An elderly patient with recently diagnosed Parkinson's disease presented with disseminated coccidioidomycosis (DCM) manifesting as ulcerative nasal lesions, pulmonary involvement and acute-on-chronic cognitive decline. Culture-confirmed cutaneous disease, positive serology, and imaging demonstrated pulmonary and central nervous system (CNS) involvement, establishing the diagnosis. The patient achieved marked clinical improvement with high-dose oral fluconazole (800 mg, later tapered to 400 mg daily), with serial imaging documenting resolution of CNS and pulmonary lesions. Lymphocyte subset ana

INTRODUCTION: The function of the respiratory microbiome during an active infection is not well characterized. Studies from the gut microbiome suggest a diverse community can aid in modulating the immune system to control infectious pathogens.
METHODS: To determine if there are microbial community compositional changes in the human lung during an infection, we conducted an analysis of both the 16S rDNA and the Internal Transcribed Spacer (ITS) region of DNA from bronchoalveolar lavage fluid (BALF) of patients from Mayo Clinic Arizona. In addition to general classification, we assessed differe

Coccidioidomycosis is caused by the dimorphic fungus Coccidioides, endemic to the southwestern United States. Most infected patients are asymptomatic or recover uneventfully, but a small proportion will develop chronic infection or dissemination. Diagnosis typically rests on organism demonstration in tissue or serology, but knowledge of Coccidioides infection imaging findings becomes important when tissue examination is not possible and serologic results are unclear. Acute coccidioidal pneumonia manifests at chest imaging with consolidation and lymphadenopathy, resolving completely or evolving

Coccidioides is a dimorphic fungus that causes coccidioidomycosis, also known as San Joaquin Valley Fever. The fungus is endemic to the southwestern United States, northern Mexico, and Central and South America. Infection is typically acquired through inhalation of dust particles that causes pulmonary disease. Direct cutaneous inoculation can occur but is rare. Histopathological examination of cutaneous lesions typically shows pseudoepitheliomatous hyperplasia with acute, suppurative granulomatous inflammation. Fungal organisms tend to be found more superficially and vary in density. Although

We encountered a case of chronic pulmonary coccidioidomycosis complicated with meningitis in a 53-year-old African-American veteran with a history of staying in Arizona, United States of America. The patient was referred from another hospital with complaints of cough, blood-stained phlegm, hemoptysis, and headache. Chronic pulmonary coccidioidomycosis was suspected based on the presence of multiple pulmonary cavity lesions on computed tomography (CT) and a relevant exposure history, and the diagnosis was confirmed by serological antibody tests (coccidioidomycosis IgG, positive; coccidioidomyco

Contemporary data on coccidioidomycosis death rates are sparse. Death certificate data for 2018-2023 from the US National Vital Statistics System were evaluated. Coccidioidomycosis deaths were identified using diagnosis codes B38.x listed anywhere on certificates. Deaths and age-adjusted mortality rates (AAMRs)/1,000,000 people, with 95% confidence intervals (CIs), were determined. We identified 1760 coccidioidomycosis-attributable deaths (AAMR = 0.75; 0.72-0.79). Most occurred in 55-74-year-olds (43.9%; corresponding AAMR = 1.72; 1.59-1.84). Males had a 2.69-fold increased AAMR versus females

BACKGROUND: Coccidioides fungemia is an uncommon but severe manifestation of disseminated coccidioidomycosis, associated with high mortality. Despite high endemicity in California's San Joaquin Valley, recent data from this region are limited. This study aims to describe the clinical characteristics, risk factors, and outcomes of patients with Coccidioides fungemia at a Central California tertiary care center.
METHODS: We conducted a retrospective review of patients diagnosed with Coccidioides fungemia between 2015 and 2025 at Community Regional Medical Centers (CRMC) in Fresno, California. D

Introduction. Coccidioidomycosis, or Valley fever, is a fungal disease caused by Coccidioides species, prevalent in parts of the southwestern United States. It usually results from inhaling spores from soil and is a common cause of pneumonia in these regions. Case Presentation. We present a unique case of coccidioidomycosis in an immunodeficient male patient secondary to human immunodeficiency virus infection with poor adherence to anti-retroviral treatment. After presenting with non-specific symptoms and pre-syncope, he was initially diagnosed with pneumonia based on chest X-ray findings, but

INTRODUCTION: Pediatric Infectious Disease (PID) clinicians involved in solid organ transplantation often assess infection risk and mitigation strategies for donor organ offers. While some guidance is available, real-life practice patterns have not been previously described.
METHODS: We surveyed PID clinicians about organ acceptance and associated posttransplantation interventions using 12 fictitious pediatric case scenarios through 3 PID-specific listservs. Descriptive statistics were employed.
RESULTS: 48 (71.6%) of 67 ID respondents were involved in organ offer assessment. Agreement was s

Coccidioidomycosis, endemic in the southwestern United States, can lead to severe pulmonary complications, particularly in chronic obstructive pulmonary disease (COPD) patients with poor lung reserves. Mycobacterium abscessus has a predisposition for structurally damaged lungs, commonly causing difficult-to-treat bronchiectasis and cavitary lesions. We present the case of a 58-year-old patient with advanced COPD and a remote history of pulmonary coccidioidomycosis diagnosed 30 years earlier, who was found to have sputum and radiographic evidence of M. abscessus cavitary lung disease 4 years pr

BACKGROUND: Balamuthia mandrillaris is a rare cause of skin and/or central nervous system (CNS) infection with a high mortality rate. Dissemination to other organs, including the lungs, is rare. Despite worldwide distribution, this disease entity is underrecognized. Delay in diagnosis may contribute to high mortality.
CASE SUMMARY: A 17-year-old Hispanic male living in Arizona on immunosuppressive therapy for Castleman's disease presented with new cutaneous lesions and cough and was found to have miliary pulmonary disease on chest imaging. He was suspected of having disseminated coccidioidomy

INTRODUCTION: Shock is a life-threatening condition amongst hospitalized patients and requires urgent management to avoid mortality. Early exposure is vital for educational and patient safety purposes.
METHODS: We developed a 90-minute shock day session that provided internal medicine interns with a cognitive framework for the initial diagnosis and management of shock, which they applied to two simulations. The first simulation involved a patient with septic shock, and the second involved a patient with cardiogenic shock. Critical action checklists were used to assess learners and guide struc

Coccidioides is the fungal causative agent of Valley fever, a primarily pulmonary disease caused by inhalation of fungal arthroconidia, or spores. Although Coccidioides has been an established pathogen for 120 years and is responsible for hundreds of thousands of infections per year, little is known about when and where infectious Coccidioides arthroconidia are present within the ambient air in endemic regions. Long-term air sampling programs provide a means to investigate these characteristics across space and time. Here we present data from > 18 months of collections from 11 air sampling sit

Coccidioidomycosis is caused by Coccidioides posadasii (Cp) and Coccidioides immitis (Ci), which have a 4-5% difference in their genomic sequences. There is an urgent need to develop a human vaccine against both species. A previously created recombinant antigen (rCpa1) that contains multiple peptides derived from Cp isolate C735 is protective against the autologous isolate. The focus of this study is to evaluate cross-protective efficacy and immune correlates by the rCpa1-based vaccine against both species of Coccidioides. DNA sequence analyses of the homologous genes for the rCpa1 antigen wer

BACKGROUND: Severe coccidioidomycosis presenting with respiratory failure is an uncommon manifestation of disease. Current knowledge of this condition is limited to case reports and small case series.
METHODS: A retrospective multicenter review of patients with coccidioidomycosis-associated acute respiratory distress syndrome (CA-ARDS) was conducted. It assessed clinical and laboratory variables at the time of presentation, reviewed the treatment course, and compared this cohort with a national database of patients with noncoccidioidomycosis ARDS. PURPOSE OF REVIEW: This review highlights the epidemiology, pathogenesis and clinical management of pulmonary infections caused by emerging fungal organisms.
RECENT FINDINGS: Emerging fungal infections have arisen as a result of population and environmental changes. An enlarging pool of immunocompromised hosts on triazole antifungal prophylaxis has led to an increased incidence of non- Aspergillus molds, such as Fusarium , Scedosporium and Lomentospora spp. Advances in diagnostic capabilities led to the identification of the Emergomyces genus and non- dermatitidis Blastomyces species, which h

This study explores the complex interplay between coccidioidomycosis (valley fever) and sarcoidosis through a detailed case study of a 54-year-old male patient. The patient presented with elevated calcium levels, chronic kidney disease (CKD), and unintended weight loss. Interdisciplinary collaboration between nephrologists and pulmonologists played a crucial role in navigating the intricate medical challenges, including hypercalcemia, renal dysfunction, and pulmonary anomalies. The diagnostic journey involved extensive laboratory findings uncovering the involvement of both infectious agents an
