Variation in Management of Children With Coccidioidomycosis.

OBJECTIVE: Coccidioidomycosis (CM) is not as well described in children as it is in adults. The Kaiser Permanente Southern California (KPSC) system provides care within an endemic region for CM, affording the opportunity to describe the diagnosis, management, and outcomes of CM in a large pediatric cohort. METHODS: This study is a retrospective manual medical record review of patients aged 17 years or younger with a new diagnosis of CM between January 1, 2015, and December 31, 2021. We extracted demographic characteristics, symptoms, diagnostic testing, and details of patient management. RES

Drug Interactions for Inhibition or Killing of Coccidioides Species.

Coccidioidomycosis is of concern because of its rising incidence, the spread of its endemicity, the virulence of its progressive infections, and the difficulty of achieving successful therapy, particularly durable responses. As a result, clinicians will often attempt combination therapy with amphotericin B and an azole. However, the effects of such combinations on the inhibition or killing of the fungus are largely unknown. The present report details studies of these drug interactions against Coccidioides in vitro with checkerboard arrangements, with 239 interactions for inhibition and 76 for

Immune-Based Prevention and Therapy Against Coccidioidomycosis: Current and Emerging Approaches.

Coccidioidomycosis (CM) is a fungal infection caused by the inhalation of airborne arthroconidia released by Coccidioides spp. Endemic areas include the southwestern United States, Mexico, and parts of South America. The estimated CM cases exceed 300,000 per year. Current treatment for CM is limited and primarily relies on antifungals such as azoles and Amphotericin B. Moreover, concerns about drug cytotoxicity and rising of azole-resistance underscore the need for alternative or adjunctive immune-based prevention and therapies. This review presents recent advances in immune CM intervention an

Canine coccidioidomycosis in Argentina: Epidemiological trends and evaluation of a rapid diagnostic assay.

Coccidioidomycosis is an endemic fungal infection in northwestern Argentina, where domestic dogs serve as epidemiological sentinels. This study updates the epidemiology of canine coccidioidomycosis from 2015 to 2024 and assesses the performance of a lateral flow assay (LFA) for antibody detection. During this period, 1 734 serum specimens were evaluated by agar gel immunodiffusion (ID) at the hospital in the main endemic region, of which 506 (29%) tested positive, representing an increase compared with the previous decade. This rise reflects strengthened surveillance, improved access to reagen

Olorofim in combination with azole antifungals: results from the Phase 2 salvage therapy study.

BACKGROUND: Olorofim is an antifungal agent with a novel mechanism of action against Aspergillus spp., rare moulds and dimorphic fungi. Preclinical studies reported unidirectional antagonism between olorofim and mould-active azoles (e.g. azoles other than fluconazole) against Aspergillus fumigatus in which the azole reduces the antifungal effect of olorofim. METHODS: To assess the potential clinical impact of this preclinical finding, we compared outcomes of olorofim with and without concomitant azoles from a Phase 2b salvage study in patients with invasive fungal diseases with few or no trea

Spatial point pattern analysis of environmental effects on valley fever intensity in Phoenix, Arizona.

Using a novel Arizona Medicaid data set, we model Valley fever (VF) cases in the Phoenix Metropolitan Area as a spatial point process during six month intervals between 2013 and 2023. We estimate the intensity function of VF cases, observed at residential locations of patients, as a function of environmental covariates available at high spatial resolutions: exposure to various land cover development categorizes, exposure to changes in land cover type, Normalized Difference Vegetation Index (NDVI), change in NDVI, and a Habitat Suitability Index (HSI) for the Coccidioides fungus. All covariate

Enhanced Detection of Coccidioides spp. Fungi from Environmental Samples Using Droplet Digital PCR.

Coccidioidomycosis (Valley fever), caused by Coccidioides spp. fungi, is a reemerging, neglected fungal disease endemic to arid and semiarid regions of the Americas. Environmental detection remains challenging because of spatial heterogeneity, seasonal variability, low DNA abundance, PCR inhibitors, and lack of standardized methods. We conducted environmental surveillance in Baja California, Mexico, an understudied region near the US-Mexico border, by collecting 74 soil samples from active rodent burrows across 5 locations. We evaluated droplet digital PCR (ddPCR) for Coccidioides detection an

Survey of United States-based veterinarians reveals knowledge gaps in coccidioidomycosis and guides future One Health-based educational initiatives.

OBJECTIVE: Coccidioidomycosis, or Valley fever, is a climate-driven fungal disease. Dogs are valuable sentinels for human disease. We sought to evaluate veterinarian-reported clinical encounters with coccidioidomycosis in dogs and the relative awareness of coccidioidomycosis by veterinarians in endemic versus nonendemic states. METHODS: Survey responses were solicited from 45,240 veterinarians subscribing to the Veterinary Information Network in June/July 2024. For US-based veterinarians, data were gathered on the number of coccidioidomycosis cases evaluated, recall of education on coccidioid

Enyzme immunoassay compared to immunodiffusion complement-fixation to assess the IgG serological response in coccidioidomycosis.

Enzyme immunoassay (EIA) is often the initial test performed for diagnosing coccidioidomycosis by serology. When EIA immunoglobulin G (IgG) optical density (OD) positive results were compared to immunodiffusion complement-fixation (IDCF) titer among 339 unique patient samples, there was only moderate correlation (R2 = 0.5345; P < 0.001). In 68 cases (20.1%), the EIA IgG was positive when the IDCF result was negative. When the medical records of 22 patients at one site were reviewed, only 14 (63.6%) with positive EIA IgG tests were associated with clinically active coccidioidomycosis. These res

DECTIN-1 stalk length determines binding ability, immune response, and survival in a mouse model of coccidioidomycosis.

Coccidioidomycosis is a leading cause of pneumonia in the endemic area of the Sonora desert and the central valley of California. The causative agent are the fungi Coccidioides immitis and C. posadasii. While most infections ultimately resolve without treatment, some infections spread to the periphery and cause serious and even fatal illness. Much work has shown that a T cell response with production of interleukin-17A and interferon ɣ is critical. Less work has focused on innate responses that ultimately set the stage for the adaptive response. Genetic analysis in both mice and man have shown

Evaluation of a Rapid Lateral Flow Assay for Coccidioidomycosis to Monitor Antibody Levels in Patients Using Fingerstick Capillary Blood.

The diagnosis of coccidioidomycosis is often achieved serologically by the detection of antibodies against fungal antigens. While several serologic tests are available for coccidioidomycosis, all of them are performed in a laboratory setting causing delays in diagnosis and therapeutic intervention. Point-of-care testing offers the ability to provide a shorter time to result by avoiding specimen send-out, minimizing processing steps, and employing expeditious immunochemical techniques. A preliminary trial of a rapid anti-coccidioidal antibody lateral flow assay (LFA) using fingerstick blood was
