Coccidioidomycosis is a fungal infection that primarily manifests as an asymptomatic condition caused by inhalation of fungal spores. Less commonly, patients can develop dissemination to extrapulmonary locations such as the skin or primary cutaneous inoculation of the fungus at a site of trauma. These cutaneous complications are primarily found among immunosuppressed individuals. Here, we present a patient with no relevant past medical history that, after living in an area to which Coccidioides is endemic, developed a slowly growing, pruritic plaque on his right thigh with well-demarcated area

Coccidioidomycosis is a significant fungal disease primarily endemic to the southern United States, northern Mexico, and South America. Coccidioidomycosis is non-endemic in China, with only approximately 47 cases reported over the past 63 years. Recognizing that increased global travel is altering regional disease epidemiology, the Chinese Health Commission has classified Coccidioides species as highly pathogenic pathogens. The diagnosis of coccidioidomycosis poses a major challenge in China for medical institutions without biosafety level 3 (BSL-3) laboratories. In November 2021, we diagnosed

We evaluated the in vivo activity of EL219 against central nervous system coccidioidomycosis. Mice were inoculated intracranially with arthroconidia of Coccidioides immitis, and treatment with EL219 (5, 10, or 20 mg/kg QD by intraperitoneal injection) or fluconazole (25 mg/kg orally BID) began 2 days later. Each dose of EL219 and fluconazole significantly improved survival. Brain fungal burden was also reduced compared to vehicle control. Further studies of EL219 against coccidioidomycosis are warranted.

Coccidioidomycosis, a disease induced by the dimorphic fungi Coccidioides immitis and Coccidioides posadasii, is endemic to the southwestern United States and occurs with a myriad of clinical and imaging manifestations, which range from limited pulmonary disease to disseminated multiorgan infections. Humans are infected when arthroconidia in the soil are disturbed and dispersed into the air. Once inhaled, arthroconidia form spherules, which trigger a host cell-mediated immune response. If this response is adequate, it results in granuloma formation, halting the infection. However, an insuffici

We analyzed inpatient data from Texas, USA, to characterize hospitalizations for disseminated coccidioidomycosis and coccidioidal meningitis from 2016-2023. Geographic mapping revealed a substantial disease effect in both western Texas and metropolitan areas. Our findings suggest the need for enhanced surveillance and increased healthcare provider awareness regarding coccidioidomycosis in Texas.

BACKGROUND: Coccidioidomycosis (Valley fever) is an endemic fungal infection of the southwestern United States, with an estimated 273,000 symptomatic infections, 23,000 hospitalizations, and 900 deaths annually. Coccidioidal meningitis, the most severe manifestation, requires lifelong antifungal therapy and carries high rates of rehospitalization, nonadherence, and mortality. Quantitative studies have documented poor outcomes, but the barriers driving these outcomes have not been examined from the patient's perspective. This study aimed to characterize structural and systemic barriers to longi

Invasive fungal infections (IFIs) are associated with high mortality rates and pose important therapeutic challenges because of the limited number of antifungal drug classes, their toxicity, and the emergence of resistance. Novel first-in-class antifungal agents have been recently approved (ibrexafungerp) or are currently being tested in clinical trials and are available via expanded access programs (olorofim, fosmanogepix). Among them, fosmanogepix (a prodrug of manogepix) represents a promising future therapeutic option for many IFIs because of its broad-spectrum antifungal activity and its

OBJECTIVE: Coccidioidal meningitis (CM) is frequently complicated by hydrocephalus, requiring cerebrospinal fluid (CSF) diversion. Loss of ventricular compliance and shunt malfunction can result in low-pressure hydrocephalus (LPH), posing challenges to definitive shunt management. We report our institutional experience using prolonged external ventricular drainage (EVD) at subatmospheric pressures to re-establish ventricular compliance prior to permanent CSF diversion in patients with CM-associated LPH.
METHODS: A retrospective review was performed of 12 adult patients with confirmed CM who u

Coccidioidomycosis (Valley Fever) is a fungal infection endemic to the southwestern United States. Meningitis occurs in approximately 1% of cases and often results in hydrocephalus necessitating shunting and lifelong antifungal therapy. Prior studies lack comparison with contemporaneous controls. In this study, we compare radiographic, laboratory, and longitudinal shunt survival data until revision and reprogramming. The objective was to compare the 7-year outcomes between patients who underwent shunting with and without coccidioidomycosis meningitis (CM). We identified neurosurgical patients

Coccidioidomycosis is typically a self-limited pulmonary infection but may cause severe disseminated disease in pregnancy. Central nervous system involvement is the most serious manifestation and can be diagnostically challenging due to nonspecific symptoms. We report a case of disseminated Coccidioides posadasii infection in a pregnant patient presenting with persistent headache subsequently complicated with coccidioidal meningitis and hydrocephalus requiring neurosurgical intervention. This case highlights the importance of maintaining suspicion for endemic fungal infections in pregnant pati

Reactivation of latent fungal infections poses a substantial risk for solid organ transplantation (SOT) and hematopoietic cell transplantation (HCT) recipients. We describe receipt of pretransplant fungal infection testing in an exploratory analysis among patients in a large US commercial health insurance database. We identified patients who received an SOT or HCT during January 1, 2018-January 31, 2025, and evaluated testing practices for selected fungal (blastomycosis, coccidioidomycosis, cryptococcosis, histoplasmosis) and non-fungal infections (including hepatitis B virus [HBV], hepatitis

Thermal dimorphic fungal pathogens are fungi that infect humans, often through the inhalation of asexual conidia, and which transition from hyphae to yeast in the human body. These fungi cause severe or chronic mycoses and are typically treated with azoles or amphotericin B. Ambruticin, a polyketide antifungal, shows promise as an alternative therapy. It targets hybrid histidine kinases (HHKs), which are fungal-specific proteins essential for osmoregulation and parasitic morphology and are conserved across thermal dimorphic species. Targeting HHKs suggests that ambruticin may therapeutically t

Coccidioidomycosis (Valley fever) is an environmentally acquired fungal infection endemic to the arid Americas, presenting a growing public health challenge as changing environmental patterns threaten to amplify exposure risks across both established and newly recognized endemic zones. Historically, forecasting efforts have relied on statistical correlations with meteorological variables. These phenomenological models often fail to capture the complex, non-linear interactions between the saprobic (environmental) and parasitic (host) life cycles of Coccidioides, particularly under non-stationar

UNLABELLED: Coccidioides is an endemic fungus that is increasing in prevalence and causes life-threatening diseases in immunocompetent people. In the environment, the spores (arthroconidia) develop into hyphae; however, when they are inhaled by a mammalian host, they develop into a unique form called the spherule. The transition to spherule can be triggered in vitro with elevated temperatures and high CO2 levels, but how the host triggers Coccidioides spherulation is not known. We used live imaging to investigate how macrophages affect the fate of Coccidioides arthroconidia. Under tissue cultu

Coccidioidomycosis is endemic to the Western Hemisphere and is rarely encountered in Asia, including Thailand. A detailed history, particularly of prior travel to endemic areas, is critical in refining the differential diagnosis and considering coccidioidomycosis. Here, we report an early adolescent male who presented with fever, non-productive cough and pleuritic chest pain of 1 month's duration. Radiologic findings were suggestive of pulmonary tuberculosis. Antituberculosis therapy was started, but there was no clinical improvement. However, on specific questioning, we learned that he had rec

BACKGROUND: Foot orthoses (FOs) are prescribed as an important conservative treatment option in patients with foot problems related to rheumatoid arthritis. However, a broad variation in FOs is used, both in clinical practice and in research. To date, there is no overview on the outcomes of the treatment with different kinds of FOs in patients with rheumatoid arthritis and a specific foot problem. The objectives of the present study were to summarize the comparative effectiveness of FOs in the treatment of various foot problems in patients with rheumatoid arthritis, on the primary outcomes foo

Leptomeningeal enhancement (LE) is an MRI neuroimaging characteristic associated with infection, malignancy, or cerebrovascular pathologies of the subarachnoid space. LE secondary to infection is most commonly a sign of meningeal tuberculosis. Fungal infections have a heightened presence in certain endemic regions in North America. We present the case of a 44-year-old male patient from Illinois with persistent headaches, hydrocephalus, and LE on MRI. He was empirically treated for tuberculosis, then blastomycosis, but later was found to have disseminated Coccidioides immitis infection via lymp

INTRODUCTION: Coccidioidomycosis is a fungal infection endemic to the southwestern United States, particularly Arizona. Fine needle aspiration biopsy (FNAB) is established as effective in diagnosing infectious diseases. However, existing literature evaluating FNAB of thoracic coccidiomycosis remains limited. We present a large single institution series of thoracic coccidioidomycosis cases diagnosed through FNAB.
MATERIALS AND METHODS: The Mayo Clinic Arizona Pathology database was searched for all FNAB cases with a diagnosis of coccidioidomycosis from January 1, 2013, to December 31, 2025. El

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. In this conte

BACKGROUND: Although robotic-assisted bronchoscopy (RAB) primarily aims to diagnose early stage lung cancer, many pulmonary nodules stem from benign causes, including atypical infections (ie, Coccidioides species, Aspergillus species, nontuberculous mycobacteria). Data on prevalence of atypical infections in patients undergoing RAB with biopsy of pulmonary nodules are limited.
RESEARCH QUESTION: What is the prevalence of atypical and bacterial infections in patients undergoing robotic-assisted bronchoscopy with biopsy of pulmonary nodules in a coccidioidomycosis endemic area?
