Dancing with the Dust Devil: Examining the Lung Mycobiome of Sonoran Desert Wild Mammals and the Effect of Coccidioides Presence.

Microbiome studies report a decrease in diversity associated with active infections. Under the endozoan hypothesis, Coccidioides can inhabit a host without causing disease. In this study, we describe and compare the lung mycobiome of Coccidioides-positive and -negative samples obtained from wildlife. If Coccidioides is not causing infection, we predict there will be no differences in the mycobiome between positive and negative samples. Lung samples were obtained from mammals previously trapped in Tucson, Arizona, USA (n = 26), and Mesa, Arizona, USA (n = 14). Samples were screened for Coccidio

Valley Fever Is an Occupational Illness: Let's Get the Data to Prevent It.

Valley fever is a fungal disease acquired through inhalation of spores of the fungus Coccidioides. Spores become airborne when soil is disturbed, making outdoor workers, such as agricultural or construction workers, at higher risk of exposure and disease. Valley fever is endemic and increasing in the American Southwest, with numerous documented outbreaks among construction crews, archeologists, prison employees, wildland firefighters, and military personnel. Valley fever is recognized as a work-related disease in California, but not Arizona or other states with endemic disease. Epidemiologists

The Use of Genetics and Immunology in the Diagnosis and Care of Advanced Coccidioidomycosis: Where Are We Going?

Coccidioidomycosis (CM) is an endemic disease in the western United States, northern Mexico, and Central and South America. The severity of coccidioidal infection is highly variable, with potential factors including comorbidities and the patient's innate and adaptive immune response. Based on data from a predominantly healthy and mainly Caucasian male survey conducted nearly a century ago, approximately 60% of infections are asymptomatic, with an estimated 40% of people experiencing some respiratory symptoms; with 10% of those diagnosed with CM. Disseminated disease occurs in approximately 1%

Mapping the environmental risk of coccidioidomycosis in Sonora, Mexico: a Bayesian spatiotemporal perspective (2010-2022).

Coccidioidomycosis, a fungal disease caused by Coccidioides spp. is endemic in arid regions of the Americas. Although northern Mexico shows increasing incidence, its environmental determinants remain understudied. This research examines the association between coccidioidomycosis and environmental factors in Sonora, Mexico, from 2010 to 2022. We integrated municipal-level hospital discharge data with mean annual temperature, total annual precipitation, wind speed and direction, soil moisture, vegetation index and land use (human settlements and agriculture). Using a Bayesian hierarchical spatio

Depression and ineffective reproduction, i.e. psychiatric aspects of infertility.

Psoriasis in the elderly will constitute a significant challenge for the practising physician in this new millennium. Special considerations for the elderly include drug-induced or drug-aggravated psoriasis, especially for patients receiving polypharmacy or with recent worsening or poor response to conventional therapy. Other frequently encountered forms of psoriasis in the elderly include psoriatic arthritis and its complications, inverse psoriasis and potentially life-threatening complications such as erythrodermic or acute pustular psoriasis, where early recognition and systemic therapy is

Single-cell analysis of human airway epithelium identifies cell-type-specific responses to Aspergillus and Coccidioides.

Respiratory fungal infections pose a significant threat to human health. Animal models do not fully recapitulate human disease, necessitating advanced models to study human-fungal pathogen interactions. In this study, we utilized primary human airway epithelial cells (hAECs) to recapitulate the lung environment in vitro and investigate cellular responses to two diverse, clinically significant fungal pathogens, Aspergillus fumigatus and Coccidioides posadasii. To understand the mechanisms of early pathogenesis for both fungi, we performed single-cell RNA sequencing of infected hAECs. Analysis r

Misuse of the unspecified coccidioidomycosis diagnosis code: An audit of electronic health records.

Widespread use of the 'unspecified coccidioidomycosis' code (B38.9) may negatively impact provider reimbursement and complicate study of disease burden. We sought to determine the frequency of B38.9 use in routine practice and assess how often it could be changed to a more specific code, what code that might be, and reasons for its initial use. To estimate the proportion of all coccidioidomycosis cases that were classified using the International Classification of Diseases-Tenth-Revision unspecified diagnosis code (B38.9), three real-world datasets were queried. Further, in January 2025, provi

Coccidioides genomes from low-incidence states reveal complex migration history across the western United States.

Coccidioidomycosis (Valley fever), caused by the fungal pathogen Coccidioides, is increasing in incidence across the western United States and parts of the Americas. However, genomic data from low-incidence regions remain scarce, limiting our understanding of the pathogen's dispersal and evolution. To address this gap, we prospectively collected Coccidioides-positive clinical isolates submitted to a national diagnostic laboratory between January 2023 and November 2024. We performed whole-genome sequencing on 27 clinical isolates from Utah, Colorado, and Nevada-states with no previously availab

Solitary thalamic abscess without meningitis from Coccidioides immitis: illustrative case.

BACKGROUND: Despite decreased mortality over the past several decades, the prognosis of brain abscesses remains dependent on intracranial location and causative organism. Deep-seated brain abscesses carry a risk of intraventricular rupture, an event with reported mortality near 80%. Coccidioidomycosis from the fungus Coccidioides immitis, endemic to the American Southwest, is growing in incidence but uncommonly produces deep brain abscesses, making management unclear. The authors report the diagnosis and management of a solitary thalamic Coccidioides abscess without meningitis. OBSERVATIONS: A

Development and Optimization of Multi-Well Colorimetric Assays for Growth of Coccidioides posadasii Spherules and Their Application in Large-Scale Screening.

Coccidioides immitis and Coccidioides posadasii, the causative agents of coccidioidomycosis, represent a major public health concern in endemic regions of North and South America. The disease spectrum ranges from mild respiratory illness to severe disseminated infections, with thousands of cases reported annually in the United States and an increasing recognition of its global impact. Despite existing antifungal therapies, treatment remains challenging due to toxicity, drug resistance, and limited therapeutic options. High-throughput screening platforms have revolutionized drug discovery for i

A case displaying the importance of JAK1 and JAK2 gene transcription in antifungal defense against coccidioidomycosis.

This case report explores the consequences of ruxolitinib via inhibition janus kinase 1 (JAK1) and JAK2 pathways in the context of fungal defense in a patient diagnosed with pulmonary coccidioidomycosis during ruxolitinib therapy for polycythemia vera. The patient experienced a relapse of pulmonary coccidioidomycosis after antifungal treatment was discontinued while continuing ruxolitinib use. This case illustrates the heightened risk of discontinuing antifungal therapy in endemic regions, emphasizing the critical need for continued monitoring. Furthermore, this case underscores the vital role

A Case of Coccidioides Meningitis in an Immunocompetent Host and the Challenges in Diagnosis.

Coccidioides is the causative organism for Coccidioidomycosis or otherwise known as "Valley fever." Valley fever usually presents as upper respiratory tract illness which can often be complicated by dissemination. Although immunosuppression increases the risk of disseminated disease, it can also be seen in immunocompetent patients. Coccidioides meningitis is the most feared outcome of disseminated coccidioides infection, rendering early diagnosis and treatment imperative. However, the diagnosis of coccidioides meningitis is challenging due to its vague presentation and its resemblance to other

When should coccidioidomycosis be in the differential?

In a recent article in ASM Case Reports, Blomquist et al. (1:e00098-24, https://doi.org/10.1128/asmcr.00098-24) describe a young woman with several years of abdominal symptoms who subsequently was found to have peritoneal coccidioidomycosis. Her story calls into focus the value of considering this endemic disease, either in patients with typical or unusual manifestations. In this patient, eventually the concern for metastatic cancer led to a surgical procedure which established the true diagnosis. However, a simple serologic test, either done in the preceding years because of her chronic sympt

Use of Intrathecal Amphotericin for Coccidioidal Meningitis.

BACKGROUND: Cases of coccidioidomycosis are increasing dramatically in the United States. The utility of intrathecal (IT) amphotericin in treating coccidioidal meningitis (CM) in the era of azole therapy is unknown. We sought to understand how IT therapy is associated with mortality. METHODS: We conducted a retrospective chart review of adult patients with laboratory-proven CM seen at Stanford Healthcare by an infectious diseases physician from 2008 to 2023. RESULTS: Fifty-seven patients met inclusion criteria. All patients received azole therapy. Twenty-seven of those patients (47.4%) addit

Diagnostic Performance of Coccidioidomycosis PCR Testing in Lung Nodules: A Retrospective Study in Central California.

BACKGROUND: Coccidioidomycosis is an endemic fungal infection in the southwestern United States that can present as solitary lung nodules, mimicking malignancy on imaging. Molecular testing, such as PCR, offers rapid diagnosis but its performance in this clinical setting remains unclear. METHODS: We conducted a retrospective analysis of patients evaluated for lung nodules at a tertiary care community-based lung nodule clinic in Central California between 2011 and 2025. Coccidioides PCR in patients with proven or probable Coccidioidomycosis was compared to those with biopsy-proven lung cancer.

Current perspectives of host-pathogen dynamics in coccidioidomycosis.

Coccidioidomycosis (CM), commonly known as Valley fever, is a respiratory infection caused by the inhalation or implantation of infectious arthroconidia produced by the dimorphic human fungal pathogens Coccidioides immitis and Coccidioides posadasii from the environment. The current endemic range includes the southwestern region of the United States and parts of South and Central America. Infected individuals may experience a spectrum of symptoms from asymptomatic to severe respiratory symptoms. Importantly, the fungus can disseminate to other tissues to produce severe symptoms, and in some ca

Effects of Soil Moisture and Soil Temperature on Coccidioidomycosis.

Coccidioidomycosis (Valley fever, VF) is a climate-sensitive infectious disease caused by inhaling soil-dwelling fungus Coccidioides, mostly reported in southwestern USA. Although soil moisture (SM) and soil temperature (ST) are known to shape the fungal lifecycle, their effects on coccidioidomycosis remain understudied. Most prior studies have relied on their proxies-precipitation and air temperature-that might not accurately capture soil hydrothermal dynamics. We conducted multivariable negative binomial regressions to estimate seasonal associations between incidence and climate drivers-incl

Drug Discovery and Repurposing for Coccidioides: A Systematic Review.

Coccidioides immitis and C. posadasii are the causative agents of coccidioidomycosis (CM) or Valley Fever, endemic to the alkaline deserts of North and South America. Clinical treatment of CM is predominantly limited to the triazole and polyene drug classes. There are limited therapeutic options for the treatment of CM, most commonly requiring prolonged courses of therapy with established antifungal agents such as azoles and Amphotericin B, which often lead to toxicity and drug resistance. Clearly, there is a need to develop novel and better antifungal drugs against CM. This review examines bo
