Successful neonatal outcome after postmortem caesarean section at Sumbawanga Regional Referral Hospital, Rukwa, Tanzania: case report.

BACKGROUND: Cardiac arrest in pregnancy can occur for a variety of reasons. During maternal cardiac arrest or shortly thereafter, the fetus may remain viable. Postmortem caesarean section can potentially save the fetus with or without long-term neurological sequelae; in the absence of timely intervention, however, fetal death is inevitable. Given the limited documentation in developing countries and the possibility of unfavourable outcomes, this report presents one of the few documented cases from resource-limited Sub-Saharan Africa describing a live birth with a sustained favourable neurologi

Ovarian preservation in a pregnant woman with a leaking dermoid cyst and ovarian torsion: a case study and review of diagnostic and therapeutic challenges.

BACKGROUND: Ovarian cysts are common during pregnancy, and dermoid cysts are common benign ovarian tumors, affecting approximately 0.3% of pregnancies, most often detected in the second trimester. Hormonal changes and pressure from the growing uterus can cause these cysts to become symptomatic, leading to complications such as torsion. Torsion presents diagnostic and management challenges, requiring prompt intervention to prevent negative outcomes for both the mother and fetus. In managing ovarian cyst torsion during pregnancy, preserving ovarian tissue is crucial to maintain fertility and end

Advanced primary abdominal pregnancy with a live birth: a case report.

OBJECTIVES: Advanced primary abdominal pregnancy, an extremely rare condition, poses diagnostic challenges and carries a risk of serious maternal and perinatal complications. In the absence of evidence-based protocols, current management approaches are predominantly guided by anecdotal reports and individual clinical judgment. We report a case of primary advanced abdominal pregnancy in a primigravida. CASE PRESENTATION: A 26-year-old asymptomatic woman at 31 + 6/7 weeks' gestation was referred for routine third-trimester ultrasonography, which revealed a non-gravid uterus, a viable extrauteri

Prenatal Diagnosis of Abdominal Aorta-Hepatic Artery-Umbilical Vein Fistula Using High-Definition Flow Render Mode and Spatiotemporal Image Correlation.

Fetal abdominal aorta-hepatic artery-umbilical vein fistulas are rare congenital vascular-related malformations. This study aimed to assess the diagnostic efficacy of ultrasound in detecting fetal aorta-hepatic artery-umbilical vein fistulas using high-definition flow (HD-flow) render mode and spatiotemporal image correlation (STIC) technology. The study emphasizes six case analyses of abdominal aorta-hepatic artery-umbilical vein fistula using HD-flow render mode and STIC. Postnatal outcome evaluations were performed at our medical facility. The fistulas formed a connection between the abdomi

Extending the Time Limit of the Second Stage of Labour Versus Delivering on Time Operatively: A Retrospective Cohort Study.

OBJECTIVES: To analyze the effect of extending the second stage on maternal outcomes compared to not exceeding the time at the expense of 100% operative delivery. METHODS: A retrospective study was conducted in a university teaching hospital on data between 2018 and 2020. Nulliparous with a singleton, viable, term fetus in a cephalic presentation at the second stage was incorporated. Study cohort was partitioned into 3 groups: group 1, nulliparous who delivered operatively (100%) by cesarean delivery or vacuum extraction within 2 hours (or 3 hours with epidural); group 2, nulliparous who proc

Successful internal fixation of a pelvic ring fracture during pregnancy: a multidisciplinary case report.

BACKGROUND: Severe pelvic trauma during pregnancy is rare and requires concurrent maternal and fetal management. Multidisciplinary coordination is essential, yet reports of definitive pelvic fixation with the fetus in utero are exceptionally uncommon. CASE PRESENTATION: A 28-week pregnant woman with no prior medical history sustained severe injuries (Injury Severity Score: 16) in a high-energy collision. Diagnostic imaging revealed a comminuted, displaced right hip fracture, sacral avulsion fractures, right-sided sacroiliac joint diastasis, and a large pre-sacral pelvic hematoma. Abdominal ult

Trauma in pregnancy: A narrative review of the current literature.

INTRODUCTION: Trauma accounts for nearly half of all deaths of pregnant women. Pregnant women have distinct physiologic and anatomic characteristics which complicate their management following major trauma. OBJECTIVE: This paper comprises a narrative review of the most recent literature informing the management of pregnant trauma patients. DISCUSSION: The incidence of trauma during pregnancy is 6-8%. The focus of clinical assessment must be on the mother, starting with the primary survey. During airway management, clinicians should consider early intubation if necessary and utilize gastric t

Broad ligament ectopic pregnancy at 18 weeks: diagnosis and management in a resource-limited setting (a case report).

Broad ligament ectopic pregnancy is a relatively rare condition described in the literature. We did not find enough data concerning this subject in Cameroon. It is therefore important to know about its existence because late management can lead to increased maternal mortality. This paper reports the case of a 22-year-old lady at 18 weeks gestation who had generalized abdominal pain for two months. Prior to her arrival at our service, an abdominal and pelvic ultrasound done revealed a viable singleton 18-weeks intra-abdominal pregnancy with a moderate amount of abdominal fluid collection. The d

Challenges in Diagnosis and Management of Second Trimester Omental Pregnancy in Limited Resource Settings: Case Report.

BACKGROUND: Abdominal pregnancies are a rare occurrence and are associated with high maternal and perinatal mortality rates compared to intra-uterine and other ectopic pregnancies. Localization of sites of abdominal pregnancies and determining the gestational age at the time of diagnosis play a fundamental role in guiding the treatment approach and minimizing potential complications. However, the vague presentation coupled with low accuracy of ultrasound detection often leads to misdiagnosis of abdominal pregnancy, and hence delaying initiation of appropriate management. We present a case of a

Guidelines for the Management of a Pregnant Trauma Patient.

OBJECTIVE: Physical trauma affects 1 in 12 pregnant women and has a major impact on maternal mortality and morbidity and on pregnancy outcome. A multidisciplinary approach is warranted to optimize outcome for both the mother and her fetus. The aim of this document is to provide the obstetric care provider with an evidence-based systematic approach to the pregnant trauma patient. OUTCOMES: Significant health and economic outcomes considered in comparing alternative practices. EVIDENCE: Published literature was retrieved through searches of Medline, CINAHL, and The Cochrane Library from Octobe

Advanced extrauterine pregnancy at 33 weeks with a healthy newborn.

Abdominal pregnancy is a very rare form of ectopic pregnancy, associated with high morbidity and mortality for both fetus and mother. It is, and often, seen in poor resource nations, where early diagnosis is often a major challenge due to poor prenatal care and lack of medical resources. An advanced abdominal pregnancy with a good fetal and maternal outcome is therefore a more extraordinary occurrence in the modern developed world. We present a case of an abdominal pregnancy at 33.4 weeks in an individual with no documented prenatal care, who arrived in a hospital in the Bronx, in June 25th 20

Full-term extrauterine abdominal pregnancy: a case report.

INTRODUCTION: Extrauterine abdominal pregnancy is extremely rare and is frequently missed during antenatal care. This is a report of a full-term extrauterine abdominal pregnancy in a primigravida who likely had a ruptured ectopic pregnancy with secondary implantation and subsequently delivered a healthy baby. CASE PRESENTATION: A 23-year-old, Middle Eastern, primigravida presented at 14 weeks gestation with intermittent suprapubic pain and dysuria. An abdominal ultrasound examination showed a single viable fetus with free fluid in her abdomen. A follow-up examination at term showed a breech p

Uterine rupture and its complications in the Netherlands: a prospective study.

OBJECTIVE: To determine maternal and perinatal morbidity and mortality after uterine rupture in the Netherlands. STUDY DESIGN: All 100 Dutch obstetric departments were asked to participate in a prospective nationwide registration of uterine rupture between 1st April 2002 and 1st April 2003. For every case, a questionnaire about obstetrical history, current pregnancy and delivery, maternal and neonatal outcome was completed. RESULTS: Eighty-nine percent of all hospitals in the Netherlands participated. Ninety-eight uterine ruptures were registered; 95 after a previous caesarean section (CS) o

Is there a greater incidence of abdominal pregnancy in developing countries? Report of four cases.

Early ectopic pregnancies often are unrecognized in rural areas of Papua New Guinea. Some of these untreated tubal pregnancies abort into the peritoneal cavity and continue as advanced abdominal pregnancies. Of 72 ectopic pregnancies that were treated surgically over a 67-month period, four fetuses were found to be implanted in the abdomen. Two of these pregnancies proceeded to term and viable normal infants were delivered. One in 18 ectopic pregnancies were of advanced gestation in the abdomen. This incidence is higher than has been reported previously and is related to the poor utilization o
