Early initiation of low-dose aspirin for the prevention of pre-eclampsia in high-risk pregnancies.

Pre-eclampsia and its related consequences last to be significant factors in maternal morbidity and mortality, particularly in high-risk pregnant women. Aspirin has revealed promise in reducing these risks, particularly when initiated early in gestation. This study evaluated the effectiveness of 75 mg aspirin, started before 12 weeks gestation, in reducing adverse pregnancy outcomes among high-risk women. We achieved a randomized controlled experiment with high-risk pregnant women. Pregnant women were randomly allocated to receive either 75 mg of Aspirin or a placebo. Daily from the point of e

Medical complications and psychosocial outcomes of adolescent pregnancy in high-income countries: a scoping review.

BACKGROUND: Adolescent pregnancy is highly prevalent in high-income countries. As this complex, multifaceted problem can be linked to multiple consequences, pregnancy in this population poses significant challenges for health and social service professionals. However, there is a lack of integrated knowledge on the topic. Thus, this study aimed to synthesize the literature published over the last decade on the medical complications and psychosocial outcomes of pregnancy, delivery, and postpartum in the 15-19 age group in high-income countries, including fetal complications. METHOD: A scoping r

Maternal Mortality Secondary to Maternal Sepsis in Ontario: A Call-to-Action.

Maternal sepsis remains a leading and rising cause of preventable maternal mortality in Ontario and globally. The Maternal and Perinatal Death Review Committee (MPDRC) investigates maternal deaths and issues recommendations; however, it remains unclear if these have been widely adopted. This review analyzes maternal deaths due to sepsis in Ontario from 2019 to 2022, as assessed by the Obstetric and Perinatal Death Review Committee, to identify recurring clinical themes and consolidate recommendations. Maternal sepsis cases were reviewed to extract common clinical features, pathogens, systemic

Why current risk factor-based approaches fall short in predicting stillbirth: a national cohort study of nulliparous women in England.

BACKGROUND: Stillbirth is a profound and devastating outcome of pregnancy that has a long-lasting emotional and physiological impact on parents and families. Current risk assessment approaches largely rely on maternal characteristics and clinical history, yet their predictive accuracy remains poor, particularly among nulliparous women (women with no previous birth beyond 24 weeks of gestation). We evaluated the extent to which routinely collected pregnancy risk factors can predict stillbirth and assessed their contribution among singleton births in nulliparous women. METHODS: We conducted a p

Perinatal mortality in eastern Africa: A systematic review and meta-analysis.

BACKGROUND: Every day, over 5000 stillbirths and more than 6000 newborn deaths are estimated to occur worldwide, the majority in sub-Saharan Africa and South Asia. Estimates, however, rely on data that might underestimate these deaths. Further, outside major categories such as preterm birth, infection, and complications at delivery, little is known about the causes of death for newborns and even less for stillbirths. OBJECTIVES: This systematic review and meta-analysis aimed to synthesize evidence on the incidence, causes, and risk factors for perinatal mortality in East Africa to inform publ

Chorioamnionitis in intrauterine fetal death: A forensic histopathological case study.

INTRODUCTION: Acute chorioamnionitis is a placental inflammatory condition often implicated in adverse pregnancy outcomes, including intrauterine death. The histopathological identification of maternal and fetal inflammatory responses (MIR and FIR) is critical for diagnosis and understanding potential causal mechanisms. Chorioamnionitis frequently warrants medico-legal investigation, particularly for suspected medical malpractice. MATERIAL AND METHODS: Two forensic cases of intrauterine fetal death with histological finding of acute chorioamnionitis are presented. Each case has been investiga

Adverse obstetric and perinatal outcomes among Palestinian adolescent mothers in the West Bank: a retrospective cohort study.

BACKGROUND: It is estimated that 13% of adolescent girls globally give birth before the age of 18. Several studies have shown that pre-eclampsia, eclampsia, puerperal infections, preterm birth, low birth weight, and neonatal death are more common in adolescent mothers than in adult mothers. This study aimed to investigate whether adolescent pregnancy increases the risk of adverse obstetric and perinatal outcomes compared with pregnancies among women aged 20-35 years in the West Bank, occupied Palestinian territory. METHODS: A retrospective cohort study utilizing data extracted from the electr

The magnitude and population burden of educational inequalities in adverse birth outcomes.

We aimed to describe the magnitude and population burden of inequality in adverse birth outcomes by maternal education (five categories). Data from 639,007 singleton births of gestational age ≥ 24 weeks or, if missing, birthweight ≥ 500 g were included from the National registry of the Netherlands, 2016-2019. One in six Dutch births had an adverse outcome. Each step down the educational ladder was associated with higher adverse outcome rates. Inequalities in stillbirth and neonatal mortality rates were large between the highest (Master's or higher) and lowest (primary) educated groups (rate ra
