Implementation of physiological interpretation of fetal heart rate changes: from scientific principles to frontline practice.

Onset of uterine contractions which become progressively more frequent, intense and last for longer durations as the labor progresses is expected to cause a gradually evolving hypoxic stress to human fetuses. This is because of the repeated constriction of maternal spiral arterioles supplying the placental bed and compression of the umbilical cord as the labor advances. The majority of fetuses are able to mount physiological compensatory responses to protect their high priority central organs by maintaining aerobic metabolism. However, fetuses who are exposed to preexisting compromise such as

Intrapartum Point-of-Care Ultrasound May Prevent Cord Prolapse in Active Labor: Report of a Case.

INTRODUCTION: Intrapartum point-of-care ultrasound (POCUS) is increasingly employed to assess fetal head position and labor progression. We describe a case in which intrapartum POCUS enabled the early detection of a compound cord presentation, preventing umbilical cord prolapse. CASE PRESENTATION: A 33-year-old primigravida at 39+4 weeks in active labor showed a persistent right occiput posterior position with slow progress despite adequate contractions. After a failed manual rotation attempt, transperineal ultrasound revealed a hypoechoic tubular structure lying between the fetal head and pu

Evaluation of the management of high-risk obstetric patients living with epilepsy: A retrospective study from KwaZulu-Natal, South Africa.

BACKGROUND: Epilepsy affects approximately 60 million individuals worldwide, with women comprising nearly 50% of the cases. Physiological changes during pregnancy can alter anti-epileptic drug (AED) pharmacokinetics and seizure thresholds, necessitating close therapeutic drug monitoring (TDM). Uncontrolled maternal seizures increase the risk of maternal morbidity, foetal hypoxia and miscarriage. Several AEDs have a known teratogenic potential, highlighting the importance of adherence to clinical guidelines to optimise maternal and foetal outcomes. METHODS: A retrospective quantitative study w

Mitochondrial dysfunction and immune microenvironment in gestational diabetes mellitus: insights from bioinformatics analysis and experimental validation.

BACKGROUND: Gestational diabetes mellitus (GDM) is a pregnancy-related disorder characterized by inflammatory dysregulation that disrupts maternal-fetal immune homeostasis, yet the contribution of mitochondrial dysfunction to this pro-inflammatory state remains incompletely understood. METHODS: This study combined transcriptomic data obtained from the GEO repository and mitochondrial gene lists from MitoCarta3.0 to pinpoint mitochondrial-related genes (Mito-RGs) exhibiting differential expression in GDM. Machine learning algorithms, including the least absolute shrinkage and selection operato

Artificial intelligence-based prediction of fetal hypoxia: a multicenter model development and nationwide AI-human comparison.

BACKGROUND: Fetal hypoxia is a leading cause of neonatal morbidity and mortality. Cardiotocography (CTG) is widely used to predict fetal hypoxia during labor, but its interpretation remains suboptimal. Artificial intelligence (AI) models have been developed for CTG interpretation, but their clinical utility is limited by two major challenges: demonstrating superiority over human experts and ensuring explainability in real-world settings. METHODS: A large dataset containing CTG traces from three tertiary hospitals between January 2014 and May 2022 was built for model development. Deep learning

Single Persistent Left Superior Vena Cava Draining Into the Left Atrium Causing Neonatal Cyanosis: A Case Report.

BACKGROUND: The persistence of the left superior vena cava (PLSVC) is the most common variant of systemic venous drainage. It is usually diagnosed through echocardiography, with the PLSVC typically draining into the right atrium via the coronary sinus without hemodynamic consequences. In cases of a single PLSVC (without right superior vena cava), it generally drains into the right atrium. However, when it drains directly into the left atrium, systemic venous return bypasses the right atrium, causing cyanosis and hypoxemia. CLINICAL FINDINGS: Term newborn was admitted to the neonatal intensive

Placenta-pulmonary coupling-guided multimodal AI for fetal lung maturity staging and individualized glucocorticoid therapy.

BACKGROUND AND AIM: Deep learning has improved medical image analysis but often produces opaque decisions and correlation-driven predictions that may diverge from clinical reasoning. We hypothesize that a physiology-informed hybrid framework, which explicitly models placenta-pulmonary interactions and integrates multimodal data, could provide interpretable and reliable guidance for assessing fetal lung maturity (FLM) and optimizing antenatal glucocorticoids (GCs). MATERIALS AND METHODS: In a prospective cohort study involving 320 pregnancies-including 160 with hypertensive disorders of pregna

Anaphylaxis in pregnancy: diagnostic blind spots, epinephrine hesitancy, and emerging prevention strategies.

PURPOSE OF REVIEW: Anaphylaxis during pregnancy is uncommon but remains one of the most critical emergencies in obstetric medicine because maternal deterioration and fetal hypoxia evolve simultaneously. Despite established treatment recommendations, diagnostic uncertainty and persistent concerns regarding epinephrine use continue to contribute to jeopardize preventable morbidity/mortality. This review examines recent advances in the recognition, management, and prevention of pregnancy-associated anaphylaxis, with emphasis on emerging concepts that may improve maternal-fetal safety. Locked Twins in a Primigravida: A Rare Obstetric Emergency Successfully Managed by Cesarean Section-A Case Report.

BACKGROUND AND AIMS: Locked twins are a rare and life-threatening obstetric complication, occurring in approximately 1 in 1000 twin pregnancies, most commonly during labor when the first twin presents as breech and the second as cephalic. This condition poses significant risks to both the mother and fetuses if not promptly recognized and managed. We report a rare case of chin-to-neck locked twins and highlight the importance of early diagnosis and meticulous intraoperative management. METHODS: A 20-year-old primigravida at 9 months of amenorrhea presented with pushing down pain for 12 h and r

Lactate values during labour and their correlation with maternal and fetal outcome.

ABSTRACT: This study investigates the relationship between lactate levels - produced during myometrial contraction and transient hypoxia - and maternal and fetal outcomes. Uterine contractions during labour are regulated by hormonal and metabolic processes, including oxytocin release, catecholamine modulation, and the hypoxia-induced force increase (HIFI) mechanism. In a cohort of 303 women receiving neuraxial analgesia, lactate was measured at three time points: at the diagnosis of the active phase of the first stage of labour (T0), at the beginning of active pushing (T1), and at delivery (T2

Placental abnormalities of malperfusion, inflammation, and meconium are associated with neonatal pulmonary vascular disease.

OBJECTIVES: The primary objective was to elucidate whether placental histopathology representative of intrauterine insults during pulmonary vascular development are associated with a diagnosis of persistent pulmonary hypertension of the newborn (PPHN). The secondary objective was to compare placental histopathologic lesions across PPHN etiologies. STUDY DESIGN: We conducted a case-control study of mother-infant dyads ≥35 weeks gestation who delivered at a tertiary care center between 2020 and 2025. Cases were infants diagnosed with PPHN and treated with inhaled nitric oxide; unaffected contro

COVID-19 and pregnancy: is a "forgotten" disease still a threat?

Coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has significantly affected global health, including maternal and perinatal outcomes. Although the acute phase of the pandemic has subsided, the continued emergence of new viral variants, waning population immunity, and suboptimal vaccination coverage among women of reproductive age suggest that COVID-19 in pregnancy may remain a clinically relevant problem. This review summarizes current evidence regarding the clinical course, complications, and prevention of SARS-CoV-2 infection during

Construction of a prediction model for retinopathy of prematurity based on placenta-derived genes and clinical variables.

BACKGROUND: Retinopathy of prematurity (ROP) is a leading cause of childhood blindness, with rising incidence as neonatal care improves. Established risk factors include low birth weight, prematurity, and supplemental oxygen, yet current clinical prediction tools have limited early accuracy. The placenta, serving as the maternal-fetal interface, may harbor transcriptomic signatures reflecting intrauterine inflammation and hypoxia that influence ROP development; however, the specific placental genes involved and their predictive value remain largely unknown. This study aimed to identify placent

Severe Mitral Stenosis in Term Pregnancy Presenting During Labor in a Resource-Limited Setting: A Case Report from Borama, Awdal Region of Somalia.

BACKGROUND: Mitral stenosis (MS), primarily from rheumatic heart disease (RHD), is a significant cause of maternal morbidity and mortality, especially in low- and middle-income countries (LMICs). Pregnancy places considerable stress on the cardiovascular system, often revealing or worsening existing cardiac issues. Diagnosing severe MS in pregnant women with acute heart failure is particularly difficult in resource-limited settings lacking specialized diagnostic modalities. Here, we report a case of severe undiagnosed MS presenting in active labor. CASE PRESENTATION: We present a 28-year-old

Persistent pulmonary hypertension, neonatal stroke, coagulopathy and multi-organ failure due to severe enterovirus sepsis: two case reports.

BACKGROUND: Disseminated neonatal enteroviral infection is a rare and life-threatening condition with significant multi-organ involvement. It often presents with nonspecific symptoms that may be mistakenly attributed to bacterial infections. Prompt recognition and treatment are crucial to prevent rapid disease progression. We report two cases of disseminated neonatal enterovirus, including a novel instance of persistent pulmonary hypertension (PPHN) and neonatal stroke. CASE PRESENTATION: A baby boy was delivered vaginally at 35 + 4 weeks, complicated by meconium-stained liquor. He developed r

Dual clinical dilemmas in late pregnancy: A multidisciplinary conservative approach to high-risk pulmonary embolism with persistent category III fetal status.

Pregnancy increases the risk of venous thromboembolism, and pulmonary embolism (PE) remains a leading cause of maternal morbidity and mortality. Diagnosis is challenging due to symptom overlap with normal pregnancy, and management is complex in severe cases without hemodynamic instability. A 32-year-old woman at 32 weeks' gestation presented with dyspnea, chest pain, and hypoxemia. CT pulmonary angiography confirmed a saddle PE with right ventricular strain. Despite high oxygen requirements, she remained hemodynamically stable. She was managed in the intensive care unit with therapeutic antico

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Advancements in Fetal Heart Rate Monitoring: A Report on Opportunities and Strategic Initiatives for Better Intrapartum Care.

Cardiotocography (CTG), introduced in the 1960s, was initially expected to prevent hypoxia-related deaths and neurological injuries. However, more than five decades later, evidence supporting the evidence of intrapartum CTG in preventing neonatal and long-term childhood morbidity and mortality remains inconclusive. At the same time, shortcomings in CTG interpretation have been recognised as important contributory factors to rising caesarean section rates and missed opportunities for timely interventions. An important limitation is its high false-positive rate and poor specificity, which underm
