An immunocompromised patient in Arizona, USA, experienced cutaneous coccidioidomycosis mimicking rosacea-like dermatitis; she was receiving vandetanib therapy. Histopathology confirmed the diagnosis despite inconclusive serologic results. Lesions resolved with fluconazole treatment, but blue-gray hyperpigmentation persisted, likely linked to vandetanib. This case highlights diagnostic challenges in endemic fungal infections and rare drug-associated hyperpigmentation.

We encountered a case of chronic pulmonary coccidioidomycosis complicated with meningitis in a 53-year-old African-American veteran with a history of staying in Arizona, United States of America. The patient was referred from another hospital with complaints of cough, blood-stained phlegm, hemoptysis, and headache. Chronic pulmonary coccidioidomycosis was suspected based on the presence of multiple pulmonary cavity lesions on computed tomography (CT) and a relevant exposure history, and the diagnosis was confirmed by serological antibody tests (coccidioidomycosis IgG, positive; coccidioidomyco

TNF-α inhibitors, including infliximab, adalimumab and etanercept, are used to treat various inflammatory diseases, such as arthritis, psoriasis and ankylosing spondylitis. However, these treatments may predispose patients to fungal infections, including histoplasmosis, candidiasis and aspergillosis. In this study, we systematically reviewed case reports to critically examine the correlations between anti-TNF-α therapies and the occurrence of invasive and superficial fungal infections. Infliximab was the most commonly used TNF-α inhibitor (50.65%). The highest number of fungal infections durin

Contemporary data on coccidioidomycosis death rates are sparse. Death certificate data for 2018-2023 from the US National Vital Statistics System were evaluated. Coccidioidomycosis deaths were identified using diagnosis codes B38.x listed anywhere on certificates. Deaths and age-adjusted mortality rates (AAMRs)/1,000,000 people, with 95% confidence intervals (CIs), were determined. We identified 1760 coccidioidomycosis-attributable deaths (AAMR = 0.75; 0.72-0.79). Most occurred in 55-74-year-olds (43.9%; corresponding AAMR = 1.72; 1.59-1.84). Males had a 2.69-fold increased AAMR versus females

BACKGROUND: Coccidioides fungemia is an uncommon but severe manifestation of disseminated coccidioidomycosis, associated with high mortality. Despite high endemicity in California's San Joaquin Valley, recent data from this region are limited. This study aims to describe the clinical characteristics, risk factors, and outcomes of patients with Coccidioides fungemia at a Central California tertiary care center.
METHODS: We conducted a retrospective review of patients diagnosed with Coccidioides fungemia between 2015 and 2025 at Community Regional Medical Centers (CRMC) in Fresno, California. D

Introduction. Coccidioidomycosis, or Valley fever, is a fungal disease caused by Coccidioides species, prevalent in parts of the southwestern United States. It usually results from inhaling spores from soil and is a common cause of pneumonia in these regions. Case Presentation. We present a unique case of coccidioidomycosis in an immunodeficient male patient secondary to human immunodeficiency virus infection with poor adherence to anti-retroviral treatment. After presenting with non-specific symptoms and pre-syncope, he was initially diagnosed with pneumonia based on chest X-ray findings, but

In this case, we present a rare occurrence of a zoonotic infection, Q fever, coexisting with a fungal infection, coccidioidal meningitis. When patients present with ambiguous symptoms, it becomes challenging to attribute their presentation to a particular disorder or disease, especially when they test negative for common infectious etiologies. This case highlights both the importance of considering rare infections early on and the possibility that multiple infectious agents can coexist, simultaneously contributing to a patient's illness.

BACKGROUND Acute myopericarditis can clinically and electrocardiographically mimic ST-elevation myocardial infarction (STEMI), leading to misdiagnosis and delayed treatment. In people living with HIV (PLWH), particularly those untreated, impaired immunity and viral-driven inflammation amplify cardiac involvement. Autopsy data suggest a myocarditis prevalence rate of up to 50% in AIDS patients, yet clinically overt cases remain rare. Diagnostic ambiguity can precipitate unnecessary invasive procedures and contribute to HIV-associated cardiomyopathy, an AIDS-defining illness. CASE REPORT A 62-ye

Taking a One Health approach to infectious diseases common to both dogs and people, pet insurance claims from 2008 to 2022 in the United States were compared to publicly available CDC-based data on human cases for Lyme disease, giardia, and Valley Fever (coccidioidomycosis). Despite having very different causative agents and etiologies, the disease trends for these three diseases were very similar between people and dogs both geographically and temporally. We furthermore demonstrated that adding dog data as a predictor variable in addition to the human data improves prediction models for those

INTRODUCTION: Pediatric Infectious Disease (PID) clinicians involved in solid organ transplantation often assess infection risk and mitigation strategies for donor organ offers. While some guidance is available, real-life practice patterns have not been previously described.
METHODS: We surveyed PID clinicians about organ acceptance and associated posttransplantation interventions using 12 fictitious pediatric case scenarios through 3 PID-specific listservs. Descriptive statistics were employed.
RESULTS: 48 (71.6%) of 67 ID respondents were involved in organ offer assessment. Agreement was s

For individuals with cell-mediated immunodeficiency (CMI), infections with opportunistic organisms such as mycobacteria, filamentous bacteria, fungi, and protozoa can lead to poor outcomes. The first part of this continuing medical education series reviews the diagnosis and management of cutaneous infections in hosts with impaired CMI. The second part addresses infections in patients with neutropenia and humoral immunodeficiency.

BACKGROUND: The burden of opportunistic infections (OIs) remains high among people living with HIV (PLWH) in Mexico, despite improvements in mortality worldwide.
OBJECTIVE: Reporting the current access to diagnostics of OIs in Mexican Health Care Centers offering health-care services to PLWH.
METHODS: An online questionnaire was sent to public health care facilities providing HIV care in Mexico. We evaluated capacities to 1) identify individuals with advanced-HIV, and 2) local and/or on-site access to: point-of-care assays, imaging studies, histological analysis, and microbiology tests usefu

Coccidioidomycosis, endemic in the southwestern United States, can lead to severe pulmonary complications, particularly in chronic obstructive pulmonary disease (COPD) patients with poor lung reserves. Mycobacterium abscessus has a predisposition for structurally damaged lungs, commonly causing difficult-to-treat bronchiectasis and cavitary lesions. We present the case of a 58-year-old patient with advanced COPD and a remote history of pulmonary coccidioidomycosis diagnosed 30 years earlier, who was found to have sputum and radiographic evidence of M. abscessus cavitary lung disease 4 years pr

BACKGROUND: Balamuthia mandrillaris is a rare cause of skin and/or central nervous system (CNS) infection with a high mortality rate. Dissemination to other organs, including the lungs, is rare. Despite worldwide distribution, this disease entity is underrecognized. Delay in diagnosis may contribute to high mortality.
CASE SUMMARY: A 17-year-old Hispanic male living in Arizona on immunosuppressive therapy for Castleman's disease presented with new cutaneous lesions and cough and was found to have miliary pulmonary disease on chest imaging. He was suspected of having disseminated coccidioidomy

INTRODUCTION: Shock is a life-threatening condition amongst hospitalized patients and requires urgent management to avoid mortality. Early exposure is vital for educational and patient safety purposes.
METHODS: We developed a 90-minute shock day session that provided internal medicine interns with a cognitive framework for the initial diagnosis and management of shock, which they applied to two simulations. The first simulation involved a patient with septic shock, and the second involved a patient with cardiogenic shock. Critical action checklists were used to assess learners and guide struc

A southern sea otter (Enhydra lutris nereis) stranded dead in central California, USA, with a distended pericardial sac containing thousands of free-floating proteinaceous masses. Serology, fungal culture, PCR, and sequencing confirmed the etiology of this novel lesion as Coccidioides immitis. Range expansion of this zoonotic pathogen is predicted with climate change.

The incidence of coccidioidomycosis continues to increase. The diagnosis frequently relies on non-invasive diagnostic testing with immunodiffusion and complement fixation (CF) testing the current gold standard. A direct comparison of quantitative immunodiffusion and CF for IgG antibodies has not been previously reported. In a comparison of 368 samples, there was close concordance observed (360/368 = 97.8%) (P-value < .001). These tests can be considerably interchangeable in the reference laboratory setting.

Coccidioidomycosis is a potentially fatal fungal disease of humans and animals that follows inhalation of Coccidioides spp. arthroconidia in the environment. The disease in dogs resembles that in people, and because dogs may be at increased risk of exposure due to their proximity to the ground and digging behavior, they are valuable models for the disease in humans. Dogs have been sentinels for identification of new regions of endemicity in Washington and Texas. Canine serosurveillance has also been used to predict variables associated with environmental presence of Coccidioides spp. Expansion

Valley Fever is a respiratory disease caused by inhalation of arthroconidia, a type of spore produced by fungi within the genus Coccidioides spp. which are found in dry, hot ecosystems of the Western Hemisphere. A quantitative microbial risk assessment (QMRA) for the disease has not yet been performed due to a lack of dose-response models and a scarcity of quantitative occurrence data from environmental samples. A literature review was performed to gather data on experimental animal dosing studies, environmental occurrence, human disease outbreaks, and meteorological associations. As a result,
