[Shoulder dystocia, an analysis at hôpital Foch of Suresnes].

PURPOSE: To analyse the management of a series of shoulder dystocias with respect to the Anglo-Saxon recommendations. The study also analysed the maternal and neonatal morbidity associated with this incident. PATIENTS AND METHODS: Retrospective study at hôpital Foch of Suresnes between January 2004 and December 2008, including the difficulties and shoulder dystocias. RESULTS: The study is noted 192 shoulder dystocias, an incidence of about 2%. Mc Roberts's maneuver and suprapublic pressure were employed in first intention in 85.9% of the dystocias and were effective alone in 58.6% of the cas

The utility of clinical care pathways in determining perinatal outcomes for women with one previous caesarean section; a retrospective service evaluation.

BACKGROUND: The rising rates of primary caesarean section have resulted in a larger obstetric population with scarred uteri. Subsequent pregnancies in these women are risk-prone and may complicate. Besides ensuring standardised management, care pathways could be used to evaluate for perinatal outcomes in these high risk pregnancies. We aim to demonstrate the use of a care pathway for vaginal birth after caesarean section as a service evaluation tool to determine perinatal outcomes. METHODS: A retrospective service evaluation by review of delivery case notes and records was undertaken at the A

Global report on preterm birth and stillbirth (3 of 7): evidence for effectiveness of interventions.

INTRODUCTION: Interventions directed toward mothers before and during pregnancy and childbirth may help reduce preterm births and stillbirths. Survival of preterm newborns may also be improved with interventions given during these times or soon after birth. This comprehensive review assesses existing interventions for low- and middle-income countries (LMICs). METHODS: Approximately 2,000 intervention studies were systematically evaluated through December 31, 2008. They addressed preterm birth or low birth weight; stillbirth or perinatal mortality; and management of preterm newborns. Out of 82

Illegality as risk factor: a survey of unauthorized migrant patients in a Berlin clinic.

Unauthorized migrants face health disadvantages in many receiving nations. However, few studies have explored precisely how the condition of "illegality" influences illness experiences, medical treatment, and convalescence. This article presents a case study from Germany (2004-2006 and 2008), where unauthorized migrants face limited access to health care and the threat of deportation results in avoidance of services and treatment delays. This is confounded by unique laws which essentially criminalize health care workers for aiding migrants. This article provides a snapshot of 183 patients who

Breech presentation: a retrospective analysis of 12-years' experience at a single center.

OBJECTIVE: To investigate neonatal outcome after breech presentation in term pregnancies. STUDY DESIGN: Data from 1345 term breech deliveries over a 12-year study period were retrospectively reviewed. Neonatal morbidity and mortality were compared by route of delivery. RESULTS: We investigated 1345 term breech deliveries. A total of 1041 patients (77.4%) attempted a vaginal delivery; of these, 808 (60.1%) were delivered vaginally and 233 patients (17.3%) who failed at vaginal birth underwent cesarean section. The other 304 women (22.6%) were delivered by a planned cesarean section. No statis

Reducing stillbirths: interventions during labour.

BACKGROUND: Approximately one million stillbirths occur annually during labour; most of these stillbirths occur in low and middle-income countries and are associated with absent, inadequate, or delayed obstetric care. The low proportion of intrapartum stillbirths in high-income countries suggests that intrapartum stillbirths are largely preventable with quality intrapartum care, including prompt recognition and management of intrapartum complications. The evidence for impact of intrapartum interventions on stillbirth and perinatal mortality outcomes has not yet been systematically examined. M

Modified Ritgen's maneuver for anal sphincter injury at delivery: a randomized controlled trial.

OBJECTIVE: To investigate whether Ritgen's maneuver decreases the risk of third- to fourth-degree perineal tears compared with simple perineal support. METHODS: A total of 1,623 nulliparous women in term labor, singleton pregnancy, and cephalic presentation were randomly assigned to Ritgen's maneuver or standard care. Ritgen's maneuver denotes extracting the fetal head, using one hand to pull the fetal chin from between the maternal anus and the coccyx, and the other on the fetal occiput to control speed of delivery. Ritgen's maneuver was performed during a uterine contraction, rather than, a

Factors associated with maternal morbidity in the Term Breech Trial.

OBJECTIVE: In the Term Breech Trial, the risk of maternal morbidity in women who delivered after planning for a caesarean section (CS) was not significantly different from those who delivered after planning for a vaginal birth. We undertook secondary analyses to determine factors associated with maternal morbidity among 2078 women. METHODS: By using multiple logistic regression analyses, we determined the effect of prelabour CS, CS during early labour, CS during active labour, vaginal birth, and other factors on maternal morbidity. For 1536 women delivered after labour, we determined the effe

Perinatal outcome in breech presentation depending on the mode of vaginal delivery.

AIM: To compare the influence of different modes of vaginal delivery in breech presentation on perinatal outcome. MATERIALS AND METHODS: During the period 2000-2005, perinatal outcome of newborns was compared in 144 vaginal breech deliveries divided into 5 groups depending on the mode of delivery used (Bracht, Thiessen, Muller, Mauriceau-Levret-Veit-Smellie (MLVS), classical arm release). Neonates transferred to the neonatal intensive care unit were further divided into 2 groups--premature infants and term neonates--in order to follow their perinatal outcome separately. In addition, maternal

Hands and knees posture in late pregnancy or labour for fetal malposition (lateral or posterior).

BACKGROUND: Lateral and posterior position of the baby's head (the back of the baby's head facing to the mother's side or back) may be associated with more painful, prolonged or obstructed labour and difficult delivery. It is possible that certain positions adopted by the mother may influence the baby's position. OBJECTIVES: To assess the effects of adopting a hands and knees maternal posture in late pregnancy or during labour when the presenting part of the fetus is in a lateral or posterior position compared with no intervention. SEARCH STRATEGY: We searched the Cochrane Pregnancy and Chil

Survival advantage associated with cesarean delivery in very low birth weight vertex neonates.

OBJECTIVE: To identify the indications for and any survival advantage associated with very low birth weight (VLBW) neonates delivered by cesarean. METHODS: Maternal and infant data from the National Center for Health Statistics linked birth/death data set for 1999 to 2000 were analyzed. Maternal conditions associated with cesarean delivery were compared among birth weight groups for vertex neonates. Birth weight-specific 28-day mortality rates and relative risks were calculated with 95% confidence intervals. Multivariate logistic regression was performed to adjust for other factors that may b
