From inequality gradients to priority bands: Translating preterm birth inequalities into targeted universalist policy in England.

Preterm Births (PTBs) are viewed as one of the major causes of neonatal morbidity, mortality and health-system cost in England. The incidence of preterm is persistently greatest among those experiencing socioeconomic and ethnic inequalities. This study aims to quantify deprivation and ethnicity gradients, using the available dataset, in PTBs and subsequently to translate deprivation-related inequality into policy-relevant priority bands. It analyses aggregated live-birth data for England (2020-2021), stratified separately by gestational age × Index of Multiple Deprivation (IMD), and gestationa

Single Persistent Left Superior Vena Cava Draining Into the Left Atrium Causing Neonatal Cyanosis: A Case Report.

BACKGROUND: The persistence of the left superior vena cava (PLSVC) is the most common variant of systemic venous drainage. It is usually diagnosed through echocardiography, with the PLSVC typically draining into the right atrium via the coronary sinus without hemodynamic consequences. In cases of a single PLSVC (without right superior vena cava), it generally drains into the right atrium. However, when it drains directly into the left atrium, systemic venous return bypasses the right atrium, causing cyanosis and hypoxemia. CLINICAL FINDINGS: Term newborn was admitted to the neonatal intensive

The relationship between prolonged travel and mortality among women with complicated deliveries in rural Kenya.

BACKGROUND: In many low- and middle-income settings, deliveries often take place in primary health facilities that are not equipped to address life-threatening complications. Emergency referral systems seek to rapidly transport women and newborns facing complications but inevitably introduce delays. Estimating the mortality impact of these delays is challenging due to potential selection bias. As an alternative approach, we examine the relationship between delays and adverse outcomes among a sample that is less likely to exhibit selection effects: mothers who started at a low-level facility be

Impaired lymphatic drainage in a child with hydrops fetalis and a novel variant in the PROX1 gene: case report and review of literature.

PROX1 is a gene that encodes a protein that may play a key role in the development of the lymphatic system. This report describes impaired lymphatic drainage, with non-immune hydrops fetalis, congenital bilateral chylothorax, chylous ascites, and the need for peritoneal dialysis in a preterm neonate born at 32 weeks of gestation. The child died at about seven months of life. Clinical Exome Sequencing revealed the novel heterozygous variant c.502C > T in the PROX1 gene, which determines the change p.Arg168Trp at the protein level. This de novo missense variant was not detected in the analyzed p

Antenatal Rh Immune Globulin Dose, Timing, and Indications After Selected Procedures and Complications: A Scoping Review.

OBJECTIVE: Studies addressing RhD alloimmunization after antenatal procedures and complications are limited, and practice of RhD immune globulin (RhIG) administration after these events is variable. Our objective was to assess evidence for RhIG efficacy for RhD-negative pregnancies after selected antenatal events. DATA SOURCES: Literature was sourced from databases, including MEDLINE, EMBASE, Cochrane Evidence Based Medical Reviews, and Cumulative Index to Nursing and Allied Health Literature (1946 to August 2024). STUDY SELECTION: Studies included those focused on RhIG effectiveness after a

Diet-Associated Regulation of Cardiac Metabolism: Molecular Determinants and Pathophysiological Consequences.

The heart is a highly energy-demanding organ that depends on metabolic flexibility to adjust substrate utilization in response to changes in nutrient availability, endocrine signals, and energetic demands. Accumulating evidence demonstrates that dietary patterns are key determinants of myocardial metabolic homeostasis, affecting substrate selection, mitochondrial function, nutrient-sensing pathways, and long-term transcriptional and epigenetic regulation. This review analyzes the molecular mechanisms through which diet regulates cardiac metabolism and explores how chronic nutritional exposures

Placenta-pulmonary coupling-guided multimodal AI for fetal lung maturity staging and individualized glucocorticoid therapy.

BACKGROUND AND AIM: Deep learning has improved medical image analysis but often produces opaque decisions and correlation-driven predictions that may diverge from clinical reasoning. We hypothesize that a physiology-informed hybrid framework, which explicitly models placenta-pulmonary interactions and integrates multimodal data, could provide interpretable and reliable guidance for assessing fetal lung maturity (FLM) and optimizing antenatal glucocorticoids (GCs). MATERIALS AND METHODS: In a prospective cohort study involving 320 pregnancies-including 160 with hypertensive disorders of pregna

Society for Maternal-Fetal Medicine Consult Series #75: Evaluation and management of non-immune hydrops fetalis.

Non-immune hydrops fetalis (NIHF) can result from a multitude of underlying causes, such as fetal genetic diseases, congenital anomalies, infections, fetal arrhythmias, placental tumors, monochorionic twin complications, and other disorders. Management is complex and rooted in the preferences of the pregnant individual and the known or suspected etiology of NIHF. In this Consult, we review general considerations and a contemporary approach to the diagnosis, evaluation, and management of NIHF, providing recommendations based on the available evidence. The following are the Society for Maternal-

Burden of Rhesus isoimmunization and pregnancy outcomes: a cross-sectional study conducted at Kenyatta National Hospital, Kenya.

INTRODUCTION: the ABO and Rhesus systems remain the most clinically significant blood group antigens on red blood cell membranes. Rhesus isoimmunization occurs when maternal Rh antibodies in a RhD-negative woman react with red blood cells of an RhD-positive fetus, leading to adverse fetal and neonatal outcomes. METHODS: a cross-sectional review of 194 medical records of RhD-negative pregnant women managed at Kenyatta National Hospital (KNH) between 2013 and 2019 was conducted. Data on sociodemographic, obstetric, and clinical characteristics were extracted and analyzed using SPSS version 23.

Fetal cardiac function before labor predicts adverse perinatal outcome.

OBJECTIVES: Current antepartum surveillance based on fetal biometry and cerebroplacental ratio (CPR) has limited ability to identify fetuses at risk of intrapartum compromise, as adverse outcomes may occur despite normal growth and Doppler findings. This study evaluated whether quantitative fetal cardiac geometry and myocardial deformation assessed before induction of labor can identify high-risk fetuses with normal CPR at increased risk of adverse perinatal outcome. METHODS: In this prospective observational study, 53 singleton high-risk pregnancies undergoing medically indicated induction o

Anaphylaxis in pregnancy: diagnostic blind spots, epinephrine hesitancy, and emerging prevention strategies.

PURPOSE OF REVIEW: Anaphylaxis during pregnancy is uncommon but remains one of the most critical emergencies in obstetric medicine because maternal deterioration and fetal hypoxia evolve simultaneously. Despite established treatment recommendations, diagnostic uncertainty and persistent concerns regarding epinephrine use continue to contribute to jeopardize preventable morbidity/mortality. This review examines recent advances in the recognition, management, and prevention of pregnancy-associated anaphylaxis, with emphasis on emerging concepts that may improve maternal-fetal safety. Clinical Significance of Maternal Anti-D Antibody Titers in Severe RhD Alloimmunization Requiring Intrauterine Transfusion: A Tertiary Referral Center Experience.

Background: Maternal anti-D antibody titers are routinely used for risk stratification in RhD alloimmunization before the development of fetal anemia. However, their association with disease severity and perinatal outcomes among pregnancies that have already progressed to severe fetal anemia requiring intrauterine transfusion (IUT) remains unclear. This study aimed to investigate the association between maternal anti-D antibody titers, disease severity, and perinatal outcomes in pregnancies complicated by severe RhD alloimmunization requiring IUT. Methods: This retrospective cohort study inclu

Understanding stillbirths and maternal deaths in Pakistan: evidence from a national household survey.

BACKGROUND: The global impact of stillbirths and maternal deaths is enormous, with over two million deaths occurring each year, affecting public health outcomes and global development practices. Despite Pakistan's commitment to the Sustainable Development Goals, the country faces a substantial burden of stillbirths and maternal deaths. However, data on cause-specific mortality and the timing of deaths remains limited. In this context, we aimed to estimate the cause distribution and proportion of stillbirths (antepartum and intrapartum) and maternal deaths among married women aged 12-49 years i

Maternal Isoimmunization-Associated Fetal Anemia After First-Trimester Abortion: A Case Report and Literature Review.

Isoimmunization and fetal anemia pose considerable challenges in obstetrics, especially when there is a documented history of RhD incompatibility or other antibodies that may threaten fetal well-being. These conditions frequently result in serious complications, including hydrops fetalis, fetal anemia, and, in certain situations, fetal loss. Effective management of such pregnancies needs careful monitoring and prompt interventions, such as intrauterine transfusion (IUT), along with thorough post-delivery follow-up to safeguard both maternal and neonatal health. Due to ongoing uncertainties con

Timed and targeted transfusion in Rh isoimmunization: A case report on early detection and neonatal outcomes.

BACKGROUND: Rhesus (Rh) alloimmunization remains an important cause of fetal anaemia and perinatal morbidity despite the widespread use of anti-D immunoglobulin, particularly in regions with inadequate antenatal surveillance. Early detection of fetal anaemia using middle cerebral artery (MCA) Doppler ultrasonography and timely intervention are essential for improving neonatal outcomes. CASE PRESENTATION: We report the case of a 35-year-old multigravida (G3P2L1) with Rh-negative blood group, managed at a tertiary-care referral centre in India. She had a previous history of neonatal death attri

Locked Twins in a Primigravida: A Rare Obstetric Emergency Successfully Managed by Cesarean Section-A Case Report.

BACKGROUND AND AIMS: Locked twins are a rare and life-threatening obstetric complication, occurring in approximately 1 in 1000 twin pregnancies, most commonly during labor when the first twin presents as breech and the second as cephalic. This condition poses significant risks to both the mother and fetuses if not promptly recognized and managed. We report a rare case of chin-to-neck locked twins and highlight the importance of early diagnosis and meticulous intraoperative management. METHODS: A 20-year-old primigravida at 9 months of amenorrhea presented with pushing down pain for 12 h and r

The Importance of a Multidisciplinary Team Approach in a Rare Case of Antenatally Diagnosed Tonne-Kalscheuer Syndrome.

Tonne-Kalscheuer syndrome (TOKAS) is a rare X-linked neurodevelopmental disorder with very few reported cases. We describe the case of a male neonate born at 27 weeks' gestation to a 41-year-old mother, with a diagnosis of TOKAS established antenatally through whole-exome sequencing after ultrasound revealed nuchal translucency, hydrops fetalis, structural cardiac abnormalities, and severe fetal growth restriction. The infant survived for nearly two hours after birth and was cared for and held by his parents and extended family. This case highlights the essential role of multidisciplinary coll
