Fetal abdominal circumference and adverse perinatal outcomes in pregnant individuals with pregestational diabetes.

BACKGROUND: Disturbances in fetal growth (ie, growth restriction and macrosomia) are associated with increased risk of perinatal morbidity and mortality. Although fetal abdominal circumference (AC) was recently added to the definition of fetal growth restriction, less is known about the performance of small AC specifically among pregnant individuals with diabetes as well as the clinical utility of large AC. Thus, we aimed to evaluate the association between fetal AC and adverse outcomes in pregnant individuals with pregestational DM. STUDY DESIGN: This was a retrospective cohort study of preg

Perinatal High-Fat Diet Induces Vascular Hypercontractility via DNA Hypomethylation of Endothelin Receptors and PKC-LTCC Axis Activation in Male Offspring.

Excessive fat intake is a well-established risk for hypertension; perinatal nutritional imbalances could increase vascular risks, but underlying mechanisms remain unclear. Endothelin-1 (ET-1), a potent vasoconstrictor, is implicated in cardiovascular diseases. This study aimed to address this gap by investigating ET-1 receptor-mediated pathways with high-fat-diet (HFD) during pregnancy and lactation. Vascular function, Ca2 + signaling, molecular expression, and DNA methylation were assessed in mesenteric arteries (MA) of offspring. HFD offspring exhibited increased fetal/adult weight, thickene

A Prospective Observational Study on Serum Lactate Dehydrogenase and Uric Acid Levels in Hypertensive Disorders of Pregnancy and Prediction of Fetomaternal Outcome.

Introduction Hypertensive disorders of pregnancy are a major cause of maternal and perinatal morbidity and mortality, particularly in low- and middle-income countries. Early identification of women at risk using simple biochemical markers may aid in timely intervention and improved outcomes. Serum lactate dehydrogenase (LDH) and uric acid have been proposed as indicators of disease severity in hypertensive pregnancies. The study aimed to evaluate the association of serum LDH and uric acid levels with the severity of hypertensive disorders of pregnancy and their relationship with maternal and f

Maternal Overnutrition and Fetal Programming: Long-Term Metabolic, Cognitive, and Epigenetic Consequences.

Maternal nutrition during pregnancy critically influences fetal programming, shaping the offspring's lifelong health and disease susceptibility. Both undernutrition and overnutrition affect fetal metabolism, predisposing offspring to obesity and cardiometabolic disorders in adulthood. This review examines current evidence on how maternal nutrition, particularly overnutrition and its complications, such as gestational diabetes mellitus (GDM) and obesity, affects offspring health. It also explores the biochemical and epigenetic mechanisms underlying aberrant fetal programming induced by an unfav

Temporal Trends in Preterm Birth Associated With Hypertensive Disorders of Pregnancy in Victoria, Australia: A Population-Based Interrupted Time-Series Study.

OBJECTIVES: Hypertensive disorders of pregnancy (HDP) are a leading reason for medically indicated preterm birth (PTB), yet their contribution to population-level PTB trends is poorly understood. We examined temporal trends in PTB overall and HDP-associated PTB in Victoria over an 11-year period spanning the COVID-19 pandemic, and distinguished changes in HDP prevalence from changes in preterm delivery among affected pregnancies. STUDY TYPE: Population-based interrupted time-series study using seasonal autoregressive integrated moving average (SARIMA) models, with July 2018 (launch of a natio

Z-Drug Use in the First Trimester of Pregnancy and Risk of Congenital Malformations.

IMPORTANCE: Sleep disturbances are common in pregnancy and often treated with nonbenzodiazepine sedative hypnotics (Z-drugs). However, there is limited evidence on the fetal safety of Z-drugs. OBJECTIVE: To evaluate whether Z-drug exposure in the first trimester of pregnancy is associated with an increased risk of congenital malformations. DESIGN, SETTING, AND PARTICIPANTS: This US population-based cohort study evaluated health care utilization data from publicly insured beneficiaries in the Medicaid database (2000-2018) and commercially insured beneficiaries in the Merative MarketScan datab

Impact of Prepregnancy Body Mass Index in Low-Risk Pregnancies with Adequate Gestational Weight Gain: Trend and Adverse Outcomes.

OBJECTIVE: To examine the temporal trend of prepregnancy body mass index (BMI) and the association between prepregnancy BMI and adverse outcomes in low-risk pregnancies with adequate gestational weight gain (GWG). STUDY DESIGN: We conducted a retrospective cohort study using U.S. vital statistics data (2016-2023). We included low-risk pregnancies with adequate GWG that delivered nonanomalous, singleton live births at 37 to 41 weeks of gestation in a hospital; those with diabetes or hypertensive disorders were excluded. The exposure variable was prepregnancy BMI. Adverse outcomes were examined

Maternal Haemoglobin and Haematocrit During Pregnancy and Stillbirth Risk: A Prospective, Population-Based Birth Cohort Study.

BACKGROUND: Maternal anaemia is a public health issue globally, associated with adverse maternal and neonatal outcomes. However, less is known about the effects of high haemoglobin (Hb) and haematocrit (Hct) on the risk of stillbirth. OBJECTIVES: We assessed the dose-response effects of dynamic changes in maternal Hb and Hct across all trimesters on stillbirth risk. METHODS: This study was based on the prospective, longitudinal, population-based China birth cohort study (CBCS) between 2018 and 2022, including 142,715 women with live births or stillbirths delivered at ≥ 20 weeks' gestation. Decision-to-delivery interval in emergency cesarean deliveries: real-world data from a tertiary obstetric care center in Turkey.

OBJECTIVE: International guidelines recommend decision-to-delivery intervals (DDIs) of ≤30 minutes (min) for emergency cesarean deliveries (ECDs), whereas the German Society recommends ≤20 min. However, the feasibility and relevance of these thresholds vary across and within healthcare systems. This study assessed the association between DDI and perinatal outcomes in ECDs performed during and outside regular working hours at a tertiary obstetric care center in Turkey. METHODS: This single-center, retrospective study included 835 ECDs with complete medical records out of 11,014 deliveries cond

TDCIPP induces placental dysfunction and fetal growth restriction via oxidative stress-mediated PINK1/Parkin mitophagy.

Tris (1,3-dichloro-2-propyl) phosphate (TDCIPP) is a widely used organophosphate flame retardant increasingly linked to reproductive toxicity, yet its placental toxic mechanisms remain unclear. This study demonstrates that TDCIPP exposure disrupts placental homeostasis by inducing oxidative stress, which triggers excessive PINK1/Parkin-mediated mitophagy in both human trophoblasts (HTR-8/SVneo) and pregnant mice. Integrated network toxicology analysis predicted mitochondrial dysfunction and oxidative stress as central pathways, validated by upregulation of ATG5 and HMOX1, downregulation of SOD

Pro-inflammatory diet in pregnancy associated with increased risk of abnormal perinatal outcomes: evidence from dietary inflammatory index in the UK biobank.

OBJECTIVES: This study aimed to investigate the association between a pro-inflammatory diet in pregnancy, assessed via the Dietary Inflammatory Index (DII), and abnormal perinatal outcomes (APOs), including miscarriage and stillbirth. Leveraging the UK Biobank cohort, we evaluated whether higher DII scores were associated with increased APOs risk. Additionally, we explored interactions between DII, lifestyle factors (physical inactivity, smoking exposure), and socioeconomic determinants to identify modifiable pathways for reducing APOs. METHODS: We analyzed 10,353 women from the UK Biobank wi

Diagnosis and management of primary fetal pleural effusion: A narrative review.

Primary fetal pleural effusion involves the accumulation of fluid in the fetal pleural space, posing considerable challenges for perinatal diagnosis and management. Primary effusions, emerging from the maldevelopment of lymphatic vessels, differ significantly from secondary effusions, which are associated with a spectrum of anomalies and conditions, affecting clinical approach and prognosis. The diagnosis of primary fetal pleural effusion is one of exclusion. The natural history of the condition ranges from spontaneous resolution in mild cases to progression to life-threatening complications.

Association of hypothyroidism with adverse pregnancy outcomes: a single-center cohort study in an iodine-sufficient country.

Hypothyroidism is reported to associate with adverse pregnancy outcomes. However, there are few reports in iodine-sufficient or -excess areas. We investigated the association of hypothyroidism in early pregnancy with pregnancy outcomes in an iodine-rich area, Japan. We conducted a retrospective cohort study at a tertiary center between 2004 to 2013. The eligible participants were classified into three groups: euthyroidism, subclinical hypothyroidism (SCH), or overt hypothyroidism (OH) groups, based on their thyroid hormone levels in the first trimester of pregnancy (median 11.6 weeks, IQR 10.6

Implementation of physiological interpretation of fetal heart rate changes: from scientific principles to frontline practice.

Onset of uterine contractions which become progressively more frequent, intense and last for longer durations as the labor progresses is expected to cause a gradually evolving hypoxic stress to human fetuses. This is because of the repeated constriction of maternal spiral arterioles supplying the placental bed and compression of the umbilical cord as the labor advances. The majority of fetuses are able to mount physiological compensatory responses to protect their high priority central organs by maintaining aerobic metabolism. However, fetuses who are exposed to preexisting compromise such as
