GABA-transaminase (GABA-T) deficiency is a rare disorder of GABA metabolism characterized by neonatal encephalopathy, epilepsy, hypotonia and intellectual disability. It is caused by biallelic pathogenic variants in the ABAT gene. We report a case of a newborn female born to a G10P5 mother, with abnormal fetal movements and polyhydramnios in utero. At birth, she presented with hypotonia, hypersomnolence, decreased level of consciousness, central hypoventilation, non-epileptic myoclonus, seizures, and neurogenic diabetes insipidus. Brain MRI on day two of life showed partial cerebellar vermis a

The evolving field of fetal neurology can offer insights into early nervous system development through antenatal dynamic ultrasonography and maternal perception of movements. The spectrum of abnormal fetal movements ranges from fetal akinesia, often associated with arthrogryposis multiplex congenita (AMC), to increased repetitive movements, typically indicating fetal seizures. Nonepileptic hyperkinetic movement disorders of the fetus are rarely documented. Here, we report a female infant who had clonus-like movements in utero and postnatally. Antenatal ultrasonography revealed polyhydramnios,

INTRODUCTION: Birth defects constitute substantial contributor to neonatal mortality and morbidity particularly in low and middle income countries. In Bangladesh, limited screening and low awareness contribute to delayed diagnoses and inadequate care. This study aimed to delineate the patterns of birth defects and identify associated risk factors to provide baseline data in a tertiary care neonatal intensive care unit in Bangladesh to support future surveillance, inform policy development, and guide preventive strategies.
METHODS: This hospital based prospective observational study was conduc

Background/Objectives: The majority of chromosome 22q11.2 deletions are de novo, resulting from meiotic non-allelic homologous recombination (NAHR). While 22q11.2 deletion syndrome (22q11.2DS)-associated phenotypes are well characterized, risk factors leading to NAHR are poorly understood, including the possible relationship with assisted reproductive technology (ART). Here we examined the prevalence of ART conceptions and medical comorbidities in patients with 22q11.2DS vs. spontaneously conceived (SC) patients with 22q11.2DS. Methods: Retrospective analysis, under IRB approval, of medical re

Epignathus, a teratoma arising from the palate or pharynx, is extremely rare and palatal fetus-in-fetu represents an even rarer anomaly. We report a prenatally diagnosed palatal fetus-in-fetu successfully managed with ex utero intrapartum treatment (EXIT). A 4 cm oral mass detected at 23 weeks caused polyhydramnios and gastric shrinkage, suggesting impaired swallowing and potential airway obstruction. Multidisciplinary evaluation determined the need for EXIT to secure the airway under placental circulation. At 34+5 weeks of gestation, caesarean delivery with EXIT was performed; a 14 cm tumour

Hydrops fetalis (HF) is the pathological accumulation of fluid in two or more fetal compartments. While immune-mediated HF was historically predominant, non-immune hydrops fetalis (NIHF) is now increasingly common. Advances in genetic testing have revealed monogenic causes, including RASopathies (a group of genetic syndromes caused by dysregulation of the RAS-mitogen-activated protein kinase signalling pathway). We report a young primigravida, referred at 20+3 weeks for unilateral fetal pleural effusion. Serial ultrasound scans showed rapid progression to bilateral effusion, hepatomegaly, poly

BACKGROUND: Pregnancy is associated with altered pharmacokinetics for several drugs. The blood concentration of infliximab increases during pregnancy, whereas that of vedolizumab tends to decrease. As autoimmune disease activity during pregnancy can lead to adverse pregnancy outcomes, monitoring drug concentrations during pregnancy is important.
METHODS: This study reports a 32-year-old female patient with severe juvenile idiopathic arthritis and Crohn disease who was treated with dual biologic therapy throughout her pregnancy. The measurements were performed as part of routine care.

INTRODUCTION: Congenital duodenal stenosis (CDS) is often associated with trisomy 21 and is typically diagnosed in the neonatal period. However, some cases may present beyond infancy with episodes of acute gastroenteritis or foreign body impaction, which makes diagnosis challenging. This report describes a case in which a young child with trisomy 21 was diagnosed with CDS following an episode of acute bowel obstruction caused by an impacted mushroom.
CASE PRESENTATION: A male infant was born at term without abnormal prenatal findings, including polyhydramnios. Although he was diagnosed with t

BACKGROUND: Nemaline myopathy (NEM) is a rare congenital muscular disorder characterized by slow progression or static neuromuscular symptoms, which is mainly caused by variants in genes encoding the myofilament protein of skeletal muscle sarcomere. This study aimed to conduct prenatal diagnosis and functional analysis of variants in the KLHL40 gene causing NEM 8.
METHODS: A Chinese family who experienced multiple pregnancies with recurrent prenatal ultrasound anomalies (such as clubfoot and polyhydramnios) and subsequent neonatal death was enrolled in this study. Etiology diagnosis was condu

INTRODUCTION: Nonimmune hydrops fetalis (NIHF) has numerous etiologies, the most common of which are cardiac anomalies and fetal infection. However, genetic disorders are also being increasingly recognized as a cause of NIHF. Here, we report a case of a neonate presenting with polyhydramnios, NIHF, structural heart disease, and diaphragmatic defect who was found to have a previously unreported mutation in the KMT2D gene.
CASE PRESENTATION: A female neonate with antenatally detected NIHF was born at 35 weeks of gestation via cesarean section. At birth, she was noted to have dysmorphic features

INTRODUCTION: Upper extremity deformation may come in isolation, such as polydactyly, or be associated with a syndrome involving other body parts. Cornelia de Lange syndrome (CDLS) is a syndrome encompassing many abnormalities with some variation and a spectrum of severity. This case report presents a case of a baby with suspected CDLS.
CASE PRESENTATION: A 28-year-old woman, Gravida 3 Para 2, came to the polyclinic referred because of her small gestational age and a possibility of congenital abnormality. The ultrasound examination showed upper extremity malformation with hypoplasia of the le

OBJECTIVES: To determine the incidence of placental chorangioma and evaluate its associated perinatal complications and mortality.
METHODOLOGY: A retrospective observational study was conducted at Fernandez Hospitals, Hyderabad, India, from March 2011 to March 2025. All singleton pregnancies with structurally normal fetuses diagnosed antenatally with placental chorangioma and delivered at our center, with postnatal confirmation by histopathological examination (HPE), were included in the study. The diagnosis was made using B-mode ultrasound examination and color Doppler flow imaging, demonstr

A preterm male neonate born at 28 + 2 weeks gestation, with a birth weight of 1520 g (large for gestational age) and antenatal polyhydramnios, presented with prematurity, early-onset sepsis, and septic shock. Between days 4 and 9 of life, the neonate developed marked polyuria, hyponatremia, and compensated metabolic acidosis with normal to transiently elevated serum potassium levels, initially obscuring the diagnosis of a tubular disorder. Subsequently, the biochemical profile evolved to hypokalemia, metabolic alkalosis, and hypercalciuria, consistent with Bartter syndrome. The clinical course

Adenylosuccinate lyase deficiency (ADSLD) is an ultra-rare autosomal recessive disorder of purine metabolism. The most severe neonatal phenotype presents with profound encephalopathy from birth, hypotonia, respiratory insufficiency, and early seizures. A preterm infant was delivered at 34+0 weeks with antenatal progressive microcephaly and mild polyhydramnios. At birth, the neonate exhibited absent spontaneous respiration and marked hypotonia, necessitating ventilation and intubation. Respiratory distress syndrome (RDS) was managed with surfactant, but two extubation attempts with non-invasive

BACKGROUND: Mitochondrial DNA depletion syndrome 13 (MTDPS13) is an autosomal recessive disorder presenting in early infancy with encephalopathy, hypotonia, lactic acidosis, and severe global developmental delay. Patient-derived cells typically exhibit impaired mitochondrial oxidative phosphorylation and a marked reduction in mitochondrial DNA (mtDNA) copy number.
CASE REPORT: We report the case of a male preterm neonate born at 31 + 3 weeks of gestation following a pregnancy marked by severe polyhydramnios. At birth, his weight was 1400 g. Physical examination revealed dysmorphic features, r

Biallelic variants in PAICS, one of the 10 genes involved in the de novo purine synthesis (DNPS), were originally associated with an extremely rare phenotype characterized by multiple and severe congenital abnormalities, such as polyhydramnios due to esophageal atresia, congenital heart disease, and urogenital anomalies, which were lethal within the first days of life. Vertebral and limb defects, as well as distinctive craniofacial features, were also described. To date, three patients with these clinical characteristics but without neurologic involvement have been described in the literature.

INTRODUCTION: PHPT in pregnancy is a rare condition that may be difficult to recognize due to nonspecific symptoms and physiologic changes that can mask hypercalcemia. Delayed diagnosis is associated with significant maternal and fetal morbidity.
CASE PRESENTATION: A 36-year-old Spanish-speaking gravida 5 para 3 woman at 30 weeks gestation presented with decreased fetal movement in the setting of absent prenatal care. Evaluation revealed severe polyhydramnios, fetal growth restriction, and nonimmune fetal hydrops. Laboratory studies demonstrated hypercalcemia with elevated parathyroid hormone

BackgroundThe prenatal "double bubble" (DB) sign is a marker of duodenal obstruction. When accompanied by distal bowel fluid on prenatal imaging, concern may arise for malrotation with volvulus. This study evaluated whether distal bowel fluid in fetuses with a prenatal DB sign is associated with malrotation or volvulus.MethodsWe performed an 11-year retrospective review (Jan 2015-Dec 2025) of fetuses with a prenatal DB sign evaluated at a single tertiary care center. Prenatal ultrasound and fetal MRI findings, operative diagnoses, and neonatal outcomes were reviewed, with specific attention to

Background and Clinical Significance: Fetal jejunoileal atresia is a congenital anomaly of the lower digestive tract. Subsequent small intestinal volvulus, which may occur secondary to intestinal malrotation or atresia, represents a common cause of neonatal intestinal obstruction. The synthesis of intestinal atresia and volvulus is a dynamic, progressive process with distinct imaging features at each stage. Serial fetal ultrasound can detect abnormalities, and imaging changes may evolve from the initial onset of atresia to the development of volvulus. Case Presentation: We report the clinical

Congenital pyloric atresia (CPA) is a rare cause of neonatal gastric outlet obstruction. Although usually isolated, CPA may coexist with additional gastrointestinal atresias. Its association with distal duodenal atresia (DA) is particularly relevant because the segment between the two obstructions may become a closed duodenal loop into which biliary and pancreatic secretions continue to drain. We report the case of a male newborn delivered at 35 + 3 weeks of gestation after prenatal detection of severe polyhydramnios, recurrent bowel dilatation, abdominal effusion, and a cystic abdominal lesio
