A multi-faceted hospital-based intervention for intrapartum care in sub-Saharan Africa: a stepped-wedge cluster-randomized trial.

Around three million babies are stillborn or die shortly after birth each year worldwide, underscoring a persistent and unacceptable burden on families and health systems. To help address this challenge, we developed the facility-based, multi-faceted Action Leveraging Evidence to reduce perinatal Mortality and Morbidity (ALERT) intervention. ALERT promoted essential practices of intrapartum childbirth using (i) co-design, (ii) training, (iii) quality improvement and (iv) leadership mentoring. We evaluated ALERT using a stepped-wedge cluster-randomized trial with five 6-month periods across 16

Effect of meso-and micro-level quality improvement interventions to improve intrapartum care and birth outcome in Nepal- stepped wedge cluster randomized trial.

BACKGROUND: High-quality intrapartum care benefits from implementation of quality improvement (QI) interventions. However, scale-up of meso- and micro-level QI interventions to improve health facility performance have shown mixed results in Nepal. OBJECTIVE: This study aims to evaluate the effect of such interventions to intrapartum care and intrapartum mortality (intrapartum stillbirth and first day neonatal death) in public health facilities of Nepal. STUDY DESIGN: A stepped-wedge cluster randomized design was applied to implement the SUSTAIN package, comprising meso- and micro-level QI in

Pregnancy after renal transplant: Truth or dare? A retrospective analysis from a tertiary care centre in India.

INTRODUCTION: Women with end-stage kidney disease (ESKD) often face fertility challenges due to hypothalamic-pituitary-gonadal axis disruption. While pregnancy is rare during dialysis, kidney transplantation restores fertility potential, albeit with increased maternal and foetal risks. This study evaluates maternal and foetal outcomes in post-renal transplant pregnancies at a tertiary care centre. METHODOLOGY: A retrospective study was conducted at our hospital on female renal transplant recipients between January 2010 and January 2024. All patients who conceived post-transplant were included

Factors Related to Late Intrauterine Fetal Death in a Tertiary Level Hospital: A Retrospective Study.

BACKGROUND: Intrauterine fetal death (IUFD) is an unfortunate perinatal occurrence with serious global and local impacts. Despite improved diagnostic methods, a significant fraction of stillbirths remains unexplained, especially in resource-constrained settings such as Nepal. Thus, the present study aimed to determine the frequency and associated characteristics of late intrauterine fetal death (?28 weeks) in a tertiary-level hospital in Nepal. METHODS: A retrospective descriptive study was conducted at Dhulikhel Hospital, Nepal, from January 2024 to January 2025. Medical records from 36 wome

Determinants of Delivery Hospital Choice in Georgia, 2016-2021: Maternal Characteristics, Hospital Attributes, and Time Price.

BACKGROUND: US maternal mortality and severe maternal morbidity (SMM) are marked by substantial disparities by race, payer, and geography. To the extent that such disparities are driven by differences across hospitals rather than within hospitals, understanding what shapes delivery hospital choice can inform policy responses. OBJECTIVES: To examine how distance, hospital attributes, and maternal characteristics jointly shape delivery hospital choice, and whether patterns differ between urban Atlanta and smaller Georgia markets. RESEARCH DESIGN: Retrospective observational study using a condi

Mechanical Thrombectomy for High-Risk Pulmonary Embolism in Pregnancy: A Case Report and a Systematic Review of the Literature.

Pulmonary embolism (PE) in pregnancy is a major clinical challenge with potentially fatal maternal and fetal outcomes. Mechanical thrombectomy (MT) offers a promising alternative when thrombolysis is contraindicated, although evidence in pregnancy is limited. We describe a 26-year-old at 29 + 5 weeks' gestation with high-risk PE causing acute right heart failure, successfully treated, achieving rapid hemodynamic recovery, and minimal fetal radiation exposure. A systematic review identified 15 MT cases in pregnancy or peripartum (11 antepartum, 4 postpartum), reporting 100% maternal survival an

Clinical Characteristics Associated with Fetomaternal Hemorrhage: A Retrospective Multicenter Descriptive Study (2009-2026).

Fetomaternal hemorrhage refers to the transfer of fetal erythrocytes into the maternal circulation. Although clinically significant fetomaternal hemorrhage is rare, it can result in severe fetal anemia with substantial morbidity and mortality. Early recognition remains challenging due to a nonspecific clinical presentation. This study aimed to identify maternal and obstetric characteristics associated with fetomaternal hemorrhage that may serve as early clinical warning signs. In this retrospective multicenter descriptive study, singleton pregnancies with a positive Kleihauer-Betke test from f

Incision Decision: Vertical or Horizontal Laparotomy in Traumatically Injured Pregnant Patients.

Background Trauma is the leading cause of non-obstetric mortality in pregnant patients. While guidelines exist for the overall management of pregnant trauma patients, there is currently no consensus regarding the optimal laparotomy incision type in those requiring urgent operative intervention. Objective The purpose of this study is to compare the presentation, operative management, and outcomes between pregnant trauma patients who underwent a vertical versus a horizontal laparotomy incision for urgent operative intervention. Study design A retrospective cohort review was conducted at an Ameri

Universal Prenatal cfDNA-Single Gene Disorder Screening for Autosomal Dominant Conditions in a Low-Risk Cohort: A Cost-Effectiveness Analysis.

OBJECTIVE: The use of cell free fetal DNA has expanded to include autosomal dominant single gene conditions (cfDNA-SGD). Our aim is to estimate the cost-effectiveness of a cfDNA-SGD panel in a general obstetric population as compared to offering this test only in the setting of anomalies. METHODS: Using TreeAge software, we created a decision-analytic model to compare outcomes of the universal cfDNA-SGD. The screening conditions included in the current panel were separated into tiers (1, 2, 3) based on severity and life expectancy of the conditions. Outcomes included miscarriage after diagnos

Introducing the Moyo device for intrapartum fetal heart rate monitoring among women with high-risk pregnancies in a low-resource setting: a quasi-experimental study.

OBJECTIVE: To assess the effect of introducing the Moyo device for continuous intrapartum fetal heart rate monitoring (FHRM) on the detection of abnormal fetal heart rate patterns and perinatal mortality. DESIGN: We conducted a quasi-experimental quality improvement project using a before-and-after approach. During the preimplementation phase, intermittent FHRM was done using the existing standard of care (Pinard or a hand-held fetal Doppler). In the postimplementation phase, we introduced the Moyo device (Laerdal Global Health, Stavanger, Norway) for continuous intrapartum FHRM. We used the

Psychological experiences of midwives during their first involvement in care following intrauterine foetal death: A mixed methods study in Japan.

BACKGROUND: Providing care following intrauterine foetal death (IUFD) can be emotionally challenging for midwives, particularly during early professional practice. However, little is known about how midwives experience and cope with their first involvement in IUFD care. AIM: To examine midwives' retrospective accounts of their first IUFD care experience within the first 3 years of practice and explore their coping processes. METHODS: A convergent mixed methods design was used. Forty-one midwives in Japan completed questionnaires assessing emotional responses to their first IUFD care and prof

Placental lesions in systemic lupus erythematosus: A retrospective histopathological study in a tertiary care center.

Systemic lupus erythematosus (SLE) is a chronic inflammatory autoimmune disease of unknown etiology. Pregnancy in patients with SLE, however, poses high-risk factors stemming from both maternal (such as lupus flare and preeclampsia) and fetal (including miscarriage and intrauterine fetal death) complications. Although the pathophysiology underlying obstetric mortality and morbidity in SLE is still under investigation, recent studies have suggested that placental dysfunction may significantly contribute. This study investigated the perinatal outcomes of pregnant women with SLE and the histopath

Improving early detection of critical congenital heart disease in a high-performing prenatal screening system: the OXYNAT interventional cohort study.

BACKGROUND: Early detection of critical congenital heart disease (CHD) is essential to prevent cardiovascular collapse and improve neonatal outcomes. While prenatal screening has advanced significantly in high-income countries, postnatal detection remains a safety net, particularly for left-sided lesions. Although pulse oximetry (PO) screening has been evaluated in several settings with high prenatal detection rates, the added value of combining it with systematic paediatric examination, and its impact on care pathways remains incompletely characterised in such contexts. OBJECTIVE: To evaluat
