Decision-to-delivery interval in emergency cesarean deliveries: real-world data from a tertiary obstetric care center in Turkey.

OBJECTIVE: International guidelines recommend decision-to-delivery intervals (DDIs) of ≤30 minutes (min) for emergency cesarean deliveries (ECDs), whereas the German Society recommends ≤20 min. However, the feasibility and relevance of these thresholds vary across and within healthcare systems. This study assessed the association between DDI and perinatal outcomes in ECDs performed during and outside regular working hours at a tertiary obstetric care center in Turkey. METHODS: This single-center, retrospective study included 835 ECDs with complete medical records out of 11,014 deliveries cond

Intrauterine interventions for severe fetal anemia due to parvovirus B19 national outbreak.

OBJECTIVE: Parvovirus B19 infection during pregnancy can cause severe fetal anemia, hydrops fetalis, and fetal death. This study describes our experience managing ten cases of severe fetal anemia due to parvovirus B19 infection during the 2023-2024 national outbreak in Israel. METHODS: This single-center descriptive cohort study included ten singleton pregnancies with fetal anemia (MCA-PSV >1.5 MoM) requiring intrauterine transfusion. Parvovirus B19 infection was confirmed by maternal IgM serology and/or amniotic fluid PCR. For pregnancies <20 weeks of gestation, we implemented a modified int

Pro-inflammatory diet in pregnancy associated with increased risk of abnormal perinatal outcomes: evidence from dietary inflammatory index in the UK biobank.

OBJECTIVES: This study aimed to investigate the association between a pro-inflammatory diet in pregnancy, assessed via the Dietary Inflammatory Index (DII), and abnormal perinatal outcomes (APOs), including miscarriage and stillbirth. Leveraging the UK Biobank cohort, we evaluated whether higher DII scores were associated with increased APOs risk. Additionally, we explored interactions between DII, lifestyle factors (physical inactivity, smoking exposure), and socioeconomic determinants to identify modifiable pathways for reducing APOs. METHODS: We analyzed 10,353 women from the UK Biobank wi

Implementation of physiological interpretation of fetal heart rate changes: from scientific principles to frontline practice.

Onset of uterine contractions which become progressively more frequent, intense and last for longer durations as the labor progresses is expected to cause a gradually evolving hypoxic stress to human fetuses. This is because of the repeated constriction of maternal spiral arterioles supplying the placental bed and compression of the umbilical cord as the labor advances. The majority of fetuses are able to mount physiological compensatory responses to protect their high priority central organs by maintaining aerobic metabolism. However, fetuses who are exposed to preexisting compromise such as

Disparities in maternal and fetal health among ethnogeographic groups-a retrospective cohort study of 30,000 women from a middle eastern multiethnic maternity population.

OBJECTIVES: Maternal ethnogeographic origins and ancestry can influence perinatal health. This study evaluated the impact of nationalities grouped into geographic categories on maternal and neonatal outcomes. STUDY DESIGN: The study employed a retrospective birth cohort design. METHODS: A total of 30,667 pregnant women and their neonates born at more than 24 weeks' gestational age in secondary and tertiary government maternity hospitals in Qatar were included. Data were extracted from a preexisting maternity registry. Women were classified into seven ethnogeographic groups based on internati

Associations between maternal injury and adverse perinatal outcomes: evidence from a retrospective cohort study.

BACKGROUND: Maternal injury affects 6% to 8% of pregnancies and is associated with perinatal complications. Some outcomes remain understudied, with limited evidence on variation of associations by injury characteristics. OBJECTIVE: To estimate associations between maternal injury and adverse perinatal outcomes. STUDY DESIGN: We conducted a retrospective cohort study using electronic health record data at the Atrium Health Wake Forest Baptist health system (2018-2024 births). We identified maternal injury using validated ICD-10-CM diagnostic codes and stratified injury by mechanism (transport

Spinal anesthesia for urgent cesarean section in a parturient with RYR1 mutation-associated malignant hyperthermia susceptibility: a case report.

INTRODUCTION: Malignant hyperthermia (MH) is a rare pharmacogenetic disorder triggered by volatile anesthetics and succinylcholine. MH susceptibility in pregnancy requires prospective anesthetic planning to ensure maternal and fetal safety. This case reports the successful management of an urgent cesarean section under spinal anesthesia in a parturient with pre-pregnancy genetic confirmation of MH susceptibility. MAIN SYMPTOMS AND CLINICAL FINDINGS: A 27-year-old primigravida (G1P0) at 38 weeks gestation presented with labor pain. She had a known pathogenic RYR1 variant (c.10561G > A) identif

A verbal and social autopsy study to improve estimates of mortality determinants in Pakistan: study design and methods.

BACKGROUND: Reducing newborn, child, adolescent, and maternal mortality is a global priority, yet Pakistan continues to bear a high burden of preventable deaths. Routine civil registration does not capture causes or social determinants for most of these deaths. The verbal and social autopsy (VASA) methodology fills this gap by combining symptom-based identification of the cause of death with structured assessment of social and care-seeking factors. Here we describe the methodological framework of a national VASA study conducted using the Pakistan National Nutrition Survey 2018 sampling frame,

A multi-target shield: Protective roles of Scutellaria baicalensis flavonoids in heavy metal-induced organ damage.

Environmental exposure to toxic heavy metals, including cadmium, lead and mercury, represents a significant global health concern, contributing to the pathogenesis of multi-organ dysfunction through interconnected mechanisms of oxidative stress, chronic inflammation and apoptotic cell death. While chelation therapy remains a conventional mainstay, its limitations underscore the need for complementary strategies that mitigate secondary cellular damage. This review synthesizes current evidence demonstrating that Scutellaria baicalensis Georgi (Huang Qin) and its principal flavonoids, including b

The relationship between prolonged travel and mortality among women with complicated deliveries in rural Kenya.

BACKGROUND: In many low- and middle-income settings, deliveries often take place in primary health facilities that are not equipped to address life-threatening complications. Emergency referral systems seek to rapidly transport women and newborns facing complications but inevitably introduce delays. Estimating the mortality impact of these delays is challenging due to potential selection bias. As an alternative approach, we examine the relationship between delays and adverse outcomes among a sample that is less likely to exhibit selection effects: mothers who started at a low-level facility be

Burden of Rhesus isoimmunization and pregnancy outcomes: a cross-sectional study conducted at Kenyatta National Hospital, Kenya.

INTRODUCTION: the ABO and Rhesus systems remain the most clinically significant blood group antigens on red blood cell membranes. Rhesus isoimmunization occurs when maternal Rh antibodies in a RhD-negative woman react with red blood cells of an RhD-positive fetus, leading to adverse fetal and neonatal outcomes. METHODS: a cross-sectional review of 194 medical records of RhD-negative pregnant women managed at Kenyatta National Hospital (KNH) between 2013 and 2019 was conducted. Data on sociodemographic, obstetric, and clinical characteristics were extracted and analyzed using SPSS version 23.

Understanding stillbirths and maternal deaths in Pakistan: evidence from a national household survey.

BACKGROUND: The global impact of stillbirths and maternal deaths is enormous, with over two million deaths occurring each year, affecting public health outcomes and global development practices. Despite Pakistan's commitment to the Sustainable Development Goals, the country faces a substantial burden of stillbirths and maternal deaths. However, data on cause-specific mortality and the timing of deaths remains limited. In this context, we aimed to estimate the cause distribution and proportion of stillbirths (antepartum and intrapartum) and maternal deaths among married women aged 12-49 years i

Timed and targeted transfusion in Rh isoimmunization: A case report on early detection and neonatal outcomes.

BACKGROUND: Rhesus (Rh) alloimmunization remains an important cause of fetal anaemia and perinatal morbidity despite the widespread use of anti-D immunoglobulin, particularly in regions with inadequate antenatal surveillance. Early detection of fetal anaemia using middle cerebral artery (MCA) Doppler ultrasonography and timely intervention are essential for improving neonatal outcomes. CASE PRESENTATION: We report the case of a 35-year-old multigravida (G3P2L1) with Rh-negative blood group, managed at a tertiary-care referral centre in India. She had a previous history of neonatal death attri

The use of the Edmonton Obesity Staging System compared to body mass index to predict adverse pregnancy outcomes.

BACKGROUND: In nonpregnant populations, obesity staging systems such as the Edmonton Obesity Staging System (EOSS) improve risk stratification of patients with obesity by also accounting for medical comorbidities. While the EOSS has been shown to better predict morbidity and mortality compared to body mass index (BMI)-based obesity classification in the nonpregnant population, its utility in pregnancy and performance compared to the current standard remain unclear. Thus, the objective of this study was to compare a modified EOSS and BMI-based obesity classification in predicting specific adver

Fetal and Neonatal Complications Associated With Acute Fatty Liver of Pregnancy.

Background Acute fatty liver of pregnancy (AFLP) is a rare but life-threatening obstetric emergency that can quickly endanger both the mother and fetus, typically presenting in the third trimester. Although greater awareness and supportive management have improved maternal survival, perinatal outcomes remain poor, particularly in resource-limited settings. This study aimed to describe fetal and neonatal complications associated with AFLP and identify maternal factors contributing to poor perinatal outcomes. Methods This retrospective study analysed medical records of 73 women diagnosed with AF
