Morphological and genetic characterization of novel Sarocladium spinificis strains in association with Coccidioides posadasii.

UNLABELLED: Sarocladium is a fungal genus containing diverse members with roles as pathogens of plants and animals, endophytes, and biocontrol agents. This study characterizes two Sarocladium spinificis strains isolated from patients diagnosed with coccidioidomycosis. Morphological, phylogenetic, and genomic analyses confirmed their classification within the Sarocladium genus, closely related to S. spinificis. Both strains exhibited mycelial growth at 24°C and 37°C, with distinct morphological features. Comparative genomic analysis identified unique orthologous clusters and functional gene fam

Linking the epidemiology of coccidioidomycosis and environmental exposure through targeted genomic enrichment of Coccidioides posadasii.

Valley fever, a disease caused by Coccidioides spp., is a fungal respiratory disease with an expanding range. Methods to culture the pathogen from soil, especially Coccidioides posadasii, are very challenging, limiting the genomics knowledge of environmental strains. In this study, we designed and tested a targeted DNA capture and enrichment system for the characterization of Coccidioides genomes without the need to culture. In this system, RNA probes are hybridized to Coccidioides DNA in a complex sample, followed by DNA amplification, sequencing, and analysis. Our enrichment system was targe

Choice of Downscaled Climate Product Matters: Projections of Valley Fever Seasonality in a Warming Climate.

Downscaled climate projections provide valuable information needed to better understand the impacts of climate change on health outcomes and to inform adaptation and mitigation strategies at local to regional scales. Because downscaled climate products vary in their representations of fine-scale spatiotemporal patterns, due to of multiple interacting factors, epidemiologic analyses need to consider how differences across downscaling approaches impact projections of health impacts into the future. We evaluate the projected seasonality of coccidioidomycosis in response to projected temperature a

Persistence of Symptoms among Commercially Insured Patients with Coccidioidomycosis, United States, 2017-2023.

Some patients with coccidioidomycosis experience prolonged respiratory and systemic symptoms. However, data on prevalence and persistence of most symptoms are lacking. Using an insurance claims database, we identified patients with coccidioidomycosis diagnoses in the United States during 2017-2023. We assessed prevalence of associated symptoms from 6 months before to 1 year after first diagnosis code (index date) and compared post-index date prevalence to baseline (within 6 to 4 months before index date). Among 2,640 patients, cough (20.8%), dyspnea (13.0%), and fatigue (8.8%) were the most co

Impact of Delays in Diagnosis on Healthcare Costs Associated With Blastomycosis, Coccidioidomycosis, and Histoplasmosis in a Commercially Insured Population.

Among patients with blastomycosis (n = 281), coccidioidomycosis (n = 1920), and histoplasmosis (n = 2180), 62% experienced diagnostic delays (mean 29 days). Patients who experienced delays incurred average excess healthcare costs of up to $15 648 (95% confidence interval: $8600-$22 695) compared with those without a delay. Earlier diagnosis may help reduce excess costs.

Fungal vaccines: so needed, so feasible, and yet so far off.

Invasive fungal infections carry high morbidity and mortality, but there are no fungal vaccines. In this issue of the JCI, Okaa et al. report that endonuclease 2 (Eng2), an antigen shared by the Blastomyces, Histoplasma, and Coccidioides species of fungi, elicits protective immunity in mice against blastomycosis, histoplasmosis, and coccidioidomycosis. These results establish a common antigen that can elicit protection against multiple mycoses, encouraging the development of a pan-fungal vaccine. The road to fungal vaccines is made difficult by the need for effectiveness in immunocompromised i

Endemic Mycoses for Pulmonary Clinicians: From Nodules to ARDS.

TOPIC IMPORTANCE: Endemic fungal infections are increasingly recognized as important causes of community-acquired pneumonia. Despite this, diagnosis is often delayed or misattributed, resulting in significant morbidity and mortality. REVIEW FINDINGS: Histoplasma capsulatum, Blastomyces species, and Coccidioides species are the most common endemic fungal infections in the United States. These infections share commonalities in modes of transmission and pathogenicity where regional climates, weather patterns, and certain exposures play a key role in infectivity. However, changes in climate and h

Imported Systemic Endemic Mycoses in Spain 1997-2021: An Analysis of a National Hospital Database.

BACKGROUND: Systemic endemic mycoses are systemic fungal infections typically found in tropical and subtropical regions. Their global incidence is rising, including in nonendemic countries, mainly due to migration and international travel. They are a major cause of morbidity and mortality worldwide, especially in immunocompromised patients. This study aimed to analyse incidence trends of endemic mycoses and their presentation in hospitalised patients in Spain from 1997 to 2021. METHODS: This retrospective, observational study drew data from the Spanish National Hospital Discharge Database. We

When to suspect and how properly early detect and treat patients with endemic mycoses.

Endemic mycoses are caused by dimorphic fungi and eventually molds, as the case of implantation mycoses. In general, these diseases are acquired through trauma or inhalation of fungal elements in the environment, and less frequently by zoonotic acquisition or transmitted during organ transplantation. The target population for endemic mycoses is usually represented by normal hosts with low-income and intensive outdoor activities. Awareness of these diseases remains limited, even in regions with high prevalence, resulting in delayed diagnosis, and affecting the quality of life and outcomes of pa

Systemic endemic mycoses: From a geographical risk to a concern expansion.

Blastomycosis, coccidioidomycosis, histoplasmosis, paracoccidioidomycosis, emergomycosis, and talaromycosis are sapronoses restricted to specific geographical areas associated with particular environmental niches that can lead to high morbidity and mortality if diagnosed or treated late. Challenges in clinical awareness arise from their nonspecific clinical presentations and the limited availability of sensitive diagnostic tools. Although advances in molecular diagnosis and antigen detection-based tests are promising, they are not available for all systemic endemic mycoses (SEM). Treatment gui

Mapping the Geographic Distribution of Dimorphic Mycoses Using a US Commercial Insurance Database.

BACKGROUND: The dimorphic mycoses (DMs) histoplasmosis, blastomycosis, and coccidioidomycosis are classically thought to be geographically restricted to specific regions of the United States. The current scientific consensus is that these geographic boundaries are expanding, but the extent of this expansion remains uncertain. METHODS: This study is a retrospective analysis of >21 million (annually) commercially insured individuals within the United States during 2007-2016. Diagnosis of histoplasmosis, coccidioidomycosis, and blastomycosis were identified using International Classification of

Trends in the epidemiology of systemic endemic mycoses in Latin America.

The changing epidemiology and diagnostic challenges of coccidioidomycosis, histoplasmosis, and paracoccidioidomycosis in Latin America are reviewed. The agents responsible for these three systemic endemic mycoses are dimorphic fungi from the Onygenales order, all of which are classified as World Health Organization fungal priority pathogens. However, the mycoses they cause span a notable diversity of contrasting etiologies, target populations, geographic distributions, and factors (including climate changes) that determine their incidence. New, updated maps are presented that reflect the curre

Disseminated Blastomycosis: A Case of Multisite Infection in an Immunocompetent Patient.

BACKGROUND Blastomycosis is a fungal infection caused by Blastomyces spp., a dimorphic fungus hyperendemic in Wisconsin, USA. CASE REPORT A 54-year-old previously healthy man presented with right foot and thigh pain, erythema, and a non-healing purulent ulcer on the right buttock. Initial treatment with oral doxycycline and cephalexin was unsuccessful, and subsequent imaging revealed a thigh hematoma and acute osteomyelitis of the forefoot with an adjacent abscess. The patient underwent 2 incision-and-drainage procedures, which uncovered a disarticulated second toe and persistent purulence. Hi

The Burden of Hospital Illness Associated with Disseminated Versus Isolated Pulmonary Coccidioidomycosis in the United States.

There are scarce data comparing inpatient mortality, length of stay (LOS) and all-cause hospital costs in disseminated coccidioidomycosis (DCM) vs. isolated pulmonary coccidioidomycosis (IPCM). We assessed the burden of hospital illness associated with DCM versus IPCM. This study was performed using National Inpatient Sample data from 2019 to 2021. DCM was defined as having a primary International Classification of Diseases-Tenth Revision (ICD-10) code for coccidioidal meningitis, a non-primary code for coccidioidal meningitis in the presence of a primary code for a meningitis complication or

Disseminated coccidioidomycosis presenting as eosinophilic ascites.

A man in his 50s with a self-reported history of liver cirrhosis was admitted to the hospital for the treatment of suspected spontaneous bacterial peritonitis (SBP). Initial imaging revealed a mildly nodular liver, a large volume of ascites and omental thickening. He was treated empirically for SBP. Ascitic fluid analysis from paracentesis demonstrated eosinophils and a low serum-ascites albumin gradient. He was discharged from the hospital with a diagnosis of peritoneal carcinomatosis pending work-up for an unknown primary malignancy. An extensive malignancy work-up was negative, but an oment
