groups of clinically used drugs. Although clinical studies have observed a gene-dose effect for some tricyclic antidepressants, it is difficult to establ
