Coccidioides posadasii is a pathogenic fungus endemic to the arid regions of the Americas, causing coccidioidomycosis, a debilitating respiratory infection. Coccidioides infection is caused by breathing arthroconidia generated by mycelia that grow in desert soil. Arthroconidia develop inside the lung into massive, multinucleated spherules that are pathognomonic. Despite its significant impact on public health, there remains a paucity of research investigating novel therapeutic approaches to combat this disease. Addressing this research gap, our study employs a multi-faceted approach integratin

Coccidioidomycosis (CM) is an emerging public health challenge due to an increasing number of cases in the U.S. Diagnosis may be challenging because of its nonspecific presentation, which may lead to treatment delays. In addition, the optimal management of severe pulmonary CM remains unclear. We present the case of an 18-year-old immunocompromised renal transplant recipient with a history of systemic lupus erythematosus who developed acute respiratory distress syndrome (ARDS) secondary to severe pulmonary CM. Following initiation of systemic corticosteroids treatment with standard-of-care anti

Suspected macrolide-refractory Mycoplasma pneumoniae pneumonia can be a diagnostic challenge in immunocompromised hosts, particularly after allogeneic stem cell transplantation. Screening serologies may further complicate diagnosis when false-positive or discordant results divert therapy. We report a 63-year-old woman with B-cell acute lymphoblastic leukemia status postallogeneic stem cell transplantation and hypogammaglobulinemia who was transferred for persistent cough, exertional dyspnea, and right upper-lobe consolidation after limited improvement with azithromycin and broad-spectrum antib

Coccidioides is a dimorphic fungal pathogen that grows as a mold in soil and produces infectious arthroconidia. Inhaled arthroconidia transition to spherules containing endospores in mammalian hosts. How distinct developmental forms of Coccidioides interact with immune cells remains poorly defined. Here, we show that arthroconidia activates TLR2 and induces NLRP3-pyrin inflammasomes in macrophages, while ferroptotic signaling promotes arthroconidia killing. Upon transition to spherules, only ruptured spherules-not intact ones-trigger IL-1β production through NLRP3-pyrin inflammasomes and GSDMD

We present a case of a previously healthy 50-year-old man with a two-week history of progressive exertional dyspnea and intermittent sharp pain in the left chest and lower rib area. He then developed cough and low-grade fever, with chest X-ray imaging demonstrating possible left lower lobe consolidation suggestive of bacterial pneumonia. Given the geographic location where the patient lived, with environmental exposure to farming dust, and symptoms that did not improve with antibiotic treatment, alternative diagnoses were considered. The patient was eventually diagnosed with coccidioidomycosis

BACKGROUND: Patients with pulmonary coccidioidomycosis often experience prolonged symptoms lasting from weeks to months. Limited data exist regarding whether fluconazole prevents development of disseminated disease or shortens symptom duration. We describe factors associated with fluconazole receipt and assess its effect on outcomes among patients with pulmonary coccidioidomycosis.
METHODS: Using the MerativeTM MarketScan® Commercial Database, we identified immunocompetent patients ages 18-64 with incident pulmonary coccidioidomycosis during 2017-2023 and continuous enrollment in the 180 days

Coccidioidomycosis, or Valley fever, is a fungal respiratory disease caused by Coccidioides immitis or Coccidioides posadasii. Clinical manifestations range from asymptomatic infection to chronic pulmonary disease. Despite the increasing endemic range and affected population, therapeutic strategies display moderate efficacy and adverse side effects. This study explores California's rich botanical diversity as a source for agents that inhibit the early stages of Coccidioides development. We established a screening protocol containing a colorimetric metabolism assay and microscopic imaging. We t

Coccidioidomycosis is a systemic fungal infection caused by Coccidioides immitis and Coccidioides posadasii. Although well recognized in parts of the southwestern United States, it remains underdiagnosed in Mexico and Central America. While most cases present as self-limited pulmonary infection, approximately 1% of immunocompetent individuals develop disseminated disease, with coccidioidal meningitis (CM) being the most severe manifestation. We describe a 46-year-old immunocompetent male from Oaxaca, Mexico, who presented with progressive headache, neurocognitive decline, and altered mental st

Recent studies suggest coccidioidomycosis can be found outside regions not considered to be endemic for Coccidioides species. We reviewed our experience with coccidioidomycosis at a large quaternary medical center in southeastern Michigan, an area not typically considered to be endemic for this infection. In the last decade, we cared for 18 patients with proven (11) or probable (7) coccidioidomycosis. All patients had a history of travel or prior residence in areas known to be endemic for Coccidioides species; for 16, the presumed source of exposure to Coccidioides was in southern Arizona. The

Coccidioidomycosis is most commonly a self-limited pulmonary infection. Disseminated disease is rare, occurring in fewer than 1% of cases, with skeletal involvement representing an uncommon manifestation in children. We report a case of skeletal coccidioidomycosis in a previously healthy 12-year-old with pelvic involvement and no antecedent pulmonary symptoms. The patient presented with 1.5 months of progressive, atraumatic left pelvic pain, night sweats, and weight loss, without respiratory complaints. Radiographs demonstrated a lytic lesion of the left ilium extending toward the acetabulum,

Coccidioidomycosis is an endemic mycosis in some regions of the Americas including the southwestern United States with recognized potential for extra-pulmonary dissemination. Nevertheless, its neuroradiologic spectrum in children, particularly with respect to longitudinal evolution and head and neck involvement, remains incompletely characterized. We retrospectively reviewed n=16 pediatric patients with definitive or probable infection evaluated at a single quaternary care center in an endemic region, analyzing clinical/laboratory data and serial neuroimaging findings. Most children were previ

A shipping container yard worker from Taiwan contracted laboratory-confirmed pulmonary coccidioidomycosis. He reported no travel history to endemic regions. Occupational exposure to aerosolized dust or soil residues on maritime shipping containers from endemic areas is likely. Our findings highlight international shipping as a potential pathway for introducing geographically restricted pathogens into nonendemic regions.

Coccidioidomycosis (Valley fever) is acquired through inhalation of spores produced by fungi in the genus Coccidioides. Coccidioides is commonly detected in and cultivated from the lung tissue of native rodents and the soils within their burrows. Coccidioidomycosis is acquired by exposure to environmentally produced spores and is not spread between hosts. Thus, determining the location of rodent burrows with soils harboring Coccidioides will be critical for understanding coccidioidomycosis incidence and modeling how Coccidioides distributions will be affected by global change. Coccidioides is

Coccidioides spp are a dimorphic fungi that are endemic to the southwestern United States. Infection with Coccidioides spp, known as coccidioidomycosis or commonly known as Valley fever, is primarily caused by inhalation of spores, with most cases occurring in California and Arizona. Patients presenting with coccidioidomycosis can have widely varying clinical manifestations, ranging from asymptomatic carriage to severe disseminated disease. Occasionally, a pulmonary cavitary lesion can form as part of pulmonary infection, occurring approximately in 5% of patients with pulmonary coccidioidomyco

Coccidioidomycosis is an endemic fungal infection that rarely presents as isolated osteoarticular disease in immunocompetent hosts. We report a case of disseminated coccidioidomycosis manifesting as chronic septic arthritis of the tibiofemoral joint without apparent pulmonary involvement in a 32-year-old male. Diagnostic delay and initial immunosuppressive therapy contributed to disease progression. Histopathology confirmed the diagnosis and combined surgical debridement and prolonged antifungal therapy led to favorable outcomes. This case highlights the importance of considering fungal etiolo

BACKGROUND: While most pulmonary infections with Coccidioides spp. are self-limited, a subset of patients is at higher risk for severe disease and extrapulmonary dissemination, including individuals with certain ethnic backgrounds. Disseminated disease can involve soft tissues, bones, and the central nervous system with high morbidity and mortality in the absence of antifungal treatment. Diagnosis of coccidioidomycosis can be challenged by the availability of specimens for culture or histopathology, confusion with, or lack of availability of various antibody tests and overall lack of considera

A 47-year-old healthy man developed disseminated coccidioidomycosis involving his mediastinal and cervical lymph nodes that was complicated by pericardial involvement despite continued fluconazole for primary pulmonary infection. Although he subsequently received an aggressive antifungal regimen with voriconazole and liposomal amphotericin B, he did not clinically improve until he underwent major debridement and drainage, highlighting the need for source-control procedures in some cases of severe disseminated coccidioidomycosis.

BACKGROUND: Coccidioidomycosis is the most prevalent endemic mycosis in the United States. Diagnosis of coccidioidomycosis can be challenging due to limitations of conventional diagnostics. Detection of fungal plasma cell-free DNA (cfDNA) is a novel non-invasive testing modality for the diagnosis of invasive fungal disease. In this study, we characterized the performance of Coccidioides plasma cfDNA PCR.
METHODS: A retrospective study was conducted in patients with suspected coccidioidomycosis who underwent plasma cfDNA PCR testing and conventional fungal testing for C. immitis and C. posadas

Coccidioidomycosis (CM) is primarily a pulmonary disease but can also present with significant extrapulmonary disease. Extrapulmonary manifestations vary across organ systems, but most frequently presents as infection of the bone, brain and skin. Due to the variety of clinical presentations in disseminated disease, CM can be underrecognized, resulting in a diagnostic conundrum. We report a case of a non-immunocompromised patient initially presenting for weight loss with imaging findings mimicking peritoneal carcinomatosis, ultimately found to have CM with peritoneal involvement.
