"How do I navigate the joy and the grief at the same time?" - Mothers' postnatal experiences following care in a specialist pregnancy after loss clinic: A qualitative study.

BACKGROUND: In a subsequent pregnancy after perinatal death, mothers likely experience anxiety, depression, grief, and post-traumatic stress. Personalised antenatal care at a specialist clinic supports mothers; however, little is known about their experiences after birth. Mothers may have anxiety and fear postnatally, and face challenges transitioning from specialist antenatal care to standard postpartum care, however this transition remains unstudied. AIM: To explore mothers' postnatal experiences in a subsequent pregnancy after previously experiencing perinatal death. METHODS: A qualitativ

Understanding stillbirth: a retrospective cohort study using WHO ICD-PM criteria at a tertiary perinatal center in Germany.

OBJECTIVES: The aim of this study was to identify maternal and fetal risk factors associated with stillbirth and to differentiate intrauterine fetal death (IUFD) from late termination of pregnancy (TOP). METHODS: Analyses were performed on births from 2014 to 2024 at Charité Berlin, characterizing the overall cohort, IUFD, and TOP, respectively. Descriptive characterizations and regressions were applied to evaluate the impact of modifiable risk factors and trends within the overall study cohort and 3rd trimester IUFDs. RESULTS: Three hundred and eighty five stillbirths and 58,631 live births

Placental apelin expression is low in early-onset fetal growth restriction but there is no association between maternal circulating apelin and pregnancy outcome.

INTRODUCTION: Fetal growth restriction increases adverse pregnancy outcomes including preterm birth and intrauterine death. Apelin is a secreted peptide expressed in placental syncytiotrophoblast and downregulated in fetal growth restriction. We tested the hypothesis that adverse pregnancy outcome is associated with low maternal plasma apelin at diagnosis of early-onset fetal growth restriction. METHODS: Plasma samples and fetomaternal blood flow Doppler velocimetry measurements were obtained from pregnant women (n = 59) at diagnosis of early-onset fetal growth restriction in the second trime

Perinatal outcomes following single intrauterine death in monochorionic twin pregnancies complicated by twin anemia polycythemia sequence: Systematic review and meta-analysis.

INTRODUCTION: Most of the studies reporting the outcome of the surviving twin after single intra-uterine fetal death (sIUFD) focused on uncomplicated pregnancies or those affected by twin-to-twin transfusion syndrome, while there is a paucity of data for those complicated by twin anemia polycythemia sequence (TAPS). The aim of this systematic review was to report perinatal outcomes in monochorionic diamniotic (MCDA) twin pregnancies complicated by TAPS after sIUFD, according to gestational age at fetal loss and the presence of pregnancy-related comorbidities. MATERIAL AND METHODS: MEDLINE, EM

Exploring pathways leading to stillbirths and gaps in postnatal care among affected women in a rural north Indian district: A qualitative study using the social autopsy lens.

BACKGROUND: Globally, stillbirth is a silent yet significant contributor to perinatal mortality. India has several national maternal and child health programs, yet, the stillbirths remain poorly reviewed, subject to suboptimal surveillance, are under-reported, and often absent from many programmatic priorities. This study aimed to explore the pathways, delays leading to stillbirths and continuum of care in postpartum phase among affected women in a rural district of North India using a social autopsy tool. METHODS: This qualitative study was a component of an ongoing implementation research t

Involving mothers in monitoring their unborn babies during labour: experience from Liberia.

AIM: To assess the impact on perinatal mortality and morbidity of involving mothers in partnership with maternity staff in monitoring their unborn babies. METHODS: Mothers attending 4 hospitals in Liberia consented and were trained to use doppler ultrasound probes to monitor for abnormalities or changes in fetal heart rates during labour. A range of 120 to 160 beats per minute was chosen as normal. Intermittent auscultation was undertaken sequentially, immediately following the end of every uterine contraction. RESULTS: One hundred eleven intrauterine fetal deaths occurred before mothers arr

Levels of in-facility maternal mortality, obstetric complications and stillbirths in three urban referral hospitals in Guinea: analysis of routine data from May 2021 to April 2023.

INTRODUCTION: Despite global efforts to reduce maternal mortality, progress has stalled, particularly in sub-Saharan Africa, where the maternal mortality ratio remains high. This study aims to assess in-facility maternal mortality, maternal and foetal complications, and stillbirths in referral hospitals at different levels of the health system in Guinea. METHODS: This was a cross-sectional study conducted at three urban referral hospitals in Guinea at the district (Hôpital de district de Forécariah or HD), regional (Hôpital Régional de Mamou or HR) and national (Hôpital National Ignace Deen o

Maternal safety outcomes of respiratory syncytial vaccination during pregnancy with a large-scale database.

This retrospective cohort study evaluated the risk of maternal safety outcomes, including preterm birth, following prefusion F protein (RSVpreF) during pregnancy compared with an unvaccinated cohort using large-scale real-world data. Propensity score matching was performed to compare the risks of preterm delivery, hypertensive disorder of pregnancy (HDP), gestational diabetes (GDM), oligohydramnios, placental abruption, intrauterine growth restriction (IUGR), and intrauterine fetal death (IUFD) to calculate the odds ratio (OR). Following the propensity score matching, 11,265 and 11,265 pregnan

Labor and Disability: A Nation-Wide Analysis of Pregnancy Outcomes after Spinal Cord Injury.

Spinal cord injury (SCI) is a challenging clinical entity necessitating multidisciplinary management. While the impact of SCI on male fertility is relatively well-understood, its impact on prepartum, peripartum, and fetal outcomes remains understudied. This study seeks to elucidate prepartum and delivery-related outcomes associated with a history of SCI in pregnant patients. We identified all pregnant patients admitted to United States hospitals with and without a history of SCI in the National Inpatient Sample from 2016 to 2019. For all patients, five outcomes were analyzed: in-hospital death

Adverse outcomes during delivery hospitalizations among patients with diabetes and an intellectual or developmental disability diagnosis.

BACKGROUND: Intellectual and developmental disabilities (IDD) are associated with increased risks for poor medical outcomes. However, there is limited national contemporary data on pregnancy outcomes in the setting of IDD and diabetes. OBJECTIVE: Diabetes in pregnancy (DIP) requires more intensive antenatal care including frequent glucose monitoring, medication adherence and titration, and more frequent visits. It is unclear whether IDD is an independent risk factor for adverse obstetric outcomes in this population. The purpose of this analysis was to evaluate whether IDD was associated with

Early neonatal transfers after moderate and late preterm birth: a nationwide cohort study based on the French National Health Data System.

OBJECTIVES: To investigate how level of maternity unit and clinical risk factors influence early neonatal transfer among moderate and late preterm (MLP; 32-36 weeks' gestation (WG)) infants. METHODS: We conducted a nationwide cohort study including all liveborn MLP infants in France between 2014 and 2021. Infants with congenital malformations, stillbirths, and out-of-hospital births were excluded. Neonatal transfers within 48 h after birth, with death before 48 h as a competing event, were analyzed by gestational age, maternity level (I/IIa/IIb/III) and major clinical risk groups, including m

Women's perspectives on fetal movement monitoring in high and low stillbirth settings: a mixed-methods study.

BACKGROUND: Maternal fetal movement monitoring during pregnancy is commonly advised to assess fetal wellbeing. However, qualitative research exploring how women perceive and implement such advice is lacking, particularly in regions with the highest burden of stillbirth. This study investigates women's experiences of, and opinions on fetal movement monitoring during pregnancy across both high and low stillbirth rate settings. METHODS: A convergent parallel mixed-methods design was used to explore women's perspectives on fetal movement monitoring. Participants (n = 234) from three low stillbirt

[Evaluation of a family-community workshop on birth and death: impact on parental attitudes].

OBJECTIVE: To evaluate a community-based workshop on birth and death, delivered by midwives for children aged 6 to 12 accompanied by one or both parents. Specifically, to analyse the adults' evaluation of the workshop and whether it modifies their attitudes towards how to approach these processes with children. METHOD: Evaluative non-experimental pre-post intervention and qualitative study conducted in community settings across nine Spanish provinces, primarily in Cantabria. Mothers and fathers (n=185) accompanied by their children (aged 6-12, n=218) attended the workshop "To be born, to die,

Postmortem examination after deaths in neonatal units in England and Wales, 2017-2022: a national observational study.

OBJECTIVE: Postmortem (PM) examination can be valuable after a baby's death, providing information about the cause of death and support for families. However, worldwide trends indicate declining neonatal PM rates. We aimed to analyse trends in PM offer and completion after deaths in neonatal units. METHODS: We used data from the National Neonatal Research Database on deaths in neonatal units in England, Wales and the Isle of Man (2017-2022). We used regression models to estimate rates of PM offer and completion and their association with selected clinical, socio-demographic and service-relate

Glucose circadian rhythm disruption is associated with preeclampsia.

Preeclampsia is a leading cause of maternal death during pregnancy, and the role of circadian rhythms in predicting preeclampsia is not well understood. We aimed to determine whether glucose circadian rhythm disruption is associated with preeclampsia and can be used to predict this disorder. We analyzed a dataset of 92 pregnant individuals recruited with Continuous Glucose Monitoring (CGM). To study rhythmicity, we performed a cosinor analysis using the packages cosinor and cosinor2, and we calculated the non-parametric circadian rhythm variables using the nparACT package in R. Furthermore, we

Lived Experiences of Labor and Delivery Nurses Caring for Mothers with Stillbirth.

PURPOSE: Stillbirth profoundly affects women and their partners. Caring for mothers after stillbirth is emotionally difficult for nurses; however, how the nurse feels and copes with this experience is underreported in the literature. The purpose of this study was to explore the lived experience of labor and delivery nurses caring for mothers with stillbirth. STUDY DESIGN AND METHODS: A qualitative study using Husserl's descriptive phenomenology was conducted. Labor and delivery nurses in the United States who cared for mothers with stillbirth were recruited through the Association of Women's
