Pulmonary Hypoplasia.

Pulmonary hypoplasia is a rare but clinically significant congenital disorder characterized by impaired fetal lung development, leading to impaired growth and maturation of lung parenchyma and pulmonary vasculature. Stemming from a diverse group of underlying etiologies including congenital diaphragmatic hernia, congenital lung malformations, and prolonged oligohydramnios, the clinical presentation of pulmonary hypoplasia is highly variable. Although many factors affect severity of disease, pulmonary hypoplasia can be life-threatening and accounts for a disproportionately high proportion of ne

Posterior urethral valves in children: Achieving renal preservation through integrated multidisciplinary care in a resource-limited region.

INTRODUCTION: Posterior urethral valves (PUV) remain the leading cause of congenital obstructive uropathy and chronic kidney disease (CKD) in male children. Outcomes vary widely across healthcare systems, with end-stage renal disease (ESRD) rates of 10-25% reported in international series. We present a single-centre PUV cohort, examining renal preservation, surgical outcomes, and the prognostic impact of detection timing. METHODS: A retrospective review of 120 consecutive patients with PUV managed between 2018 and 2024 was conducted. Patients were stratified by antenatal versus postnatal dete

Successful management of a large feline skin defect using xenogeneic bovine amniotic membrane: a case report.

BACKGROUND: Amniotic membrane has gained interest as a biological dressing due to its antimicrobial, anti-inflammatory, and pro-regenerative properties. However, its short shelf life and limited availability often constrain its clinical use in veterinary medicine. This report describes, for the first time, the use of bovine-derived AM preserved in a 1% propolis medium as a xenogeneic biological dressing for the management of extensive skin necrosis in a cat. CASE PRESENTATION: An eight-month-old female domestic cat (1.9 kg, unvaccinated) presented with severe dorsal skin sloughing and purulent

A randomized control trial comparing the clinical outcome of thyroxine supplementation in thyroid peroxide antibody- negative cases in patients with subclinical hypothyroidism in pregnancy.

BACKGROUND: A normal free thyroxine level in the presence of an increased thyroid-stimulating hormone is what constitutes subclinical hypothyroidism (TSH). Changes in thyroid physiology during pregnancy result in the normal TSH range being lower than non-pregnant. Subclinical thyroid disease during pregnancy is associated with adverse outcomes, including, risk of adverse pregnancy and neonatal outcomes. Prevalence in India is 6-8%. The prevalence of antibodies varies with ethnicity. Pregnancy with TSH concentrations >2.5 mIU/L needs evaluation for thyroid peroxidase antibody (TPO) antibody sta

Postoperative Pulmonary Hypertension after Primary Abdominal Wall Closure for Small Omphalocele: A Case Series.

Primary closure is a low-risk option for small omphaloceles (SOs). However, despite the absence of liver herniation and limited volume of herniated viscera, we encountered two cases of postoperative pulmonary hypertension (PH) and respiratory failure after closure for SO. This report discusses the safety of primary closure and potential factors that may be associated with postoperative PH in infants with SOs. Cases 1 and 2 presented with SOs without chromosomal abnormalities or liver herniation (gestational ages 36 and 37 weeks, birth weights 3,650 and 2,577 g, and defect sizes 4 and 2 cm, res

Efficacy of Cryopreserved Amniotic Membrane Allograft in the Management of Refractory Chronic Venous Leg Ulcers: A Randomized Controlled Trial.

Background and aims Chronic venous leg ulcers present a clinical challenge due to persistent inflammation, proteolytic imbalance, and senescence of the extracellular matrix. Dehydrated amniotic products are utilized; however, cryopreserved amniotic membrane retains the native heavy chain hyaluronic acid and viable growth factors that are often altered during heat dehydration processing. This study evaluated the efficacy of cryopreserved amniotic membrane allografts in promoting wound closure and reducing pain in patients with refractory venous ulcers compared to standard compression therapy. Impact of prolonged preterm premature rupture of membranes and oligohydramnios on neonatal outcomes in extremely preterm infants: A nationwide cohort study.

BACKGROUND: Prolonged preterm premature rupture of membranes (PPROM) may support fetal maturation but it also increases the risk of complications such as oligohydramnios. The aim of this study was to assess the effect of PPROM latency and oligohydramnios on neonatal outcomes in extremely preterm infants. METHODS: This study analyzed 940 infants born at < 28 weeks of gestation after PPROM using data from the Korean Neonatal Network. The infants were divided into two groups based on latency period: short (1-20 days, SP) and long latency (≥ 21 days, LP). Each group was further stratified by the

Foley's Catheter as a Simple Adjunct for Membrane Reduction in Late Rescue Cerclage: A Case Report.

Advanced cervical dilatation with prolapsed membranes in the late second trimester is associated with imminent pregnancy loss or extreme prematurity. Rescue cerclage may improve outcomes in selected patients with intact membranes. We report a case of a 32-year-old G2A1 (gravida 2, abortion 1) woman at 27+4 weeks of gestation with painless cervical dilatation and bulging membranes. After excluding infection and uterine activity, emergency Shirodkar cerclage was performed under spinal anaesthesia. The prolapsed amniotic sac was reduced using a Foley catheter balloon technique prior to suture pla

Pre-Pregnancy Obesity Phenotypes and the Risk of Oligohydramnios: A Large Population-Based Cohort.

Background/Objective: Maternal obesity is an established risk factor for adverse obstetric outcomes. However, its association with oligohydramnios remains unclear and has been examined using body mass index (BMI) alone. We assessed pre-pregnancy general and abdominal obesity in relation to oligohydramnios. Methods: We analyzed 762,104 women who delivered between 2010 and 2018 and had undergone a national health examination within two years before conception. Women were classified into four phenotypes by BMI (general obesity: ≥25 kg/m2) and waist circumference (abdominal obesity: ≥85 cm). Odds

The effect of amniotic membrane application on post-cesarean wound healing and cosmetic outcomes.

Post-cesarean wound complications-such as surgical site infection (SSI), dehiscence, and hypertrophic scarring-remain a challenge in obstetric surgery. We evaluated whether intraoperative autologous amniotic membrane application improves postoperative outcomes. In a single-blind, randomized controlled trial, 372 women undergoing elective cesarean section were allocated to an amniotic membrane group (n = 174) or control group (n = 198). SSI incidence was lower with amniotic membrane (4.3% vs. 10.8%; p < 0.05), and wound dehiscence was reduced (2.1% vs. 6.4%; p < 0.05). Postoperative pain scores
