BACKGROUND: Drug rash with eosinophilia and systemic symptoms syndrome is a severe T-cell-mediated adverse drug reaction characterized by a skin rash, fever, hematologic abnormalities, and internal organ involvement following prolonged exposure to a causative medication. Drugs associated with drug rash with eosinophilia and systemic symptoms syndrome include anticonvulsants, allopurinol, antibiotics, and nonsteroidal anti-inflammatory drugs. Fluconazole is an exceedingly rare cause of drug rash with eosinophilia and systemic symptoms syndrome, with only one previously reported case in abstract

Coccidioidomycosis is a fungal infection endemic to arid regions. Although it usually presents as a pulmonary infection, extrapulmonary dissemination is rare, and cervical lymphadenitis is even more unusual with fewer than five cases reported in the literature. Diagnosis of this disease remains a clinical challenge due to its rarity and similarity to other clinical entities. A 34-year-old woman with lupus nephritis on immunosuppressive therapy presented with a painful cervical mass associated with unquantified fever of six weeks of evolution. The patient is a resident of Coahuila, Mexico, in a

Coccidioidomycosis ("Valley fever") is a fungal disease that causes a wide range of illness severity in animals and people. Here, we use U.S. Census data, surveys of pet ownership demographics, and results of a nation-wide dog serologic study to estimate the financial burden of the disease to dog owners. We estimate that the one-year cost of diagnosis and treatment for uncomplicated pulmonary coccidioidomycosis in dogs across 6 endemic southwestern U.S. states ranged from $60,117,875 to $74,858,713 before adjusting for inflation. The financial burden to dog owners for uncomplicated illness was

Coccidioidomycosis (CM) has been a recognized disease for about 130 years. The organisms (Coccidioides spp. fungi) inhabit desert soil in the southwestern USA, Mexico, and parts of Central and South America. Natural events such as dust storms, wildfires or outdoor activities including construction and gardening can disrupt the fungal arthroconidia, which easily become airborne and inhaled by the host. Approximately 60% of those exposed to arthroconidia are asymptomatic and do not require medical attention, but 30% show signs of pulmonary infection with symptoms ranging from a flu-like illness

BACKGROUND: Valley fever (VF) is a common cause of community-acquired pneumonia in the American Southwest. Diagnosis is challenging due to VF's similarities to other pulmonary diseases. The VF T-cell response is important to control the disease, and measuring this response holds promise as an improved diagnostic. We aim to identify of VF T-cell epitopes, and to develop disease and efficacy diagnostics for the clinic and vaccine trials.
METHODS: We identified VF-specific T-cell epitopes by using their ability to bind the major histocompatibility complex (MHC) class II molecules and T-cell stim

Coccidioidomycosis, or Valley fever, is an emerging respiratory disease caused by soil-dwelling fungi of the Coccidioides genus that is expected to spread from the southwest into the central U.S. by 2050. While 60% of infections are asymptomatic, the other 40% of patients experience a range of symptoms, from self-limiting pneumonia to life-threatening disseminated disease. The immunological events that underlie the progression to severe disease remain underdefined. Here, we probed the early immune response to Coccidioides using a high dose of an attenuated strain of Coccidioides posadasii in a

BACKGROUND: Only a small number of antifungal therapies for invasive fungal disease (IFD) are currently available, and many pathogens are resistant to one or more of these therapies. Olorofim, the first orotomide antifungal agent to be developed, is active against fungi that are resistant to registered therapies. It impairs fungal pyrimidine biosynthesis, leading to cell death. We sought initial data on the efficacy and safety of olorofim as a therapy for IFD.
METHODS: In this single-arm, open-label, phase 2b study, patients aged 16 years or older with few or no treatment options for proven I

Coccidioides species are thermally dimorphic fungal pathogens that cause coccidioidomycosis (Valley Fever) primarily in North and South America. Coccidioides grow as hyphae that differentiate into arthroconidia, which can be aerosolized upon soil disturbance, and inhaled by the mammalian host to cause pulmonary infections with occasional dissemination to other organs. In the context of mouse models, current methods of infection include intranasal, intravenous, and intraperitoneal delivery of the arthroconidia into mice. To explore an aerosol route of infection, we compared the intranasal metho

Microbiome studies report a decrease in diversity associated with active infections. Under the endozoan hypothesis, Coccidioides can inhabit a host without causing disease. In this study, we describe and compare the lung mycobiome of Coccidioides-positive and -negative samples obtained from wildlife. If Coccidioides is not causing infection, we predict there will be no differences in the mycobiome between positive and negative samples. Lung samples were obtained from mammals previously trapped in Tucson, Arizona, USA (n = 26), and Mesa, Arizona, USA (n = 14). Samples were screened for Coccidio

Respiratory fungal infections pose a significant threat to human health. Animal models do not fully recapitulate human disease, necessitating advanced models to study human-fungal pathogen interactions. In this study, we utilized primary human airway epithelial cells (hAECs) to recapitulate the lung environment in vitro and investigate cellular responses to two diverse, clinically significant fungal pathogens, Aspergillus fumigatus and Coccidioides posadasii. To understand the mechanisms of early pathogenesis for both fungi, we performed single-cell RNA sequencing of infected hAECs. Analysis r

This case report explores the consequences of ruxolitinib via inhibition janus kinase 1 (JAK1) and JAK2 pathways in the context of fungal defense in a patient diagnosed with pulmonary coccidioidomycosis during ruxolitinib therapy for polycythemia vera. The patient experienced a relapse of pulmonary coccidioidomycosis after antifungal treatment was discontinued while continuing ruxolitinib use. This case illustrates the heightened risk of discontinuing antifungal therapy in endemic regions, emphasizing the critical need for continued monitoring. Furthermore, this case underscores the vital role

BACKGROUND: Coccidioidomycosis is an endemic fungal infection in the southwestern United States that can present as solitary lung nodules, mimicking malignancy on imaging. Molecular testing, such as PCR, offers rapid diagnosis but its performance in this clinical setting remains unclear.
METHODS: We conducted a retrospective analysis of patients evaluated for lung nodules at a tertiary care community-based lung nodule clinic in Central California between 2011 and 2025. Coccidioides PCR in patients with proven or probable Coccidioidomycosis was compared to those with biopsy-proven lung cancer.

TOPIC IMPORTANCE: Endemic fungal infections are increasingly recognized as important causes of community-acquired pneumonia. Despite this, diagnosis is often delayed or misattributed, resulting in significant morbidity and mortality.
REVIEW FINDINGS: Histoplasma capsulatum, Blastomyces species, and Coccidioides species are the most common endemic fungal infections in the United States. These infections share commonalities in modes of transmission and pathogenicity where regional climates, weather patterns, and certain exposures play a key role in infectivity. However, changes in climate and h

BACKGROUND: The dimorphic mycoses (DMs) histoplasmosis, blastomycosis, and coccidioidomycosis are classically thought to be geographically restricted to specific regions of the United States. The current scientific consensus is that these geographic boundaries are expanding, but the extent of this expansion remains uncertain.
METHODS: This study is a retrospective analysis of >21 million (annually) commercially insured individuals within the United States during 2007-2016. Diagnosis of histoplasmosis, coccidioidomycosis, and blastomycosis were identified using International Classification of

BACKGROUND Blastomycosis is a fungal infection caused by Blastomyces spp., a dimorphic fungus hyperendemic in Wisconsin, USA. CASE REPORT A 54-year-old previously healthy man presented with right foot and thigh pain, erythema, and a non-healing purulent ulcer on the right buttock. Initial treatment with oral doxycycline and cephalexin was unsuccessful, and subsequent imaging revealed a thigh hematoma and acute osteomyelitis of the forefoot with an adjacent abscess. The patient underwent 2 incision-and-drainage procedures, which uncovered a disarticulated second toe and persistent purulence. Hi

There are scarce data comparing inpatient mortality, length of stay (LOS) and all-cause hospital costs in disseminated coccidioidomycosis (DCM) vs. isolated pulmonary coccidioidomycosis (IPCM). We assessed the burden of hospital illness associated with DCM versus IPCM. This study was performed using National Inpatient Sample data from 2019 to 2021. DCM was defined as having a primary International Classification of Diseases-Tenth Revision (ICD-10) code for coccidioidal meningitis, a non-primary code for coccidioidal meningitis in the presence of a primary code for a meningitis complication or

Coccidioidomycosis is predominantly a pulmonary disease, however dissemination can include unique cutaneous presentations. We report a previously healthy 11-year-old female with delayed diagnosis of cutaneous scalp coccidioidomycosis with disseminated disease with secondary wound myiasis and bacterial superinfection. She required extensive surgical debridement, eventually requiring a skin graft and prolonged hospitalization for antifungal therapy, experiencing a unique adverse effect.

Disseminated peritoneal coccidioidomycosis is rare and can mimic peritoneal carcinomatosis. Ustekinumab, an IL-12/23 inhibitor, impairs Th1/Th17 immunity which is critical for fungal control. No prior cases link ustekinumab use to disseminated fungal disease. We report a 45-year-old woman from an endemic region with prior pulmonary coccidioidomycosis and psoriatic arthritis on ustekinumab who presented with malignant-appearing ascites and omental caking. Fluid studies were non-diagnostic, but laparoscopic biopsies confirmed disseminated Coccidioides infection. She improved with liposomal ampho
