The perinatal mortality rate of infants of diabetic mothers (IDMs) has declined dramatically from 250 per 1000 live births in the 1960s to a near-normal 20 per 1000 live births in the 1980s. Five to 8% of all IDMs suffer from major congenital malformations, and it is the latter that are responsible for 50% of these perinatal deaths. It has been shown that tight glycemic control prior to conception and during pregnancy can prevent an excess rate of congenital malformations, fetal macrosomia, birth trauma, and neonatal respiratory distress syndrome. We briefly review the short- and long-range co

OBJECTIVE: To investigate the concentrations of amniotic fluid erythropoietin in normal and risk pregnancies.
MATERIAL AND METHODS: The concentrations of erythropoietin were measured in 150 samples of amniotic fluid. The samples were obtained by amniocentesis and amniotomia and were analysed using an enzyme-linked immunosorbent assay (ELISA). Results were available within 6 hours. The intra-assay variation was 6.4%, the inter-assay variation 7.2%.
RESULTS: The range of erythropoietin concentration in all samples was between 0.23 and 80 U/L and in a defined group of normal pregnancies between

The purpose of this retrospective study was to determine the risk factors for the morbidity of the mothers and their fetus in patients with diabetic retinopathy and/or nephropathy with an open family planning. We compared the course of pregnancies, complications as well as the maternal and neonatal morbidity in 76 patients with diabetic retinopathy or nephropathy (White R F) with 85 patients without severe microangiopathy (White C D). We found a correlation between retinopathy progression and hyperglycaemia during the first trimester (p < 0.05). There was an increase in the deterioration of vi

OBJECTIVE: To test the hypothesis that long-term postnatal development may be modified by metabolic experiences in utero.
RESEARCH DESIGN AND METHODS: We enrolled offspring of women with pregestational diabetes (this included insulin-dependent diabetes mellitus [IDDM] and non-insulin-dependent diabetes mellitus [NIDDM]) and gestational diabetes in a prospective study from 1977 through 1983. Fetal beta-cell function was assessed by measurement of amniotic fluid insulin (AFI) at 32-38 weeks gestation. Postnatally, plasma glucose and insulin were measured yearly from 1.5 years of age after fasti

In the study covering 551 female inhabitants of Lodz, a model for clinical and control examinations was used in order to identify risk factors (estimated by means of odds ratio) for fetus hypotrophia and preterm birth. Logistic regression was employed to determine independent variables and those contributing directly to the occurrence of both pathologies. The following risk factors for preterm birth and fetus hypotrophia were identified: smoking during pregnancy, bleeding in more than one trimester of pregnancy and low weight gain by mothers during pregnancy. Late reporting to a doctor, uterus

OBJECTIVE: To determine 1) whether macrosomia in the fetus of the diabetic mother is related to fetal hyperinsulinemia and 2) whether hyperinsulinemia and macrosomia are related to maternal metabolic control.
RESEARCH DESIGN AND METHODS: Normal pregnant women (n = 95) were compared with insulin-treated pregnant women (n = 155), who were subdivided according to White's class, hypertension, and mode of delivery. All women were treated to achieve optimal metabolic control. HbA1c was determined at each visit. At delivery, umbilical plasma was analyzed for glucose, insulin antibodies, total insuli

The purpose of this study was to review the maternal and neonatal outcomes of patients with preterm premature rupture of membranes (PPROM) with a latency period greater than 7 days. Using the perinatal database, there were 11,007 deliveries in a 25-month period, in which 81 patients (0.7%) with singleton pregnancies were admitted with documented PPROM between 20 and 30 weeks' gestation. Twenty-one patients (26%) remained undelivered after 1 week, and 19 of these patients were discharged to outpatient care. Twelve patients (57%) delivered at term and nine patients (43%) delivered prematurely. I

There is a strong epidemiological association between poor nutritional status and low-birthweight (LBW) newborns. According to the World Health Organization (WHO), 95% of total LBW in the world are born in developing countries. Nevertheless, the nutritional etiology of intra uterine growth retardation (IUGR) is still controversial and not yet established largely because of contradictory nutritional studies in pregnancy. In 1981 Menon et al described an animal model of IUGR due to mild deficiency in essential fatty acids (EFA, linoleic and alpha linolenic acids), with a strong correlation betwe

In order to find out which criteria could be useful for pregnancy screening and referral, the maternal weight gain curves of 251 patients have been reviewed. Three alarm signs (gain greater than 1.0 kg/w; no gain for 4 weeks and weight loss) have been assessed and correlated with the occurrence of macrosomia, twin pregnancy, fetal intrauterine growth retardation and intrauterine death. Although excessive weight gain appears statistically linked to preeclampsia, macrosomia and twin pregnancy the sensitivity and specificity of a system using these criteria are unacceptably low.

Variations in nutritional intake during pregnancy have measurable effects on the circulating levels of maternal nutrients, maternal weight gain, and birth weight of the offspring. A growing body of evidence indicates that alterations in maternal metabolism can also have long-term consequences in the offspring in relation to adult adiposity, glucose tolerance, and perhaps intellectual development. Therefore, recommendations for diet during pregnancy must be made with great care, and with as much scientific understanding as possible. Nutritional advice traditionally given to all pregnant women,

Sixteen infants in our intensive care nursery (ICN) were inadvertently exposed to an adult who presented the next day with chickenpox. Since a majority of the patients were extremely premature, we were concerned that they might not have received transplacental varicella-zoster virus (VZV) antibody, and consequently would be at risk for contracting chickenpox. Serum samples were obtained from each infant and examined for the presence or absence of VZV antibody. Fourteen of the 16 infants had antibody levels equal to or in excess of 1:16 by indirect fluorescent antibody, including six of eight i

Infants of women who smoke during pregnancy have lower birthweights and have been observed to have higher rates of perinatal mortality than infants of non-smokers. It is not clear whether this increased risk of mortality is due to an excess of small births among smokers or to an independent effect of smoking. Although infants of smokers have overall higher mortality rates than non-smokers, low birthweight (< 2500 g) infants of smokers have lower mortality rates than low birthweight infants of non-smokers. However, comparison of birthweight-specific mortality between two groups is problematic w

The 1988 National Maternal and Infant Health Survey was conducted by the National Center for Health Statistics to study factors related to poor pregnancy outcome, such as adequacy of prenatal care; inadequate and excessive weight gain during pregnancy; maternal smoking, drinking, and drug use; and pregnancy and delivery complications. The survey is a nationally representative sample of 11,000 women who had live births, 4000 who had late fetal deaths, and 6000 who had infant deaths in 1988. Mothers were mailed questionnaires based on information from certificates of live birth, reports of fetal

Pregnancy weight gains were examined at 4-week intervals from 12-36 weeks' gestation and total gain assessed at delivery in a cohort of 2008 pregnant women aged 18 or less at entry to prenatal care. As early as 12 weeks' gestation, there was a significant association between the amount of weight gained and infant birth weight measured at the time of delivery. At 16 weeks' gestation, gains below the 25th percentile were associated with an increased risk of low birth weight (LBW) (adjusted odds ratio 1.56; 95% confidence interval 1.01-2.43), and by 20 weeks' gestation, the risk of LBW was double

This paper provides basic epidemiological data on smoking and drinking patterns before pregnancy amongst 1117 pregnant women attending a London antenatal clinic. Similar data are already available from North America and surveys have been done in Scotland but there is a dearth of information from England and Wales. In the early stages of pregnancy, prior to recognition, heavy alcohol consumption (in excess of 56 units of alcohol per week) is associated with the Foetal Alcohol Syndrome. More moderate levels of intake (as little as 10 units of alcohol per week) have been associated with Foetal Al

The dramatic excess of low birthweight and infant deaths among black babies compared to whites cannot be completely explained by differences either in sociodemographic factors or in prenatal care patterns. It is suggested that part of the explanation resides in characteristics of the mothers' own intrauterine and childhood environment which interfere with their optimal growth and development and become manifest later in suboptimal reproductive outcome. Conditions in the adult environment under which pregnancies occur also appear to interfere with fetal development. This evidence points to the
