Disseminated Musculoskeletal Coccidioidomycosis in an Immunocompetent Host: A Rare Case Requiring Extensive Spinal and Pelvic Surgery.

Coccidioidomycosis, also known as "Valley Fever," is an endemic fungal infection caused by Coccidioides species, typically presenting as a respiratory illness. Disseminated disease, particularly with extensive skeletal involvement, is rare and typically seen in immunocompromised individuals. We report a unique and severe case of disseminated musculoskeletal coccidioidomycosis in a previously healthy immunocompetent 33-year-old African American man, involving multiple spinal levels, pelvic bones, and the right chest wall. The patient required multiple orthopedic surgeries, including anterior an

Type-2 immune skewing in patients with disseminated coccidioidomycosis.

BACKGROUND: Disseminated coccidioidomycosis (DCM) is an often fatal and otherwise intractable condition requiring lifelong antifungal treatment. We have previously shown that a deranged polarization of CD4+ T cells toward a Type-2 phenotype can exist in the context of DCM. Here we studied a large population of subjects to determine the frequency of abnormal Type-2 skewing of CD4+ T cells in patients with coccidioidomycosis and to identify underlying genetic mechanisms supporting this skew. METHODS: We collected peripheral blood mononuclear cells from 204 patients with coccidioidomycosis, incl

Disseminated Skeletal Coccidioidomycosis: A Case Report and Comprehensive Review.

Coccidioidomycosis, caused by the dimorphic fungi Coccidioides immitis and Coccidioides posadasii, is endemic to arid regions such as the southwestern United States. Infection typically occurs via inhalation of aerosolized arthroconidia from disrupted soil. In the majority of cases, infection is asymptomatic or results in self-limiting respiratory symptoms; however, dissemination beyond the lungs may occur rarely. Disseminated disease is a rare occurrence, most commonly affecting the skeletal and central nervous systems, with skeletal involvement often manifesting as osteomyelitis in axial loc

Pediatric Coccidioidal Meningitis: A Systematic Review and Proportional Synthesis of Cases Reported in the Fluconazole Era (2000-2025).

Coccidioidal meningitis (CM) is a rare but life-threatening complication of disseminated coccidioidomycosis, occurring in ~16% of cases, particularly among children in endemic regions such as the southwestern US and northern Mexico. Without timely diagnosis and antifungal therapy, pediatric CM is almost universally fatal within the first year. Hydrocephalus develops in up to 50% of cases. In 2000, Galgiani et al. established fluconazole as first-line therapy for CM. Subsequent guidelines refined management but did not specifically address pediatric patients (>1 month-≤19 years). No studies in

Disseminated coccidioidomycosis mimicking malignant lymphoma in a 14-year-old female.

Coccidioidomycosis is a fungal infection that is often asymptomatic in immunocompetent individuals. When symptoms do occur, they often resemble a mild, flu-like illness. However, this disease can become clinically severe and disseminate, especially in immunocompromised patients or other high-risk groups. Failure to consider coccidioidomycosis in the differential diagnosis may lead to missed or delayed diagnosis, resulting in postponement of appropriate treatment.

A case of delayed diagnosis of disseminated coccidioidomycosis highlights opportunities for improved awareness.

BACKGROUND: As Coccidioides spp. are endemic in the southwestern United States, awareness of the risk factors for severe disease and dissemination is important to avoid delays in diagnosis and treatment. In addition to immunocompetent patients, individuals of certain ethnic backgrounds, especially African and Filipino ancestry, are at higher risk for dissemination. Diagnostic testing may be complicated by the lack of available invasive specimens for histopathologic examination and culture. CASE SUMMARY: A 29-year-old African American woman presented with a 6-year history of abdominal bloating

A Case of Reactivated Disseminated Coccidioidomycosis in an Elderly Patient.

Reactivation of extrapulmonary disseminated coccidioidomycosis infection is rare. When it does happen, it often happens soon after the primary infection. Reactivation of latent disease is usually seen in patients who later become immunocompromised. Here we discuss a case of an 88-year-old male patient with a distant history of extrapulmonary coccidioidomycosis infection 50 years prior, who developed reactivated coccidioidomycosis presenting as a soft tissue mass in his arm and new polyarthritis. Despite rapid initiation of fluconazole treatment, the patient struggled with his recovery, which w

A scalp to make you squirm: Cutaneous myiasis hiding a unique presentation of disseminated coccidioidomycosis.

Coccidioidomycosis is predominantly a pulmonary disease, however dissemination can include unique cutaneous presentations. We report a previously healthy 11-year-old female with delayed diagnosis of cutaneous scalp coccidioidomycosis with disseminated disease with secondary wound myiasis and bacterial superinfection. She required extensive surgical debridement, eventually requiring a skin graft and prolonged hospitalization for antifungal therapy, experiencing a unique adverse effect.

Disseminated peritoneal coccidioidomycosis mimicking malignancy in an immunosuppressed patient: a case report.

Disseminated peritoneal coccidioidomycosis is rare and can mimic peritoneal carcinomatosis. Ustekinumab, an IL-12/23 inhibitor, impairs Th1/Th17 immunity which is critical for fungal control. No prior cases link ustekinumab use to disseminated fungal disease. We report a 45-year-old woman from an endemic region with prior pulmonary coccidioidomycosis and psoriatic arthritis on ustekinumab who presented with malignant-appearing ascites and omental caking. Fluid studies were non-diagnostic, but laparoscopic biopsies confirmed disseminated Coccidioides infection. She improved with liposomal ampho

Disseminated coccidioidomycosis with cutaneous, pulmonary and suspected CNS involvement in an elderly patient with Parkinson's disease.

An elderly patient with recently diagnosed Parkinson's disease presented with disseminated coccidioidomycosis (DCM) manifesting as ulcerative nasal lesions, pulmonary involvement and acute-on-chronic cognitive decline. Culture-confirmed cutaneous disease, positive serology, and imaging demonstrated pulmonary and central nervous system (CNS) involvement, establishing the diagnosis. The patient achieved marked clinical improvement with high-dose oral fluconazole (800 mg, later tapered to 400 mg daily), with serial imaging documenting resolution of CNS and pulmonary lesions. Active fungal infections alter the respiratory microbiome profiles of Mayo Clinic Arizona patients.

INTRODUCTION: The function of the respiratory microbiome during an active infection is not well characterized. Studies from the gut microbiome suggest a diverse community can aid in modulating the immune system to control infectious pathogens. METHODS: To determine if there are microbial community compositional changes in the human lung during an infection, we conducted an analysis of both the 16S rDNA and the Internal Transcribed Spacer (ITS) region of DNA from bronchoalveolar lavage fluid (BALF) of patients from Mayo Clinic Arizona. In addition to general classification, we assessed differe

Pearls and Pitfalls of Intrathecal Amphotericin B Therapy for Refractory Coccidioidal Meningitis in Children: An Illustrative Pediatric Case Series.

INTRODUCTION: Incidence of pediatric coccidioidomycosis has risen over the last 20 years, and coccidioidal meningitis is the most severe form of disseminated disease. Intrathecal amphotericin B deoxycholate (IT AmBd) has historically improved outcomes and remains a mainstay of therapy when response to azole therapy is inadequate. However, the dosing, route of administration and duration of therapy in young pediatric patients pose unique challenges due to their anatomy, inability to communicate and the need for hospital-based administration. We present 4 instructive cases over 30 years to provi

Real-world performance of diagnostic testing for coccidioidal meningitis in initial sampling of the cerebrospinal fluid.

Early diagnosis and treatment are essential for improving outcomes of coccidioidal meningitis. While the detection of IgG by serologic testing of the cerebrospinal fluid (CSF) has been a mainstay of diagnosis for years, the diagnosis of coccidioidal meningitis in clinical practice can be very challenging due to suboptimal sensitivity of laboratory tests. We reviewed the results of the CSF diagnostic test results from the initial lumbar puncture in 110 patients with proven, probable, and likely coccidioidal meningitis from 1989 to 2024. One hundred four patients were diagnosed with coccidioidal

Intravenous Liposomal Amphotericin as an Adjunct to Fluconazole in Pediatric Patients With Central Nervous System Coccidioidomycosis: A Single-Center Case Series.

BACKGROUND: Coccidioidomycosis is a fungal infection endemic to the southwestern United States. Central nervous system (CNS) coccidioidomycosis is a severe manifestation that requires lifelong antifungal therapy. While fluconazole is currently recommended as the initial therapy in pediatric patients, a rise in reports of fluconazole failure has led to use of alternative treatment approaches. In this paper, we evaluated our institutional preference to use intravenous (IV) liposomal amphotericin B (L-AmB) as part of the initial therapy for pediatric CNS coccidioidomycosis. METHODS: We conducted

Insights into current coccidioidomycosis therapeutic pathways.

Coccidioidomycosis is a systemic fungal infection caused by Coccidioides immitis and Coccidioides posadasii, which are endemic to the Southwestern United States and parts of Mexico, Central, and South America. While oral fluconazole is widely recommended as first-line therapy, treatment failure is not uncommon for severe manifestations. This study aims to characterize antifungal treatment pathways, identify factors associated with treatment switches, and compare outcomes based on treatment courses. A retrospective cohort study was conducted using the TriNetX database with individual de-identif

Common and Uncommon Imaging Manifestations of Thoracic and Disseminated Coccidioidomycosis Infection.

Coccidioidomycosis is caused by the dimorphic fungus Coccidioides, endemic to the southwestern United States. Most infected patients are asymptomatic or recover uneventfully, but a small proportion will develop chronic infection or dissemination. Diagnosis typically rests on organism demonstration in tissue or serology, but knowledge of Coccidioides infection imaging findings becomes important when tissue examination is not possible and serologic results are unclear. Acute coccidioidal pneumonia manifests at chest imaging with consolidation and lymphadenopathy, resolving completely or evolving

A Unique Case of Disseminated Cutaneous Coccidioidomycosis Years After Initial Infection.

Coccidioides is a dimorphic fungus that causes coccidioidomycosis, also known as San Joaquin Valley Fever. The fungus is endemic to the southwestern United States, northern Mexico, and Central and South America. Infection is typically acquired through inhalation of dust particles that causes pulmonary disease. Direct cutaneous inoculation can occur but is rare. Histopathological examination of cutaneous lesions typically shows pseudoepitheliomatous hyperplasia with acute, suppurative granulomatous inflammation. Fungal organisms tend to be found more superficially and vary in density.
