The impact of bioinformatic choices on Coccidioides variant identification accuracy.

Emerging fungal pathogens, such as Coccidioides, the causative agent of Valley fever, or coccidioidomycosis, pose significant clinical and public health challenges. While advances in genomic epidemiology have enhanced our understanding of Coccidioides evolutionary history, effective variant identification is complicated by the genome's structural complexity. Repetitive elements, transposable sequences, and regions of low complexity can lead to incorrect variant calls, affecting downstream analyses. Further, accurate species identification is essential for understanding the spread of C. immitis

Lightning Can Strike Twice: Management of an Infant With Anomalous Left Coronary Artery from the Pulmonary Artery Complicated by Coccidioidomycosis Infection.

Cardiac disease in young children can be unrecognized until symptoms are unmasked by a precipitating event, such as an infection. We present a case of anomalous left coronary artery from the pulmonary artery causing clinically significant disease in a four-month-old male with concomitant mitral regurgitation and pulmonary coccidioidomycosis who required modification of his surgical management due to the infection. This case highlights how timely diagnosis and perioperative management and recovery can be affected by concurrent infections in patients with congenital heart disease.

Outcomes of Coccidioides Seropositive Solid Organ Transplant Recipients After Self-discontinuation of Antifungal Prophylaxis: The EIA-IgM-only Conundrum.

BACKGROUND: Solid organ transplant recipients are at risk of severe coccidioidomycosis and are given prophylaxis to mitigate the risk. Patients with seropositive testing typically receive lifelong prophylaxis; currently, this prophylaxis strategy includes patients who are positive only for IgM by enzyme immunoassay (EIA-IgM-only), although this result may be falsely positive. METHODS: We conducted a retrospective study at a large-volume transplant center in an endemic coccidioidomycosis region to compare outcomes of non-lung transplant recipients who were seropositive for Coccidioides but dis

The profound effect of diabetes mellitus control on outcomes of coccidioidomycosis.

Coccidioidomycosis is an endemic fungal infection caused by the soil-dwelling Coccidioides. One prior study showed that persons with diabetes mellitus (DM) with elevated glucose at the time of coccidioidomycosis had poorer coccidioidal outcomes compared to persons without DM. We aimed to assess the association between hemoglobin A1C (A1C) and outcomes among persons with coccidioidomycosis and coexisting DM. We conducted a retrospective case-control study of patients with DM (cases) and without DM (controls) with coccidioidomycosis to assess the effect of glycemic control on outcomes in patient

Sensitivity of Coccidioides serologic tests among culture-proven coccidioidomycosis patients with hematological malignancy compared to a matched immunocompetent cohort.

Coccidioidomycosis is associated with an increased risk of mortality and morbidity among immunosuppressed patients. The sensitivity of current commercial Coccidioides serologic tests is not well evaluated in patients with hematological malignancy. We conducted a retrospective study, including patients with culture-proven coccidioidomycosis, from October 1, 2017, to December 12, 2023. Cases with hematological malignancy and hematopoietic stem cell transplant (HM) were matched with an immunocompetent cohort (1:2) to compare the sensitivity of serology (enzyme immunoassay IgG/IgM or complement fi

Investigating Asthma After Coccidioidomycosis Among Patients With Commercial Health Insurance, United States, 2017-2022.

BACKGROUND: The relationship between asthma and coccidioidomycosis has not been fully described. We have hypothesised that Coccidioides could trigger inflammatory airway responses, similar to other fungi. OBJECTIVES: To estimate the frequency of new-onset asthma-related symptoms after coccidioidomycosis and identify potentially associated factors. PATIENTS/METHODS: We used a large health insurance claims database to identify patients with coccidiomycosis with and without an asthma diagnosis code or a short-acting β2 agonist prescription in the year after diagnosis. RESULTS: Thirteen per cen

Cavitary Pneumonia Causes in the Arid Landscapes of Southern Arizona.

Cavitary pneumonia, characterized by gas-filled spaces within lung tissue, is commonly associated with bacterial pathogens, such as Staphylococcus aureus, Streptococcus, Pseudomonas, and Klebsiella species. This study aimed to investigate the most common causes of cavitary pneumonia in southern Arizona, where coccidioidomycosis is endemic. Fungal infections were the predominant cause of cavitary pneumonia (69%), either alone or in association with bacterial infections. Seven percent of patients had concurrent fungal and bacterial infections. Overall, coccidioidomycosis was the most common caus

Impact of glycemic control on coccidioidomycosis outcomes in patients with underlying diabetes mellitus in central California.

Diabetes mellitus (DM) is a known risk factor for severe coccidioidomycosis. Central California has some of the highest rates of DM in the USA, according to CDC data. This study examines the impact of glycemic control on the severity and outcomes of coccidioidomycosis in this high-DM-prevalent region. A retrospective analysis was conducted on patients with both coccidioidomycosis and DM from 2014 to 2022 at a large referral center in Fresno, California. Data collected included demographics, presentation, HbA1c levels, management, and outcomes. Fisher's exact test and the Wilcoxon-Rank sum test

Pulmonary Coccidioidomycosis in a Ruxolitinib-Treated Polycythemia Vera Patient: A Case Study and Literature Review.

This case report explores the epidemiology and clinical implications of coccidioidomycosis (Valley fever), with a focus on cases involving the immunosuppressive agent ruxolitinib (RUX) in patients with polycythemia vera (PV). The incidence of coccidioidomycosis has increased in the past decade in endemic regions, particularly in immunocompromised individuals. RUX, a Janus-associated kinase 1/2 (JAK1/2) inhibitor used in PV treatment, has been associated with various infections, but its link to coccidioidomycosis remains underexplored. We present a rare case of coccidioidomycosis in a PV patien

Three in One: A Case Report of Pulmonary Co-infection With a Virus, a Bacterium, and a Fungus in an Immunocompetent Adult.

Pneumonia is a common cause of morbidity and mortality in the United States and worldwide. It is most commonly caused by a single organism, but co-infection can occur. Nevertheless, there are no or very few reported cases of triple acute co-infection with a virus, a bacterium, and a fungus in an immunocompetent patient without underlying chronic lung disease. Here, we discuss a case in which an otherwise healthy, elderly male exhibited a triple co-infection with coronavirus NL63, Coccidioides immitis or Coccidioides posadasii (also known as a cause of Valley fever), and Mycoplasma pneumoniae.

Coccidioidomycosis Lymphadenitis in a patient with lupus nephritis: a case report.

Coccidioidomycosis is a fungal infection endemic to arid regions. Although it usually presents as a pulmonary infection, extrapulmonary dissemination is rare, and cervical lymphadenitis is even more unusual with fewer than five cases reported in the literature. Diagnosis of this disease remains a clinical challenge due to its rarity and similarity to other clinical entities. A 34-year-old woman with lupus nephritis on immunosuppressive therapy presented with a painful cervical mass associated with unquantified fever of six weeks of evolution. The patient is a resident of Coahuila, Mexico, in a

The costs of the diagnosis and treatment of canine coccidioidomycosis in endemic regions, USA, 2022.

Coccidioidomycosis ("Valley fever") is a fungal disease that causes a wide range of illness severity in animals and people. Here, we use U.S. Census data, surveys of pet ownership demographics, and results of a nation-wide dog serologic study to estimate the financial burden of the disease to dog owners. We estimate that the one-year cost of diagnosis and treatment for uncomplicated pulmonary coccidioidomycosis in dogs across 6 endemic southwestern U.S. states ranged from $60,117,875 to $74,858,713 before adjusting for inflation. The financial burden to dog owners for uncomplicated illness was

Coccidioidomycosis: a growing global concern.

Coccidioidomycosis (CM) has been a recognized disease for about 130 years. The organisms (Coccidioides spp. fungi) inhabit desert soil in the southwestern USA, Mexico, and parts of Central and South America. Natural events such as dust storms, wildfires or outdoor activities including construction and gardening can disrupt the fungal arthroconidia, which easily become airborne and inhaled by the host. Approximately 60% of those exposed to arthroconidia are asymptomatic and do not require medical attention, but 30% show signs of pulmonary infection with symptoms ranging from a flu-like illness

Identification of Coccidioidomycosis Immunoreactive Peptides That Recall T-Cell Responses Indicating Past Exposure.

BACKGROUND: Valley fever (VF) is a common cause of community-acquired pneumonia in the American Southwest. Diagnosis is challenging due to VF's similarities to other pulmonary diseases. The VF T-cell response is important to control the disease, and measuring this response holds promise as an improved diagnostic. We aim to identify of VF T-cell epitopes, and to develop disease and efficacy diagnostics for the clinic and vaccine trials. METHODS: We identified VF-specific T-cell epitopes by using their ability to bind the major histocompatibility complex (MHC) class II molecules and T-cell stim

Early immune response to Coccidioides is characterized by robust neutrophil and fibrotic macrophage recruitment and differentiation.

Coccidioidomycosis, or Valley fever, is an emerging respiratory disease caused by soil-dwelling fungi of the Coccidioides genus that is expected to spread from the southwest into the central U.S. by 2050. While 60% of infections are asymptomatic, the other 40% of patients experience a range of symptoms, from self-limiting pneumonia to life-threatening disseminated disease. The immunological events that underlie the progression to severe disease remain underdefined. Here, we probed the early immune response to Coccidioides using a high dose of an attenuated strain of Coccidioides posadasii in a

Olorofim for the treatment of invasive fungal diseases in patients with few or no therapeutic options: a single-arm, open-label, phase 2b study.

BACKGROUND: Only a small number of antifungal therapies for invasive fungal disease (IFD) are currently available, and many pathogens are resistant to one or more of these therapies. Olorofim, the first orotomide antifungal agent to be developed, is active against fungi that are resistant to registered therapies. It impairs fungal pyrimidine biosynthesis, leading to cell death. We sought initial data on the efficacy and safety of olorofim as a therapy for IFD. METHODS: In this single-arm, open-label, phase 2b study, patients aged 16 years or older with few or no treatment options for proven I

Development of a Mouse Model of Coccidioidomycosis Using an Inhalation Exposure System.

Coccidioides species are thermally dimorphic fungal pathogens that cause coccidioidomycosis (Valley Fever) primarily in North and South America. Coccidioides grow as hyphae that differentiate into arthroconidia, which can be aerosolized upon soil disturbance, and inhaled by the mammalian host to cause pulmonary infections with occasional dissemination to other organs. In the context of mouse models, current methods of infection include intranasal, intravenous, and intraperitoneal delivery of the arthroconidia into mice. To explore an aerosol route of infection, we compared the intranasal metho
